Impact of vitamins B12, B6 and folate supplementation on cardiovascular risk markers in an elderly community of Sharpeville

Submitted in fulfillment of the requirements of the degree of Doctor of Technology: Health Sciences, Durban University of Technology, Durban, South Africa, 2015. / Background: In a vulnerable low-income group with a confirmed high risk of cardiovascular disease, like the elderly in the Sharpeville care centre, an acute intervention is needed in order to improve their health profile. Previous studies suggested homocysteine lowering by vitamin B12, B6 and folate supplementation. The effect of vitamin B12, B6 and folate supplementation on the inflammatory response, thrombotic risk, lipid profile, hypertension, risk of metabolic syndrome and homocysteine metabolism in an elderly, black South African population has never been reported. Objectives: The main aim of this interventional study was to assess the effect of vitamins B12, B6 and folate supplementation at 200% RDA for six months on cardiovascular risk markers of an elderly semi-urbanised black South African community. Design: This study was an experimental intervention non-equivalent control group study design in 104 purposively selected samples of all the elderly attending the day-care centre. Setting and participants: A homogeneous group of respondents was included in the study. All subjects were equivalent in age (>60 years), race (black), unemployed/pensioners (socio-demographic) and 60 years and older attending a day care centre in Sharpeville, situated in the Vaal region, Gauteng, SA. Measurements: The distinctiveness of this study lies in the broad panel of parameters evaluating the CVR in correlation with the increased nutritional intake of vitamin B6, B12 and folate. These included: weight, height, waist, serum cholesterol, high density lipoprotein (HDL), low-density lipoprotein (LDL), triglycerides, blood pressure, fibrinogen, high-sensitivity C-reactive protein (HS–CRP), homocysteine, vitamin B12, folate, glucose, insulin, adiponectin and fibronectin. Results: A very high incidence (66.36%) of hyperhomocysteinaemia is present in the sample. The mean serum homocysteine level in hyperhomocysteinaemic individuals decreased statistically significantly from 25.00±8.00 umol/l to 18.80±12.00 umol/l after the intervention. The number of respondents with an increased homocysteine level decreased from 100% (baseline) to 67% (follow-up). The supplementation was beneficial (statistically significant changes) to the glucose levels, fibrinolytic status, vitamin B6 serum levels, fibronectin levels and haemopoeiesis (decreased macrocytosis) of all the individuals (regardless of their homocysteine status). Conclusion: It is concluded that supplementation of vitamins B6, B12 and folate at 200% RDA for six months is an effective homocysteine-lowering approach as a strategy to reduce hyperhomocysteinaemia in an elderly population and thereby reduce cardiovascular risk (CVR). The supplementation intervention mentioned is not an effective multifactorial strategy to decrease CVR although beneficial effects were found with other CVR markers independent of homocysteine status.

This thesis deals with the policy of apartheid in the SAU and later in the SAR and its reflexion in the foreign countries. Massacre in Sharpeville, which took place on March 21, 1960 close to Johannesburg had an important impact on the approach to the policy of racial segregation. South African police opened fire on the crowd of protesters against pass law, 69 people were killed and many others wounded. Sharpeville became a symbol of the policy of racial segregation and oppression of the non - white population of South Africa and it gained worldwide response. Thesis is focused on the impact of this event on the short - term domestic crisis. The main part deals with the influence in international relations. It analyzes the impact of worldwide critique of apartheid on the relation with Commonwealth, United Nations Organization, Organization of the African Unity and USA. The biography of "access" as an expression of human rights in South African education policies

This study In an attempt to promote equal access to education, we in South Africa, have adopted an instrumentalist approach to the debate of the right to education. In other words, we have provided an enabling legal framework and we simply assume that access to education has been granted to every one. We continue to pretend that we understand what exactly the concept of “access to education” means. We also assume that we all have a common understanding of what the Constitution means by the right to education. On 26 June 1955 the historic Freedom Charter of the African National Congress (ANC) was adopted. This charter declared “the doors of learning and culture shall be opened.” Over the next four decades, the demand for open and equal access to education became central platform in the anti-apartheid struggles that brought an ANC-led government to power in 1994. Yet, ten years later (2004) the problem of access continues to preoccupy education planners and activists against the backdrop of some of the most progressive policy positions including a Constitution that recognizes education as a basic right. The intellectual puzzle that motivates this study is to explain, therefore, why despite its prominence, it continues to be regarded as an intractable problem. The research strategy adopted in pursuit of this puzzle is to trace the changing meanings of the concept of “access to education” under and after apartheid, and its expression in the practices of two case study schools (comparative case studies). Data was collected from different sources to trace the concept of access to education in education from the apartheid era to the policies and practices that affirm access to education as a basic human right today. This study hopes to contribute significantly to the dialogue of “access” as a realisation of the basic right to education. For the conceptual framework Morrow’s distinction between epistemological access and physical access was used. Formal access to education refers to enrolment or registration at an education institution, in this case, a school, whereas epistemological access refers to access to knowledge and information that these education institutions hold. I expanded the conceptual framework to include dimensions of epistemological access such as how the topic is taught, who selected the topic, the value and political basis. I undertook documentary analysis and a series of interviews with individuals who were involved in the struggle either through intellectual contributions in the NEC and NEPI processes or in the National Education Co-ordinating Committee. I also conducted two case studies of schools located in vastly different social and political contexts. At these schools, I collected data through classroom and school observations, semi-structured interviews with principals, history teachers and learners. Findings: The first finding of this study is that the ways in which students experience access to knowledge (epistemological access) is strongly dependent on the history and politics of the school context and the institutional culture, rather than the formal prescriptions laid down in the school curriculum The second finding of this study is that even when students enjoy physical access to schools, they have highly uneven, even unequal, access to knowledge within those schools. The third finding is that despite the awareness and understanding of what good education entails, without physical access, it is difficult for individuals to entertain discussions about epistemological access. The fourth finding is that despite claims that the policy promotes increased access to education, it was not possible to find reports that refer to any significant degree of quality outcomes as a result of the implementation of the principle of “equality of access” to education. Increased access to education has not resulted in quality output. This dissertation contributes to knowledge by its nuanced exploration of the complexities of access to education as a human right. A Dissertation Submitted to the Faculty of Arts University of the Witwatersrand, Johannesburg for the Degree of Master of Arts. / Thesis examines cultural expressions and community attachment, and their relation to each other, in the creation and maintenance of urban identity. In examining this, the thesis considers a number of key cultural forms in Sharpeville such as boxing, football, musicial performance, youth. gangs, and styles of dress. It argues that, conceptually, "community" is never static; rather it is a state of existence, a perception, for a grouping of people. At a given time they may consider themselves to be collectively part of or constitute a community; at another, their attachments may be to a different entity - the local neighbourhood, for example. The empirical data was derived mainly from primary sources although due to the historical time-period examined - namely 1943 to 1985 - there was some reference to secondary sources. The research involved mainly in-depth interviews and participant observation. By administering a questionnaire, "key" informants within the various cultural areas examined were identifed and interviewed at length, sometimes more than once. The thesis argues that "communities" only gain a sense of cohesion, "identity" and unity at certain specific historical moments; at other times the cultured focus within them may in fact express quite other meanings than those of "community" for their members. This identity is seen thus as both a product of the structural features which inform, influence and even dictate its direction as well as the responses and actions of the residents themselves, in shaping its outcome. / Andrew Chakane 2018

This dissertation studies the role of clothing manufacturing co-operatives in job creation and poverty alleviation in Sharpeville. Ttie study focuses on three areas, namely, the theories of unemployment, poverty and clothing manufacturing, the state of unemployment and poverty in Sharpeville and clothing manufacturing co-operatives as one of the contributors to the solution to unemployment and poverty. Unemployment is identified, among others, as a major determinant of poverty; therefore, the main component of any policy aimed at eradicating poverty should focus on employment creation. The clothing industry being the most labour-intensive industry in South Africa with low capital entry requirement creates opportunities for entrepreneurially driven employment creation in Sharpeville. The approach in the dissertation was to define and measure unemployment and poverty and determine the profile of the poor in Sharpeville. This is done by employing household-level indicators. For measuring poverty the following tools are used: the Household Subsistence Level (HSL) as poverty line, the, headcount index, the poverty gap and the dependency ratio. Unemployment is also used to determine poverty levels. The dissertation shows that Sharpeville experiences high unemployment rates and low levels of poverty based on the indicators employed. Compared to Bophelong, most indicators (except for unemployment) show that Sharpeville is better off. Comparing the operations of clothing businesses in Sharpeville and Mitchell's Plain, the dissertation shows that clothing manufacturing has the potential to create job opportunities in Sharpeville if assistance could be offered. Assuming that assistance is offered to form clothing manufacturing cooperatives. The dissertation shows that 374 jobs could be created and that the unemployment rate could decrease from 59.2 percent to 52.9 percent. The expenditure of the whole population of Sharpeville on clothing (also considering nearby communities) shows a potential big market for clothing manufacturing co-operatives in the township. At an average income of R600 per month the impact will be that the poverty rate will be reduced from 43.1 percent to 40.8 percent and at an average income of R1 500 the impact will be that the poverty rate decrease from 43.1 percent to 39.1 percent. Finally, the dissertation concludes that clothing manufacturing co-operatives can indeed create job opportunities and alleviate poverty, but assistance from different structures is needed. The dissertation suggests major support systems like involvement by industry and training institutions, the formation of associations working directly with operators, the addressing of the problem of geographic location of service providers and government involvement. / Thesis (M.Com. The impact of government grants on poverty in Sharpeville / Boitumelo Reneilwe Hatla

South Africa, like international countries, has been experiencing an increase in the levels of poverty over the years. Poverty affects vulnerable groups of society more intensely and these groups include children, the old, disabled people and women, especially those who are single parents. This dissertation studies the role social grants have on the level of poverty in households of Sharpeville. This study focuses on two areas namely the theoretical background of poverty and social grants; and what the impact is of income from social grants. The South African government provides its citizens with eight different social grants to help those in need and/or vulnerable. From these social grants only six are investigated for the purpose of this study. These grants include the old age grant, child dependency grant, foster care grant, child support grant, disability grant and the war veteran grant. Poverty is defined as the inability to attain a minimal material standard of living by the World Bank. The different indicators used in this study to profile poor households in Sharpeville include the Household Subsistence Level (HSL) as the poverty line, the poverty gap ratio, the headcount index and the dependency ratio. This dissertation shows that poverty within the township has increased over the five years. And to do this the results from the data survey conducted in 2009 are compared to the results from Sekatane‘s 2004 data. The poverty gap ratio and the headcount index for the township in 2009 were estimated at 0.86 and 0.654 respectively. In the year 2004 the headcount index was estimated at 0.431 indicating a 22.3 percent increase in the number of people living in poverty. This means that an estimated 5 477 households in Sharpeville, in 2009, were regarded to be poor When government grants are excluded from the household‘s income within the township both the poverty gap ratio and the headcount index decrease to 0.93 and 0.705 respectively. This means that when government grants are excluded from households‘ income within Sharpeville, the depth of poverty within household‘s increases. The income from government grants might be regarded as minimal, however it assists in moving households further from the poverty line. This study recommends that activities within the informal sector should be encouraged as this will increase employment opportunities for those unemployed in the township. As the vast majority of the unemployed people have skills from trading/retail sector; employment creation should be focused in this sector. Lastly, the income threshold used in the means test equation to check affordability of social grant applicant should be decreased as people meeting the current criteria are already living in dire poverty. / Thesis (M.Com. The relationship between poverty and inflation in Sharpeville / Tiyeselani Clara Maluleke

All over the world, the level of poverty is increasing. In South Africa it is mainly concentrated in rural areas and differs significantly from whether considering race, sex, provinces or community areas. This dissertation studies the relationship between poverty and inflation in Sharpeville by determining the impact of rising prices on the poor households in Sharpeville. The study focuses on three areas, namely the theoretical background of poverty and inflation, the impact of rising prices in expenditure patterns and the relationship to poverty. There are different approaches in defining poverty. Poverty can either be absolute or relative. For the purpose of this study, poverty is defined as absolute. Thus the study defines individuals as poor due to their inability to attain a minimum material standard of living. This minimal standard of living is normally referred to as the poverty line. Inflation may be defined in different ways. For the purpose of this dissertation, inflation is defined as the rise in the general price levels over a specific period of time. Changes in expenditure patterns are caused by an increase in inflation. This study uses the regression model to determine the impact of inflation on poverty in Sharpeville. According to the macroeconomic theory’s implication, the same level of inflation on the same basket of commodities has a different level of effect on each household. Accordingly, in this study, all households are assumed to be faced with the same inflation rate. Household size is positively related to poverty gap squared. This means that the more members there were in a household in Sharpeville the poorer they were. Households with the highest number of members were poorer than those with few members. Statistically, the null hypothesis that there is no relationship between household size and poverty gap is rejected, even at the 1% level of significance. EXPINFL is negatively related to poverty gap. The correlation matrix confirms the results in the regression analysis. The correlation coefficient between The relationship between poverty and inflation in Sharpeville Page EXPINFL and PGAP is -0.34467. Although it is relatively weak, the fact that there is a negative correlation confirms that inflation negatively affects poverty. Finally, the study recommends that government provides more job opportunities for the individuals without any source of income in Sharpeville. The government could also provide business funding to the unemployed individuals to enable them to start their own businesses. This would enable those individuals to create additional employment. In addition, measures should be introduced to determine the effect of inflation on those households who are not employed (that is, not receiving any form of income, not even through any form of grant), but do benefit from some form of feeding scheme administered by either government or non-profit organisations. / MCom, Economics, North-West University, Vaal Triangle Campus, 2012
