Marketization in Swedish Eldercare : Implications for Users, Professionals, and the State

During the last decades, Swedish policy makers have implemented various marketization reforms into the public welfare sector in order to make it more cost-efficient and to improve its quality. The aim of this dissertation is to investigate what implications this marketization trend has had for the organization of Swedish eldercare. In particular, the research question addressed is how marketization reforms such as privatized provision, increased competition, and user choice have transformed the relationship between the service users, the professionals, and the state. To answer the research question, four articles are presented in the dissertation, each corresponding to a separate empirical investigation. Together, the articles demonstrate that the increased reliance on marketization in Swedish eldercare has made it more difficult for the local authorities to directly control the quality of the services, since it reduces their ability to allocate public resources and expects them to govern the provision of eldercare through the entering of contracts. This development has also implied that service users themselves become increasingly responsible for ensuring that the quality of their care is high. Moreover, the articles show that the increased reliance on audit by the national government and its agencies has tended to undermine the professionalization of eldercare staff, thereby limiting their autonomy and ability to ensure service quality. As a whole, the dissertation contributes with a more comprehensive understanding of how marketization has altered the organization of Swedish eldercare and under what conditions it might undermine the goals of social equality and ensuring that all citizens have equal access to good quality care.

Informal carers' attitudes to pensions and retirement savings

Concerns about population ageing have been central to reforms of pensions policy and social care policy in the UK over recent decades. However, policy responses to these social problems are in tension, as individuals have been expected to take greater responsibility for both pension saving and provision of care, even when care involves a reduction in earnings and savings. While the pension system protects carers’ state pensions, little attention has been paid to their non-state pensions. This thesis therefore explores the attitudes and beliefs carers hold regarding pensions, and the effect of caring on pensions planning, particularly planning a non-state pension. It focuses on the assumptions made about agency and decision-making contained within social policy. Semi-structured interviews were held with those aged 35-64 who were providing 20 hours of care or more a week in the Thames Valley and Greater London. Almost all of the carers reported disruption to their private pension savings as their employment was impacted by caring. Few felt able to make a choice regarding either caring or employment. Caring also affected the importance many attached to saving, although there was no uniform effect on decision-making. Some carers became more reflexive, attempting to take more control in response to the uncertainty in their lives; others felt powerless due to this uncertainty and limited financial resources. The study provided support for the theory of an ethic of care, which suggests individuals make decisions based on relationships rather than calculations of expected outcomes. The findings challenge the assumptions made in pensions policy. Carers were classed in four categories of approach to pensions savings: reflexive planners, non-reflexive planners, reflexive non-planners and non-reflexive non-planners. A range of financial and social resources corresponding to Bourdieu’s habitus and economic and cultural capital was required for carers to act as reflexive planners.

The CLASS act and long-term care policy : the politics of long-term care financing reform in the United States

This thesis seeks to contribute to the knowledge base about social policy in the United States, using long-term care (LTC) financing policy reform as an illustrative example. Specifically, this thesis explores LTC financing reform efforts during three U.S. Presidential administrations: Bill Clinton (1993-2001), George W. Bush (2001-2009), and Barack Obama (2009-2010). Within this historical framework, the LTC provisions of the Health Security Act of 1993, the development of the Community Living Assistant Services and Supports or 'CLASS' Act during the Bush Administration, and the legislative success of the CLASS Act as a part of the Patient Protection and Affordable Care Act of 2010 provide comparable cases to compare the drivers of social policy. Drawing on the explanatory frameworks of the welfare state such as ideology, historical institutionalism, and an actor-centered approach to policy analysis, this thesis argues that successful path-departing legislation is difficult to achieve due, in part, to the presumed high costs of social programs and the complex institutional framework of the American political system. Policy outcomes result from the interaction between the complex processes and dynamics of the political system through which policy change (or the failure to change) actually occurs. The fact that the CLASS Act was politically successful, yet administratively inoperable as designed, reinforces the argument that social policy outcomes in the United States are reflective of a complex, enduring struggle of competing ideologies. This continual struggle, coupled with a heightened concern over cost control and fiscal austerity, helps to ensure that policies which are legislatively successful within the institutional architecture of the American political system are unlikely to produce major expansions of the welfare state. Social change is therefore highly difficult to achieve, even in the face of significant unmet social needs. Comprehensive reform of U.S. LTC financing arrangements will remain an elusive goal for the foreseeable future. Instead, incremental, highly pro-market solutions are likely to be the types of policies promoted in the years of ahead.

Vers une politique de conciliation travail- famille au Québec : des enjeux complexes et en évolution

Au cours des dernières décennies, le thème de la conciliation travail-famille s’est taillé une place importante dans le discours populaire, médiatique et politique tant au Québec, dans les pays industrialisés que dans les organismes internationaux. L’expression désigne les défis que posent pour les individus, les couples, les familles, les milieux de travail et la société en général la relation nouvelle qui s’est développée entre ces deux sphères de vie avec le passage d’une société industrielle à une société dite postindustrielle. Ces défis perçus, ressentis, identifiés et définis différemment par l’un ou l’autre de ces acteurs se sont traduits par l’inscription de cette question à l’agenda politique des gouvernements, ici comme ailleurs. L’objet de la recherche est de comprendre les dynamiques entourant le développement des actions de l’État québécois sur le thème de la conciliation travail-famille. La recherche s’intéresse aux acteurs (Intérêts) qui ont participé aux processus de développement des mesures de conciliation travail-famille et au contexte Institutionnel qui encadre leurs interactions avec les décideurs. À ce titre, la recherche permet de vérifier si le développement de la politique familiale québécoise peut être situé dans ce qui a été désigné comme le « modèle québécois de développement ». La variable « Idée » complète le cadre théorique de l’analyse du développement de la politique de conciliation travail-famille au Québec selon l’approche dite « des 3 I ». / In recent decades, the topic of balancing work and family has earned an important place in popular discourse, media and politics in Québec, as well as in industrialized countries and in international organizations. The term refers to the challenges for individuals, couples, families, workplaces and society in general, and the new relationship that has developed between these two spheres of life with the transition from an industrial to a post-industrial society. These perceived challenges, identified and defined differently by each of these players have resulted in the inclusion of this issue on the political agenda of governments, both here and abroad. The purpose of this research is to understand the dynamics surrounding the development of actions by the Québec government on the issue of reconciling work and family. The research focuses on the actors (Interests) who participated in the process of developing measures to reconcile work and family and the Institutional environment in which they interact with policy makers. As such, research to verify whether the development of Québec's family policy can be located in what was designated the "Québec model of development." The variable "Idea" completes the theoretical analysis of the development policy of balancing work and family in Québec.

Trygghet som handelsvara : Privat folkförsäkring i det framväxande välfärdssamhället 1900–1950 / The Business of Welfare : Industrial Life Insurance and the Emerging Swedish Welfare State 1900–1950

Industrial Life Insurance (ILI) was introduced in Sweden in the beginning of the 1900s. Following models already used in the United Kingdom and the United States, this insurance was specifically aimed at manual labourers, promising pension savings and compensation to surviving relatives. The insurance was an immediate success, with almost three million insurance policies in force by the mid-1900s. ILI was characterised by extensive and carefully monitored marketing practices. By managing an army of agents, the companies sold policies and collected premiums on a regular basis in the homes of the insured. The purpose of the dissertation is to analyse the development of a commercial business with social policy aspirations, and how it interacted with other social security institutions. How could ILI thrive in the emerging Swedish welfare state that, according to existing research, allowed little space for market-based welfare alternatives? The dissertation also seeks to contribute to a broader understanding of the contemporary “welfare market” in Sweden today. From a perspective of welfare formation as a social process, the emergence and expansion of ILI is interpreted as a phenomenon that has shaped, and been shaped by, the social policy arena. The insurance industry’s capacity to adapt to the changing ambitions of the state in this arena is emphasised. Furthermore, its leading representatives’ ability to continuously locate the role of life insurance in the shifting landscape of social policy is underlined. By locating welfare in separate but complementary public and private spheres, the industry contributed to the shaping of the compulsory pension scheme introduced in 1913 and the overall regulation of insurance in the mid-twentieth century. The social security of Swedish citizens was now to be ensured according to the vision of complementary spheres that the insurance industry had advocated for almost fifty years. The insurance companies’ commercial activities are analysed as a form of governmentality, where the agency system is scrutinized as an interventionist practice that created a long lasting relationship between the companies and the working classes. The dissertation shows how the industrialists’ role as “insurance experts” was used to influence public policies. As public figures and experts on various committees, representatives of the industry advocated a welfare formation that left ample space for their own business interests. The scientisation of security was also essential in creating a product where social aspirations and commercial logics could be united. The success of ILI thus rested on the interaction with the state apparatus. An arena of social policy was established where commercial companies were to be the supplier of all welfare above the level of “meagre basic security”. Through intense marketing measures, commercial actors influenced the perceptions of security and welfare. The process of welfare formation led to the internalisation of commercial ideals about social security that now constitutes an essential dimension of the Swedish welfare state.

Analyse comparative entre la famille de l’habitant de Saint-Justin de Léon Gérin (1886) et une famille rurale québécoise d’aujourd’hui (2010) selon la perspective des temps sociaux

L’agent régulateur des temps sociaux de la famille québécoise a été la religion catholique pendant des décennies. Les mouvements d’après-guerre et la Révolution tranquille, ont amené un changement de paradigme avec la laïcisation des institutions et la multiplication des programmes sociaux de l’État-providence. Pour cerner l’importance de ce changement, nous avons utilisé la clef analytique de Léon Gérin pour produire une analyse comparative entre deux familles à deux époques : en 1886 et en 2010. Or, au moment où l’État est de plus en plus présent dans toutes les sphères de la vie de la famille tout en leur laissant une autonomie et une liberté, la population se montre de plus en plus sceptique envers les institutions étatiques. Un fossé s’est créé entre la société officielle, celle du social, des institutions, et la société officieuse, celle de la culture populaire qui agit à partir de l’esprit de corps de son groupe, virtuel ou non. La crise de confiance des gens envers les institutions annonce un nouveau changement de paradigme. La rationalité qui a été le moteur de la modernité fait graduellement place à l’émotionnel et à la valeur du temps présent. Le travail, agent régulateur qui comblait les besoins matériels à la modernité, serait remplacé progressivement par des besoins immatériels devenant l’agent régulateur des temps sociaux de la famille. / During decades, the regulating agent of social times of the Quebec family was the catholic religion. The post-war the Révolution tranquille, brought a change of paradigm with the secularization of institutions and the multiplication of the social programs under the Welfare State. To investigate the importance of this change of paradigm, we used Léon Gérin's method of clef analytique [analytic key] to produce a comparative analysis between two families and two periods: in 1886 and in 2010. Today, as the Welfare state is more and more present in all the family’s spheres of the life while leaving autonomy and freedom the population shows itself more and more sceptical to the state institutions. A great distance appears between the official society, that of the social, institutions, and the unofficial society, that of the popular culture who acts from the esprit de corps of its group, virtual or not. The crisis towards institutions at present announces another change of paradigm. Rationality, which was the mainspring of the modernity, gradually gives way to the emotional and to the value of the present time. Work time, the regulating agent who filled the material needs in the modernity would be gradually replaced by immaterial social needs as regulating agent of social times.

Social Policy Reforms in Turkey : Uses of Europe

Ce mémoire analyse trois réformes majeures de politique sociale en Turquie, en deux domaines: emploi et sécurité social. En utilisant l'approche "Usage de l'Europe", ce mémoire developpe une analyse empirique et apporte une explication théorique de ces changements qui ont été introduits au cours du processus d'adhésion de la Turquie à l'Union européenne. "Les usages de l'Europe" est une approche d'européanisation qui se concentre sur le rôle des acteurs domestiques, au sein des États membres et candidats, ainsi que de leur utilisation des ressources de l'Union européenne. Les études de cas utilisées dans cette thèse démontrent l'introduction de changements au niveau de l'État-providence; ainsi, l'approche originelle est suppléée par des concepts provenant de la littérature sur la politique partisane, les institutions formelles et l'héritage des politiques. Cette recherche utilise la méthode de l'analyse de processus pour suivre la réforme des règlements du travail par la voie de reconstitution des droits individuels des travailleurs et de l'Agence d'emploi en Turquie jusqu'en 2003, ainsi que la transformation du système de sécurité sociale en 2008. Ces trois réformes représentent des changements majeurs tant sur le plan institutionnel que politique en Turquie depuis 2001. Afin de comprendre "les usages de l'Europe" dans ces réformes politiques, l'analyse empirique questionne, si, quand et comment les acteurs turcs ont utilisé les ressources, les références et les développements politiques de l'Union européenne lors de ce processus dynamique de réforme. Les réformes du système de sécurité sociale, des règlements du travail, en plus de la reconstitution de l'Agence d'emploi étaient à l'ordre du jour en Turquie depuis les années 1990. La réforme des règlements du travail ont entraîné l'introduction des accommodements flexibles au travail et une révision de la Loi du travail permettant l'établissement d'une législation de la sécurité d'emploi. La reconstitution de l'Agence d'emploi visait à remplacer la vieille institution défunte par une institution moderne afin d'introduire des politiques d'activation. La réforme de sécurité sociale comprend les pensions de retraite, le système de santé ainsi que l'administration des institutions de sécurité sociale. Les principaux résultats révèlent que la provision des ressources de l'Union européenne en Turquie a augmenté à partir de la reconnaissance de sa candidature en 1999 et ce, jusqu'au lancement des négociations pour son adhésion en 2005; ce qui fut une occasion favorable pour les acteurs domestiques impliqués dans les processus de réformes. Cependant, à l'encontre de certaines attentes originelles de l'approche de "les usages de l'Europe", les résultats de cette recherche démontrent que le temps et le sort de "les usages de l'Europe" dépendent des intérêts des acteurs domestiques, ainsi de leurs stratégies tout au long de ce processus de réforme, plutôt que des phases du processus ou la quantité des ressources fournies par l'Union européenne. / This dissertation analyses three major social policy reforms in Turkey in two policy domains: employment and social security. By adopting the Uses of Europe theoretical approach, it aims to analyze empirically and to explain theoretically the uses of Europe in two domains of social policy during the EU membership process in Turkey. Uses of Europe is an actor-centered approach to Europeanization that focuses on the role of national actors, in member and candidate states, and their use of EU resources. The case studies in this thesis involve welfare state changes. Thus the original approach is complemented by concepts from the welfare state literature on formal institutions, partisan politics and policy legacies. This research uses a process-tracing methodology to follow the reform of labor regulations via the restructuring of individual labor rights, restructuring of the Turkish employment agency up through 2003 and the transformation of the social security system by 2008. Both represent major institutional and policy changes in the post-2001 period in Turkey. In order to understand the uses Europe in these policy reforms, the empirical analysis asks whether, where, and how Turkish actors were using EU resources, references and policy developments within the dynamic processes of reform. The reforms of the social security system, labor regulation and the restructuring of the employment agency have been on the agenda in Turkey since the mid-1990’s. The reform of labor regulations involved the introduction of flexible work arrangements and job security legislation into a revised Labor Act. The restructuring of the employment agency aimed to replace the old institution that had become defunct with a modern institution oriented towards active labor market policies. The social security reform comprising pension, healthcare and administrative components aimed to ensure financial sustainability and increase the coverage of the system. The main findings were that the supply of EU resources in Turkey increased from the recognition of its candidate status in 1999 to the launch of accession negotiations in 2005. This supply offered opportunities for national actors involved in the reform processes, via legitimizing uses of Europe, obfuscation and credit claiming, among other practices. However in contrast to some of the expectations of the original Uses of Europe approach, the findings of this research demonstrate that the type and timing of uses of Europe depend on the national actors’ interests and coalition-building strategies in the reform process rather than on the stage of the reform process or amount of resources supplied by the European Union.

Sex and the party : gender policy, gender culture, and political participation in unified Germany

This thesis explores the relationship between gender policy, gender culture, and political participation in unified Germany. It investigates the extent to which political regimes shape citizens' attitudes towards gender roles and examines the effect of such attitudes on women's participation in politics. The thesis is divided into three parts: The first part explores the differences in gender regime types between the former German Democratic Republic and Federal Republic of Germany during the Cold War period. Building on existing studies, the analysis considers how generations that were socialised in the divided Germany differ in their attitudes toward gender roles. It finds that citizens from West Germany are more socially conservative than citizens from the East. The second part of the thesis tests the effects of these traditional gender attitudes on citizens' participation, focusing on party membership. The analysis highlights that gender gaps in formal political participation in unified Germany still exist, but that these gaps are smaller in the new federal states. The investigation further shows that traditional gender attitudes exert a negative effect on women’s political engagement beyond the predictive power of socio-economic and demographic factors. The final part of this thesis casts a critical look at the political controversy in Germany over the introduction of a cash-for-care subsidy (the so-called Betreuungsgeld). It explores the normative assumptions and ideas about gender roles that have been promoted by Germany's main political parties throughout the policy negotiation process. Using a combination of qualitative and quantitative methods, the research presented in this thesis draws on, and contributes to, studies on gender, welfare states, political socialisation, and political participation.

Welfare state v rozvojových zemích: případová studie Botswany, Ghany a Indie / Welfare state in developing countries: Case study of Botswana, Ghana and India

This diploma thesis explores the emergence of the welfare state in developing countries, in particular shown on the example of Botswana, Ghana and India. The inquiry is focused on the period beginning in 1990, when the neoliberal paradigm was dominating, untill 2010. The recent years are in token of the shift from the neoliberalism to the post-neoliberalism characterized by a number of concepts, which are taking into account. The common feature of these concepts is the diversion from the narrow focus on GDP, as the only indicator of the growth, to the social dimension of the development. This shift is also being distinguished as a transition from the basic-needs concept to the rights-based approach. Therefore, the thesis explores, if these shifts are remarkable in the analyzed countries, eventually, if there are any divergences as compared to the theoretical concepts. To achieve this goal, a broader analysis of the welfare state was used, which involves social, health and education policy. Based on this analysis, the diploma thesis tries to classify the analyzed countries into the welfare state typology; eventually, in case such classification is impossible, it describes the weaknesses of this welfare state the typology. The Knowledgeable Parent : Ideologies of Communication in Swedish Health Discourse / Den kunniga föräldern : Kommunikationsideologier i svensk hälsodiskurs

This thesis explores the communication of health knowledge among new parents in Sweden. Based on three separate studies, the thesis employs a selection of theoretical concepts and methodological approaches, mainly originating from mediated discourse analysis and linguistic anthropology. Study 1 takes a broad view on the object and asks how knowledge circulates and emerges in a particular arena for parental knowledge. Drawing on nine months of online fieldwork on a discussion forum thread for expectant parents, the study shows that communication of knowledge is engendered by entextualizations and recenterings of previous experiences, including encounters with discourse. This fact challenges categorical conceptions that construct some sources of health knowledge as trustworthy and others as unreliable, and thus, potentially harmful. Study 2 narrows the focus to professionals typically perceived as producers of parental health knowledge, namely, midwives who give prenatal education classes. Drawing on a dataset comprising observations of classes as well as interviews with midwives, the study throws analytical light on anticipatory discourse, that is, discourse designed to dictate and influence the future, and elucidates some of the ways in which midwives prepare the participants for their upcoming delivery by discursively constructing links to these future events. Study 3, finally, takes the perspective of a single individual in whose life several forms of communicated parental knowledge converge as she becomes a mother. The study focuses on a period during which this individual struggles with breastfeeding problems. A combination of the notions of interdiscursivity and the historical body is here employed to grasp this experience as shaped in relation to discourse regarding child care and health. Looking at narrative data through this lens, the study shows how this individual connects failure to follow official breastfeeding recommendations to failure to perform child care in an appropriate way. At heart, the study makes a case for the moral loading of health knowledge and cautions against the assumption that authoritative medical knowledge is the only means for taking action that a new mother might need. In conclusion, the present thesis utilizes a combination of theoretical and methodological tools from MDA and linguistic anthropology to enable a discourse analysis of health communication that privileges a view of language in use as accumulating vis-à-vis engendering meaning over time and in relation to social action. Invoking the notion of ideologies of communication, it demonstrates that parents’ knowledge about their children’s health is a non-neutral issue, and that instrumental aspects of parental health knowledge can never be isolated from moral ideas regarding how particular parenting practices are to be carried out. At the same time, the thesis points out that while representatives of institutions of the welfare state may produce messages to communicate health knowledge, the knowledge obtained by individuals is the product of myriad discursive encounters and other experiences, of which the discourse produced by representatives of state institutions constitutes only one share. / <p>At the time of the doctoral defense, the following papers were unpublished and had a status as follows: Paper 2: Submitted.
