An input-output analysis of the economic impacts of chronic wasting disease and bovine spongiform encephalopathy in Alberta and Canada

This thesis utilizes input-output analysis to calculate the economic impacts from potential prion diseases outbreaks in Alberta and Canada. Both chronic wasting disease and bovine spongiform encephalopathy have the capacity to not only affect the farmed cervid and cattle industries, but to impact all industries with direct and indirect links to these sectors. Cervid sector shocks consistently yield small spillover effects on the economy in all models. In contrast, the cattle sector generates larger multiplier effects. A worst-case scenario that reduces cervid sector output to zero yields total economic losses of $11.5 million in Alberta, and $43.7 million in Canada. A reduction of cattle sector output to zero results in total economic losses of $6.4 billion in Alberta, and $34.9 billion in Canada. / Agricultural and Resource Economics

Introdução: A artrite reumatoide é uma doença inflamatória autoimune associada à complicações sistêmicas como fadiga e perda muscular. Perda muscular pode estar relacionada com a ativação do sistema ubiquitina-proteossomo. O objetivo deste trabalho foi avaliar a perda muscular e o evolvimento do proteossomo no modelo de artrite induzida por colágeno (CIA), com ou sem o tratamento de metotrexato ou inibidor de TNF (etanercepte). Métodos: Camundongos DBA1/J machos foram divididos em 4 grupos (n=8 cada): CIA (salina); ETN (etanercepte, 5.5 /) e MTX (metotrexato, 35 /), tratados duas vezes por semana por 6 semanas, e um grupo controle saudável (CO). Tratamentos iniciaram uma semana após a injeção do booster. Escore clínico, edema da pata traseira e peso corporal foram analisados durante o período experimental. Músculo gastrocnêmio (GA) foi pesado após a morte e usado para quantificar a atividade, níveis proteicos e expressão de mRNA das diferentes subunidades do proteossomo através de ensaio fluorogênico, Western blot e rtPCR, respectivamente. Resultados: Tratamentos reduziram o desenvolvimento da doença, observado através do menor escore clínico e edema da pata traseira nos grupos ETN e MTX. ETN apresentou maior peso corporal do que MTX nas semanas 5 e 7. Músculo GA estava aumentado em ETN do que CIA e MTX, um resultado também observado no peso muscular normalizado. As propriedades catalíticas do proteossomo 26S muscular mostraram um aumento na atividade do tipo caspase nos grupos CIA e MTX. Tecidos musculares de animais MTX demonstraram maiores níveis proteicos das subunidades do proteossomo PSMB8 e PSMB9 e maior expressão gênica de Psmb5, Psmb8 e Psmb9. Por outro lado, a expressão de Psmb6 estava diminuída e de Psmb9 estava aumentada em CIA. Conclusões: Apesar de ambos os medicamentos melhorarem o escore da doença, ETN apresentou um afeito anti-artrítico mais forte e foi o único tratamento capaz de prevenir parcialmente a perda muscular. Ao contrário de ETN, CIA e o tratamento com MTX apresentaram perda muscular e atividade e expressão do proteossomo aumentadas. / Background: Rheumatoid arthritis is an autoimmune inflammatory disease associated with systemic complications like fatigue and muscle wasting. Methods: Male DBA1/J mice were divided into 4 groups (n=8 each): CIA (saline); ETN (etanercept, 5.5 /) and MTX (methotrexate, 35 /), treated twice a week for 6 weeks, and a healthy control group (CO). In contrast, expression of Psmb6 was decreased and of Psmb9 was enhanced in CIA. Conclusions: Although both drugs improved the disease score, ETN presented a stronger anti-arthritic effect and was the only treatment able to partially prevent muscle wasting. In contrast to ETN, CIA and MTX treatment did not prevent muscles loss due to increased proteasome expression and activity.

FundaÃÃo de Amparo à Pesquisa do Estado do Cearà / IntroduÃÃo: Apesar do reconhecimento das alteraÃÃes intestinais associadas à desnutriÃÃo e dos efeitos trÃficos da alanil-glutamina (AG) na funÃÃo de barreira intestinal, medida pela taxa de excreÃÃo urinÃria de Lactulose: Manitol, ainda sÃo escassos os estudos para determinar o efeito de suplementaÃÃo oral da AG em crianÃas desnutridas. Objetivos: Examinar o efeito de suplementaÃÃo oral de AG ou placebo glicina (G) na funÃÃo de barreira intestinal e crescimento em crianÃas sob risco nutricional residentes na comunidade do Parque UniversitÃrio do Pici. MÃtodos: Ensaio clÃnico randomizado controlado em crianÃas maiores de 6 meses e menores de 8 anos de idade, com pelo menos um dos escores z para os indicadores antropomÃtricos (IAs) (peso-para-idade, estatura-para-idade e peso-para-estatura) < -1. Cento e sete crianÃas foram randomizadas, entre julho de 2003 a novembro de 2004, para receberem AG (24g/dia) ou G (25g/dia) em quantidades isonitrogÃnicas por 10 dias. A excreÃÃo urinÃria de Lactulose:Manitol foi utilizada como medida da permeabilidade intestinal e realizada nos 1 e 10 dias do protocolo de estudo. O peso e estatura das crianÃas foram coletados nos 1Â, 10Â, 30 e 120 dias do protocolo de estudo para cÃlculo dos IAs. Resultados: O percentual da excreÃÃo urinÃria de lactulose diminuiu significantemente no grupo que recebeu AG, mas nÃo no grupo que recebeu G ( p < 0,05 teste t de Student). A melhora cumulativa nos indicadores antropomÃtricos, peso-para-estatura e peso-para-idade, no 120 dia do protocolo de estudo foi significante quando realizado a anÃlise de covariÃncia no grupo que recebeu AG versus o grupo que recebeu glicina (respectivamente, p <0,04 e <0,029). Durante o perÃodo de estudo foram observados um total de vinte (19%) Eventos Adversos (EAs) na seguinte ordem de freqÃÃncia: vÃmitos (8,4%), diarrÃia (2,8%), nÃuseas (1,9%), infecÃÃo respiratÃria (1,9%), escabiose (1,9%), asma (0,9%), epistaxe (0,9%). Os EAs nÃo foram diferentes na anÃlise estatÃstica entre os grupos estudados e somente trÃs dos EAs foram considerados como SÃrios Eventos Adversos (SEAs ), dois no grupo da alanil-glutamina (1 asma e 1 diarrÃia) e um no grupo da glicina (1 diarrÃia). ConclusÃes: A suplementaÃÃo oral de AG por 10 dias melhorou a permeabilidade intestinal e o estado nutricional em crianÃas sob risco nutricional residentes em uma comunidade de Fortaleza / Introduction: In spite of the recognition of alterations in intestinal barrier function associated with malnutrition and the trophic effects of alanyl-glutamine (AG) on intestinal barrier function, measured by the rate of urinary excretion of Lactulose:Manitol, there are still few studies determining the effect of oral supplementation of AG in malnourished children. Objectives: Examine the effect of oral AG supplementation or the placebo Glycine (G) on intestinal barrier function and growth in children who are at risk for malnutrition and are residents of the University Park of Pici community. Methods: Double blind randomized study in children between 6 months and 8 years of age, with at least one of the z scores for the anthropometrical indicators (AIs) (weight-for-age, height-for-age, and weight-for-height) less than minus one. One hundred and seven children were randomized between July 2003 and November 2004, to receive AG (24 g/day) or G (25 g/day) in iso-nitrogenic quantities for 10 days. The urinary excretion of Lactulose:Mannitol was used as a measure of intestinal permeability, and was performed on the first and tenth days of the study protocol. The weight and height of the children were collected on the 1, 10, 30, and 120 days of the study protocol to calculate the AIs. Results: The percentage of urinary excretion of Lactulose decreased significantly in the group that received AG (p < 0.05 Student T test), but not in the group that received G. The cumulative improvement in the Anthropometrical Indicators, weight-for-height and weight-for-age on the 120 day of the study protocol were significant when covariant analysis was performed in the group which received AG versus the control group G (p < 0.04 and < 0.029, respectively). During the study period a total of 20 (19%) Adverse Events (AEs) were observed in the following order of frequency: vomiting (8.4%), diarrhea (2.8%), nausea (1.9%), respiratory infection (1.9%), scabies (1.9%), asthma (0.9%), epistaxis (0.9%). The AEs were not statistical different between the study groups, and only three of the AEs were considered Serious Adverse Events (SEAs), two in the AG group (1 asthma and 1 diarrhea) and one in the G group (1 diarrhea). Conclusions: The oral supplementation of AG for 10 days improved intestinal permeability and nutritional status in children at risk for malnutrition in a Fortaleza community

Acceptabilité de la récupération nutritionnelle ambulatoire chez les enfants de sept ans et plus infectés par le VIH suivis dans douze sites de prise en charge au Sénégal / Acceptability and feasability of outpatient nutritional rehabilitation among HIV-infected children and adolescents in Senegal

La malnutrition est une pathologie récurrente en Afrique, qui touche particulièrement les enfants et adolescents vivant avec le VIH. Le Sénégal a été l’un des premiers pays africains à mettre en œuvre un programme national d’accès aux antirétroviraux à partir de 1998. Néanmoins, comme dans la majorité des pays, la prise en charge pédiatrique a connu un retard par rapport à celle des adultes. La malnutrition et l’infection à VIH agissent en interaction et aggravent le risque de morbidité et de mortalité des enfants. Pour autant, les recommandations internationales se sont surtout intéressées à la prise en charge de la malnutrition chez les enfants de moins de cinq ans. Il existe peu de directives concernant les modalités de récupération nutritionnelle chez les enfants de plus de cinq ans et les adolescents infectés par le VIH.C’est dans ce contexte qu’a été mise en place l’étude Snac’s qui avait pour objectif d’évaluer l’efficacité et l’acceptabilité de la récupération nutritionnelle ambulatoire par l’administration d’Aliments prêts à l’emploi (APE) chez les enfants et adolescents infectés par le vih dans 12 sites de prise en charge à Dakar et dans les régions du Sénégal. Cette thèse a pour objectif d’évaluer, dans le cadre de cette étude 1/l’acceptabilité d’un dispositif innovant d’information des enfants et des parents pour la participation à la recherche 2/ l’acceptabilité de l’intervention de récupération nutritionnelle chez les enfants et adolescents et d’identifier les facteurs et obstacles à cette acceptabilité 3/ l’acceptabilité de l’intervention chez les soignants impliqués dans l’étude.Trois enquêtes ont été menées au cours du projet Snac’s, dans les deux sites de Dakar et les dix sites régionaux auprès des enfants, des parents/tuteurs et des professionnels de santé. Les entretiens avec les enfants en succès ou en échec de traitement de la malnutrition et avec les parents se sont déroulés par focus group. Ils ont concerné 112 enfants au moment de l’inclusion et 71 enfants au moment de la sortie de l’étude. Des entretiens individuels ont concerné 30 professionnels de santé.Le Dispositif standardisé d’information à la recherche (dsir) avait pour intérêt de standardiser et rendre facilement compréhensible l’information des participants. Il a été apprécié par les enfants/adolescents, et par les parents/tuteurs. 68 % des parents/tuteurs et 58 % des enfants/adolescents, ont répondu correctement à au moins 7/8 questions. La notion qui a été la moins bien comprise par les parents/tuteurs et les enfants/adolescents était le droit de quitter l’étude, avec des taux respectifs de réponses correctes de 54 % et 36 %. L’enquête sur l’acceptabilité à permis d’identifier trois déterminants qui peuvent représenter un obstacle à l’adhésion des enfants/adolescents à une prise en charge nutritionnelle ambulatoire à base d’ape : le dégoût des ape, les effets secondaires et la durée de l’attente avant la consultation. Les entretiens avec les équipes soignantes ont mis en évidence les difficultés rencontrées dans la prise en charge du VIH pédiatrique. Les analyses ont montré une bonne acceptabilité de l’intervention, mais une incertitude quant à la possibilité de la pérenniser à la fin du projet.Ce travail a permis d’expérimenter la mise en place du dsir, qui s’avère intéressant même s’il demande à être amélioré. Il a permis de décrire les difficultés et les enjeux de la prise en charge de l’infection à vih pédiatrique notamment en région, qui constituent l’environnement des interventions de récupération nutritionnelle. Les résultats de cette étude montrent que le dispositif de récupération nutritionnelle ambulatoire est acceptable par les principaux acteurs (enfants/adolescents, parents/tuteurs et équipes soignantes), mais que sa pérennisation n’est envisageable qu’avec l’appui et l’engagement des autorités sanitaires, la mise en place d’un approvisionnement régulier en ape et d’un accompagnement, notamment financier, adéquat. / Malnutrition is a recurrent disease in Africa, particularly affecting children and adolescents living with HIV. Senegal was one of the first African countries to implement a national program for access to antiretrovirals (ISAARV) from 1997. Nevertheless, as in most countries, pediatric in relation to that of adults. Malnutrition and HIV infection interact and increase the risk of child morbidity and mortality. However, international recommendations have focused on the management of malnutrition among children under five years of age. There are few guidelines for nutritional recovery in children over five years of age and adolescents infected with HIV.It was in this context that the Snac's study was set up to evaluate the effectiveness and acceptability of ambulatory nutrition recovery through the administration of Ready-to-Use Foods ) in HIV-infected children and adolescents in 12 treatment sites in Dakar and Senegal. The objective of this thesis is to evaluate, within the framework of this study 1 / the acceptability of an innovative information system for children and parents for participation in research 2 / the acceptability of the recovery intervention nutritional status in children and adolescents and to identify factors and barriers to acceptability 3 / the acceptability of the intervention among the caregivers involved in the study.Three surveys were conducted during the Snac's project at the two sites in Dakar and the ten regional sites for children, parents / caregivers and health professionals. Interviews with children on successful or unsuccessful treatment of malnutrition and with parents were conducted by focus group. They included 112 children at the time of inclusion and 71 children at the time of study exit. Individual interviews were held with 30 healthcare professionals. The quantitative data were processed with SAS and qualitative data using the Dedoose ™ software.Results: The Standardized Information System for Research (dsir) had the advantage of standardizing and making participants' information easily understandable. It was enjoyed by children / teenagers, and by parents / guardians. 68% of parents / guardians and 58% of children / adolescents, correctly answered at least 7/8 questions. The notion that was least well understood by parents / guardians and children / adolescents was the right to leave the study, with respective rates of correct answers of 54% and 36% respectively. The Acceptability Survey identified three determinants that may represent an obstacle to adherence of children / adolescents to ambulatory ape-based nutritional management: ape disgust, adverse effects, and the duration of the waiting period before the consultation. Interviews with healthcare teams highlighted the difficulties encountered in the management of pediatric HIV. The analyzes showed a good acceptability of the intervention, but an uncertainty as to the possibility of perpetuating it at the end of the project.This work allowed us to experiment with the implementation of the desire, which is interesting even if it needs to be improved. It made it possible to describe the difficulties and the stakes of the management of the pediatric hiv infection in particular in the region, which constitute the environment of the interventions of nutritional recovery. The results of this study show that the ambulatory nutritional recovery device is acceptable to the main actors (children / adolescents, parents / caregivers and healthcare teams), but that its sustainability is only possible with the support and commitment health authorities, the establishment of a regular supply in ape and adequate financial and financial support.

Household food security and the anthropometric status of children under five: evidence from the Kenya integrated household budget survey (2005/2006)

Magister Economicae - MEcon / Kenya has continued to record decreasing child mortality rates in recent years, with available data showing that the under-five mortality rate was 85 per 1000 live births in 2010, down from 117 in 1997 (World Bank, 2012). However, the country continues to battle with poor anthropometric status of children (stunting, wasting and underweight). The country also faces high incidences of food insecurity. It is estimated that one third of the country’s population is food and nutrition insecure, with about 10 million of Kenyans suffering from chronic food insecurity (ROK, 2011). The worst affected are children, who are deprived of sufficient nutrients required for proper growth and development. This study seeks to analyse the relationship between household food security and the anthropometric status of children. Specific objectives include identifying the prevalence and predictors of poor anthropometry, identifying the extent of food insecurity in the country and investigating the link between food security and the anthropometric status of children. The research uses data from the Kenya Integrated Household Budget Survey (2005/2006). The survey which was carried out for a period of 12 months covered the entire country and collected data on the demographic and socio-economic characteristics of the households. It also collected data on child anthropometric measurements and households’ food consumption patterns. The statistical software package STATA SE v.12 is used to run ordinary linear (OLS) and logistical regressions in order to analyse the relationship between household food security and the anthropometric status of children. Results show that the prevalence of malnutrition is high in the country with stunting coming out as the main form of malnutrition. Dietary diversity, a measure of food security, is found to be highest in the urban areas. Results from the regression analysis show that a Household Dietary Diversity Score (HDDS) is positively associated with better anthropometry, with the prevalence of malnutrition decreasing with an increase in the score. The study also found that other risk factors associated with poor anthropometry are age of the child, gender, area of residence, diarrhoea, education, household size and income. The paper concludes with suggestions on measures that need to be put in place to curb child malnutrition.

Also there is an introduction of the company and its production portfolio. Crucial part of this thesis is it‘s practical part focused on improving of current situation on the production line with an economical and technical assessment. The main goal was to create more compact and smaller area of product line with lower work in progress.
