Para avaliação da migração verifica-se a posição da endoprótese em relação à determinada região anatômica. Considerando o aneurisma da aorta abdominal infra-renal, a área proximal de referência consiste na origem da artéria renal mais baixa e, na região distal, situa-se nas artérias ilíacas internas. Os pacientes deverão ser monitorizados por longos períodos a fim de serem identificadas migrações visto que estas ocorrem normalmente após 2 anos de implante. Para se evitarem migrações, forças mecânicas que propiciam fixação e determinadas por características dos dispositivos e a incorporação da endoprótese, devem predominar sobre forças gravitacional e hemodinâmica que tendem a arrastar a prótese no sentido caudal. Angulação, extensão e diâmetro do colo, além da medida transversa do saco aneurismático, são importantes aspectos morfológicos do aneurisma relacionados à migração. Com relação à técnica, o implante de endopróteses com sobredimensionamento excessivo (>30%) não é recomendado por provocar dilatação do colo do aneurisma, além de dobras e vazamentos proximais que também contribuem para migração. Por outro lado, endopróteses com mecanismos adicionais de fixação (ganchos, farpas e fixação supra-renal) parecem estar menos associadas às migrações. O processo de incorporação das endopróteses ocorre parcialmente e parece não ser suficiente para impedir migrações tardias. Neste sentido, estudos experimentais com endopróteses de maior porosidade e uso de substâncias que permitam maior fibroplasia e aderência da prótese à artéria vem sendo realizados e parecem ser promissores. Nesta revisão estes aspectos serão discutidos, em especial o uso da cola de fibrina. / Migration of the endoprosthesis is a complication of the endovascular treatment defined as misplacement of its initial fixation. In order to assess such migration, one shall verify the position of the endoprosthesis in relation to the determined anatomic site. In this sense, experimental assessment with higher porosity endoprosthesis as well as the use of substances that allow higher fibroplasia and adherence of the prosthesis to the artery have been made and look promising. In this review, such aspects are discussed, specially the use of fibrin glue.

Implante experimental de substituto arterial de polidimetilsiloxano com reforço de tecido de poliéster em coelhos / Experimental implant of arterial substitute of polydimethylsiloxane reinforced with polyester fabric in rabbits

O presente estudo avaliou próteses vasculares de pequeno calibre feitas de polidimetilsiloxano (PDMS) com reforço de poliéster implantadas em aorta de coelhos através de fluoroscopia. Os objetivos do estudo foram: (1) analisar tubo de PDMS com reforço de poliéster implantado em aorta de coelhos como possível material para prótese vascular, (2) avaliar a patência das próteses através de fluoroscopia, (3) avaliar a condição de implante cirúrgico; (4) avaliar o comportamento macroscópico biológico do implante e (5) analisar o modelo experimental proposto. Próteses vasculares de PDMS foram implantadas na aorta infrarrenal de 64 coelhos cedidos pelo biotério da Faculdade de Medicina da Univesidade de São Paulo. Os exames de fluoroscopia foram realizados em até 150 dias após implantação cirúrgica. As próteses de PDMS foram classificadas em ocluídas e patentes, sendo essas sub-classificadas em diferentes graus de estenose. O tempo de clampeamento da aorta foi aferido durante os procedimentos cirúrgicos. De maneira amostral peças foram encaminhadas para microscopia eletrônica para verificação de endotelização das próteses. As análises estatísticas foram feitas através de teste t-Studant, teste ANOVA e Kaplan-Meier. Dos 64 animais que receberam a prótese, trinta (46,88%) apresentaram boa evolução clínica, vinte e três (35,94%) morreram e onze (17,18%) apresentaram paraplegia de membros posteriores. A patência das próteses em 30 dias foi de 87% (±6,7), em 60 dias foi de 73% (±9,3), em 90 dias foi de 57% (±11) e em 120 dias foi de 48% (±12). Cinquenta por cento (oito) das próteses patente não apresentaram nenhum grau de estenose, 35,5% (seis) apresentaram cinquenta por cento ou menos de estenose e 12,5% (dois) apresentaram estenose entre cinquenta e setenta por cento. Nenhum animal apresentou estenose maior que 70%. O tempo médio de clampeamento da aorta foi de 52 minutos. Não houve diferença significante nem na associação entre tempo de clampeamento da aorta e a evolução clínica dos animais (p=0,67) nem na associação entre o tempo de clampeamento e a patência das próteses (t=1,35; p=0,18). As peças encaminhadas para microscopia apresentaram crescimento endotelial a partir do vaso nativo em direção à prótese de PDMS. Foi possível concluir com este estudo que o PDMS demonstrou-se um material adequado para futuras pesquisas no ramo de próteses vasculares e que o uso da fluoroscopia na avaliação dessas próteses foi de fundamental importância na determinação da patência dos implantes / The present study evaluated small caliber vascular prostheses made of polydimethylsiloxane (PDMS) with polyester reinforcement implanted in the aorta of rabbits by fluoroscopy. The study objectives were: (1) analyze PDMS tube with polyester reinforcement implanted in the aorta of rabbits as possible material for vascular prosthesis, (2) assess the patency of the prosthesis through fluoroscopy, (3) assess the condition of surgical implant (4) evaluate the macroscopic behavior of biological implant and (5) analyze the proposed experimental model. Vascular prostheses were implanted in PDMS infrarenal aorta of 64 rabbits assigned by the animal house of the Faculty of Medicine of the University of São Paulo. Fluoroscopic examinations were performed within 150 days after surgical implantation. The prosthesis of PDMS were classified into occluded and patents, these being sub-classified into different degrees of stenosis. The time of aortic clamping was measured during surgical procedures. Sample pieces were sent for electron microscopy to check endothelialization of prostheses. Statistical analyzes were performed using t-test Studant, ANOVA and Kaplan-Meier. Of the 64 animals that received the prosthesis, thirty (46.88%) showed clinical improvement, twenty-three (35.94%) died and eleven (17.18%) had paraplegia of hind limbs. The patency of the prosthesis in 30 days was 87% (± 6.7), at 60 days was 73% (± 9.3), at 90 days was 57% (± 11) and at 120 days was 48 % (± 12). Fifty percent (eight) of the prosthesis patent did not present any degree of stenosis, 35.5% (six) had fifty percent or less of stenosis and 12.5% (two) had stenosis between fifty and seventy percent. No animal showed stenosis greater than 70%. The mean aortic clamping was 52 minutes. There was no significant difference in the association between duration of aortic clamping and clinical evolution of animals (p = 0.67) nor the association between clamping time and patency of the prosthesis (t = 1.35, p = 0.18). The parts sent for microscopy showed endothelial growth from the native vessel toward the prosthesis PDMS. It can be concluded from this study that the PDMS proved to be a suitable material for future research in the field of vascular prostheses and the use of fluoroscopy in the evaluation of these prostheses was of fundamental importance in determining the patency of the implants

Estudo piloto do impacto da terapia antiproliferativa com everolimus administrado por via oral na diminuição de reestenose após implante de stent auto-expansível de nitinol para tratamento de lesões oclusivas da artéria femoral superficial / Pilot study of the impact of antiproliferative therapy with everolimus administered orally in the reduction of restenosis after implantation of selfexpandable nitinol stent for treatment of occlusive lesions of the superficial femoral artery

INTRODUÇÃO: A implantação de stent auto-expansível de nitinol para o tratamento das lesões oclusivas femoro-poplíteas tem sido associado com maus resultados a longo prazo. O everolimus administrado via oral para inibir reestenoses do stent foi investigado recentemente em animais com bons resultados, porém sua segurança e eficácia não têm sido estudada em seres humanos. O propósito deste estudo piloto foi avaliar o impacto da terapia antiproliferativa com everolimus administrado via oral por 28 dias na diminuição de reestenose após implante de stent auto-expansível de nitinol para tratamento de lesões oclusivas da artéria femoral superficial. MÉTODOS E RESULTADOS: Trinta e quatro pacientes foram recrutados para este estudo randomizado, prospectivo. O grupo que recebeu everolimus via oral foi constituído por 15 pacientes e o grupo que não recebeu medicação composto por 19 pacientes. As características basais e do procedimento foram similares entre os dois grupos. Todos os pacientes tinham isquemia crônica do membro inferior e oclusão da artéria femoral superficial (média da lesão de 83,14 mm no grupo sem medicação e 105 mm no grupo everolimus). O objetivo primário do estudo foi a redução da porcentagem média do diâmetro da reestenose intra-stent após seis meses da angioplastia avaliada por angiografia quantitativa. A porcentagem média do diâmetro das reestenoses foi 46,9% no grupo tratado com everolimus e 44,5% no grupo que não recebeu a medicação (p=0,81). Não foram observados efeitos colaterais graves nos grupos. No acompanhamento clínico aos 24 meses não houve diferenças significativas entre os grupos em relação a eventos clínicos. A patência primária, primária assistida e secundária em 24 meses, foi 42%, 74% e 79% no grupo sem medicação e 27%, 73% e 73% no grupo tratado com everolimus. CONCLUSÃO: O everolimus via oral por 28 dias em doses altas é seguro e bem tolerado, com baixo índice de efeitos colaterais, porém não é eficaz na redução da porcentagem média do diâmetro da reestenose intra-stent em pacientes com implante de stents auto-expansíveis de nitinol nas lesões oclusivas complexas da artéria femoral superficial. / INTRODUCTION: The implantation of a self-expanding of nitinol stent in the treatment of femoropopliteal occlusive lesions has been associated with a poor outcome in a long term setting. Everolimus administered orally to inhibit restenosis of the stent was investigated recently in animals with good results, but its safety and efficacy has not been studied in humans. The purpose of this pilot study was to evaluate the impact of antiproliferative therapy with everolimus administered orally for 28 days in the reduction of restenosis after implantation of self-expandable nitinol stent for treatment of occlusive lesions of the superficial femoral artery. METHODS AND RESULTS: Thirty-four patients were recruited for this randomized, prospective study. The group that received oral Everolimus was consisted of 15 patients and the group that received no medication was 19 patients. The baseline characteristics and procedure were similar in both groups. All the patients had chronic lower limb ischemia and occlusion of the superficial femoral artery (mean of the lesion of 83.14 mm in the group without medication and 105 mm in the everolimus group). The primary objective of the study was to evaluate the reduction of the average percentage of the diameter of in-stent restenosis six months after angioplasty assessed by quantitative angiography. The in-stent mean percent diameter stenosis was 46.9% in the group treated with everolimus and 44.5% in the group that received no medication (p = 0.81). There were no serious side effects seen in either group in the clinical follow up at 24 months. There was no significant difference between groups in relation to clinical events. The primary patency, assisted primary and secondary in 24 months was 42%, 74% and 79% in the group without medication and 27%, 73% and 73% in the group treated with Everolimus. CONCLUSION: Everolimus administered orally for 28 consecutive days to stent implantation in high doses proves to be safe and well tolerated, with low rate of side effects, but it is not effective in reducing the average percentage of diameter of in-stent restenosis in patients with implantation of self-expandable nitinol stent in complex occlusive lesions of the superficial femoral artery.

The group that received oral Everolimus was consisted of 15 patients and the group that received no medication was 19 patients. The baseline characteristics and procedure were similar in both groups. All the patients had chronic lower limb ischemia and occlusion of the superficial femoral artery (mean of the lesion of 83.14 mm in the group without medication and 105 mm in the everolimus group). The primary objective of the study was to evaluate the reduction of the average percentage of the diameter of in-stent restenosis six months after angioplasty assessed by quantitative angiography. The in-stent mean percent diameter stenosis was 46.9% in the group treated with everolimus and 44.5% in the group that received no medication (p = 0.81). There were no serious side effects seen in either group in the clinical follow up at 24 months. There was no significant difference between groups in relation to clinical events. The primary patency, assisted primary and secondary in 24 months was 42%, 74% and 79% in the group without medication and 27%, 73% and 73% in the group treated with Everolimus. CONCLUSION: Everolimus administered orally for 28 consecutive days to stent implantation in high doses proves to be safe and well tolerated, with low rate of side effects, but it is not effective in reducing the average percentage of diameter of in-stent restenosis in patients with implantation of self-expandable nitinol stent in complex occlusive lesions of the superficial femoral artery.

Evaluation du traitement antibiotique des infections de prothèse vasculaire à Staphylococcus aureus : apport d'un modèle murin / Assessment of different antibiotic therapies in a murine model of Staphylococcus aureus vascular prosthesis infection

Les infections de prothèses vasculaires (IPV) sont des maladies particulièrement graves. Malgré une fréquence finalement assez importante, elles demeurent mal connues. Staphylococcus aureus en est l’agent responsable principal. Les données concernant le traitement antibiotique à administrer pour ces infections sont excessivement pauvres. L’objectif de notre travail était donc de comparer l’efficacité de différents protocoles d’antibiothérapie à l’aide de divers modèles expérimentaux d’IPV. Six souches différentes de S. aureus ont été évaluées : 3 sensibles (SAMS) et 3 résistants à la méticilline (SARM). Nous avons comparé les concentrations minimales inhibitrices et éradicatrices (CMIB et CMEB) au sein du biofilm obtenues avec des techniques classiques sur polystyrène à ceux obtenus à l’aide d’un modèle original in vitro sur Dacron® (dCMIB et dCMEB) ®. Nous avons ensuite utilisé un modèle original d’infection de Dacron chez la souris pour comparer l’efficacité de différents protocoles thérapeutiques. Enfin nous avons visualisé l’effet de ces antibiotiques in vivo par microscopie confocale. Nous avons montré que les mesures classiques de CMIB et CMEB obtenues sur polystyrène pouvaient surestimer la baisse d’efficacité des antibiotiques dans le biofilm et que des mesures sur le matériel d’intérêt pouvaient être plus pertinentes. Dans notre modèle in vivo, la daptomycine pouvait être supérieure que les comparateurs pour certaines souches de SARM et de SAMS mais pas pour toutes. Par contre, si l’ajout de rifampicine était bénéfique pour la cloxacilline et la vancomycine, cela n’était pas le cas pour la daptomycine. Enfin, nous avons visualisés des effets totalement différents sur le biofilm selon les antibiotiques utilisés mais également selon les souches testées. Nos modèles ont permis d’obtenir des informations nouvelles concernant l’antibiothérapie des IPV qui, nous l’espérons, permettront d’aider à la prise en charge des patients. / Prosthetic vascular graft infection (PVGI) is an emerging disease, mostly due to staphylococci, with limited data regarding efficacy of current antistaphylococcal agents. We aimed to assess the efficacy of different antibiotic regimens. Six different strains of methicillin-susceptible (MSSA) and methicillin-resistant S. aureus (MRSA) were used. We compared results of minimal biofilm inhibitory and eradicating concentrations (MBICs and MBECs) obtained with a Calgary Biofilm Pin lid Device (CBPD) to those yielded by an original Dacron®-related minimal inhibitory and eradicating concentrations measure model. We then used an original murine model of Staphylococcus aureus vascular material infection to evaluate efficacy of different antibiotic regimens. We finally visualized the effect of antibiotics on biofilm by confocal microscopy. We demonstrated that classical measures of MBICs and MBECs with CPBD could overestimate the decrease of antibiotic susceptibility in material related infections and that the nature of the support used to measure biofilm susceptibility might be influent since results yielded by our Dacron®-related minimal eradicating assay were lower than those found on a plastic device. In our in vivo model, we shown that daptomycin was significantly more bactericidal than comparators for some strains of MRSA or MSSA but not for all. For the majority of strains, it was as efficient as comparators. The addition of rifampicin to daptomycin did not enhance daptomycin efficacy in our model. Finally, we highlighted an in vivo differential effect on biofilm depending on the antibiotic used but also on the bacterial strain evaluated. Our models represent an option to better define the best antibiotic options for PVGIs.

Introdução: Apesar de bons resultados descritos na literatura, o substituto ideal para a utilização como remendo no fechamento arterial ainda não existe. Por este motivo, ainda há espaço para a busca por remendo que seja biocompatível, e que apresente facilidade de manuseio e resultados satisfatórios. Avaliamos remendos arteriais de silicone (polidimetilsiloxano com reforço em poliéster - PDMSr) em comparação com remendos arteriais de PTFEe. Objetivo: O objetivo deste trabalho é comparar, em modelo experimental, em coelhos, os resultados de remendos arteriais feitos em PDMSr com remendos de PTFEe. Materiais e Método: A amostra foi definida em 10 animais que completassem todas as etapas da pesquisa em cada grupo. Os animais foram submetidos à laparotomia mediana e abertura longitudinal da aorta de aproximadamente 8mm, realizando-se o seu fechamento com remendo do grupo selecionado, seguido do fechamento por planos. Os animais foram mantidos em biotério até o 60º PO, quando, então, realizou-se arteriografia de controle, e análise macro e microscópica de peça. Resultados: Para se atingir a amostra desejada, foram necessários 12 procedimentos no grupo PDMSr e 16 no grupo PTFEe. Ocorreram 2 óbitos no grupo PDMSr e 6 no grupo PTFEe. Apesar do número maior de óbitos no grupo PTFEe não houve diferença estatística na sobrevida entre os grupos. Um animal do grupo PDMSr apresentou monoparesia em pata posterior direita e um animal do grupo PTFEe apresentou hérnia incisional. Não houve diferença estatística nas complicações entre os grupos. O tempo operatório foi estatisticamente maior no grupo PTFEe quando comparamos todos os animais, fato que não se repetiu quando excluímos os animais que faleceram. O peso e o tempo operatório foram estatisticamente maiores nos animais que morreram. Todos os animais que chegaram ao final do tempo de estudo apresentavam aorta patente. Na análise macroscópica da peça, houve, estatisticamente, maior reação tecidual periprótese no grupo PTFEe. A microscopia eletrônica de varredura evidenciou cobertura de todo o remendo de PDMSr por tecido similar ao endotélio, enquanto, nos remendos de PTFEe, o crescimento limitou-se às bordas da linha de sutura e ilhas isoladas no seu centro. Conclusão: O material estudado apresentou resultados comparáveis ao do PTFEe, porém com menor reação tecidual local e maior proliferação celular para a luz do vaso. Apesar de novos estudos serem necessários, inclusive para avaliação de uso em humanos, o presente estudo apresenta resultados promissores que encorajam a continuidade de sua pesquisa / Introduction: Although good results are reported for various materials for use as patches for arterial closure, as yet none of these is ideal. Therefore, research is continuing into development of a patch that is biocompatible and provides ease of handling, while having satisfactory outcomes. A new silicone arterial patch (polydimethylsiloxane reinforced with polyester fabric, PDMSr) was compared with patches made of expanded polytetrafluoroethylene (ePTFE). Objective: To compare the outcomes between arterial patches made of PDMSr with those made of ePTFE, in an experimental rabbit model. Materials and Method: Rabbits were placed in two groups, and received either PDMSr or ePTFE arterial patches (PDMSr group and ePTFE group, respectively). The animals underwent laparotomy and longitudinal opening of the aorta, which was then closed with the selected patch, followed by suture of all layers. The animals were kept in their cages until the 60th postoperative day, when arteriography, removal of the aorta, and macroscopic and optic and scanning electron microscopic analyzes of the aorta were performed. Ten rabbits from each group that had completed all stages of the research were included in analyses. Results: Twelve procedures were performed in the PDMSr group and 16 in the ePTFE group. There were 2 deaths in the PDMSr group and 6 in the ePTFE group. Despite the higher number of deaths in the ePTFE group, there was no statistical difference in survival rate between the groups. One animal in the PDMSr group developed monoparesis in its right hind paw and 1 animal in the ePTFE group had an incisional hernia. There was no statistical difference in complications between the groups. The operative time was significantly longer in the ePTFE group when all animals were included in analysis, but not when animals that died were excluded. Body weight was significantly greater and operative time was significantly longer in animals that died. Postsurgical aortic patency in the survival animals was 100% in both groups by arteriography. Macroscopically, tissue reaction around the prosthesis was greater in the ePTFE group (statistically significant). Microscopically, the PDMSr patches were entirely covered with a cellular endothelium-like tissue, while tissue growth on the ePTFE patches was limited to the edges of the suture line and to isolated central islands. Conclusion: The two materials showed comparable outcomes; however, PDMSr showed cellular proliferation to the entire graft, and less local inflammatory reaction compared with ePTFE. Although further studies are required, including assessment in humans, the results of the present study indicate that PDMSr shows promise as an arterial patch material

Utilisation d'une décharge à barrière diélectrique pour développer une matrice polymère plasma dégradable pour des applications vasculaires / Using a dielectric barrier discharge to develop a degradable plasma polymer matrix for vascular applicationsg

Chaque année, environ 1,5 million de patients requièrent un remplacement vasculaire en réponse à une athérosclérose avancée, causant le rétrécissement interne des vaisseaux sanguins. Malheureusement, encore aujourd'hui les matériaux synthétiques utilisés pour remplacer les artères de petits diamètres (inférieurs à 6 mm) restent associés à un haut taux d'échecs, démontrant ainsi un manque de biocompatibilité certain. L'une des principales complications observées est l'hyperplasie néointimale artérielle caractérisée par l'obstruction du vaisseau sanguin due à la prolifération tridimensionnelle de cellules sur la paroi interne de la prothèse. Différentes stratégies visant à limiter cette réaction naturelle sont aujourd'hui envisagées, notamment l'utilisation d'un système à libération contrôlée de médicament intégré localement aux prothèses vasculaires. En parallèle, l'essor des technologies plasma a permis de montrer qu'il était possible de revêtir la surface de matériel biomédical pour améliorer son interaction avec un environnement biologique. La stratégie consiste à utiliser l'énergie et la réactivité d'un plasma pour polymériser un précurseur gazeux. En sélectionnant la structure moléculaire du précurseur et les conditions expérimentales du plasma appropriées, il est possible de déposer un polymère plasma à la surface du matériel sélectionné pour lui conférer des propriétés sur mesure. C'est dans ce contexte que cette thèse a consisté à synthétiser, à l'aide d'un plasma, une matrice polymère plasma biodégradable pour revêtir la paroi interne d'une prothèse vasculaire, dans le but d'y incorporer un médicament choisi de façon à limiter l'hyperplasie néointimale. Ce projet a permis d'une part de réaliser une preuve de concept en déposant un revêtement polymère plasma dégradable par décharge à barrière diélectrique en configuration planaire. En utilisant le lactate d'éthyle en tant que précurseur et après de nombreuses analyses, des conditions de dépôt optimales ont pu être élues pour leur potentiel dans le cadre d'applications vasculaires. D'autre part, grâce à une caractérisation approfondie de la décharge, une corrélation étroite entre la physico-chimie du plasma et les dépôts dégradables obtenus a pu être établie. Afin d'élargir les possibilités de vitesse de dégradation, l'influence d'une alimentation impulsionnelle sur la décharge et sur le dépôt a de plus été étudiée. Si la manière d'apporter l'énergie a eu une forte influence sur la décharge, aucune influence majeure n'a été notée sur la chimie et la morphologie des dépôts faits à partir de lactate d'éthyle. Enfin, la construction d'un réacteur plasma tubulaire permettant de déposer la matrice développée à l'intérieur de prothèses artérielles a permis de s'étendre aux conditions réelles de dépôt. Dans l'ensemble, ce projet de recherche a mis en évidence le potentiel des procédés plasma pour le développement de matrices polymères plasma dégradables, notamment dans le cadre de systèmes à libération contrôlée et locale de médicaments pour des applications en chirurgie vasculaire. D'un point de vue de la physique des plasmas, ce travail a de plus souligné l'importance de l'étude de la décharge dans de véritables conditions de dépôt de couches minces. / Every year, about 1.5 million patients need a vascular replacement due to advanced arteriosclerosis, which causes the internal narrowing of blood vessels. Unfortunately, even today the synthetic materials used to replace small diameter arteries (below 6 mm) remain associated with low patency rate, which demonstrates an evident lack of biocompatibility. One of the main observed complications is arterial neointimal hyperplasia, which is characterized by the blood vessel obstruction due to the tridimensional proliferation of cells on the graft internal wall. Different strategies aiming at limiting this body reaction are currently considered, in particular the use of a drug delivery system locally integrated to the vascular grafts. Concurrently, the rise of plasma technologies enabled to demonstrate the possibility to coat the surface of biomedical devices to improve their interaction with a biological environment. The strategy consists in using the plasma energy and reactivity to polymerize a gaseous precursor. By selecting the appropriate precursor molecular structure and plasma experimental conditions, one can build up a plasma polymer with tailored properties. It is in this context that this thesis consisted in synthesizing, using plasma, a biodegradable polymeric plasma polymer matrix to coat the internal wall of a vascular graft, with the goal to incorporate a drug chosen to limit neointimal hyperplasia. On one hand, this project acted as proof of concept by developing a degradable plasma polymer coating using a planar dielectric barrier discharge. After extensive studies using ethyl lactate as precursor, optimal chemical vapor deposition conditions were elected for their potential in terms of vascular applications. On the other hand, thanks to an extended discharge characterization, a strong correlation was established between the plasma physico-chemistry and the properties of the degradable coatings synthesized. In addition, to broaden possibilities in terms of degradation rate, the influence of a squared pulse power supply on the discharge and the coating was studied. If changing the way to bring the energy had a strong influence on the discharge, no major influence was noticed on the ethyl lactate-based coatings' chemistry and morphology. Finally, a tubular plasma reactor was build up to empower the internal wall of vascular prosthesis to be coated, which enabled to extend this project to the deposition conditions of its final application. Overall, this research project highlighted the potential of plasma processes for the development of degradable plasma polymer matrices, particularly for local drug delivery systems for vascular applications. On a physics perspective, this work emphasized the importance of studying the discharge under actual thin layer deposition conditions.
