The Efficacy of a Novel Silver-Containing Bioresorbable Microfilm Matrix in At-Risk Surgical Wounds: A Clinical Case Series

INTRODUCTION: For persons with diabetes, surgery is fraught with complications; of primary concern is postoperative infection. A postoperative infection rate of up to 13% has been noted in patients with diabetes undergoing elective surgical procedures compared with less than 3% in nondiabetic populations. OBJECTIVE: The objective of this study was to provide preliminary evaluation of the efficacy of a novel bioresorbable microfilm matrix (20 µm thick) containing very low amounts of silver (0.16 mg/in²) in preventing surgical site infections when placed at the level of subcutaneous tissue and dermis prior to primary closure in the patient with diabetes undergoing elective surgery. MATERIALS AND METHODS: Twenty-two patients with diabetes undergoing nonemergent or elective foot or ankle surgery and who met at least 1 of the following 6 criteria were included in the study: neuropathy, infection, open wound, history of recurrent infection, nonhealing wound, or peripheral vascular disease. Patients underwent amputation, removal of exostosis, midfoot bone removal, Achilles tendon repair, bunionectomy, or an elevating osteotomy with primary closure of the wound. After hemostasis was obtained and subcutaneous closure achieved, if applicable, the bioresorbable microfilm matrix was applied just deep to the incision at the level of subcutaneous tissue and dermis, and the incision primarily closed. A nonadherent cover dressing was applied over the suture line, and routine follow-up was scheduled for 3 to 5 days later. RESULTS: No patient exhibited signs of infection at initial follow-up, and all adherent patients achieved complete healing during the 3-month follow-up period. Eighteen patients healed at a rate typical for the respective procedure. In 2 patients, time to healing was delayed secondary to weight-bearing dehiscence. Two patients were not included in the results secondary to multiple infractions of nonadherence with the postoperative protocol. CONCLUSIONS: The application of microfilm matrix in surgical incisions at the level of subcutaneous tissue and dermis prior to primary closure is safe for and has the potential to prevent postoperative surgical site infections in at-risk patients with diabetes..

Estudo comparativo entre as osteossínteses de tornozelo com implantes convencionais e bioabsorvíveis / Comparative study of osteosynthesis of ankle with conventional and bioabsorbable implants

O padrão ouro no tratamento cirúrgico das fraturas de tornozelo são as sínteses metálicas, sejam de titânio ou aço. Os implantes bioabsorvíveis foram desenvolvidos para evitar o efeito do stress shielding, ou escudo de carga, e a retirada de material de síntese. O uso de materiais absorvíveis é bem documentado, e com bons resultados em grande número de fraturas craniofaciais. O objetivo deste estudo foi comparar os resultados funcionais de fraturas de tornozelo tratadas com placas metálicas e absorvíveis. Os pacientes foram randomizados e seguidos de forma prospectiva em dois grupos, metálico e absorvível. No período pós operatório imediato os pacientes foram imobilizados com tala gessada por uma semana, substituída por órtese removível por mais 4 semanas. Carga parcial foi autorizada com 3 semanas, e carga total com 6 semanas. Dezenove pacientes foram seguidos por nove meses, e avaliados radiográfica e funcionalmente aos 6 e 9 meses. Todos consolidaram entre 8 e 12 semanas. A recuperação funcional foi similar nos dois grupos. Aos 6 meses, três pacientes no grupo metálico queixaram-se de problemas locais, e tiveram seus implantes removidos. Um paciente do grupo absorvível teve uma deiscência da sutura, tratado com boa evolução. Aos 9 meses foi aplicado o escore da AOFAS para todos os pacientes. Os resultados funcionais foram semelhantes nos dois grupos, após a retirada de implante de três pacientes no grupo metálico. Os implantes absorvíveis permitiram resultados clínicos e funcionais semelhantes aos metálicos em fraturas de tornozelo / The current gold standard fixation system for surgical ankle fractures are the metallic implants, steel or titanium made. Resorbable implants have been developed to avoid stress shielding and a new surgery for implant removal, common with metal implants. Use of resorbable implants is well documented in the literature for a range of craniofacial fractures, with good results. The purpose of this study was to compare the outcome of ankle fractures using biodegradable and metal plates. Patients were randomly and prospectively treated with resorbable or metal fixation system. Post-operatively, the ankle was immobilized with a plaster cast for 1 week, removable cast for other 4 weeks. Half and full weight bearing were allowed at weeks 3 and 6. nineteen patients were followed for 9 months. Functional recovery was similar in both groups at 6 and 9 months, three patients in metal group complained implant-related problems, and had their implants removed, and one in resorbable had an acute dehiscence, solved with surgical debridement and closure. At 6 and 9 months, the AOFAS score was applied for all patients. The functional results were similar in both groups. Implant removal was necessary in three patients from the metallic group. The resorbable plate provided qualitatively similar fracture healing results as the metal plate

Foram criados defeitos cirúrgicos nos 3os pré-molares inferiores (P3) de cinco cães, de ambos os lados, totalizando 10 dentes. Após um período de três meses de cronificação, as lesões foram divididas aleatoriamente em dois grupos de tratamento. (1) Grupo teste: PRP + enxerto ósseo autógeno + membrana; (2) Grupo controle: enxerto ósseo autógeno + membrana. Após um período de cicatrização de 90 dias, os animais foram sacrificados. A análise histológica, a partir da marcação, mostrou formação de novo cemento celular, mínima quantidade de novo osso e ligamento periodontal, em ambos os grupos. Nenhuma diferença estatisticamente significante foi observada entre os grupos teste e controle. A associação do PRP ao enxerto ósseo autógeno e membrana, ou somente, do enxerto ósseo autógeno e membrana, não apresentou vantagens estatisticamente significantes no tratamento das lesões, resultando em preenchimento parcial das furcas e regeneração periodontal limitada à marca experimental das lesões. / Abstract: The objective of this study was to evaluate histomorphometrically the platelet-rich plasma (PRP) effects, associated with autogenous bone graft and absorbable membrane (Resolut®), for the treatment of Class III furcation defects, in 5 dogs. Surgical defects were created in the 3nd mandibular premolars (P3) on both sides, of each dog, totaling 10 teeth. After a period of three months of chronified, the defects were randomly divided in two treatment groups. (1) Group Tests: PRP + graft bone autogenous + membrane; (2) Group Control: graft bone autogenous + membrane. Animals were sacrificed after 90 days healing. Histologic evaluation revealed new cellular cementum, low amount of bone and periodontal ligament formation in both groups, starting from the demarcation. No statistically significant difference was observed between Groups Test and Control. PRP + autogenous bone graft + membrane or autogenous bone graft + membrane no present statistically significant advantages on the lesions treatment, resulting in furcation partial filling and periodontal regeneration limited to the experimental mark of the lesions. / Doutor

An Investigation on Biocompatibility of Bio-Absorbable Polymer Coated Magnesium Alloys

Advances in biomaterials have enabled medical practitioners to replace diseased body parts or to assist in the healing process. In situations where a permanent biomaterial implant is used for a temporary application, additional surgeries are required to remove these implants once the healing process is complete, which increases medical costs and patient morbidity. Bio-absorbable materials dissolve and are metabolized by the body after the healing process is complete thereby negating additional surgeries for removal of implants. Magnesium alloys as novel bio-absorbable biomaterials, have attracted great attention recently because of their good mechanical properties, biocompatibility and corrosion rate in physiological environments. However, usage of Mg as biodegradable implant has been limited by its poor corrosion resistance in the physiological solutions. An optimal biodegradable implant must initially have slow degradation to ensure total mechanical integrity then degrade over time as the tissue heals. The current research focuses on surface modification of Mg alloy (MZC) by surface treatment and polymer coating in an effort to enhance the corrosion rate and biocompatibility. It is envisaged that the results obtained from this investigation would provide the academic community with insights for the utilization of bio-absorbable implants particularly for patients suffering from atherosclerosis. The alloying elements used in this study are zinc and calcium both of which are essential minerals in the human metabolic and healing processes. A hydrophobic biodegradable co-polymer, polyglycolic-co-caprolactone (PGCL), was used to coat the surface treated MZC to retard the initial degradation rate. Two surface treatments were selected: (a) acid etching and (b) anodization to produce different surface morphologies, roughness, surface energy, chemistry and hydrophobicity that are pivotal for PGCL adhesion onto the MZC. Additionally, analyses of biodegradation, biocompatibility, and mechanical integrity were performed in order to investigate the optimum surface modification process, suitable for biomaterial implants. The study concluded that anodization created better adhesion between the MZC and PGCL coating. Furthermore, PGCL coated anodized MZC exhibited lower corrosion rate, good mechanical integrity, and better biocompatibility as compared with acid etched.

[pt] Membranas de politetrafluoretileno (PTFE) são empregadas na odontologia como barreiras biocompatíveis para impedir o crescimento de células impróprias para a regeneração óssea guiada. Uma das membranas comerciais mais usadas mundialmente é a Gore-Tex, desenvolvida e comercializada desde a década de oitenta. Membranas similares têm surgido nos últimos anos, desde o término do período de proteção da patente da membrana Gore-Tex. No entanto, não existe ainda membrana com propriedades similares fabricadas no Brasil. A pesquisa proposta objetivou desenvolver uma membrana de PTFE com características semelhantes às da membrana Gore-Tex, em particular, a morfologia superficial e a presença de porosidade. A fabricação da membrana depende do uso de uma fita que pode ser obtida por extrusão de partículas de PTFE. A fita é tracionada em duas direções distintas, uma no sentido original das fibras do PTFE e outra perpendicular, sob condições térmicas apropriadas. A morfologia final da membrana resulta da superposição e união de duas fitas processadas. O material inicialmente foi analisado calorimetria diferencial de varredura. A morfologia das membranas foi analisada por microscopia eletrônica de varredura e a porosidade estimada por processamento digital de imagens, empregando o programa KS400. Foi possível estabelecer um procedimento de fabricação envolvendo tracionamento e tratamento térmico capazes de fornecer membranas com morfologia e porosidade semelhantes às das membranas comercializadas pela W.L. Gore and Associates. / [en] Polytetrafluoroethylene (PTFE) Membranes are used in dentistry as biocompatible barriers to prevent undesired cells growth for guided bone regeneration (GBR). One of the most worldwide used commercial membrane is Gore-Tex. This membrane was developed and marketed since the eighties decade of the last century. Similar membranes have emerged in recent years, since the end of the patent protection period of the Gore-Tex. However, there is not any membrane with similar properties manufactured in Brazil. The proposed research aimed to develop a PTFE membrane with similar characteristics to the Gore-Tex membrane, in particular, the surface morphology and the porosity presence. The manufacturer of the membrane uses a tape that can be obtained by extrusion of PTFE particles. The tape is pulled in two different directions; one in the original fiber is direction and the other in a perpendicular, direction under appropriate thermal conditions. The final membrane morphology resulted from the overlapping and union of two processed tapes. The starting material was analyzed by differential scanning calorimetric. The morphology of the membranes was analyzed by scanning electron microscopy and the porosity estimated by digital image processing, using the KS400 program. It was possible to establish a procedure involving pulling, tractioning and heat treatment that provides membranes with similar morphology and porosity of the PTFE-e membranes marketed by W.L. Gore and Associates.

In-vitro degradation of calcium phosphate bone substitutes : Coupled monitoring of the evolution of mechanical, microstructural and physico-chemical properties of DCPD and β-TCP samples / Dégradation in-vitro de substituts osseux à base de phosphates de calcium : Suivi couplé de l’évolution des propriétés mécaniques, microstructurales et physico-chimiques d’échantillons de DCPD et β-TCP

Ce travail de thèse a eu comme objectif la mise en place et la validation d’une méthodologie expérimentale pour le suivi de l’évolution in-vitro de substituts osseux à base de phosphates de calcium. Du phosphate dicalcique dihydraté (DCPD, CaHPO4·2H2O) et du phosphate tricalcique bêta (β-TCP, β-Ca3(PO4)2) ont été choisis comme matériaux modèles de deux grandes classes de substituts osseux: les “biosolubles” (sujets à dissolution après implantation) et les “biorésorbables” (sujets à résorption cellulaire après implantation). Pour l’étude des phénomènes de dissolution et de reprécipitation observés lorsque les phosphates de calcium sont plongés en solution, ces matériaux ont été produits sous forme d’échantillons microporeux (60% de porosité pour le DCPD, 75% pour le β-TCP) et soumis à des tests de dissolution in-vitro en conditions statiques ou dynamiques (sans ou avec renouvellement du liquide) dans différentes solutions tamponnées à pH physiologique (TRIS et PBS) et pour des durées s’étalant entre 30 minutes et 2 mois. L’analyse des propriétés physico-chimiques, microstructurales et mécaniques des échantillons avant et après immersion a permis d’évaluer l’influence du type de milieu et des conditions de test choisies sur l’évolution des échantillons. Une attention particulière a été prêtée à la caractérisation mécanique: la technique de micro-indentation instrumentée sphérique a été préférée à autres essais plus conventionnels. Cette technique permet d’évaluer plusieurs paramètres tels que la dureté et le module de Young de façon quasi non-destructive et à une échelle locale. En conséquence, l’utilisation de la micro-indentation s’est avérée d’une grande aide pour le suivi des caractéristiques d’échantillons dégradés qui présentaient un gradient de propriétés entre la surface (où le processus de dégradation commence) et le cœur (sujet à des changements sur plus long terme). La dernière partie de cette étude a été dédiée à l’étude du deuxième phénomène qui entraine la résorption de substituts osseux in-vivo, à savoir la résorption cellulaire. Pour cela des essais cellulaires avec des cellules précurseurs d’ostéoclastes ont été réalisés sur des échantillons denses de β-TCP pur ou dopé avec 5% molaire de magnésium. L’addition de cet élément est censée modifier les propriétés du matériau (notamment sa solubilité) et, par conséquence, modifier le comportement cellulaire. Les résultats des tests ont confirmé la cytocompatibilité des deux types de β-TCP, mais ont également mis en avant une difficulté d’activation des ostéoclastes. Deux des causes possibles seraient liées à la topographie de surface des échantillons et au relargage des ions calcium suite à la dissolution du matériau. / The present Ph.D. thesis work was aimed to establish and assess an experimental methodology to monitor the in-vitro evolution of calcium phosphate (CaP) bone substitutes. Dicalcium phosphate dihydrate and beta-tricalcium phosphate were chosen as model of two main classes of bone substitutes: “biosoluble” ones (which undergo dissolution after implantation) and “bioresorbable” ones (which undergo cellular resorption after implantation). In order to study the dissolution and precipitation phenomena, which take place once CaPs are immersed in solution, these materials were produced in the form of micro-porous samples (60% of porosity for DCPD, 75% for β-TCP) and used for dissolution tests in-vitro in static and dynamic conditions (without or with liquid renewal) in different buffered solutions at physiologic pH (TRIS and PBS) and for periods of time ranging between 30 minutes and 2 months. The analysis of the physico-chemical, microstructural and mechanical properties of the samples before and after immersion allowed to evaluate the influence of the chosen medium and immersion conditions on the evolution of the specimens. Particular attention was paid to the mechanical characterisation: instrumented spherical micro-indentation was preferred to other more conventional tests. This technique enables the evaluation of several parameters such as the hardness and the Young’s modulus in a quasi-non-destructive way and on a local scale. As a consequence, the use of micro-indentation proved to be of great help for monitoring the characteristics of the degraded specimens, which presented a gradient of properties between the surface (where the degradation process starts) and the core (subject to changes on a longer period). The last part of this work was focused on the study of the second main phenomenon, which takes part in the in-vivo resorption of bone substitutes, that is to say the cellular resorption. For this purpose, cellular tests with osteoclast-precursor cells were carried out on dense samples made of pure and magnesium-doped β-TCP (5 mol.% of Mg). The addition of magnesium was aimed to modify the properties of the material and, as a consequence, the cellular behavior. The results confirmed the cytocompatibility of both types of β-TCP, but they also showed a difficult activation of osteoclasts. Two of the possible causes would be linked to the topography of the surface of the specimens and to the release of calcium ions due to the dissolution of the material.

O trauma renal de alto grau e as nefrectomias parciais para remoção de tumores malignos induzem a uma reconstrução renal de difícil execução e possuem um potencial elevado para causar perda completa do órgão. A utilização de substâncias auxiliares na reconstrução renal, tais como: selantes biológicos e hemostáticos têm fundamental importância para a sua preservação. OBJETIVO: Avaliar o efeito hemostático e selante da esponja do biopolímero da cana-de-açúcar em trauma renal de alto grau induzido em coelhos. MÉTODOS: Foi realizado um estudo experimental, prospectivo e pareado, com vinte e quatro unidades renais de doze coelhos da linhagem California. Após indução de trauma renal bilateral e de alto grau (lesão da via coletora), utilizamos, para a reconstrução e hemostasia do parênquima renal direito, a esponja do biopolímero da cana-de-açúcar, e à esquerda, foi utilizada a esponja de Surgifoam®. Nos dois lados, foi realizada uma sutura com fio de poliglactina 4/0 sobre as esponjas. Desta forma, cada coelho se tornou controle de si mesmo. Durante o procedimento cirúrgico, foram avaliados: o tempo cirúrgico total, o tempo para hemostasia e o grau de perda sanguínea. Os animais foram sacrificados após quatro semanas, sendo os rins retirados e encaminhados para a análise histológica. Foi avaliada a presença de fístulas, urinoma, hematomas e aderências aos órgãos adjacentes no momento da nefrectomia. A análise histológica observou o grau de inflamação e fibrose, assim como presença de necrose. RESULTADOS: Em relação ao tamanho dos rins, não houve diferença estatística entre os grupos (p=0,058), o que configura uma homogeneidade entre eles. Da mesma forma, o tempo para hemostasia foi similar (p=0,195) e a perda sanguínea (p=0,480). Por outro lado, foi notada uma maior incidência de aderências aos órgãos adjacentes no grupo do BCPA, assim como, uma maior intensidade da resposta inflamatória a corpo estranho (p=0,004). Todavia, sem influência nos resultados finais. Não foi evidenciada a ocorrência de hemorragia maciça, formação de hematomas, abscessos, fístulas ou cálculos em nenhuma das unidades renais. A presença de fibrose e necrose foi semelhante nos dois grupos do ponto de vista estatístico (p=0,500 e 0,414, respectivamente). CONCLUSÃO: A esponja do biopolímero da cana-de-açúcar demonstrou eficácia em relação ao controle hemostático e selante sobre o parênquima renal de coelhos, sem causar complicações importantes. Futuros estudos podem direcionar seu uso rotineiro na prática urológica. / Introduction: The reconstruction of the renal parenchyma requires, in some situations, a surgical technique more complex and delicate. The high grade renal trauma and partial nephrectomy for removal of malignant tumors induce a kidney reconstruction difficult to implement and have a high potential to cause complete loss of the organ. The use of auxiliary substances in renal reconstruction, such as biological sealants and hemostatic are of enormous value to their preservation. Objective: To evaluate the effect of sealant and hemostatic sponge biopolymer of sugarcane in high grade renal trauma induced in rabbits. Methods: We conducted an experimental study, prospective, matched, with twenty-four renal units of twelve rabbits from California lineage. After induction of bilateral renal trauma and high-grade lesions, it was used for the reconstruction of right renal parenchymal, the biopolymer sponge of cane sugar, and in left kidney, we used the sponge Surgifoam®. On both sides, we performed a suture with polyglactin 4/0 on the sponges. Thus, each rabbit has control of himself. During surgery, we evaluated the total surgical time, time to hemostasis and the degree of blood loss. The animals were sacrificed after four weeks, and the kidneys removed and sent for histological analysis. We evaluated the presence of fistula, urinoma, hematoma, and adhesions to adjacent organs at the time of nephrectomy. Histological analysis showed the degree of inflammation and fibrosis, as well as necrosis. Results: In relation to the size of the kidneys, there was no statistical difference between groups (p = 0.058), which is one commonality between them. Similarly, time to hemostasis was similar (p = 0.195) and blood loss (p = 0.480). Moreover, it was noted an increased incidence of adhesions to adjacent organs in BCPA group, as well as a greater inflammatory response to foreign bodies (p = 0.004). However, no influence on final results. There was no evidence the occurrence of massive bleeding, hematomas, abscesses, fistulas, or in any calculations of renal units. The presence of fibrosis and necrosis was similar in both groups a statistically (p = 0.500 and 0.414, respectively). Conclusion: The biopolymer sponge of sugarcane has shown efficacy in the hemostatic control and sealant on the renal cortex of rabbits, without causing major complications. Future studies may direct its routine use in urologic practice.
