Les missions spatiales d'observation de la Terre de nouvelle génération telles que Sentinel-2 (préparé par l'Agence Spatiale Européenne ESA dans le cadre du programme Copernicus, auparavant appelé Global Monitoring for Environment and Security ou GMES) ou Venµs, conjointement développé par l'Agence Spatiale Française (Centre National d 'Études Spatiales CNES) et l'Agence Spatiale Israélienne (ISA), vont révolutionner la surveillance de l'environnement d' aujourd'hui avec le rendement de volumes inédits de données en termes de richesse spectrale, de revisite temporelle et de résolution spatiale. Venµs livrera des images dans 12 bandes spectrales de 412 à 910 nm, une répétitivité de 2 jours et une résolution spatiale de 10 m; les satellites jumeaux Sentinel-2 assureront une couverture dans 13 bandes spectrales de 443 à 2200 nm, avec une répétitivité de 5 jours, et des résolutions spatiales de 10 à 60m. La production efficace de cartes d'occupation des sols basée sur l'exploitation de tels volumes d'information pour grandes surfaces est un défi à la fois en termes de coûts de traitement mais aussi de variabilité des données. En général, les méthodes classiques font soit usage des approches surveillées (trop coûteux en termes de travaux manuels pour les grandes surfaces), ou soit ciblent des modèles locaux spécialisés pour des problématiques précises (ne s'appliquent pas à autres terrains ou applications), ou comprennent des modèles physiques complexes avec coûts de traitement rédhibitoires. Ces approches existantes actuelles sont donc inefficaces pour l'exploitation du nouveau type de données que les nouvelles missions fourniront, et un besoin se fait sentir pour la mise en œuvre de méthodes précises, rapides et peu supervisées qui permettent la généralisation à l'échelle de grandes zones avec des résolutions élevées. Afin de permettre l'exploitation des volumes de données précédemment décrits, l'objectif de ce travail est la conception et validation d'une approche entièrement automatique qui permet l'estimation de la couverture terrestre de grandes surfaces avec imagerie d'observation de la Terre de haute résolution spatiale, spectrale et temporelle, généralisable à des paysages différents, et offrant un temps de calcul opérationnel avec ensembles de données satellitaires simulés, en préparation des prochaines missions. Cette approche est basée sur l'intégration d'algorithmes de traitement de données, tels que les techniques d'apprentissage de modèles et de classification, et des connaissances liées à l'occupation des sols sur des questions écologiques et agricoles, telles que les variables avec un impact sur la croissance de la végétation ou les pratiques de production. Par exemple, la nouvelle introduction de température comme axe temporel pour un apprentissage des modèles ultérieurs intègre un facteur établi de la croissance de la végétation à des techniques d'apprentissage automatiques pour la caractérisation des paysages. Une attention particulière est accordée au traitement de différentes questions, telles que l'automatisation, les informations manquantes (déterminées par des passages satellitaires, des effets de réflexion des nuages, des ombres ou encore la présence de neige), l'apprentissage et les données de validation limitées, les échantillonnages temporels irréguliers (différent nombre d'images disponible pour chaque période et région, données inégalement réparties dans le temps), la variabilité des données, et enfin la possibilité de travailler avec différents ensembles de données et nomenclatures. / The new generation Earth observation missions such as Sentinel-2 (a twin-satellite initiative prepared by the European Space Agency, ESA, in the frame of the Copernicus programme, previously known as Global Monitoring for Environment and Security or GMES) and Venµs, jointly developed by the French Space Agency (Centre National d'Études Spatiales, CNES) and the Israeli Space Agency (ISA), will revolutionize present-day environmental monitoring with the yielding of unseen volumes of data in terms of spectral richness, temporal revisit and spatial resolution. Venµs will deliver images in 12 spectral bands from 412 to 910 nm, a repetitivity of 2 days, and a spatial resolution of 10 m; the twin Sentinel-2 satellites will provide coverage in 13 spectral bands from 443 to 2200 nm, with a repetitivity of 5 days, and spatial resolutions of 10 to 60m. The efficient production of land cover maps based on the exploitation of such volumes of information for large areas is challenging both in terms of processing costs and data variability. In general, conventional methods either make use of supervised approaches (too costly in terms of manual work for large areas), target specialised local models for precise problem areas (not applicable to other terrains or applications), or include complex physical models with inhibitory processing costs. These existent present-day approaches are thus inefficient for the exploitation of the new type of data that the new missions will provide, and a need arises for the implementation of accurate, fast and minimally supervised methods that allow for generalisation to large scale areas with high resolutions. In order to allow for the exploitation of the previously described volumes of data, the objective of this thesis is the conception, design, and validation of a fully automatic approach that allows the estimation of large-area land cover with high spatial, spectral and temporal resolution Earth observation imagery, being generalisable to different landscapes, and offering operational computation times with simulated satellite data sets, in preparation of the coming missions.

Factors contributing to the commission of errors and omission of standard nursing practice among new nurses

Every year, millions of medical errors are committed, costing not only patient health and satisfaction, but thousands of lives and billions of dollars. Errors occur in many areas of the healthcare environment, including the profession of nursing. Nurses provide and delegate patient care and consequently, standard nursing responsibilities such as medication administration, charting, patient education, and basic life support protocol may be incorrect, inadequate, or omitted. Although there is much literature about errors among the general nurse population and there is indication that new nurses commit more errors than experienced nurses, not much literature asks the following question: What are the factors contributing to the commission of errors, including the omission of standard nursing care, among new nurses? Ten studies (quantitative, qualitative, and mixed-mode) were examined to identify these factors. From the 10 studies, the researcher identified the three themes of lack of experience, stressful working conditions, and interpersonal and intrapersonal factors. New nurses may not have had enough clinical time, may develop poor habits, may not turn to more experienced nurses and other professionals, may be fatigued from working too many hours with not enough staffing, may not be able to concentrate at work, and may not give or receive adequate communication. Based on these findings and discussion, suggested implications for nursing practice include extended clinical experience, skills practice, adherence to the nursing process, adherence to medications standards such as the five rights and independent double verification, shorter working hours, adequate staffing, no-interruption and no-phone zones, creating a culture of support, electronically entered orders, translation phones, read-backs, and standardized handoff reports.

La surveillance sentinelle pour les maladies vectorielles : une étude de cas de la maladie de Lyme au Canada Sentinel surveillance for vector-borne disease : a case study of Lyme disease in Canada

Les maladies vectorielles sont en processus d’émergence à travers le monde. Au Canada, la maladie de Lyme (ML) a été identifiée comme une maladie infectieuse émergente prioritaire. La surveillance, si elle est efficace, peut suivre le portrait d’une maladie en évolution afin d'informer les autorités de santé publique; toutefois, en raison de ressources limitées, elle doit être optimisée. La surveillance sentinelle permet de réduire les coûts, car un nombre limité d'unités statistiques sont mesurées de manière répétée dans le temps. Néanmoins, ces unités sentinelles doivent être représentatives du portrait épidémiologique de la maladie pour assurer l'efficacité du système de surveillance. Le premier objectif de cette thèse était d'évaluer la représentativité de la surveillance sentinelle pour décrire le risque de ML au Québec, Canada. Deux types de systèmes de surveillance active acarologique sont déjà en place au Québec : 1) un système sentinelle, où les sites de terrain sentinelles sont maintenus constants et visités à chaque saison de terrain, et 2) un système de surveillance basé sur le risque où les sites accessoires sont priorisés en fonction de leur profil de risque. Des mesures de danger acarologique, en termes d'estimations de la densité de nymphes, ont été dérivées des sites sentinelles entre 2015 et 2019 et comparées à celles obtenues dans les sites accessoires. Les mesures de danger acarologique dérivées des sites sentinelles ont également été corrélées avec le nombre de cas humains rapportés à l'échelle municipale pour déterminer si elles étaient représentatives du risque de ML pour la population humaine. Il a été démontré que le système de surveillance sentinelle était capable de suivre les tendances spatio-temporelles d’incidence de ML dans les populations humaines de la zone d'étude et fournissait un meilleur indicateur de l’incidence de ML par rapport au système de surveillance basé sur le risque. Cependant, bien que les modèles aient pu prédire le risque de maladie de Lyme, les sites sentinelles n'ayant pas été choisis selon une approche validée, on peut émettre l’hypothèse que l’utilisation d'une approche holistique standardisée pour la sélection de sites sentinelles pourrait optimiser le design spatial du système de surveillance. Le deuxième objectif de cette thèse était de développer une telle approche et de l'appliquer à une étude de cas : la surveillance acarologique sentinelle pour le risque de ML à travers le Canada. Une revue de la portée a été utilisée pour inventorier les initiatives précédentes de surveillance sentinelle pour les maladies vectorielles, et pour cataloguer les critères qui ont été utilisés pour sélectionner les emplacements des unités sentinelles dans la zone d'étude. Les articles pertinents ont ensuite été analysés à l'aide d'une revue du type réaliste afin de créer un outil décisionnel permettant de sélectionner des critères pertinents pour la planification du design spatial d'un système de surveillance sentinelle pour les maladies vectorielles. Enfin, l'outil a été utilisé lors de la création d'un nouveau réseau de surveillance sentinelle pour le risque de ML au Canada; les critères retenus ont été incorporés dans une analyse multi-critères spatiale afin de sélectionner les régions sentinelles pour le réseau de surveillance acarologique active. Dans l'ensemble, cette thèse a exploré la surveillance sentinelle pour les maladies vectorielles, et a développé et testé une approche pour optimiser et standardiser la planification du design spatial des systèmes de surveillance sentinelle pour les maladies vectorielles. Dans de futurs travaux, cette approche devrait être mise en œuvre, évaluée et validée pour d’autres maladies et contextes épidémiologiques. / Vector-borne diseases are emerging all over the globe. In Canada, Lyme disease (LD) has been identified as a priority emerging infectious disease. Public health surveillance, if effective, can track disease risk to inform public health authorities; however, due to finite resources, it must be optimized. Sentinel surveillance can cut costs, since a limited number of statistical units are measured repeatedly through time. Nonetheless, these sentinel units must be representative of the risk landscape to ensure an effective surveillance system. The first objective of this thesis was to evaluate the representativeness of sentinel tick surveillance for the risk of LD in Québec, Canada. Two types of tick-based active surveillance systems are already in place in Québec: 1) a sentinel system, where sentinel field sites are kept constant and visited every field season, and 2) a risk-based surveillance system where accessory sites are prioritized according to their risk profile. Acarological hazard measures, in the form of nymph density estimates, were derived from sentinel sites between 2015 and 2019 and compared with those obtained from accessory sites. Hazard measures derived from sentinel sites were also correlated with LD incidence at the municipal scale to see if they were representative of LD risk to human populations. It was shown that the sentinel tick-based surveillance system was able to follow spatiotemporal LD incidence trends in human populations across the study zone and provided a better indicator of LD incidence in comparison with the risk-based surveillance system. However, as sentinel sites were not chosen using a validated approach, it can be hypothesized that the spatial design for the system could be optimized through the development of a standardized, holistic approach for sentinel site selection. The second objective of this thesis was therefore to develop such an approach and apply it to a case example: sentinel surveillance of LD across Canada. A scoping review was used to inventory previous sentinel surveillance initiatives for vector-borne diseases, and catalogue criteria which had been used to select sentinel unit locations across the study zone. Relevant papers were subsequently analyzed using a realist-type review to create a decision tool to select relevant criteria for planning the spatial design of a sentinel surveillance system for vector-borne diseases. Finally, the tool was applied to guide the creation of a new sentinel tick-based surveillance network for LD risk in Canada; the retained criteria were incorporated into a spatial multi-criteria decision analysis to select sentinel regions for the active surveillance network. Overall, this thesis has explored sentinel surveillance for vector-borne disease and has developed and applied an approach to optimize and standardize spatial design planning for vector-borne sentinel surveillance systems. In future work, this approach should be implemented, evaluated, and validated for other types of diseases and epidemiological contexts.

The Application of Differential Synthetic Aperture Radar Interferometry Dataset for Validation, Characterization and Flood Risk Analysis in Land Subsidence-Affected Areas

This interdisciplinary doctoral dissertation addresses land subsidence in different and diverse study cases in the world, employing advanced techniques and methodologies to measure their magnitude and comprehensively explore its causes, and implications. Investigating areas such as the San Luis Potosi metropolitan area, Alaşehir-Sarıgöl sub-basin (ASSB) in Türkiye, and the Alto Guadalentín Valley in Spain, the research unveils critical insights into the complex dynamics of subsidence phenomena. Utilizing advanced remote sensing techniques like Persistent Scatterer Interferometry (PSI) and Coherent Pixels Technique (CPT), the study assesses subsidence rates and correlates them with factors such as trace faults, groundwater extraction, and soft soil thickness. Validation methodologies were developed and proposed to the scientific community on the first stage, integrating Global Navigation Satellite System (GNSS) benchmarks, enhance the reliability of Differential Synthetic Aperture Radar Interferometry (DInSAR) measurements, ensuring a robust foundation for subsequent analyses. The research aims to contribute to the understanding of land subsidence and contribute to create a decision-support framework to mitigate the phenomenon while addressing specific research objectives within each identified topic of inquiry. The research topic 1 includes the “DInSAR for monitoring land subsidence in overexploited aquifers”. In the San Luis Potosi metropolitan area (Mexico), the application of CPT technique reveals intriguing correlations between trace faults, land subsidence, and groundwater extraction. Specifically, areas in the municipality of Soledad de Graciano Sánchez exhibit subsidence values ranging between -1.5 and -3.5 cm/year, while in San Luis Potosi, values range from -1.8 to -4.2 cm/year. The validation of CPT results against five Global Navigation Satellite System (GNSS) benchmarks establishes a robust correlation of 0.986, underlining the reliability of InSAR-derived deformations. Additionally, in regions like the Alaşehir-Sarıgöl sub-basin (Türkiye), where water stress is heightened due to intensive agricultural irrigation, the study explores the roles of tectonic activity and groundwater withdrawal in land subsidence. Utilizing the P-SBAS algorithm, 98 Sentinel-1 SAR images in ascending orbits and 123 in descending orbits were analysed, covering the period from 2016 to 2020. Independent Component Analysis was applied to distinguish long-term displacements from seasonal variations in the DInSAR time series data. Displacement rates of up to -6.40 cm/year were identified, thus, the proposed P-SBAS algorithm facilitates the monitoring of displacement, revealing direct correlations between DInSAR displacement and critical factors like aquitard layer compaction. These findings contribute valuable insights into the dynamic interactions shaping overexploited aquifers. The research topic 2, developing parallelly to topic 1, consists of the “Validation of DInSAR data applied to land subsidence areas”. Addressing the imperative for validation methodologies in subsidence assessments, a systematic approach introduces statistical analyses and classification schemes. This methodology is designed to validate and refine DInSAR data, enhancing the reliability of subsidence assessments. By normalizing Root Mean Square Error (RMSE) parameters with the range and average of in-situ deformation values and employing the squared Pearson correlation coefficient (R²), a classification scheme is established. This scheme facilitates the acceptance/rejection of DInSAR data for further analyses through the application of automatic analysis supported by a Matlab © code, ensuring a more accurate representation of land subsidence phenomena. The research topic 3 covers the exploitation of DInSAR data for assessing flooding potential and determining characteristic parameters of aquifer systems. The first one is “Impact of land subsidence on flood patterns”. The study in the Alto Guadalentín Valley, a region experiencing extreme flash floods jointly with high-magnitude land subsidence, integrates flood event models, Differential interferometric SAR (DInSAR) techniques, and 2D hydraulic flow models. Through Synthetic Aperture Radar (SAR) satellite images and DInSAR, land subsidence's magnitude and spatial distribution are quantified. The results demonstrate significant changes in water surface elevation between the two 1992 and 2016 temporal scenarios, leading to a 2.04 km² increase in areas with water depths exceeding 0.7 m. These outcomes, incorporated into a flood risk map and economic flood risk assessment, underscore the pivotal role of land subsidence in determining inundation risk and its socio-economical implications. The research offers a valuable framework for enhancing flood modelling by considering the intricate dynamics of land subsidence. The second application of DInSAR data is about the “Automatic calculation of skeletal storage coefficients in aquifer systems”. In response to the need for automating data analysis for specific storage coefficients in aquifer systems, a MATLAB© application is introduced. This application streamlines the correlation between piezometric levels and ground deformation, significantly reducing analysis time and mitigating potential human interpretation errors. The developed application integrates temporal groundwater level series from observation wells and ground deformation data measured by in-situ or remote sensing techniques (e.g., DInSAR). Through the automatic construction of stress-strain curves, the application contributes to the estimation of skeletal storage coefficients, offering a valuable tool for evaluating aquifer system behaviours. This comprehensive research, guided by the complexities of these three distinct research topics, yields detailed insights and methodological advancements. A participação de um serviço público na atenção e implementação de ações à saúde do viajante no Brasil / The participation of a public service in attention and implementation of traveler health actions in Brazil

A medicina de viagem (MV) surgiu em resposta ao crescente deslocamento populacional, com o objetivo de prevenir os agravos à saúde relacionados às viagens. No Brasil teve inicio no final da década de 90, momento em que reformas socioeconômicas levaram a melhorias das condições de vida dos brasileiros. O Núcleo de Medicina do Viajante (NMV), do Instituto de Infectologia Emilio Ribas (IIER), foi o primeiro serviço de atenção à saúde do viajante criado na cidade de São Paulo, em maio de 2000. O presente estudo visa: descrever a população de viajantes que procuraram orientação pré-viagem no Núcleo de Medicina do Viajante (NMV) do Instituto de Infectologia Emilio Ribas (IIER) no período de janeiro de 2006 a dezembro de 2010; descrever as medidas de prevenção recomendadas em relação às doenças infecciosas; descrever as atividades de ensino realizadas e a participação do serviço na discussão de diretrizes em políticas públicas em medicina de viagem. No período estudado, 2744 viajantes procuraram orientação pré-viagem no NMV do IIER. Foram realizados 2836 atendimentos de orientação pré-viagem, 92 viajantes procuraram o serviço mais de uma vez. A faixa etária entre 18 e 34 anos (54,2%), o sexo feminino (51,1%) e grau de educação superior (75,5%) foram as principais características demográficas desses viajantes. Os destinos mais procurados foram: África (24,5%), Europa (21,2%), Ásia (16,6%) e Brasil (19,2%). O turismo (35,7%) e o trabalho (35,7%) foram os motivos de viagem mais referidos. O tempo de permanência menor ou igual a 30 dias foi referido pelos viajantes em que o objetivo de viagem foi o turismo, enquanto os viajantes que referiram o trabalho ou estudo apresentaram maior tempo de permanência (p < 0,001). O meio de transporte mais referido foi o aéreo (62,8%). Os viajantes relataram durante a consulta pré-viagem dificuldade de acesso ao serviço. As fontes de informação mais referidas foram: informação a partir de amigos, indicação por profissional da saúde e mídia eletrônica. As medidas de prevenção recomendadas variaram conforme o destino. O tratamento autoadministrado para diarreia foi mais recomendado aos viajantes com destino à Ásia. As vacinas de febre amarela, poliomielite e antimeningocócica A e C foram mais recomendadas aos viajantes com destino à África, assim como a quimioprofilaxia para malária, que foi recomendada para 26,4% dos viajantes para esse destino. A quimioprofilaxia (QPX) para malária foi recomendada em 10,3% de todas as orientações. Houve diferença com significância estatística na recomendação segundo a finalidade (p < 0,30), o destino (p < 0,001) e a duração da viagem (p < 0,001). Das 422 orientações realizadas aos viajantes com destino ao Brasil, a QPX foi recomendada somente para 30 (7,1%). Dos 2744 viajantes atendidos, 664 (24,2%) relataram pelo menos uma morbidade prévia; 66 (2,4%) eram menores de 10 anos de idade; e 157 (5,7%) tinham 60 anos ou mais. Em relação às atividades de ensino, no período do estudo, 83 médicos residentes estagiaram no NMV e foram orientadas onze monografias de conclusão de residência médica. O NMV participou de 12 reuniões para discussão de diretrizes sobre a saúde do viajante e de iniciativas como a Carta de São Paulo (documento em defesa da saúde do viajante elaborado por acadêmicos e profissionais de saúde participantes do SUS). Da criação da Sociedade Brasileira de Medicina de Viagem e da criação do Comitê Estadual de Saúde do Viajante, pela Secretaria de Estado da Saúde de São Paulo. Essas atividades foram passos decisivos para corroborar a implementação de políticas públicas em saúde do viajante no Brasil / Travel medicine (TM) arose in response to the growing population displacement and its objective is the prevention of health problems related to travel. Began in Brazil in the in the late 1990s, at which socioeconomic reforms have led to improvements in of life of Brazilians. Travel Medicine Center (TMC) at \"Instituto de Infectologia Emilio Ribas\" (IIER) was the first health of traveler service created in São Paulo city in May 2000. The present study aims: to describe the traveler population who sought pre-travel guidance in TMC at IIER from January 2006 to December 2010; to describe the recommended preventive measures to travelers concerned with infectious diseases; to describe the teaching activities performed and the service involvement in debating guidelines about public policies in travel medicine. In the time period studied, a total of 2744 travelers sought pre-travel guidance in TMC of IIER, but 2836 assistances were provided for pre-trip orientation since 92 travelers sought the service more than once. The age group between 18-34 years (54.2%), female (51.1%), and university level degree (75.5%) were the main demographic characteristics. The more popular destinations were: Africa (24.5%), Europe (21.2%), Asia (16.6%) and Brazil (19.2%). Tourism (35.7%) and work (35.7%) were the main purposes of trip for travelers. For tourism purpose travelers would stay 30 or less days, while for work or study they stayed for a longer time (p <0,001). Commercial air travel was the preferred alternative (62.8%). The main sources of information were friends, health professionals, and electronic media. The recommended preventive measures varied according to the destination. The self-treatment for diarrhea was more recommended for travelers to Asia. Vaccination against yellow fever, polio, and anti-meningococcal (A and C) was more recommended for travelers to Africa, as well as chemoprophylaxis (CP) of malaria was recommended for 26.4% of travelers to the same country. The CP of malaria was indicated for 10.3% of all assistances. There was statistically significant difference in the recommendation according to purpose (p < 030), destination (p < 0.001) and trip duration (p < 0.001). From 422 assistances to travelers to Brazil, CP was only recommended for 30 (7.1%) travelers. From the 2744 travelers assisted, 664 (24.2%) reported at least one previous morbidity; 66 (2.4%) were under age 10; and 157 (5.7%) were 60 years or older. During the research period and relating to study activities, 83 residents were interns in TMC, and eleven monographs for completion of the Medical Residency were supervised. TMC participated in 12 meetings to discuss guidelines in travel medicine, and participated in initiatives such as \"Carta de São Paulo\" (a document in defense of traveler health prepared by academics and health professionals participating in the Unified Health System); in the institution of the Brazilian Society of Travel Medicine, and of the São Paulo State Travel Medicine Committee by the Secretariat for Health of São Paulo State. The role of wild deer in the epidemiology and management of bovine tuberculosis in New Zealand

The eco-epidemiology of bovine tuberculosis (Tb) in wild deer (mainly red deer Cervus elaphus) in New Zealand was investigated. Bovine Tb is caused by Mycobacterium bovis. Specific aims were to clarify the likely routes of infection in deer, and to determine the status of deer as hosts of Tb, the likely rates and routes of inter- and intra-species transmission between deer and other wildlife hosts, the role of deer in spreading Tb, and the likely utility of deer as sentinels of Tb presence in wildlife. As the possum (Trichosurus vulpecula) is the main wildlife host of Tb, the research also included some investigation of transmission routes in possums. Patterns of infection were measured in 994 deer killed between 1993 and 2003. Tb prevalence varied between areas (range 8–36%). Few deer had generalised infection, with 21–68% of infected deer having no visible lesions, depending on the area. The retropharyngeal lymph nodes and oropharyngeal tonsils were commonly infected. No dependent fawns less than 0.75 years old were infected, indicating intra-species transmission is rare in wild deer. Where possums were not controlled, the net (cumulative) force of infection in young (1–4 y) deer was 0.10–0.24 per year in males and 0.09–0.12 per year in females, but much lower in older deer (less than 0.05 per year). Possum control reduced the net force of infection quickly, and eventually to zero. However, Tb persisted in possum-controlled areas through immigration of infected deer and, for almost a decade, through the survival of resident deer infected before possum control. Tb was lost from infected deer at an exponential rate of 0.13 per year, mostly as a result of deer recovering from infection rather than dying from it. Wild deer do die of Tb, but there was no discernible effect on age structure. The occurrence of infection in deer was not linked to the local deer or possum density at their kill sites (i.e. in their home range), but the area-wide prevalence of Tb in deer was closely correlated with Tb levels in possums, which were in turn correlated with area-wide measures of possum density. For wild deer in New Zealand, Tb is a persistent but usually inconsequential disease of the lymphatic system. It is acquired mainly by young independent deer, usually orally via the tonsils, and probably as a result of licking infected possums. Many species fed on deer carrion, including possums. Most possums encountering carrion did not feed on it, but a few fed for long periods. Other scavengers such ferrets (Mustela furo), hawks (Circus approximans), and weka (a hen-sized flightless native bird; Gallirallus australis) fed in a way that probably increased the infectivity of carrion to possums. Commercial deer hunting may have facilitated the historical establishment of Tb in possums. Scavenging (including cannibalism) and interactions with dead and dying possums are identified for the first time as potentially important routes for transmission of Tb to possums, and I develop new hypotheses involving peri- and post-mortem transmission in possums that explain many of the epidemiological patterns that are characteristic of the disease in possum. In continuous native forest, deer home range size averaged 250 hectares for six young females, and over twice that for two males. Over 90% of infected deer are likely to die within 2 km (females) or 6 km (males) of where they acquired Tb, but deer could occasionally carry Tb up to 30 km. Deer will be useful as sentinels, but only where other sentinels are rare, because the force of infection for a deer with a single infected possum in its home range is only 0.004 per year, compared to greater than 0.2 per year for deliberately released pigs. Deer are occasionally capable of initiating new cycles of infection in wildlife, but deer control is not essential to eradicate Tb from wildlife.

A participação de um serviço público na atenção e implementação de ações à saúde do viajante no Brasil / The participation of a public service in attention and implementation of traveler health actions in Brazil

A medicina de viagem (MV) surgiu em resposta ao crescente deslocamento populacional, com o objetivo de prevenir os agravos à saúde relacionados às viagens. No Brasil teve inicio no final da década de 90, momento em que reformas socioeconômicas levaram a melhorias das condições de vida dos brasileiros. O Núcleo de Medicina do Viajante (NMV), do Instituto de Infectologia Emilio Ribas (IIER), foi o primeiro serviço de atenção à saúde do viajante criado na cidade de São Paulo, em maio de 2000. O presente estudo visa: descrever a população de viajantes que procuraram orientação pré-viagem no Núcleo de Medicina do Viajante (NMV) do Instituto de Infectologia Emilio Ribas (IIER) no período de janeiro de 2006 a dezembro de 2010; descrever as medidas de prevenção recomendadas em relação às doenças infecciosas; descrever as atividades de ensino realizadas e a participação do serviço na discussão de diretrizes em políticas públicas em medicina de viagem. No período estudado, 2744 viajantes procuraram orientação pré-viagem no NMV do IIER. Foram realizados 2836 atendimentos de orientação pré-viagem, 92 viajantes procuraram o serviço mais de uma vez. A faixa etária entre 18 e 34 anos (54,2%), o sexo feminino (51,1%) e grau de educação superior (75,5%) foram as principais características demográficas desses viajantes. Os destinos mais procurados foram: África (24,5%), Europa (21,2%), Ásia (16,6%) e Brasil (19,2%). O turismo (35,7%) e o trabalho (35,7%) foram os motivos de viagem mais referidos. O tempo de permanência menor ou igual a 30 dias foi referido pelos viajantes em que o objetivo de viagem foi o turismo, enquanto os viajantes que referiram o trabalho ou estudo apresentaram maior tempo de permanência (p < 0,001). O meio de transporte mais referido foi o aéreo (62,8%). Os viajantes relataram durante a consulta pré-viagem dificuldade de acesso ao serviço. As fontes de informação mais referidas foram: informação a partir de amigos, indicação por profissional da saúde e mídia eletrônica. As medidas de prevenção recomendadas variaram conforme o destino. O tratamento autoadministrado para diarreia foi mais recomendado aos viajantes com destino à Ásia. As vacinas de febre amarela, poliomielite e antimeningocócica A e C foram mais recomendadas aos viajantes com destino à África, assim como a quimioprofilaxia para malária, que foi recomendada para 26,4% dos viajantes para esse destino. A quimioprofilaxia (QPX) para malária foi recomendada em 10,3% de todas as orientações. Houve diferença com significância estatística na recomendação segundo a finalidade (p < 0,30), o destino (p < 0,001) e a duração da viagem (p < 0,001). Das 422 orientações realizadas aos viajantes com destino ao Brasil, a QPX foi recomendada somente para 30 (7,1%). Dos 2744 viajantes atendidos, 664 (24,2%) relataram pelo menos uma morbidade prévia; 66 (2,4%) eram menores de 10 anos de idade; e 157 (5,7%) tinham 60 anos ou mais. Em relação às atividades de ensino, no período do estudo, 83 médicos residentes estagiaram no NMV e foram orientadas onze monografias de conclusão de residência médica. O NMV participou de 12 reuniões para discussão de diretrizes sobre a saúde do viajante e de iniciativas como a Carta de São Paulo (documento em defesa da saúde do viajante elaborado por acadêmicos e profissionais de saúde participantes do SUS). Da criação da Sociedade Brasileira de Medicina de Viagem e da criação do Comitê Estadual de Saúde do Viajante, pela Secretaria de Estado da Saúde de São Paulo. Travel Medicine Center (TMC) at \"Instituto de Infectologia Emilio Ribas\" (IIER) was the first health of traveler service created in São Paulo city in May 2000. The present study aims: to describe the traveler population who sought pre-travel guidance in TMC at IIER from January 2006 to December 2010; to describe the recommended preventive measures to travelers concerned with infectious diseases; to describe the teaching activities performed and the service involvement in debating guidelines about public policies in travel medicine. In the time period studied, a total of 2744 travelers sought pre-travel guidance in TMC of IIER, but 2836 assistances were provided for pre-trip orientation since 92 travelers sought the service more than once. The age group between 18-34 years (54.2%), female (51.1%), and university level degree (75.5%) were the main demographic characteristics. The more popular destinations were: Africa (24.5%), Europe (21.2%), Asia (16.6%) and Brazil (19.2%). Tourism (35.7%) and work (35.7%) were the main purposes of trip for travelers. For tourism purpose travelers would stay 30 or less days, while for work or study they stayed for a longer time (p <0,001). Commercial air travel was the preferred alternative (62.8%). The main sources of information were friends, health professionals, and electronic media. The recommended preventive measures varied according to the destination. The self-treatment for diarrhea was more recommended for travelers to Asia. Vaccination against yellow fever, polio, and anti-meningococcal (A and C) was more recommended for travelers to Africa, as well as chemoprophylaxis (CP) of malaria was recommended for 26.4% of travelers to the same country. The CP of malaria was indicated for 10.3% of all assistances. There was statistically significant difference in the recommendation according to purpose (p < 030), destination (p < 0.001) and trip duration (p < 0.001). From 422 assistances to travelers to Brazil, CP was only recommended for 30 (7.1%) travelers. From the 2744 travelers assisted, 664 (24.2%) reported at least one previous morbidity; 66 (2.4%) were under age 10; and 157 (5.7%) were 60 years or older. During the research period and relating to study activities, 83 residents were interns in TMC, and eleven monographs for completion of the Medical Residency were supervised. TMC participated in 12 meetings to discuss guidelines in travel medicine, and participated in initiatives such as \"Carta de São Paulo\" (a document in defense of traveler health prepared by academics and health professionals participating in the Unified Health System); in the institution of the Brazilian Society of Travel Medicine, and of the São Paulo State Travel Medicine Committee by the Secretariat for Health of São Paulo State. The sentinel lymph node biopsy is part of recommended surgical staging guidelines in patients with early stages of cervical cancer. High success rates of bilateral detection of SLN are achieved in sites with adequate experience with this procedure. The sentinel lymph node biopsy without systematic pelvic lymph node dissection is currently considered inadequate procedure for stages IB to IIA of the disease. One of the benefits of sentinel lymph node detection is extensive histopathological examination using the ultrastaging protocol enabling detection of small metastases (i.e. micrometastases). At the moment, there is lack of evidence about oncological safety of sentinel lymph node biopsy which might replace systematic lymph node dissection in the future. Prognostic significance of micrometastases is also controversial due to the lack of data about their potential presence in non-sentinel lymph nodes in cases with negative sentinel lymph nodes. This dissertation deals with the concept of sentinel lymph node biopsy in the cervical cancer and focuses on several topics. We have shown that the presence of micrometastasis is associated with significant negative impact on patients' prognosis on the largest retrospective cohort of patients ever published.
