A escrita desta dissertação foi agenciada privilegiando o encontro circunstancial entre campos teóricos distintos, na tentativa de articular a teoria e os conceitos à dimensão do acontecimento, da experiência. Para tanto, o trabalho apostou na aproximação ou na fusão entre arte e vida, em que proposições dadaístas e surrealistas foram contribuições significativas. As experiências da clínica, tratadas como ambientações, foram expostas em forma de narrativa, método que embasou esta pesquisa. Com inspiração em autores da psicanálise, da filosofia, das artes e da literatura, para este trabalho alguns autores tornaram-se pontos de apoio frequentes - Freud, Lacan, Guattari, Foucault, Benjamin e Lyotard, além de artistas e escritores. As questões propostas ou desenvolvidas nesta dissertação foram problematizadas a partir de uma perspectiva interdisciplinar, articulando conexões entre os ambientes clínicos e uma estética da existência. / The writing of this dissertation was based on the privilege of the circumstantial meeting among distinct theoretical fields in an attempt of articulation of theory and concepts, in view of the event and the experience. For this purpose, the task kept on the approach or the fusion between art and life in which the statements of dadaism and surrealism have made expressive contributions. The clinical experiences, considered settings, were exposed on narrative form that is the method on which this research was based. Inspired on authors from psychoanalysis, philosophy, arts, and literature, some of them have become constant supports for this research Freud, Lacan, Guattari, Foucault, Benjamin and Lyotard, besides artists and writers. The issues proposed or developed on this dissertation were problematized from an interdisciplinary perspective that surmises connections between the clinical environments and an aesthetics of existence.

Improving the quality of maternal and child health service delivery in resource-poor settings: case study of project fives alive! in GhanaAkaligaung, John Akalpa 08 April 2016 (has links)
BACKGROUND: Quality improvement (QI) efforts in which providers from various health facilities, with support from coaches, work together to implement innovations for improving health care are rapidly spreading from high-income settings to resource-poor settings. However, limited research exists regarding the factors which drive teams to succeed or fail, spread, and sustain best practices. The purpose of this study was to understand the factors facilitating or inhibiting the performance of QI teams of maternal and child health (MCH) service delivery in resource-poor settings.
METHODS: A qualitative multi-site case study was conducted in northern Ghana to determine the views and beliefs of QI teams, coaches, and beneficiaries of Project Fives Alive (PFA) about the application of QI to MCH service delivery. Using key informant interviews, document review, and group interviews, the study elicited information about: 1) the factors that affect QI team success and failure when implementing QI methods designed to improve MCH service delivery; 2) the interplay of factors that facilitate or inhibit the spread of best practices among QI teams in northern Ghana; and 3) steps being taken by stakeholders to sustain these best practices. Grounded theory processes were used to identify themes from the data.
RESULTS: Testing of changes using Plan-Do-Study-Act (PDSA) cycles and adoption of a client-centered model to the delivery of services has helped teams to identify the felt needs of clients, resulting in perceived increased uptake of MCH services. Key contributory factors to successes were: availability and accessibility of midwives, training of QI teams, incentive packages for providers and clients, community support groups, and PFA partners. Key challenges inhibiting success were: staff turnover, inadequate supervision, cultural practices, and inadequate infrastructure. Some of the key factors that shaped successes have dissipated, threatening sustainability of QI efforts two years after PFA ended.
CONCLUSION: The presence of a midwife, providing leadership for organizing team members and implementing PDSA cycles, can facilitate success. Project staff support is important, but teams and coaches need space and time to drive the QI process independently and practice QI methods in ways that foster continuity beyond donor support.

Focamos a atenção nos processos de construção de cenários. A elaboração teórica considera a ótica do design estratégico na abordagem dos processos e das capacidades de design na criação de projetos de inovação social. Para tanto, foram fundamentais as pesquisas iniciadas por Ezio Manzini e os conceitos por ele desenvolvidos. Vislumbrando a aproximação com as questões do desenvolvimento social e ambiental, consideramos a necessidade de aprofundar o conhecimento sobre as práticas de trabalho das organizações da sociedade civil, empenhadas em se capacitar para incidir em políticas públicas. Pesquisamos como estas podem se beneficiar da construção de cenários de design e como a ferramenta pode contribuir para o aprofundamento de sua reflexão estratégica. Nosso objetivo é analisar os cenários Sociedade Civil 2023 pela perspectiva do design estratégico e sua relação com os Cenários DOS. Buscamos entender a contribuição da ferramenta para a organização estratégica da sociedade civil na proposição de políticas públicas. Para tanto, este trabalho de caráter qualitativo tem foco na investigação da possibilidade das organizações da sociedade civil em absorverem as práticas dos cenários e transformarem a tecnologia dessa ferramenta em avanço na condução de suas estratégias. / To conceive this work, we start with the choice of axis of Social Innovation research on Strategic Design. We focus attention on scenario-building processes. The theoretical elaboration considers the perspective of the strategic approach in the design of processes and design capabilities to create social innovation projects. The start up researches and the concepts developed by Ezio Manzini were fundamental. Glimpsing the approach to the issues of social and environmental development, we consider the need to deepen the knowledge about the working practices of civil society organizations committed to empowering it to influence public policies. We researched how these can be benefited from building design scenarios and how the tool can contribute to the deepening of their strategic thinking. Our goal is to analyze the Civil Society 2023 scenarios from the perspective of the strategic design and its relation to the DOS scenarios. We try to understand the tool's contributi on to the strategic organization of civil society in public policy proposition. Therefore, this qualitative work focuses on the investigation of the possibility of civil society organizations in absorbing the practices of scenarios and transform this breakthrough technology tool into the conduct of their strategies. Can PRUs work? : a search for an answer from within a lived experienceDodman, Hilary Frances January 2016 (has links)
This study draws on theories of punishment (Cavadino and Dignan 2007), leadership (West-Burnham 2013), social rules, (Burns and Machado 2014) and resilience (Fredrikson and Branigan 2005) to develop an understanding of Pupil Referral Units (PRUs). PRUs came into being through statute in 1993. They were set up to provide formal educational settings for young people who had been excluded from school. LEAs have responsibility for the education and welfare of all children in their catchment areas, irrespective of which school they attend. If an exclusion occurs, the LEA is obliged to assume responsibility, under section 19(1) of the 1996 Education Act, for the child's education by whatever means seems appropriate to its designated officers. Placement in a Pupil Referral Unit is a course of action they may pursue. This study sets out to discover through a series of narrative interviews conducted within a Key Stage 4 PRU, whether the multiple purposes of the PRU can be achieved, given the issues that present themselves in the isolated setting of the Unit, the resources available and the complex needs of the young people concerned. Fifteen interviews were conducted within a PRU in the academic year 2011-2012. Four were held with the Head; 11 further interviews involved 12 people; 8 members of staff and 4 pupils. A study of the evidence they provided led to a qualified positive response to the research question; i.e. that PRUs can' work' given a number of factors that are listed in the conclusion.
