Epidemiology and prevention of sepsis in young infants and the potential impact of maternal HIV infection on neonatal sepsisCutland, Clare Louise January 2016 (has links)

Johannesburg, South Africa 2016 / Introduction: Neonatal infections contribute to 25% of all neonatal deaths, which
account for approximately 44% of all under-5 childhood deaths globally. Pathogens
responsible for sepsis in neonates and young infants can be acquired vertically prior to
or during labour, or from the environment (community or hospital).
This project evaluated the burden and aetiology of sepsis in neonates and young
infants (≤90 days), and explored this association to in-utero exposure to human
immunodeficiency virus. The study also included a specific focus on the epidemiology
of invasive Group B Streptococcal disease in young infants.
Additionally, we assessed the efficacy of intrapartum chlorhexidine vaginal washes for:
(i) preventing early-onset neonatal sepsis; and (ii) vertical transmission of potentially
pathogenic bacteria to the newborns. Furthermore, we evaluated risk factors for poor
outcomes due to neonatal sepsis.
Materials and methods: (i) A bacterial surveillance system was established at Chris
Hani Baragwanath Academic Hospital (CHBAH) from 2004-2008 to identify young
infants with bacterial sepsis hospitalised in the neonatal and paediatric wards. Medical
and microbiological records were utilised to obtain clinical and laboratory data. Maternal
HIV results were obtained from antenatal testing records or admission records.
(ii) A blinded, randomised, placebo-controlled trial of 0.5% chlorhexidine maternal
vaginal intrapartum wipes and newborn skin wipes was conducted at CHBAH between
2004 and 2007. Consented, eligible participants were randomised during labour to
receive either chlorhexidine vaginal wipes or water external genitalia wipes. Newborns
received either chlorhexidine full-body wipes (intervention arm) or foot wipes (control
arm). Maternal and infant participants were followed up for admissions during the first
month after delivery/ birth. A subset of 5144 maternal participants had an intrapartum
lower vaginal swab collected, and skin swabs were collected from their newborns to
assess colonisation with potentially pathogenic bacteria (Group B streptococcus,
Escherichia coli and Klebsiella pneumoniae).
Results: Group B streptococcus (GBS) was the most commonly isolated bacterial
pathogen, causing 35.2% of culture-confirmed sepsis in infants ≤90 days, 41.6% of
early-onset disease (EOD, 0-6 days), 40.5% of late-onset neonatal disease (LOD, 7-27
days) and 18.7% of young-infant community-acquired disease (YI-CAD, 28-90 days).
Staphylococcus aureus (S. aureus), Escherichia coli (E. coli) and Klebsiella
pneumoniae (K. pneumoniae) contribute 16.2%, 12.2% and 3.4% to sepsis in young
infants.
Overall, incidence (per 1000 live births) of invasive GBS disease was 2.72 (95%
confidence interval [95% CI]: 2.46 to 3.01), including an incidence of 1.50 and 1.22,
respectively, in infants 0-6 days and 7-90 days of age. HIV-exposed infants were at
greater risk of EOD (Relative risk [RR]: 1.69; 95% CI: 1.28-2.24) and LOD (RR= 3.18;
95% CI: 2.34-4.36) than HIV-unexposed infants. GBS serotypes Ia and III caused
84.0% of invasive GBS disease in young infants.
Intrapartum chlorhexidine interventional wipes was not efficacious in prevention of any
of: (i) vertical transmission of pathogenic bacteria (54% vs. 55%; efficacy -0.05, 95% CI:
-9.5 to 7.9) to the newborns; (ii) sepsis in first 3 days of life (3% vs. 4%; p=0.65,); (iii)
sepsis in the later neonatal period (both <1%; p=0.4444); or (iv) maternal puerperal
sepsis(both <1%; p=0.56).
Conclusion: GBS, S. aureus, E. coli and K. pneumoniae are the most commonly
isolated bacterial pathogens in neonates and infants ≤90 days old. HIV-exposed infants
are at greater risk of GBS sepsis. Intrapartum chlorhexidine intervention was not
efficacious in reducing vertical transmission of pathogenic bacteria, neonatal or
maternal sepsis. Alternative interventions to prevent sepsis in young infants, including
maternal immunisation, need to be investigated in setting such as ours where there is a
high prevalence of maternal HIV infection. / MT2017

Sepsis är ett tillstånd med hög mortalitet. Tidig identifiering och administrering av antibiotika i samband med adekvat omvårdnad är väsentligt angående bemötandet av patienter med sepsis. Det är därför viktigt att sjuksköterskan är uppmärksam samt visar god kompetens när det gäller omvårdnad vid sepsis. Noggrann och frekvent bedömning är också en viktig del hos sjuksköterskan, framför allt när det gäller att minska risken för komplikationer och försämring av tillståndet. Syftet var att belysa sjuksköterskans kompetens att identifiera riskfaktorer för sepsis samt att utföra omvårdnadshandlingar vid kliniska tecken på sepsis. Arbetet är en litteraturöversikt som inkluderar 15 vetenskapliga artiklar. De vetenskapliga artiklarna hittades med hjälp av sökningar i databaserna CINAHL och PubMed. Dessa granskades sedan utefter en vetenskaplig kvalitetsbedömning och beskrivs närmre i arbetets matris. En tematisk analys gjordes för att urskilja artiklar som ansågs vara väsentliga för syftet. Ett huvudtema identifierades: sjuksköterskans kompetens. Utefter detta utvecklades tre subteman: utbildning och utveckling, sjuksköterskans egen upplevelse av kompetens samt riskfaktorer. Brist i kompetens gällande tidig identifiering av sepsis kunde urskiljas. Införande av riktlinjer och visade på förbättring gällande mortalitet vid sepsis samt kompetens hos sjuksköterskor. Sjuksköterskor är inte medvetna om de tecken som är karaktäristiska för sepsis samt att SIRS-symtom är associerade med sepsis. Idag är kompetensen hos sjuksköterskor inte tillräcklig för att uppnå en god kvalitet gällande vård av patienter med sepsis. Utbildning samt tydligare rutiner om vad som gäller vid sepsis behöver utvecklas.

Design & Fabrication of a Microfluidic Device for Clinical Outcome Prediction of Severe SepsisYang, Jun 06 1900 (has links)
Sepsis is an uncontrolled response to infection. Severe sepsis is associated with
organ dysfunction, and has mortality rate of 30-50%. Identification of severity of sepsis
and prediction on mortality is crucial in making clinical decisions. Recently, cell-free
DNA (cfDNA) in blood was found to have high discriminative power in predicting ICU
mortality in patients with severe sepsis. In an analysis of 80 severely septic patients, the
mean cfDNA level in survivors (1.16±0.13μg/ml) was similar to that of healthy
volunteers (0.93±0.76μg/ml), while that of non-survivors (4.65±0.48μg/ml) was notably
higher. Therefore, rapid quantification of cfDNA concentration in blood will enable
physicians to quickly predict mortality of sepsis and decide on treatment.
Current methods for quantification of cfDNA involve multiple steps including
centrifugation, DNA-extraction from plasma, and its quantification either through
spectroscopic methods or quantitative PCR. The whole process is time consuming, thus is
not suitable for immediate bedside assessment. To solve the problems, a microfluidic
device is designed and fabricated in this thesis, which is potential for cfDNA
quantification directly using blood in 5 minutes. The goal is to use this device for
distinguishing survivors or healthy donors from non-survivors in patients with severe
sepsis. The two-layer device consists of a sample channel (top) and an accumulation
channel (bottom) that intersect each other. The accumulation channel is preloaded with
1% agarose gel, and the blood containing cfDNA and intercalating fluorescent dye is
loaded in the sample channel. Fluorescently labeled DNA is able to be trapped and
concentrated at the intersection using a DC electric field, and fluorescent intensity of the
accumulated DNA is representative of its concentration in the blood. The simulated
electric field in the sample channel reveals that both the magnitude and the gradient of
electric field reach their maximum values at the intersection. Force analysis shows that
DNA was driven into the gel by the dominate electrophoretic force, while red blood cells
moved away from the gel due to a strong dielectrophoretic force.
In this thesis, 4 types of samples have been used to characterize the performance
of the device. It showed that DNA was efficiently accumulated at the intersection, and the
fluorescent intensity could be measured using a fluorescent microscope. Samples from
healthy donors were able to be distinguished from that of severely septic patients in 5
minutes. However, better resolution was needed for differentiating various cfDNA
concentrations in patient samples. The discussion on the effect of applied voltage showed
that 9V is an optimized setting compared with 3V and 15V. In addition, it has been
proved that the fluorescent reagent could be immobilized in the device and the sample
preparation could be absolutely eliminated. In summary, the device proposed in this thesis is capable of distinguishing
severely septic patients from healthy donors using clinical plasma in 5 minutes, and is
potential to be applied in clinical blood samples. It has low cost, and is ready to be
developed into a fully functioned system. This tool can be a valuable addition to the ICU

Objective – Sepsis is a syndrome in which infection triggers a systemic inflammatory and procoagulant response. Cell-free DNA (CFDNA) is elevated in sepsis, and correlates with mortality. This DNA may come from nuclear, mitochondrial, or bacterial sources. CpG motifs on bacterial and mitochondrial DNA can stimulate inflammatory responses via TLR9. CFDNA can activate coagulation via the contact pathway. CFDNA may thus play an important pathogenic role in sepsis. This study elucidates the relative effects of nuclear, mitochondrial, and bacterial DNA on inflammatory and pro-coagulant pathways with relevance to sepsis.
Results – Mitochondrial DNA as well as nuclear DNA are elevated in plasma from septic patients compared to healthy controls. Bacterial, but not mitochondrial or nuclear, DNA increased neutrophil IL-6 secretion. Both mitochondrial and bacterial DNA increased neutrophil viability. Nuclear, mitochondrial, and bacterial DNA increased thrombin generation in both platelet-poor plasma and platelet-rich plasma to a similar degree. This effect was reduced by addition of corn-trypsin inhibitor and in FXII-depleted plasma, and abolished in FXI-depleted plasma, indicating dependence on the intrinsic pathway of coagulation. Independently of coagulation, DNA from all three sources was capable of causing activation of platelet integrin αIIbβ3.
Conclusions – CFDNA from nuclear, mitochondrial, and bacterial sources have varying pro-inflammatory effects, although all three have similar pro-coagulant potential. The pathophysiological effects of CFDNA in sepsis may vary with the source of DNA. / Thesis / Master of Science (MSc)

Myeloid-derived suppressor cells (MDSCs) contribute to high mortality rates during sepsis, but how sepsis induces MDSCs is unclear. Previously we reported that microRNA (miR)-21 and miR-181b reprogram MDSCs in septic mice by increasing levels of DNA binding transcription factor, nuclear factor 1 (NFI-A). Here, we provide evidence that miR-21 and miR-181b stabilize NFI-A mRNA and increase NFI-A protein levels by recruiting RNA-binding proteins HuR and Ago1 to its 3′ untranslated region (3′UTR). We also find that the NFI-A GU-rich element (GRE)-binding protein CUGBP1 counters miR-21 and miR-181b dependent NFI-A mRNA stabilization and decreases protein production by replacing 3′UTR bound Ago1 with Ago2. We confirmed the miR-21 and miR-181b dependent reprogramming pathway in MDSCs transfected with a luciferase reporter construct containing an NFI-A 3′UTR fragment with point mutations in the miRNA binding sites. These results suggest that targeting NFI-A in MDSCs during sepsis may enhance resistance to uncontrolled infection.

The most common cause of death in patients with sepsis/septic shock is deterioration of the function of multiple organs, termed multiple organ dysfunction syndrome. Although our understanding of mechanisms involved in the pathophysiology of sepsis-induced tissue damage has improved substantially, therapy of these syndromes still remains largely supportive. The hallmark of sepsis is an overwhelming systemic production of both pro- and anti-inflammatory mediators leading to generalized endothelial and epithelial damage, microcirculatory-mitochondrial distress, altered endocrine and coagulation homeostasis and cellular immune hyporesponsiveness. Therefore, the hypothesis that modulation of this excessive immunological and biological response to infection might improve patient outcome appears reasonable. Hemoelimination techniques represent biologically plausible way to provide non-specific removal of soluble pro- and anti-inflammatory mediators, although the concept of blood purification in sepsis remains a matter of considerable debate. Introduktion/bakgrund: I Sverige drabbas 50 000 personer årligen av sepsis. Mortaliteten var hög och tillståndet är möjligt att häva i tidigt skede. Sjuksköterskans kompetens och patientnära arbete gör yrkesrollen central för tidig upptäckt av sepsis. Syfte: Syftet med litteraturstudien var att belysa sjuksköterskans kliniska verktyg och förutsättningar i relation till tidig identifikation av sepsis. Metod: Litteraturstudie med induktiv ansats. Artiklar söktes fram genom databaserna Cinahl och PubMed, abstrakt och titlar granskades och kvarvarande artiklar fulltextgranskades. Åtta vetenskapliga artiklar utgjorde resultatet, vilka kvalitetsgranskades. Databearbetning och analys skedde enligt SBU:s metodbok. Resultat: Kunskap inom området är varierande och påvisades vara centralt för identifikation. Den senaste definitionen skapade ytterligare osäkerhet i relation till komplexitet och patofysiologi. Kunskap och erfarenheter visade sig vara betydelsefulla vid bedömning av sepsis. Diffusa symtom medför svårigheter att upptäcka tillståndet. Kliniska verktyg uppgavs vara den främsta faktorn för tidig identifiering av sepsis. Inget specifikt kliniskt verktyg utvecklat för sepsisidentifikation fanns, det mest effektiva var NEWS2. Slutsats: Litteraturstudien påvisar kunskapsluckor vid sepsisidentifiering, ospecifika kliniska verktyg och låg kännedom om det patofysiologiska förloppet samt problem vid definitionstolkning. Klinisk betydelse: Då sjuksköterskan utgör en central roll vid identifiering behövs bättre kännedom om diffusa symtom samt specifika kliniska verktyg anpassade till yrkesrollen.

Bakgrund: Sepsis är ett livshotande tillstånd som drabbade 48,9 miljoner människor under 2017. Snabb identifiering och behandling är viktiga aspekter gällande överlevnad och av den anledningen är det viktigt att sjuksköterskor besitter kunskap om sepsis. Syfte: Syftet med studien var att beskriva sjuksköterskors erfarenheter och kunskap av att identifiera sepsis hos patienter. Metod En deskriptiv design har använts i litteraturstudien. Studien består av 12 vetenskapliga artiklar med kvantitativ och kvalitativ ansats. Huvudresultat: Det framkom att sjuksköterskor har stora kunskapsbrister om sepsis och hade svårigheter att identifiera tillståndet hos patienter. Kunskapsbristen var mycket framträdande gällande symtom och screeningsverktyg samt vilken betydelse sjuksköterskors yrkeserfarenhet hade vid identifieringen av sepsis. I och med bristen på kunskap sågs ett behov av utbildning inom ämnet sepsis bland sjuksköterskorna. Utbildning kunde öka kunskapen hos sjuksköterskorna vilket kan komma att leda till en bättre sepsisvård. Många sjuksköterskor upplevde begränsningar och hinder som förhindrade eller försenade identifieringen av sepsis. Organisationen hade en stor betydelse gällande sjuksköterskornas förutsättningar. Även sjuksköterskors arbetsbelastning ansågs som hög vilket bidrog till stress och tidsbrist. Ett ytterligare hinder var överlämningssvårigheter och kommunikationsfel som bidrog till en försening av identifikationen av sepsis. Slutsats: Sjuksköterskor med längre yrkeserfarenhet är bättre på att identifiera sepsis. Det finns behov av mentorskap för nyblivna sjuksköterskor för att kunna utvecklas i sin yrkesroll. För att förbättra sjuksköterskornas kunskap om sepsis har utbildningsprogram varit en bra metod. Sjuksköterskors arbetsmiljö behöver förbättras eftersom hög arbetsbelastning, brist på personal och högt arbetstempo påverkar sjuksköterskors förmåga att identifiera sepsis samt hindrar sjuksköterskornas yrkesutveckling.

Sjuksköterskan har i sitt patientnära arbete en betydande roll i upptäckten och omvårdnaden av sepsis. Tidigare forskning visar på svårigheter att identifiera sepsis i tid och vilken betydelse tidiga åtgärder har för patienters chanser till överlevnad. Därför har sjuksköterskans en stor utmaning i omvårdnaden av patienter med sepsis. SyfteStudiens syfte var att undersöka sjuksköterskors erfarenheter av omvårdnaden vid sepsis hos vuxna patienter. MetodArbetet är en litteraturöversikt med 10 vetenskapliga originalartiklar, varav 4 var kvalitativa, 5 kvantitativa och 1 mixad metod. Sökbaserna som användes var CINAHL och PubMed. Artiklarna kvalitetsgranskades och analyserades genom kvalitativ innehållsanalys. ResultatTre teman identifierades: Organisation och resurser, Kommunikation och Värdet av erfarenhet och kunskap. SlutsatsUtbildning i omhändertagandet av sepsis ge mer kunskap som behöver kontinuerlig fördjupning i praktiken. Kommunikationen inom vården behöver förbättras, samt underlättas med användningen av standardiserade kommunikationsverktyg. Stress är ett hinder för effektiv kommunikation i vården. Sjuksköterskan behöver ha en trygghet i sig själv samt mer stöd från organisationen och resurser för att kunna ge en säkrare vård med bibehållet engagemang och medlidande för patienterna.
