Aspirin Improves the Patency Rate of Seeded Vena Cava Grafts

The purpose of this study was to evaluate the effectiveness of aspirin (ASA) and porcine endothelial cell seeding in improving the patency rate of vena cava grafts. Thirty-nine dogs underwent infrarenal vena cava replacement by 10 cm lengths of 8 mm I.D. ringed polytetrafluoroethylene grafts. Thirty-one grafts were seeded with 1-1.5 x 10(6) porcine aortic endothelial cells while eight were not (GIII). Of the seeded group, 16 animals received no ASA (GI), while 15 others (GII) were given ASA (325 mg) daily starting two days preoperatively and continuing until sacrifice. Venograms were performed on the fourth postoperative day. Grafts were harvested 32 days after insertion and evaluated for patency rate and endothelialized surfaces. The 32-day patency rate was significantly higher for GII than for GI and III animals (67% vs. 13 and 25% respectively). Endothelialized surface was higher in GII than Gi and III (67% vs. 16% and 18% respectively). We conclude that endothelial cell seeding alone does not prevent graft closure and that a combination of ASA and cell seeding significantly increases the patency rate of vena cava grafts.

Rezultati lečenja aortoilijačne bolesti endovaskularnim procedurama i klasičnim hirurškim pristupom / Results of the treatment of diseases of the aortoiliac endovascular procedures and conventional surgical access

<p>UVOD: Uspe&scaron;nost revaskularizacije aortoilijačne bolesti procenjuje se prohodno&scaron;ću (patentno&scaron;ću) nakon revaskularizacije i brojem komplikacija. Osnovni cilj ove studije je bio utvrditi da li postoji razlika u učestalosti komplikacija, prohodnosti (patentnosti) i uticaj prohodnosti arterija donjeg vaskularnog korita na rekonstrukcije aortoilijačne bolesti nakon lečenja endovaskularnim i klasičnim hirur&scaron;kim pristupom. MATERIJAL I METODOLOGIJA: uzorak je činilo 229 bolesnika lečenih od aortoilijačne bolesti koji su pripadali TASC-u B i C a nakon procene prohodnosti femoropoplitealnog segmenta kao i broja prohodnih potkolenih arterija, bolesnici su stratifikovani u dve glavne grupe a svaka grupa od po tri podgrupe prema kvalitetu utočnog korita. Prvu grupu čine bolesnici sa jednoetažnom ili multiplim kraćim lezijama aortoilijačne signifikantne stenoze, sa ne signifikantnom aortoilijačnom stenozom i sa aortoilijačnom okluzijom. Drugu grupu čine bolesnici sa vi&scaron;eetažnom aortoilijačnom signifikantnom stenozom uz signifikantnu femoropoplitealnu stenozu i/ili popliteokruralnu stenozu. Studija je kreirana kao retrospektivna i prospektivna gde smo pratili rezultate revaskularizacije endovaskularnim procedurama i klasičnim hirur&scaron;kim pristupom. REZULTATI: Računanjem Mahalanobisove distance između homogenost jednoetažne, multiplih kraćih lezija i vi&scaron;eetažne aortoilijačne bolesti kod ispitivanih bolesnika dobija se najmanje rastojanje između homogenost jednoetažne, multiplih kraćih lezija i vi&scaron;eetažne aortoilijačne bolesti zato smo se i rukovodili da jednoetažne lezije, multiple kraće lezije tretiramo endovaskularnim procedurama a vi&scaron;eetažne lezije klasičnim hirur&scaron;kim pristupom. Nije bilo statističke značajne razlike među komplikacijama endovaskularnih procedura i klasičnog hirur&scaron;kog pristupa p&gt;0,1. U lečenju aortoilijačne bolesti endovakularne procedure imaju manji broj bolničkih dana, podjednaku primarnu asistiranu patentnost ali ne&scaron;to slabiju primarnu patentnost u odnosu na klasični hirur&scaron;ki pristup dok je sekundarna patentnost neznatno lo&scaron;ija od klasičnog hirur&scaron;kog pristupa. Da bi smo odreditli uticaj prohodnosti arterija donjeg vaskularnog korita na rekonstrukcije aortoilijačne bolesti kreirali smo matematički model kome smo dali obeležja prema kome bi mogli prognozirati smanjenje primarne patenosti nakon aortoilijačne revaskularizacije endovaskularnim procedurama i klasičnim hirur&scaron;kim pristupom. Postojanjem značajnih razlika između stepena patentnosti u odnosu na obeležja faktora rizika, definisali smo funkciju razgraničenja svih mogućih kombinacija od dva stepena patentnosti. Na osnovu te funkicije (x) moguće je izvr&scaron;iti prognozu kom stepenu patentnosti pripada (dobra,solidna, lo&scaron;a) za bolesnike koji nisu obuhvaćeni ovom studijom, kao i pouzdanost tom stepenu. ZAKLJUČAK: Aortoilijačna bolest je u većini slučajeva udružena sa bole&scaron;ću donjeg vaskularnog korita. Bez obzira na ekstenzivnost aortoilijačne bolesti endovaskularne procedure su efektivna i bezbedna, adekvatna alternativa klasičnom hirur&scaron;kom pristupu. Ova studija pokazuje da su endovaskularne procedure i klasični hirur&scaron;ki pristup u lečenju aortoilijačne bolesti komplementarne a ne komparativne metode.</p> / <p>BACKGROUND. The effectiveness of revascularization aortoiliac occlusive disease is the estimated patency after revascularization and is connected to the number of complications. The main objective of this study was to determine whether there is a difference in the incidence of complications and patency of lower artery vascular tree on the reconstruction aortoiliac occlusive disease after treatment of endovascular and conventional surgical approach. MATERIAL AND METHODOLOGY: The sample consisted of 229 patients treated for the aortoiliac occlusive disease who belonged in TASC B and C after a mobility assessment of the femoropopliteal segment and the number of walk-on below knee arteries, patients were stratified into two groups and each group of three subgroups according to the quality of the inflow. The first group consists of patients with one storey or multiple shorter lesions, and aortoiliac significant stenosis with aortoiliac not as significant stenosis and occlusion aortoiliac. The second group consists of patients with multi-floor aortoiliac significant stenosis with a significant femoropopliteal stenosis and/or popliteocrural stenosis. The study was designed as a retrospective and prospective study, where we track the results of endovascular revascularization procedures and compare to the conventional surgical approach. RESULTS: By calculating the Mahalanobis distance between the homogeneity of one story, multiple lesions and multi-storey short aortoiliac occlusive disease the studied patients received a minimum distance between the homogeneity of one story, multiple lesions and multi-storey short aortoiliac disease, multiple lesions treated fewer endovascular procedures and multi-storey lesions than a classical surgical approach. There were no statistically significant differences among the complications of endovascular procedures and classic surgical approach p&gt; 0.1. The patients treated with aortoiliac (endovascular) procedure have fewer hospital days, equal-assisted primary patent protection or slightly less primary patent protection compared to a traditional surgical approach while secondary patent protection is slightly worse than the classic surgical approach. In order to determine the impact of vascular patency of the arteries of the lower stories to the reconstruction treatment of aortoiliac occlusive diseases, we have created a mathematical model to which we gave the characteristics by with which it could predict the reduction of primary patency of aortoiliac revascularization after endovascular procedures and the conventional surgical approach. The existence of significant differences between the degree of patent protection in relation to the characteristics of the risk factors, we have defined the function of the demarcation of all possible combinations of two degrees of patent protection. Based on this function of (x) it is possible to forecast to what extent patent protection belongs (good, solid&sbquo; bad) for patients who are not included in this study, as well as the reliability of this level. CONCLUSION: the aortoiliac occlusive disease is in most cases associated with vascular disease of the lower artery vascular tree. Regardless of the extensiveness of the disease, aortoiliac occlusive disease endovascular procedures are safe and effective, an adequate alternative to the conventional surgical approach. This study shows that endovascular procedures and the conventional surgical approach in the treatment of aortoiliac occlusive diseases with a complementary rather than competitive method.</p>

Estudo prospectivo e randomizado da revascularização do miocárdio minimamente invasiva com dissecção da artéria torácica interna esquerda por videotoracoscopia robótica / Robotic left internal mammary artery harvesting for single vessel minimally invasive coronary bypass: a randomized controlled trial

Objetivos: O objetivo desse estudo foi comparar a perviedade da artéria torácica interna esquerda (ATIE) dissecada por videotoracoscopia robótica para revascularização minimamente invasiva do ramo interventricular anterior (RIA) com a revascularização do miocárdio convencional. Métodos: De 2007 a 2010, 36 pacientes foram randomizados para revascularização do miocárdio minimamente invasiva (RMMI) ou revascularização do miocárdio convencional (RMC). Pacientes randomizados para o grupo RMMI foram submetidos à dissecção da ATIE por videotoracoscopia auxiliada pelo braço robótico AESOP seguida de uma minitoracotomia anterior esquerda no 4º espaço intercostal para anastomose com o RIA. Pacientes randomizados para o grupo RMC foram submetidos a revascularização do miocárdio convencional com esternotomia mediana completa, dissecção aberta da ATIE e anastomose ao RIA. Fluxometria por tempo de trânsito (FTT) foi utilizada para avaliação da perviedade da ATIE imediata. Após 24 meses uma tomografia multislice foi utilizada para avaliar a perviedade a médio prazo da ATIE. Resultados: O tempo médio de dissecção da ATIE no grupo RMMI foi de 50,1 ± 11,2 vs. 22,7 ± 3,3 min no grupo RMC. Não houve diferença significativa no fluxo médio da ATIE para o RIA entre os grupos estudados (46,17 ± 20,11 vs. 48,61 ± 23,42 mL/min, p=0,86) respectivamente. Não houve diferença significante na incidência de infecção de ferida profunda (0 vs. 2, p=0,48) e necessidade de reoperação por sangramento (0 vs. 1, p=1,00) nos grupos RMMI e RMC respectivamente. A angiotomografia mostrou perviedade da ATIE em 100% dos pacientes do grupo RMMI vs. 94,1% no grupo RMC (p=1,00). Não houve mortalidade nos grupos estudados. Conclusão: A revascularização do miocárdio minimamente invasiva do ramo interventricular anterior com dissecção da artéria torácica interna esquerda por videotoracoscopia robótica foi segura e factível. A perviedade da artéria torácica interna esquerda imediata e a médio prazo foi similar entre ambas as técnicas / Objective: The aim of this study was to compare the patency of left internal mammary artery (LIMA) robotically harvested for left anterior descendent (LAD) artery minimally invasive bypass with conventional LIMA to LAD off-pump bypass. Method: From 2007 to 2010, 36 patients were randomized to either LIMA robotically harvested to LAD artery minimally invasive bypass or standard LIMA to LAD off-pump bypass. Patients assigned to robotic group underwent robotic endoscopic harvesting of LIMA with the AESOP system followed by a small left thoracotomy in the 4th intercostal space for off-pump LAD bypass. Patients assigned to standard group underwent full median sternotomy, open LIMA harvesting followed by off-pump LAD bypass. Transit time flow measurement was used for intraoperative evaluation of LIMA to LAD patency. After a mean 24-month follow-up, Multislice Computed Tomography was used to evaluate LIMA to LAD midterm patency. Results: The mean LIMA harvesting time in robotic group was 50.1 ± 11.2 min vs. 22.7 ± 3.3 min in conventional group. Estudo experimental comparativo de remendos arteriais de polidimetilsiloxano com reforço de tecido de poliéster (PDMSr) versus politetrafluoretileno expandido (PTFEe) em aorta de coelhos / Patch of polydimethylsiloxane reinforced with polyester fabric for aortic angioplasty in rabbits

Introdução: Apesar de bons resultados descritos na literatura, o substituto ideal para a utilização como remendo no fechamento arterial ainda não existe. Por este motivo, ainda há espaço para a busca por remendo que seja biocompatível, e que apresente facilidade de manuseio e resultados satisfatórios. Avaliamos remendos arteriais de silicone (polidimetilsiloxano com reforço em poliéster - PDMSr) em comparação com remendos arteriais de PTFEe. Objetivo: O objetivo deste trabalho é comparar, em modelo experimental, em coelhos, os resultados de remendos arteriais feitos em PDMSr com remendos de PTFEe. Materiais e Método: A amostra foi definida em 10 animais que completassem todas as etapas da pesquisa em cada grupo. Os animais foram submetidos à laparotomia mediana e abertura longitudinal da aorta de aproximadamente 8mm, realizando-se o seu fechamento com remendo do grupo selecionado, seguido do fechamento por planos. Os animais foram mantidos em biotério até o 60º PO, quando, então, realizou-se arteriografia de controle, e análise macro e microscópica de peça. Resultados: Para se atingir a amostra desejada, foram necessários 12 procedimentos no grupo PDMSr e 16 no grupo PTFEe. Ocorreram 2 óbitos no grupo PDMSr e 6 no grupo PTFEe. Apesar do número maior de óbitos no grupo PTFEe não houve diferença estatística na sobrevida entre os grupos. Um animal do grupo PDMSr apresentou monoparesia em pata posterior direita e um animal do grupo PTFEe apresentou hérnia incisional. Não houve diferença estatística nas complicações entre os grupos. O tempo operatório foi estatisticamente maior no grupo PTFEe quando comparamos todos os animais, fato que não se repetiu quando excluímos os animais que faleceram. O peso e o tempo operatório foram estatisticamente maiores nos animais que morreram. Todos os animais que chegaram ao final do tempo de estudo apresentavam aorta patente. Na análise macroscópica da peça, houve, estatisticamente, maior reação tecidual periprótese no grupo PTFEe. A microscopia eletrônica de varredura evidenciou cobertura de todo o remendo de PDMSr por tecido similar ao endotélio, enquanto, nos remendos de PTFEe, o crescimento limitou-se às bordas da linha de sutura e ilhas isoladas no seu centro. Conclusão: O material estudado apresentou resultados comparáveis ao do PTFEe, porém com menor reação tecidual local e maior proliferação celular para a luz do vaso. Apesar de novos estudos serem necessários, inclusive para avaliação de uso em humanos, o presente estudo apresenta resultados promissores que encorajam a continuidade de sua pesquisa / Introduction: Although good results are reported for various materials for use as patches for arterial closure, as yet none of these is ideal. Therefore, research is continuing into development of a patch that is biocompatible and provides ease of handling, while having satisfactory outcomes. A new silicone arterial patch (polydimethylsiloxane reinforced with polyester fabric, PDMSr) was compared with patches made of expanded polytetrafluoroethylene (ePTFE). Objective: To compare the outcomes between arterial patches made of PDMSr with those made of ePTFE, in an experimental rabbit model. Materials and Method: Rabbits were placed in two groups, and received either PDMSr or ePTFE arterial patches (PDMSr group and ePTFE group, respectively). The animals underwent laparotomy and longitudinal opening of the aorta, which was then closed with the selected patch, followed by suture of all layers. The animals were kept in their cages until the 60th postoperative day, when arteriography, removal of the aorta, and macroscopic and optic and scanning electron microscopic analyzes of the aorta were performed. Ten rabbits from each group that had completed all stages of the research were included in analyses. Results: Twelve procedures were performed in the PDMSr group and 16 in the ePTFE group. There were 2 deaths in the PDMSr group and 6 in the ePTFE group. Despite the higher number of deaths in the ePTFE group, there was no statistical difference in survival rate between the groups. One animal in the PDMSr group developed monoparesis in its right hind paw and 1 animal in the ePTFE group had an incisional hernia. There was no statistical difference in complications between the groups. The operative time was significantly longer in the ePTFE group when all animals were included in analysis, but not when animals that died were excluded. Body weight was significantly greater and operative time was significantly longer in animals that died. Postsurgical aortic patency in the survival animals was 100% in both groups by arteriography. Macroscopically, tissue reaction around the prosthesis was greater in the ePTFE group (statistically significant). Microscopically, the PDMSr patches were entirely covered with a cellular endothelium-like tissue, while tissue growth on the ePTFE patches was limited to the edges of the suture line and to isolated central islands. Conclusion: The two materials showed comparable outcomes; however, PDMSr showed cellular proliferation to the entire graft, and less local inflammatory reaction compared with ePTFE. Although further studies are required, including assessment in humans, the results of the present study indicate that PDMSr shows promise as an arterial patch material

Apesar de novos estudos serem necessários, inclusive para avaliação de uso em humanos, o presente estudo apresenta resultados promissores que encorajam a continuidade de sua pesquisa / Introduction: Although good results are reported for various materials for use as patches for arterial closure, as yet none of these is ideal. Therefore, research is continuing into development of a patch that is biocompatible and provides ease of handling, while having satisfactory outcomes. A new silicone arterial patch (polydimethylsiloxane reinforced with polyester fabric, PDMSr) was compared with patches made of expanded polytetrafluoroethylene (ePTFE). Objective: To compare the outcomes between arterial patches made of PDMSr with those made of ePTFE, in an experimental rabbit model. Materials and Method: Rabbits were placed in two groups, and received either PDMSr or ePTFE arterial patches (PDMSr group and ePTFE group, respectively). The animals underwent laparotomy and longitudinal opening of the aorta, which was then closed with the selected patch, followed by suture of all layers. The animals were kept in their cages until the 60th postoperative day, when arteriography, removal of the aorta, and macroscopic and optic and scanning electron microscopic analyzes of the aorta were performed. Ten rabbits from each group that had completed all stages of the research were included in analyses. Results: Twelve procedures were performed in the PDMSr group and 16 in the ePTFE group. There were 2 deaths in the PDMSr group and 6 in the ePTFE group. Despite the higher number of deaths in the ePTFE group, there was no statistical difference in survival rate between the groups. One animal in the PDMSr group developed monoparesis in its right hind paw and 1 animal in the ePTFE group had an incisional hernia. There was no statistical difference in complications between the groups. The operative time was significantly longer in the ePTFE group when all animals were included in analysis, but not when animals that died were excluded. Body weight was significantly greater and operative time was significantly longer in animals that died. Postsurgical aortic patency in the survival animals was 100% in both groups by arteriography. Macroscopically, tissue reaction around the prosthesis was greater in the ePTFE group (statistically significant). Microscopically, the PDMSr patches were entirely covered with a cellular endothelium-like tissue, while tissue growth on the ePTFE patches was limited to the edges of the suture line and to isolated central islands. Conclusion: The two materials showed comparable outcomes; however, PDMSr showed cellular proliferation to the entire graft, and less local inflammatory reaction compared with ePTFE.
