Hard Time and Hard Love: Issues and Challenges of Visitation for Men of Incarcerated Women

abstract: The United States prison population is rapidly rising. Consequently, more families are losing loved ones to the system. While many researchers have focused on women of incarcerated men and children of incarcerated parents, none have looked at the partners of incarcerated women. This paper explores the issues and challenges of prison visitation for the significant others of women incarcerated at Perryville Prison in Goodyear AZ. It is known that prison visitation is important for supporting and maintaining romantic relationships. It is also beneficial to the prison institution. Visitation assists in social control and high inmate morale; both of which lower the instances of violent acts. However, it has been reported that visitation is a daunting task for the visitors. Many sources of information and data were used for this study; formal and informal interviews with family members and others with prison visitation experience, government websites that contain visitation policies, online forums for family and friends of inmates to discuss their concerns, existing research literature, direct observations, and discussions with scholar experts and prison activists. These resources act as a window to visitation at Perryville. With insights derived from symbolic interactionism and previous research guiding the project, it was found that visitation is a good experience for the significant others, incarcerated women, and Perryville. However, the troubles the significant others have with money, the institution and social support strongly suggest that these men encounter hurdles that make the positive act of visitation at times nearly impossible. / Dissertation/Thesis / M.S. Mental Health Stigma and Military Spouses: The Influence of Marital Conflict and Career Consequences on Help-seeking Encouragement

abstract: Approximately one-third of Iraq and Afghanistan veterans develop mental health problems, yet only 35-40% of those with mental disorders are seeking mental healthcare (Hoge, et al., 2004; Vogt, 2011). Military spouses may be an important resource for facilitating treatment seeking (Warner, et al., 2008), especially if service member mental health issues are impacting the marriage. Military spouses might be hesitant to encourage service member help-seeking, however, due to perceived threat of adverse military career consequences. For this study, 62 military wives completed an online survey. As part of the survey, participants were randomly assigned to one of four vignettes containing a description of a hypothetical military husband with mental health symptoms. Each vignette presented different combinations of marital conflict (high versus low) and service member concerns about adverse career consequences (high versus low). Wives rated on a five-point scale how likely they were to encourage the hypothetical military husband to seek help. It was hypothesized that spouses would be more willing to encourage help-seeking when concerns about adverse military career consequences were low and marital distress was high. No main effects or interaction effect were found for marriage and career. Perceived stigma about seeking mental health treatment in the military, psychological identification as a military spouse, and experience and familiarity with military mental healthcare policies failed to moderate the relationship between marital conflict, career concerns, and encouragement of help-seeking. Correlational analyses revealed that (1) greater experience with military mental healthcare (first- or secondhand), and (2) greater perceptions of stigma regarding seeking mental healthcare in the military each were associated with decreased perceptions of military supportiveness of mental healthcare. Therefore, although the experimental manipulation in this study did not lead to differences in military spouses' encouragement of a hypothetical military service member to seek mental health services, other findings based on participants' actual experiences suggest that experiences with military mental healthcare may generate or reinforce negative perceptions of military mental healthcare. Altering actual experiences with military mental healthcare, in addition to perceptions of stigma, may be a useful area of intervention for military service members and spouses. / Dissertation/Thesis / M.S. Apesar de pais e mães de crianças com câncer estarem em um processo interativo nos cuidados ao filho, pouca atenção tem sido dada às maneiras como a doença da criança afeta a relação conjugal desses casais. O objetivo deste trabalho foi compreender as repercussões do câncer infantil na relação conjugal de pais e mães de crianças com câncer, na experiência da doença de seus filhos. Trata-se de um estudo exploratório, de abordagem qualitativa. Participaram do estudo nove casais, totalizando 18 sujeitos. Utilizamos a entrevista em profundidade como principal técnica de coleta de dados, e a consulta em prontuários como recurso auxiliar para obter dados importantes da criança e de seus pais. A análise dos dados empíricos foi feita por meio da análise de conteúdo indutiva. Os resultados foram agrupados por similaridades nas seguintes categorias: a trajetória do casal na experiência do câncer infantil: as primeiras mudanças; o câncer infantil, o relacionamento conjugal dos pais e o papel da comunicação: fragilidades e potencialidades; alterações na dinâmica familiar e implicações para o casal: trabalho, lazer, papéis e relações de gênero; e enfrentando o desafio juntos: balanço da experiência e expectativas do casal. O presente estudo não objetivou distinguir se as mudanças ocorridas na relação conjugal desses pais e mães foram positivas ou negativas, mas, sim, mostrar que alguns aspectos contribuem para o estremecimento da relação desses homens e mulheres. Entretanto, alguns casais vivenciam aspectos que promovem o fortalecimento da relação com o cônjuge no contexto de doença da criança. O estudo evidenciou a importância da comunicação como um dos pilares do relacionamento conjugal desses casais, a necessidade da oferta adequada de apoio social, principalmente vindo do próprio cônjuge e a relevância das questões de gênero nos aspectos relacionados à sexualidade e intimidade do casal e no intercâmbio de papéis que se faz necessário durante a trajetória da doença. Os resultados deste estudo contribuem para que os profissionais de saúde, dentre eles o enfermeiro, tenham o casal como importante foco de seu cuidado, voltando-se para suas necessidades e ajudando-os a reconhecer as fortalezas no relacionamento com o cônjuge durante as adversidades, oportunizando e incentivando o diálogo e a parceria para lidar com os desafios impostos pela doença da criança. / Although fathers and mothers of children with cancer are going through an interactive process in care for their child, little attention has been given to how the childs disease affects these couples marital relationship. This research aimed to understand the repercussions of childhood cancer on the marital relationship between fathers and mothers of children with cancer, in the experience of their childrens disease. We carried out an exploratory study with a qualitative approach. Participants were nine couples, totaling 18 subjects. In-depth interviews were used as the main data collection technique, and patient files were consulted as an auxiliary resource to obtain important data about the children and their parents. Inductive content analysis was applied for empirical data analysis. The results were grouped according to similarity in the following categories: the couples trajectory in the experience of childhood cancer: the first changes; childhood cancer, the parents marital relationship and the role of communication: fragilities and potentials; changes in family dynamics and implications for the couple: work, leisure, roles and gender relations; and facing the challenge together: balancing the experience and the couples expectations. This research did not intend to distinguish whether the changes occurred in these fathers and mothers marital relationship were positive or negative, but to show that some aspects contribute to shake up these men and womens relationship. On the other hand, some couples experience aspects that promote the strengthening of the spousal relationship in the context of the childs disease. This study evidenced the importance of communication as one of the pillars of these couples marital relationship, the need for adequate social support supply, mainly from the spouse and the relevance of gender issues in aspects related to the couples sexuality and intimacy and in the role exchange needed along the disease trajectory. The results of this study contribute for health professionals, including nurses, to adopt the couple as an important focus in care delivery, looking at their needs and helping them to acknowledge the strengths in their spousal relationship during adversities, permitting and stimulating dialogue and partnership to deal with the challenges imposed by the childs disease.

Introdução: o câncer não afeta unicamente o paciente, mas também o cônjuge. Assim, torna- se necessário identificar as demandas oriundas da doença e o tratamento, a fim de ter uma visão completa do paciente e cônjuge para aprimorar a assistência clínica e facilitar às equipes a tomada de decisão para a realização de cuidados. Ao perceber a falta de instrumentos em português brasileiro que pudessem identificar essas demandas, foi escolhido o Demands of Illness Inventory (DOII) Patient Version e Partner Version, que se caracteriza por abordar de forma abrangente as demandas, preocupações e necessidades dos pacientes com câncer e cônjuges. Objetivo: adaptar culturalmente e avaliar as propriedades psicométricas dos instrumentos DOII, na versão do paciente e esposo, em pacientes portadores de câncer e cônjuges, para o português brasileiro. Método: estudo metodológico desenvolvido em duas etapas, a adaptação cultural e a avaliação psicométrica dos instrumentos. A adaptação cultural foi a preconizada por Beaton et al. As propriedades psicométricas da versão do paciente foram avaliadas em 658 pacientes com câncer em tratamento quimioterápico de um hospital público de grande porte de São Paulo. Já a versão do esposo foi realizada em 170 esposos de pacientes da mesma instituição. Para a validade de conteúdo utilizaram-se o IVC (Indice de Validade de Conteúdo) e o coeficiente Kappa. Para a validade de construto foi realizada a Análise Fatorial Exploratória (AFE). Para a validade convergente e divergente foi utilizada a análise de correlação, na versão do paciente foi aplicado o Termômetro de Distress e o instrumento que avalia a qualidade de vida EORTC-CQC-30, respectivamente; na versão do esposo foi aplicada a Escala de Estresse Percebido (PSS-14) e o Inventário de estratégias de Coping de Folkman e Lazarus. A confiabilidade foi testada pelos coeficientes alfa de Cronbach e ômega de McDonald. Resultados: todas as etapas de adaptação cultural foram realizadas satisfatoriamente de ambas versões adaptadas. Para a validade de conteúdo, o IVC e Kappa apresentaram valores superiores a 0,75 e 0,72 respectivamente, para a maioria dos itens. Para a validade de construto, obteve-se medidas de KMO (Kaiser-Meyer-Olkin)=0,930 e 2 de Bartlett = (8256) 47209,9; p < 0,001. A AFE mediante a análise paralela foram retidos 11 fatores que explicam 46,5% da variância total do fenômeno de estudo. A confiabilidade para a escala total foi de 0,961 para o alfa de Cronbach e 0,952 para ômega de McDonald. Na versão do esposo, a validade de construto obteve indicadores de KMO= 0,818 e 2 de Bartlett= (528) 2414,1, p < 0,001. De acordo com a análise paralela 4 fatores foram retidos que explica 52,9% da variância total do fenômeno de estudo. A confiabilidade para a escala total foi de 0,894 para o alfa de Cronbach e 0,876 para ômega de McDonald. Na versão do paciente, as correlações das validades convergente e divergente foram r=0,605 (p<0,01) e r=0,660 (p<0,01), respectivamente. Na versão do cônjuge, foram r=0,334, p0,01 e r=0,260, p0,01, respectivamente. Conclusões: o DOII, na versão do paciente e esposo, mostrou-se adaptado culturalmente a pacientes com doenças crônicas como o câncer no contexto brasileiro e aos cônjuges, apresentando evidências de validade de conteúdo, construto e confiabilidade. Recomenda-se realizar a Análise Fatorial Confirmatória. / Introduction: cancer affects not only the patient, but also the spouse. Thus, it becomes necessary to identify the demands arising from the disease and the treatment, in order to have a complete view of the patient and spouse to improve clinical care and facilitate the decision- making of the healthcare teams. By observing the lack of instruments in Brazilian Portuguese that could identify these demands, the Demands of Illness Inventory (DOII) - Patient Version and Partner Version has been chosen, which is characterized by comprehensively addressing the demands, concerns and needs of cancer patients and spouses. Objective: to culturally adapt and to evaluate the psychometric properties of the DOII instruments, in the patient and in the spouses version; in patients with cancer and their spouses, in Brazilian Portuguese. Method: A methodological study developed in two stages, the cultural adaptation and the psychometric evaluation of the instruments. The cultural adaptation has been advocated by Beaton et al. The psychometric properties of the patient\'s version have been evaluated in 658 cancer patients in chemotherapy treatment of a large public hospital in São Paulo. The spouse\'s version has been used in 170 spouses of patients from the same institution. For the content validity, the CVI (Content Validity Index) and the Kappa coefficient have been used. For the construct validity, the Exploratory Factor Analysis (EFA) has been performed. For the convergent and divergent validity, the correlation analysis has been performed in the patient\'s version with the Distress Thermometer and the EORTC-CQC-30 instrument, respectively; in the spouse\'s version, with the Perceived Stress Scale (PSS-14) and the Folkman and Lazarus Coping Inventory, respectively. Reliability has been tested by Cronbach\'s alpha and McDonald\'s omega coefficients. Results: all the stages of cultural adaptation have been satisfactorily performed in both adapted versions. For the content validity, IVC and Kappa have presented values higher than 0.75 and 0.72, respectively, for most items. For the construct validity, the following KMO (Kaiser-Meyer-Olkin) measurements have been obtained= 0.930 and Bartlett\'s 2= (8256) 47209.9; p <0.001. The EFA, through the parallel analysis, has comprised the 11 retained factors that account for 46.5% of the total variance. The reliability for the full scale was 0.961 for Cronbach\'s alpha and 0.952 for McDonald\'s omega. In the spouse\'s version, the construct validity has obtained indicators of KMO= 0.818 and 2 of Bartlett= (528) 2414.1, p <0.001. According to the parallel analysis, has comprised the 4 retained factors that account for 52.9% of the total variance. The reliability for the full scale was 0.894 for Cronbach\'s alpha and 0.876 for McDonald\'s omega. In the patient\'s version, the convergent and divergent validity correlations were r= 0.605 (p <0.01) and r= - 0.660 (p <0.01), respectively. Conclusions: The DOII, in the patient and in the spouse\'s version, has been culturally adapted to patients with chronic diseases such as cancer in the Brazilian context and to the spouses, presenting evidence of content validity, construct and reliability. It is recommended to perform the Factorial Confirmatory Analysis.

This thesis discusses insolvency proceedings regarding an insolvency of one or both spouses. The author aims to explain problems occurring during such insolvency proceedings. The thesis is not limited to a description of these procedural peculiarities, but also discusses their impact on the matrimonial property. The author addresses above all a debt discharge and a bankruptcy, for these are the most frequent mechanisms to resolve an insolvency of the debtor. On the contrary the author does not address the reorganization, for it is not very frequently used in practise. Furthermore, the author does not address a position of legal persons in insolvency proceedings, since it is not of any importance for this thesis. The author only describes the current legislation as it is. The author therefore neither discusses legislation and court rulings prior the 2013 insolvency act revision, nor considers the revision of the insolvency law that is currently (2017) under way. The thesis is divided into four chapters. In the first chapter, the author briefly discusses matrimonial property law, since it is essential in order to understand legal arguments in the third and fourth chapter of this thesis.
