Prevention of recidivism and relapse of addiction after LVM : A qualitative study of the treatment work at the Swedish National Board of Institutional Care.

Drug abuse often correlates with a criminal career. Compulsory care of addicts aims to motivate the individual to come to terms with their addiction. However, crime can be part of the lifestyle that the addict gets involved in. This essay examines how the preventive treatment at the State Board of Institutions in Sweden works to prevent relapse for both addiction and crime. The issues also concern collaboration in addition to treatment work. The material was gathered through qualitative semi-structured interviews with five people in the treatment staff at one of the State Department Board's LVM homes. The analysis was a thematic analysis and the collection of material resulted in three themes: "treatment work", "lack of knowledge among staff" and "collaboration". The informants believe that the treatment work has a good effect through the social bonds that are created between the client and treatment staff, but also so called “treatment conversations”. Clients are perceived to have high motivation during the time for LVM, but this often decreases after LVM completion. The result of the study shows that there is a lack of knowledge among treatment staff, mainly regarding crime prevention and psychiatry. Collaboration is perceived to work deficient, and informants highlight shortcomings in the provision of information between the actors involved. The results are subsequently attributed to social bond theory, the age-graded theory of informal social control, routine activity theory and to previous research on addiction and crime. The results are intended to be used instrumentally in order to improve the treatment work at the Swedish National Board of Institutional Care, which in turn improves prevention for both substance abuse and crime.

Background: Adolescents in need of care that cannot be provided by Child and Adolescent Psychiatry, or by other aid provided by the Social service can be placed and cared for at the youth facilities of the Swedish National Board of Institutional Care (SiS). Adolescents being cared for at the youth facilities of SiS have complex needs of care, including both psychiatric conditions and psychosocial issues. At every institution at least one nurse with responsibility of the adolescents' care is employed. There is a lack of research on how these nurses experience the care which they provide at the youth facilities. Purpose: The aim of the study was to describe the experiences of the nurses caring for adolescents in the SiS. Method: A qualitative interview study was conducted. Eight nurses employed by the SiS were interviewed. The data was then analyzed according to Lundberg and Graneheim's model for qualitative content analysis. Results: The results are presented in three main themes and nine subthemes: Challenges and opportunities with working within SiS. Lack of collaboration and information, resources and competence and the work environment within SiS. The role of the nurse. Experience of working under different laws and assuming different work roles, leading and training other staff. Meeting the care needs of adolescents. To be able to see the entire adolescent, create an alliance and educate the youth about their own health. Conclusion: The nurses experienced an inexplicit health care assignment within SiS. They experienced a lack of health care competence among the staff working at the wards. A better collaboration with other health care departments, such as Child and Adolescent Psychiatry and primary care centers, was considered desirable. The adolescents are considered to present with great suffering and complexcare needs, and their psychiatric needs are considered the most difficult to meet. The nurses need to create an alliance with the adolescents, and to see the individual beyond criminal history, diagnosis and social issues. This essay contributes to the visibility of the nurses' perspective and of how they can contribute to the development of health care within the institutions, but more studies and more extensive research on the subject are sorely needed.

​Behandling i skuggan av våldsvågen. : En kvalitativ studie om programverksamheten på SiS. / Treatment in the shadow of the wave of violence. : A qualitative study of the program activities at the National Board of Institutional Care

​Denna studie syftar till att undersöka förutsättningar, hinder och möjligheter att bedriva behandling inom ungdomsvården på Statens institutionsstyrelse. Studien lutar sig mot bakgrund av en samhällsutveckling där allt fler unga begår grova brott och får allt svårare problem. Samtidigt kritiseras Statens institutionsstyrelse av media och andra myndigheter för att inte kunna hantera sitt uppdrag. Samtidigt som SiS behöver tvångsvårda fler ungdomar involverade i grov brottslighet, vilket i sin tur inneburit ett behov av skärpt säkerhet, behöver myndigheten också ge god omsorg och behandling till de ungdomar som placeras utifrån andra behov. Uppdraget är svårt, och i verksamheterna arbetar människor som valt att jobba på SiS utifrån en vilja och ambition att kunna påverka och hjälpa ungdomar med svåra problem. Med hjälp av Lipskys teori om gräsrotsbyråkrater och begreppet handlingsutrymme, undersöks hur personal som arbetar med behandlingsprogram hanterar de dubbla kraven från organisationens riktlinjer och ungdomarnas individuella behov. Med en kvalitativ ansats analyserades intervjuer med tematisk analys. Resultaten visar på organisatoriska hinder men också på en tro på förändring genom professionell erfarenhet och kunskap. Resultaten visade också att rätt kompetens är avgörande för att kunna bedriva behandling för myndighetens målgrupper och att finns en brist på behandlingsmetoder för grovt kriminella ungdomar. Resultaten betonar också ett behov av ökad medvetenhet, utbildning och möjligheter till för en kontinuerlig vårdkedja för att öka kvalitén på behandlingsarbetet. Slutligen belyser resultaten maktaspekter inom tvångsvården som ett hinder för behandlingsarbetet och föreslår ett förändringsarbete mot mer vårdande än straffande och kontrollerande strategier.

Rollen som verksamhetscontroller i statliga myndigheter

I problemdiskussionen lyfts att litteraturen och forskningen utgår från att en controller är en ekonom som arbetar med ekonomistyrning. Enligt min erfarenhet stämmer inte detta för verksamhetscontrollers i statliga myndigheter. Syftet med uppsatsen är att öka kunskapen om rollen som verksamhetscontroller i statliga myndigheter. Forskningsfrågorna är vilka krav ställer myndigheterna vid rekrytering av verksamhetscontrollers och hur beskriver de arbetsuppgifterna? Vad innehåller tertialuppföljningarna? Metoden har varit att genom en diskursanalys studera innehållet i tertialuppföljning på Socialstyrelsen, Statens institutionsstyrelse och Tullverket. Dessutom har myndigheternas rekryteringsannonser som avser verksamhetscontroller analyserats. Slutsatsen är att arbetsuppgifterna för verksamhetcontrollers på myndighetsnivå är likartade och det ingår inte ekonomiska arbetsuppgifter. De förefaller dock ha olika mandat. Arbetsuppgifterna på avdelnings-/ kompetenscenternivå skiljer sig mellan och inom myndigheterna och inte heller dessa verksamhetscontrollers har ekonomiska arbetsuppgifter. Kraven som ställs på utbildning och erfarenhet har stor spännvidd och ibland har man krav på att den sökande ska ha ekonomisk bakgrund. Min slutsats är att rollen som verksamhetscontroller på avdelnings-/ kompetenscenternivå är under utveckling och att rollen på myndighetsnivå är tydligare. / In literature and research the definition of a controller is an economist working with financial control. According to my knowledge this is not correct for controllers focused on operations management in government agencies The purpose of this thesis is to increase the knowledge of the role as a controller focused on operations management in state agencies. The questions to be answered are what demands does government agencies have when they recruit operations management controllers and how do they describe the  assignments? What is included in the four month follow ups? The method has been by discourse analysis study the content in the four month follow ups at the National Board of Health and Welfare, the National Board of Institutional Care and the Swedish Customs. In the thesis the agencies recruitment ads for controllers focused on operations management is also analyzed. The conclusion is that the assignments for controllers focused on operational management on the agency-wide level is similar between the agencies and few financial assignments are included. The assignments for the lower levels differ within and between the agencies and financial assignment are not included. The controllers at the agency-wide level seem to have different mandates between the agencies. The agencies demands for education and experience varies widely and some of the ads have demands for a background in  finances. My conclusion is that the controller at the department level is under development and the role at the agency-wide level is more distinct.

Psykologer har ett brett uppdrag vid särskilda ungdomshem. Trots det saknas det i nuläget forskning som undersöker särskilda ungdomshem utifrån psykologernas perspektiv. Syftet med förevarande studie var att öka kunskapen och förståelsen för psykologers erfarenheter av att arbeta vid särskilda ungdomshem samt vilka möjligheter och svårigheter som följer med yrkesrollen. Tio semistrukturerade intervjuer genomfördes och analyserades utifrån tematisk analys. Analysen resulterade i tre huvudteman: Psykolog vid särskilda ungdomshem - en mångfacetterad roll, Ett utmanande vård- och behandlingsuppdrag samt Spänningar inom ramen för tvångsvård. Resultatet visade att deltagarna beskrev att psykologrollen kan variera mellan olika institutioner och att det inte finns en enhetlig mall för vad det innebär att arbeta som psykolog vid särskilda ungdomshem. Deltagarna i studien uppgav att psykologerna har en unik kompetens som behövs vid särskilda ungdomshem för att skapa förståelse för ungdomarna och underlätta för övrig personal i deras arbete. Resultatet visade att de utmaningar deltagarna beskrev i vård- och behandlingsarbetet kunde förklaras utifrån den svårbehandlade målgruppen, tvångsvårdkontexten- och institutionsmiljön, brister i själva behandlingsarbetet men även yttre faktorer så som avsaknaden av andra vårdformer samt problem i samverkan med barn- och ungdomspsykiatrin eller socialtjänsten. Svårigheterna i vård- och behandlingsarbetet samt tvångsvårdkontexten beskrevs som etiskt utmanande av deltagarna. Arbetet med ungdomarna sågs samtidigt som positivt och meningsfullt. / Psychologists have a broad mission in special youth homes. Research regarding psychologists working in special youth homes is lacking. The purpose of this study was to increase the knowledge and understanding of psychologists' experiences of working at special youth homes and the opportunities and difficulties that are included in the professional role. Ten semi-structured interviews were conducted and analyzed based on thematic analysis. The analysis resulted in three main themes: Psychologist in special youth homes – a multifaceted role, A challenging care- and treatment assignment and Tensions within the framework of compulsory care. The results showed that the participants description of the professional role of psychologist can vary between different institutions and that there is no homogeneous template for what it means to work as a psychologist at special youth homes. The participants in the study stated that psychologists have a unique competence that is needed to benefit both adolescents and other staff. The challenges within care and treatment described by the participants were linked to the difficult target group, the compulsory care context and the institutional environment, but also external factors such as lack of other forms of care and difficulties in collaboration with child psychiatry and social services. The caregiving context was seen as ethically challenging by the participants, as it involves a major intervention in young people's lives. The work involving the adolescents was seen as positive and meaningful, which the participants emphasized.

<p>The intention of this study was to examine the interaction between authorities. More specifically it had a purpose to look at the ongoing cooperation between social workers and staff of special homes of closed institutional youth care. The sanction system for young offenders’ reform of 1999 where young people between the ages of 15-17 who have committed serious crimes, can be sentenced to secure institutional treatment instead of prison, aims to counteract the harm which can result from a stay in prison. The social welfare board in the young person´s local authority and the national board of institutional care shall cooperate during the youth’s placement at the special home of closed institutional youth care. This study was based on eight qualitative interviews with social workers chosen randomly from all over Sweden and staff members from different special homes of closed institutional youth care. To interpret the material we have been using theory of collaboration and cooperation of Danermark and Kullberg. We have also used earlier studies within the area. The study has resulted that the social welfare board are more satisfied with the corporation than the national board of institutional care are. The study also shows that there has been an improvement. Significant reasons for this result has proved to be; the two different employers with different mission statement who are cooperating, unclear guidelines and the lack of knowledge within in the social services. Today there’s an ongoing cooperation that needs to be developed.</p>

Relationer under tvång : - En kvalitativ intervjustudie om personalens perspektiv på relationen till de unga flickorna på SIS-hem / RELATIONSHIPS UNDER DURESS

Följande studie syftar till att undersöka behandlingspersonalens perspektiv om hur arbetet på särskilda ungdomshem (SIS-hem) för flickor ser ut i praktiken samt öka kunskapen om hur det relationsskapande arbetet fungerar inom tvångsvård. Mer specifikt kommer relationen mellan behandlingspersonalen och flickorna på SIS-hem att undersökas utifrån behandlingspersonalens perspektiv. Studien har genomförts genom semistrukturerade intervjuer med behandlingspersonal, som är eller har varit anställda under det senaste året på något av de SIS-hem i Sverige som tar emot flickor. Resultaten från intervjuerna har analyserats med hjälp av begreppet makt, Michel Foucaults teori om maktens olika former och Arlie Russell Hochschilds emotionssociologi. Studiens slutsatser belyser att fenomenet makt ständigt förekommer i relationen mellan personalen och flickorna, samtidigt som makten dessutom kommer till uttryck i alla dess olika former likväl i relationen som i det dagliga arbetet. Relationerna utgörs av kommersialiserade relationer där personalens relationsskapande arbete präglas av ett emotionellt lönearbete, där de anpassar både sina känslor och emotioner utifrån vad som anses acceptabelt utifrån de organisatoriska målen. Anledningen till att studien har genomförts är för att belysa personalens perspektiv samt bidra med andra aspekter av tvångsvård. / The following study aims to examine the treatment staff's perspective on how the work at special youth homes (SIS-homes) for girls looks like in practice and to increase knowledge of how the relationship-building work functions within compulsory care. More specifically, the relationship between the treatment staff and the girls in SIS-homes will be examined from the perspective of the treatment staff. The study was conducted through semi-structured interviews with treatment staff, who are or have been employed during the past year at one of the SIS-homes in Sweden for girls. The results from the interviews have been analyzed using the concept of authority, Michel Foucault's theory of the different forms of power and Arlie Russell Hochschild's sociology of emotions. The study's conclusions highlight that the phenomenon of authority constantly occurs in the relationship between the staff and the girls, while power is also expressed in all its different forms, both in the relationship and in the daily work. The relationships consist of commercialized relationships where the staff's relationship-building work is characterized by emotional wage work, where they adjust both their feelings and emotions based on what is considered acceptable in accordance with the organizational goals. The reason why the study has been conducted is to shed light on the staff's perspective and to contribute with other aspects of compulsory care.
