A lesão medular gera uma condição crônica de saúde ao indivíduo que sofre a injúria e pode acarretar complicações, como o intestino neurogênico. A depender do nível neurológico da lesão, há expectativas de resultados para o funcionamento intestinal e medidas educativas devem ser implementadas ainda na fase aguda da lesão, visando a reabilitação. Estudo de delineamento metodológico que objetivou produzir vídeo educativo com as manobras de esvaziamento intestinal a ser utilizado no preparo de indivíduos com intestino neurogênico. A pesquisa, aprovada pelo Comitê de Ética, foi realizada em quatro etapas: etapa 1: Produção do roteiro e storyboard elaborados a partir das diretrizes do Guia traduzido: \"Intestino Neurogênico: Guia para Pessoas com Lesão Medular\"; etapa 2: Produção dos instrumentos; etapa 3: Coleta de dados; e por fim etapa 4: Produção do vídeo educativo. Foram utilizados dois instrumentos para a validação do roteiro e storyboard. O instrumento para validação de conteúdo continha 15 questões, divididas em quatro sessões e o de validação técnica possuía 11 questões, divididas em três sessões. Em cada sessão dos instrumentos tinha um campo para sugestões. A validade de conteúdo do roteiro e storyboard foi realizada por um comitê de dez juízes de conteúdo e três juízes técnicos. Os resultados demonstraram que todos os juízes de conteúdo eram do sexo feminino, com idade entre 30 a 58 anos. Quanto à atuação profissional atual e maior titulação acadêmica, a maioria trabalhava na área de reabilitação e tinha mestrado. As alternativas \"discordo fortemente\" e \"não sei\", não foram sinalizadas e a porcentagem de respostas total à opção \"concordo fortemente\" foi de 57,3% e \"concordo\" foi de 36,7%. A maioria das sugestões foi relacionada à mudança na linguagem, visando adequação ao público- alvo e para incrementar o uso de fotografias, figuras e animações. Quanto aos juízes técnicos, a maioria era do sexo masculino, com idade entre 37 a 45 anos. Atuavam como analista em mídias digitais e tecnologias educacionais, produção multimídia e em sistemas de informação, com predomínio do título de especialização. A porcentagem total de respostas \"concordo fortemente\" foi de 78,8% e \"concordo\" de 21,2%. Ressaltaram nas sugestões, o cuidado na edição e escolha dos recursos audiovisuais, de maneira a favorecer o interesse e a atenção do usuário. Cada item do instrumento de validação do roteiro e storyboard do vídeo obteve mais de 70% do conceito \"concordo fortemente\" e/ou \"concordo\" pelos juízes. Posteriormente o roteiro e storyboard foram adequados e as sugestões analisadas e acatadas conforme pertinência. A gravação do vídeo ocorreu no Centro de Simulação e no Centro de Informática de Ribeirão Preto da Universidade de São Paulo. Methodological design study aimed to produce an educational video with intestinal emptying maneuvers to be used in the preparation of patients with neurogenic bowel. The research, which was approved by the Ethics Committee, was performed in four steps: Step 1: Production and script storyboard drawn from the Guide guidelines translated: \"Neurogenic Bowel: A Guide for People with Spinal Cord Injury\"; Step 2: Production of instruments; Step 3: Data collection; and Step 4: Production of educational video. Two instruments for the validation script and storyboard were used. The instrument for content validation contained 15 questions divided into four sessions and the technical validation possessed 11 questions, divided into these three sessions. In each session of the instruments had a field for suggestions. The validity of the script and storyboard content was performed by an expert panel of 10 content and 3 technical judges. The results showed that all the contents of judges were female, aged 30-58 years. Padronização do modelo experimental de lesão da medula espinal e avaliação da lesão neurológica em camundongos / Standardization of a spinal cord lesion model and neurologic evaluation using micePaulo Alvim Borges 16 January 2018 (has links)
INTRODUÇÃO: A lesão da medula espinal é um dos grandes desafios da medicina. Apesar de décadas de pesquisa sobre o assunto, seu tratamento ainda não é satisfatório. A padronização de modelos de lesão da medula espinal permite a reprodutibilidade e a análise dos resultados sendo importante para a pesquisa sobre o tema. OBJETIVO: Validar a padronização de um modelo de lesão da medula espinal e avaliação da lesão neurológica em camundongos. MÉTODOS: Submetemos 30 camundongos BalbC divididos em 4 grupos experimentais e um grupo controle à lesão da medula espinal torácica por queda de peso de diferentes alturas (gerando lesões de graus variados). O grupo controle (SHAM) foi submetido apenas à laminectomia. Os camundongos foram avaliados por seis semanas durante as quais foram aplicadas escalas de avaliação funcional motora. Após seis semanas os animais foram sacrificados para avaliação histológica das medulas espinais lesadas. Os achados foram correlacionados entre si para validar se a lesão foi efetiva e se os grupos diferenciaram-se entre os diferentes graus de lesão. Adicionalmente avaliamos se as escalas utilizadas são aplicáveis e se são fiéis aos achados histológicos. RESULTADOS: Seis dos trinta camundongos do experimentos evoluíram para óbito sendo um do Grupo 3, um do Grupo 4 e quatro do Grupo 5. Um camundongo do Grupo 4 apresentou autofagia. O Grupo 5 foi excluído do experimento por alta mortalidade e perda de dados. Todas as escalas funcionais estudadas foram estatisticamente diferentes entre si e demonstraram evolução durante o experimento. Os achados foram confirmados por histologia e apresentaram uma correlação forte com as escalas BBB e BMS e moderada a forte com a escala MFS. A Escada Horizontal apresentou forte correlação com a degeneração neurológica porém não apresentou correlação com os demais parâmetros histológicos estudados. CONCLUSÃO: O modelo de lesão da medula espinal em camundongos apresentado neste estudo é efetivo, confiável e reprodutível, com exceção da lesão causada por queda de peso (10g) de 50mm de altura, que traz mortalidade inaceitável. Das escalas estudadas, BBB e BMS são as mais confiáveis, enquanto que a Escada Horizontal tem seu uso discutível / INTRODUCTION: Spinal cord lesion is a great medical challenge. Even with many decades of research, no satisfactory treatment is available yet. The standardization of animal experimentation models makes the spinal cord lesion reproducible allowing a reliable analysis of the results. Hence, standardization is a major concern in spinal cord lesion research. OBJECTIVE: To validate the standardization of a spinal cord lesion model with neurologic evaluation using mice. METHODS: Thirty BalbC mice were divided in four experimental groups and one control group and submitted to spinal cord lesion produced by weight drop from different heights (producing different severity lesions). The control group (SHAM) was submitted to laminectomy only. Every mice was followed up for six weeks during which functional motor scales were applied. After six weeks the animals were sacrificed for histological examination. Findings were correlat-ed to confirm if the spinal cord lesion was effective and if the groups were dif-ferent between themselves. Additionally all functional motor scales were corre-lated with the histological findings to confirm if the scales are reliable and truly represented the spinal cord lesion. RESULTS: Six mice died during the experi-mentation period (one mouse from the Group 3, one mouse from the Group 4 and four mice from Group 5). One mouse from Group 4 presented autophagia and was excluded from the experiment. Group 5 was excluded from the exper-iment for high mortality rates and data loss. All functional motor scales applied demonstrated significant results with moderate or strong correlation with the histological findings. The Horizontal Ladder scale had strong correlation with neurologic degeneration but had weak or worse correlation with the rest of the histological parameters studied. CONCLUSION: The spinal cord lesion model using mice presented in this study is reliable and reproducible, excluding the lesion produced by a weight drop (10g) from 50mm, which brings unacceptable mortality rate. Of all fuctional motor scales studied, BBB and BMS scales are the most reliable. The use of the Horizontal Ladder scale, however, must be carefully evaluated

A new perspective on the design of pressure relief cushions for those with spinal injuriesLance, Philip Thomas January 2010 (has links)
The aim of this study is to develop new insights which pressure relief (PR) cushion designers can use to guide the design of new cushions with greater efficacy at preventing pressure ulcers than contemporary cushions. A methodological framework was formulated which incorporated a number of research techniques from the user-centred methodology USERfit, and included methodological triangulation. Exploratory interviews and observational work were conducted in a specialist unit for spinal cord injury (SCI). This involved ten patients, four physiotherapists, two nurses and an outpatient technician. Additionally, two questionnaires were designed and circulated amongst SCI patients and staff with completed responses from 41 patients and 31 staff. From the analyses of the data gathered from the literature, observational work, interviews and questionnaire responses, 28 recommendations for cushion design were formulated. These recommendations covered the principles which underpin cushion design, cushion usability and the future direction of cushion design.

Relationship Between Doctor-Patient Communication and Sexual Functioning Among Women With Spinal Cord InjuryLafferty, Melissa 01 January 2019 (has links)
After individuals sustain a spinal cord injury, all aspects of their lifestyle must change for them to manage their new life roles. One important area of recovery that is often not addressed during the rehabilitation process is sexual functioning. The purpose of this quantitative study was to examine how doctor communication about sexual health with women who have sustained spinal cord injuries predicts their levels of sexual functioning and sexual self-esteem. The theoretical framework was the sexual health model. Questionnaires were used to gather data from 45 women who had completed rehabilitation from spinal cord injuries. Level of current sexual functioning was measured using the Female Sexual Function Index. Sexual self-esteem was measured using the Multidimensional Sexual Self-Concept Questionnaire. Satisfaction with doctor-patient communication was measured using the Patient Satisfaction Questionnaire and Perceived Self-Efficacy in Patient-Physician Interactions-Sex. Findings from correlation analysis indicated a positive correlation between general satisfaction with doctor-patient communication and confidence to communicate with the doctor about sexual health. Results also indicated a negative correlation between sexual self-esteem and sexual functioning. Findings may be used to improve communication between doctors and patients about sexual health, which may reduce the stigma of talking about sexuality and may promote more holistic treatment for women recovering from spinal cord injuries.

Sciatic nerve remyelination and nodal formation following olfactory ensheathing cell transplantationDombrowski, Mary A. 14 February 2008 (has links)
Transplantation of olfactory ensheathing cells (OECs) into injured spinal cord results in improved functional outcome through axonal regeneration, remyelination, and neuroprotection. However, because little is known of the fate of OECs transplanted into injured peripheral nerve, their myelin forming potential requires investigation. To study these issues OECs were isolated from the olfactory bulbs of adult green fluorescent protein (GFP)-expressing transgenic rats and transplanted into a sciatic nerve crush lesions. Five weeks to six months after transplantation the nerves were studied histologically and it was determined that GFP-expressing OECs survived in the lesion and distributed longitudinally across the lesion zone. Immunostaining revealed a high density of isoform Nav1.6 at the newly formed nodes of Ranvier which were flanked by paranodal Caspr staining. Immuno-electron microscopy for GFP revealed transplanted OECs form peripheral type myelin. These results indicate that transplanted OECs extensively integrate into transected peripheral nerve, form myelin on regenerated peripheral nerve fibers, and reconstruct nodes of Ranvier with proper sodium channel structure.
