Introdução: As fissuras orofaciais estão entre as anomalias congênitas mais comuns e compreendem uma grande variabilidade de formas clínicas, sendo as fissuras labiopalatinas as mais comuns. A incidência de patologias da orelha média é elevada em pacientes com fissuras labiopalatinas ou palatina isolada. Nestes pacientes, pode haver uma inserção anômala da musculatura do véu palatino na tuba auditiva além de alterações estruturais na cartilagem e no lúmen da tuba. A disfunção tubária persistente ocasiona pressão intratimpânica negativa a qual leva ao início de sucessivas alterações na orelha média, muitas vezes irreversível. Objetivo: Descrever as alterações otológicas de 180 pacientes com fissura labiopalatina ou palatina isolada acompanhados no Hospital de Clínicas de Porto Alegre. Métodos: Analisaram-se 180 pacientes que consultaram no Ambulatório de Otorrinolaringologia e Fissura Palatina no período de agosto de 2001 a outubro de 2002. Além de uma história detalhada, foi realizada otoscopia com descrição detalhada dos achados e otomicroscopia sempre que possível. A análise estatística foi efetivada com o teste t de Student e o Quiquadrado, e quando necessário, foi utilizado o teste exato de Fisher. Resultados: Detectou-se 130 pacientes (72,2%) sem queixas otorrinolaringológicas na primeira avaliação. Observamos que 109 pacientes (70,3%) apresentavam fenda envolvendo o lábio e o palato e 46 pacientes (29,7%) apresentavam apenas comprometimento isolado do palato. A prevalência geral de alterações à otoscopia foi de 77,5%. Efusão na orelha média esteve presente em 80 pacientes (46,2%), otite média crônica colesteatomatosa em 11 (6,4%) e não colesteatomatosa em 9 (5,2%). Retração moderada/ severa foi o principal achado em 24 pacientes (13,9%). Observou-se uma associação linear entre a faixa etária e a presença dessas patologias. À medida que aumenta a faixa etária diminui a ocorrência de efusão na orelha média e aumenta a ocorrência de patologias mais crônicas como retração moderada/ severa, otite crônica com ou sem a presença de colesteatoma. A avaliação otorrinolaringológica continuada do paciente com fenda no palato permite a intervenção sempre que necessária e a manutenção da integridade da orelha média. / Introduction: Orofacial clefts are among the most frequent congenital anomalies, and encompass a wide variety of clinical presentations, of which labiopalatine clefts are the most common. The incidence of middle ear diseases is high in patients with labiopalatine or palatine clefts. In these patients, there is an anomalous insertion of the musculature of the palatine veil in the auditory tube, and also structural abnormalities in the cartilage and lumen of the tube. Persistent tubarian dysfunction causes a negative intratimpanic pressure which in turn gives rise to a series of sequential and often irreversible abnormalities in the middle ear. C.G. Conn Tuba Designs from 1880-1940: An Investigation of Early Tuba Product Lines and Construction Techniques

abstract: The C.G. Conn instrument manufacturing company is known as one of the most successful and innovative band instrument manufacturers in the history of the United States. Many of C.G. Conn's instrument product lines have undergone significant changes throughout the company's history, especially in the brass family. The C.G. Conn tuba product lines are no exception to this company's extraordinary success, and have been significantly redesigned since the company began manufacturing these instruments in circa 1880. This research project investigates the tuba product lines that C.G. Conn manufactured between 1880 and 1940. C.G. Conn designed six different tuba product lines during this timeframe, including an unnamed tuba product line with Stölzel valves, the Wonder Valve line, the New American line, the Wonder Model line, the 20-J, and the 22-J instrumental product lines. These tuba product lines have been investigated using extant publications and patent information because the majority of C.G. Conn's internal records prior to 1970 have been lost. In addition to investigating each of C.G. Conn's early tuba product lines, this project also explores the particularly anomalous design in the top-action valve apparatus of the Conn Wonder Model tuba product line. This anomalous design was implemented in the all of C.G. Conn's top-action tuba and tuba-like product lines from circa 1890-1940. This author's measurements of period instruments and analysis of data taken from these measurements indicates that this anomalous top-action valve apparatus design utilized interchangeable parts with other front-action C.G. Syftet med detta arbete var att undersöka hur sång kan appliceras i övning och instudering. Genom att sjunga, spela på munstycke och slutligen spela utvalda passager ur Vaughan Willams tubakonsert sats 1 och 2, som är väl använd vid tubaprovspelningar, försökte jag överföra sångaspekter och kvaliteter på mitt tubaspel. Områden jag fokuserade på var klang, användning av olika vokaler och musikaliskt uttryck. För att kunna höra resultatet och eventuella skillnader spelade jag in mig själv. För att utöka min kunskap har jag läst om de fysiologiska aspekterna om sång och vokalljud, Arnold Jacobs metod “Song and Wind” och bakgrund kring Vaughan Williams och hans tubakonsert. Jag kom fram till att sång som instuderingsmetod ger mycket goda resultat främst musikaliskt. Det sätt och de vokaler som var lättast att sjunga på var inte alltid de som var bäst lämpade att använda i tubaspelet

Exercises for Excerpts: A Prescriptive Approach for the Developing Orchestral Tubist

abstract: This document details the conception and creative process of fundamental exercises intended for the refinement of orchestral repertoire for the tuba. Performance practices and study materials that relate to the orchestral tubist have evolved significantly over the instrument’s history. Although much of its current methodology is derived from the pedagogical insights of the brass family, resources intended to specifically address the unique musical and technical challenges of the tuba have become more prevalent in the past decade. This study aims to detail the conception of exercises that target the skills necessary for the realization of eight excerpts. It also begs the question: what might a new resource encompass that would differentiate its quality and intent from existing materials? To create a resource that is reflective of current trends and standards in tuba performance, a dialogue was established with several professional tubists through the creation of an online survey. Respondents’ interpretations of each included excerpt were assessed by generalized, specific, and quantifiable feedback. This data was then utilized to directly inform the creative process of supplementary exercises for the included repertoire. This thesis thrives to promote community health and wellbeing through smart design, celebrating culture(s), and offering efficacious and real-world solutions to mitigate certain challenges arising from the imminent threat of climate change and the gradual depletion of our planet's natural resources. The projected building harnesses naturel forces, minimizes energy consumption, and uses natural/passive strategies like thermal mass and natural ventilation. Interior spaces enjoy an abundance of Natural lighting, biophilic attributes, and thera-serlized or uninterrupted views. It generates electrical energy due to adequate solar power and clear skies, especially in hot and arid climates like the proposed location of the project in Tuba City, AZ. In my proposal of a sustainable, community-focused, wellness center, this project will attempt to embrace diversity, celebrate the Navajos heritage through incorporating their belief system and culture into the genius Loci of the place, which will have a long-term healing effect on patients during their journey of recovery. The Navajo nation is a native American reservation and a self-governing community located in the southwest of the US between four states (UT, AZ, NM, CO). Since it's an Underserved, marginalized, and medically under-resourced community, the Navajo Nation was prone to COVID-19 onslaught in 2020, which resulted in substantial number of cases compared to other US states. / Master of Architecture / In Dec 2020, the World witnessed the first case of Coronavirus disease or COVID-19 in Wuhan, China. The disease has since spread rapidly worldwide, leading to an ongoing pandemic. Like many countries across the globe, the health system in the United States of America has to grabble with this deadly virus by inducing measures such as mask mandates and lockdowns in many US states. Unfortunately, and due to economic and social disparities, COVID-19 pandemic has brought injustice and inequity to the forefront of public health. Some communities were hit hard due to lack of emergency response, the availability of health professionals, and healthcare infrastructure. Tuba city, which is the Diné or the Navajo nation second-largest community in Coconino County, AZ, was majorly hit with COVID-19 resulting in a significant number of cases compared to other US cities. This project is a critical component of an emergency preparedness matrix that can firstly; help absorb the shock of such outbreaks by providing primary and outpatient services. Secondly; it offers community-focused and wellness service that can empower underserved, under-resourced and valuable communities like the Navajo Nation. This project is unique due to its inherited and embedded characteristics of bringing the Navajo tradition into the spirit of the building, by celebrating their culture making it a key component in a patent's healing process.
