Therapist identity formation is an important part of the development of the therapist training programme. It results in numerous changes for the therapist and is often an emotive developmental stage. The therapist develops his identity by doing therapy, but before he is able to do that, he needs to understand what he sees the role of therapy to be, as well as developing his own therapeutic skills. Fortunately there are personality traits which enhance this identity formation process. It is by his own actions and involvement that the therapist accomplishes this multifaceted process. The effects of this identity formation are varied and require the therapist to undergo introspection on a number of issues which affect his own life-world. The lived experiences and support needs of a mainstream high school learner with a speech-flow difficulty

Thesis (MEd)--Stellenbosch University, 2014. / ENGLISH ABSTRACT: The aim of this study was to better understand the lived experiences of a learner in a mainstream high school, who is experiencing a speech-flow difficulty, in order to gain insight into how best to support such a learner within an inclusive classroom. The theoretical framework on which this study was based is the bio-ecological model, inclusive education, positive psychology, resilience as well as developmental psychology, specifically the developmental phase of adolescence. The purpose of this was to view the learner holistically, taking into account all of the positive support structures in her life, especially, her levels of resilience, whilst taking cognisance of the fact that she is in the adolescent developmental phase, within a mainstream high school. Inclusive classrooms should ideally be structured in such a way that they accommodate a learner’s specific individual learning needs. The methodology employed in this study was based on a basic qualitative research approach, and viewed through an interpretive paradigmatic lens. Purposive sampling was used to select a learner who was experiencing a speech-flow difficulty. Various methods of data collection were employed, such as: a semi-structured interview with the learner and her mother, diary entries from the learner, a timeline of the learner’s life drawn by the learner herself, the researcher’s observations (both in the classroom and during a break time), as well as the researcher’s own reflective notes. Documents were also made available to the researcher, and these were the learner’s school reports, the learner’s speech therapy workbook, as well as a report on the learner from her speech therapist. This data was analysed through a qualitative coding process. The research findings indicated that the learner had various experiences, both positive and negative, within all spheres of her life. The majority of her experiences were positive, as the learner had strong support structures in her life, in the form of her mother, her friends, her sound academic capabilities, as well as her level of resilience. The learner is also currently receiving positive intervention in the form of speech therapy, where she is learning various strategies in order to assist her with her speech-flow difficulty. However, there are few factors that make the learner feel uncomfortable, especially in the classroom. The researcher has thus recommended ways in which educators can better support learners who are experiencing speech-flow difficulties within their classroom. / AFRIKAANSE OPSOMMING: Die doel van hierdie studie was om die geleefde ervaringe van ʼn leerder in ʼn hoofstroomskool, en wat ʼn spraakvloeiversteuring ervaar, te probeer verstaan. Die doel hiermee was om insig te verkry in hoe so ʼn leerder ondersteun kan word binne ʼn inklusiewe klaskamer. Die teoretiese raamwerk waarop hierdie studie berus is die bio-ekologiese model, inklusiewe onderwys, positiewe sielkunde, veerkragtigheid, sowel as ontwikkelingsielkunde, spesifiek die adolessente ontwikkelingsfase. Die mikpunt was om die leerder holisties te beskou, deur al die positiewe ondersteuningstrukture in haar lewe in ag te neem veral haar vlakke van veerkragtigheid, terwyl die feit dat sy haar in die adolessente ontwikkelingsfase bevind, en in ʼn hoofstroomskool is, verder lig op haar ervaringe kan werp. Inklusiewe klaskamers behoort dus in so ʼn mate gestruktureer te wees dat individuele behoeftes van leerders in ag geneem word. Die navorsingsmetodologie in hierdie studie het berus op ʼn basiese kwalitatiewe benadering, en beskou deur ʼn interpretatiewe paradigmatiese lens. ʼn Doelgerigte steekproef is gebruik om ʼn leerder te identifiseer wat ʼn spraakvloeiversteuring ervaar. Verskeie metodes van data-insameling is gebruik, byvoorbeeld semi-gestruktureerde onderhoude met die leerder en haar moeder, dagboekinskrywings van die leerder, ʼn tydlynoefening wat die leerder van haarself geteken het, die navorser se waarnemings (binne die klaskamer sowel as op die speelgrond), asook die navorser se reflektiewe notas gedurende die proses afgeneem. Dokumente is beskikbaar gestel aan die navorser, naamlik die leerder se skoolrapporte, haar werkboek wat sy gedurende spraakterapie sessies gebruik, sowel as ʼn verslag deur die spraakterapeut wat die deelnemer tans konsulteer. Die data is geanaliseer met behulp van ʼn kwalitatiewe koderingsproses. Die navorsingsbevindinge dui ʼn verskeidenheid van ervarings (positief sowel as negatief) aan wat die leerder binne al die areas van haar lewe ondervind. Die meerderheid van haar ervaringe is positief, aangesien sy sterk ondersteuningsstrukture in haar lewe het, veral haar moeder, haar vriende, haar sterk akademiese vermoeëns sowel as haar veerkragtigheid. Die leerder ontvang ook tans ʼn baie positiewe intervensie van ʼn spraakterapeut, waar sy ʼn verskeidenheid strategieë aanleer, wat haar help om die spraakvloeiversteuring mee te hanteer. Daar is egter ʼn paar faktore wat haar ongemaklik maak, veral in die klaskamer. Die navorser kan dus ‘n verskeidenheid van riglyne aanbeveel waardeur leerders met spraakvloeiversteurings ondersteun kan word binne die klaskamer.

Esta dissertação tem como objetivo geral criticar o modelo relacional vigente entre o terapeuta e o paciente, a partir de elementos fundamentais da Religião Cristã, do ponto de vista terapêutico. Por meio de pesquisa bibliográfica, estuda-se a relação entre terapia e religião, buscando-se apreender o modo de interagir que pacientes e terapeutas reclamam. A pesquisa tem como base o Evangelho de Marcos, a partir de autores bíblicos atuais como Ekkehard W. Stegemann, Gerd Theissen e John Meier. São consideradas, também, para fundamentação teórica de conceitos, idéias de pensadores como Juan Luis Segundo, Paul Tillich e Victor Frankl. Beyond cybernetics : connecting the professional and personal selves of the therapist

Text in English / This research explores the meaning of the first and second-order therapeutic stances with reference to the therapist's professional and personal development. The dominant positivist paradigm was reflected in the therapist's initial position of expert observer, outside of the observed. The observed phenomena were a group of children suffering from thalassemia major, a terminal genetic disease, and their mothers. The initial idea of short-term intervention and focus on the observed evolved into six-year journey where the observer and the observed became an interconnected unit of observation, understanding and change. A first-order stance led to therapeutic stuckness, where the therapist's confrontation with her therapeutic failure and the limitations of the dominant paradigm provoked a deconstruction of the expert position and promoted a self-reflexive therapeutic stance. The author's self-searching process took her back to her personal self, her family of origin and the ''wounded healer". The researcher moved from an initial disconnection between her professional and personal selves to an awareness of the interface between the two and, ultimately, to a unification of her professional and personal selves. Such development involved an individuation process moving from a narcissistic belief in her objective stance towards a therapeutic stance where she sees herself less as a powerful agent of change and moves to an increasingly higher order of integration of the professional and personal selves (Skovholt & Ronnestad, 1992). The process with the children and mothers shifted from a focus on compliance and medical issues to more personal and emotional stories. The therapist's participation and collaborative stance created a context for change, where greatly improved medical compliance was just one of the many transformations experienced by all the participants. The researcher speculates that development of a second-order stance requires second-order change, which comes "at the end of long, often frustrating mental and emotional labor" (Watzlawick et al., 1974, p. 23), promoting integration between the professional and personal selves of the therapist. Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Esta pesquisa é fruto de inquietações e questionamentos da pesquisadora em uma UTI geral/adulto, seu campo de ação clínica. A questão que norteou a pesquisa buscou compreender a experiência de uma equipe de saúde em uma UTI/geral adulto. A interrogação inicial desdobrou-se em novas perguntas: como as ações da equipe podem ser complementares no contexto da UTI, cuja prioridade é salvar a vida do paciente, considerando, também, o cuidado integral do paciente. Como a equipe lida com as questões da morte e do morrer em seu cotidiano de trabalho, e quais as repercussões dessa experiência no modo como acontecem as intervenções junto ao paciente e familiares? Quais os prejuízos decorrentes da fragmentação do saber na modernidade, nas ações da equipe de saúde na situação de intervenção em uma UTI geral/ adulto. E quais as possíveis contribuições da ação clínica do psicólogo, fundada numa perspectiva fenomenológica existencial, no modo de funcionar da equipe de saúde. A dissertação está organizada em três artigos: dois teóricos e um empírico. O primeiro artigo teórico, trata da ação de uma equipe de saúde em UTI, teve como metodologia a pesquisa bibliográfica. Em seu desenvolvimento, busca traçar o surgimento da UTI e a evolução da assistência hospitalar ao doente grave, ressaltando os eminentes personagens da história do intensivismo; caracteriza o modelo atual de UTI no Brasil e descreve como a equipe multiprofissional foi se estabelecendo neste contexto de ação; apresenta, ainda, o cotidiano de trabalho da equipe e o quanto sua rotina é atravessada pelas questões da vida e da morte. O segundo artigo, também com metodologia bibliográfica, enfoca o acontecer da equipe de saúde em uma Unidade de Terapia Intensiva. Para tanto, aborda a fragmentação do saber na modernidade e sua ressonância na prática do profissional de saúde, delineando o fenômeno da hiperespecialização e as relações disciplinares; além disso, demarca a ação do psicólogo na equipe da UTI, numa perspectiva da fenomenologia existencial. Por fim, o terceiro artigo foi desenvolvido a partir de uma pesquisa qualitativa de cunho fenomenológico. A questão da pesquisa, vinculada aos objetivos da dissertação, foi trabalhada em um encontro com seis profissionais de saúde que fazem parte de equipes multidisciplinares em um Centro de Terapia Intensiva de um hospital da rede privada na cidade de Recife, cujos dados foram analisados a partir da Analítica de Sentido de Critelli. Como resultado observou-se que uma única área de conhecimento não pode dar conta da complexidade do fenômeno do adoecimento; que o trabalho em equipe exige colaboração, de modo que as ações no campo possam revelar uma fusão de horizontes, com possibilidades para o iniciar de uma outra/nova compreensão da ação em con-junto.

The question that guided the research sought to understand the experience of a team of health care at ICU / general adult. The paper is organized into three parts: two theoretical papers and empirical one. The initial interrogation lead us to new questions: how the team's actions may be complementary in the context of the ICU, whose priority is to save the patient´s life, considering also the holistic care of the patient. As the team deals with issues of death and dying in their daily work, and what the repercussions of this experience happen in the way the interventions with the patient and family? What are the negative effects of fragmentation of knowledge in modernity, the actions of the health team in a position to intervene at the ICU / general adult. And what are the possible contributions of the psychologist's clinical action based on an existential phenomenological perspective, mode of functioning of the health team. The first theoretical paper deals with the action of a health team in the ICU, had the methodology literature.About the development outlines around the emergence of the historical evolution of the ICU and hospital care to the ill person, emphasizing the prominent characters in the history of intensive care. It also aims to characterize the current model of ICU in Brazil and it describes how the multidisciplinary team was establishing itself in this context of action. It also displays the daily work of the team and how your routine is crossed by the issues of life and death. The second article, also with the methodology literature, focuses on the happening of the health team at intensive care unit. To do so, addresses the fragmentation of knowledge in modernity and resonances in the practice of health professionals outlining the phenomenon of hyperdisciplinary and relationships. Moreover, the action marks the psychologist in ICU team perspective of existential phenomenology. Finally, the third article was developed from a qualitative study of a phenomenological. The question of research, linked to the aims of the dissertation, was crafted in a meeting with six health professionals who are part of multidisciplinary teams in the intensive care unit of a private hospital network in the City of Recife and the data were analyzed from the Analytical Sense Critelli. As a result it was observed that a single area of knowledge does not account for the complexity of the phenomenon of illness that requires teamwork, collaboration, so thatthe actions in the field may prove a "fusion of horizons" with possibility to start another /new understanding of action in a set. / Esta pesquisa é fruto de inquietações e questionamentos da pesquisadora em uma UTI geral/adulto, seu campo de ação clínica. The relationship between touch sensation of the hand and occupational performance in individuals with chronic stroke

Indiana University-Purdue University Indianapolis (IUPUI) / Stroke is the main cause of disability in the United States. Individuals with stroke commonly report sensory impairment affects their recovery. Motor recovery and sensory impairment are related and impact individuals’ ability to perform valued occupations. Despite the prevalence of sensation impairment after stroke, many occupational therapists fail to include sensation assessment and intervention in treatment planning. The exclusion of sensation in occupational therapy interventions during stroke rehabilitation may be due to the lack of literature supporting the association between sensation and occupational performance. The current study aimed to determine the relationship between touch sensation of the affected hand and occupational performance and satisfaction in individuals with chronic stroke. Using a cross-sectional study design, this study associated factors related to hand sensation and function in individuals with chronic stroke. Fifty individuals with chronic stroke participated in a one-time testing session in which assessments related to sensation, movement of the hand and engagement in daily activities were administered. Correlation analyses were utilized to determine relationships between touch sensation of the affected hand with individuals’ abilities to engage in valued daily activities, arm and hand disability, and manual abilities. The main finding of the study was that individuals with intact sensation reported greater ability to perform valued occupations and satisfaction with their performance, as compared with individuals with touch sensation impairment. For individuals with impaired touch sensation of the affected hand, impairment of touch sensation of the hand did not correlate with individuals’ performance or satisfaction with valued occupations, arm or hand movement, or manual abilities. Collectively, the results of this study reflect the complex interaction between touch sensation, occupational performance, motor functioning, and manual abilities of the affected hand for individuals’ who have experienced a stroke. This study informs therapists, rehabilitation scientists, and other healthcare professionals that client-centered, individualized approaches, including a wide array of clinical assessments and intervention, including assessment of occupational performance and sensation, remain important components in stroke rehabilitation.
