AN EXPLORATION OF THE EXPERIENCES OF TRANSGENDER AND NONBINARY PEOPLE WHO USE NONTRADITIONAL OR NON-STANDARDIZED GENDER-AFFIRMING HORMONE THERAPY

Transgender and nonbinary (TNB) people identify with a gender identity that differs from cultural expectations based on sex assigned at birth. Many TNB people pursue social and/or medical transition with the intent of affirming their gender identity to themselves and the world around them. Medical transition describes the process of utilizing one or more forms of gender-affirming medical care (e.g., hormone therapy, gender confirmation surgery, hair removal, breast augmentation, mastectomy) in order to change the physical body such that it is more congruent with an individual’s affirmed gender identity. Researchers suggest that many TNB individuals currently use or desire to use gender-affirming hormone therapy (GAHT), and that some TNB individuals desire or use nontraditional or non-standardized (e.g., low dose) hormone therapy regimens. Unfortunately, many TNB people encounter barriers in the process of accessing gender-affirming healthcare, especially individuals who are nonbinary and/or desire gender transition steps that are nontraditional or non-standardized (e.g., low-dose hormone therapy). In current research literature, little is currently known about the experiences and healthcare needs of TNB people who currently or desire to take nontraditional GAHT. To address this gap in the research literature, the present study was a qualitative study involving semi-structured interviews with 10 TNB people to explore their experiences connected to nontraditional GAHT. The grounded theory analysis revealed a core category: Nontraditional Gender Affirmation Model. The core category is comprised of five axial level categories: Social Processes, Medical Processes, Barriers to Accessing Medical Transition Care, Facilitators to Accessing Medical Transition Care, and Coping with Distress and Barriers. The five axial level categories are comprised of 30 open-coding level categories and 33 open-coding level subcategories. The grounded theory model developed depicts a framework of various phenomena involved in the process of gender affirmation for transgender and nonbinary individuals with nontraditional gender transition needs, including nontraditional approaches to GAHT. Implications for healthcare providers, psychological practice, and future research are discussed.

As global mental health initiatives continue, the need for culturally responsive approaches to research and mental health inquiry and intervention as increasingly important. However, most counseling programs and research preparation do not include indigenous approaches to address training and clinical practice. Liming and Ole Talk has emerged as a culturally responsive methodological approach with some Caribbean populations, however, at the time of this study, it had not been used with people from the Bahamas. Therefore, the purpose of this study is to continue to explore Liming and Ole Talk as a pathway to inquiry with Caribbean people leading to the research question; how do Bahamian immigrants experience liming and ole talk as a approach to mental health inquiry? For this study, 5 Bahamians signed up to lime, 4 came on the day of and 1 limer returned to process after. Utilizing a storytelling and reflective approach to analysis 5 connection points emerged: Stigma, Immigration as enlightenment, Spirituality as cause and intervention, Helplessness, and Hopefulness. The findings of the study are relevant for counselor education programs, clinical practice, and research methodological training.

A Queer Miracle in Georgia: The Origins of Gay-Affirming Religion in the South

The intersection of homosexuality and faith values, a very controversial topic in the United States, has generated both social accommodation as well as “culture war.” In the past forty years this nation has witnessed the establishment of predominantly gay congregations, gay “welcoming” and “affirming” mainstream congregations, as well as virulently anti-gay religious organizations. This study investigates the origins and evolving history of gay and gay-affirming religious traditions in America with an emphasis on Atlanta and Georgia. Primarily an oral history, this project draws from eighty-two interviews as well as primary and secondary documents to construct this history. Several conclusions unfold: 1) Southern culture, though uniquely religious, has been more accommodating of gays and lesbians than heretofore appreciated; 2) citizens of Atlanta and the state of Georgia have been primary historical producers of gay and gay-affirming religious culture and institutions in America; 3) gay religious history pre-dates the Stonewall Rebellion, thus troubling and adding nuance to the traditional metanarrative of LGBTQ history; and 4) the paths of and to gay-affirming religious activism and institution building follows several distinct patterns.

Os resultados do estudo revelam a existência de uma formação fora da universidade, construída e articulada a núcleos culturais afro-baianos, que contribuem para a formação de pessoas, em geral, em desprestígio e desvantagem social, por isso, precisam ser criadas condições de ampliação, manutenção e proteção através de ações afirmativas e políticas públicas. Exploring Sexual and Reproductive Health Care in Central Appalachia Through the Experiences of Transgender and Gender Non-Binary People

Background: Transgender or nonbinary (TNB) individuals in the United States experience higher rates of sexual and reproductive health disparities when compared to their cisgender counterparts, along with lower access to care. TNB people in the south experience higher rates of disparities. Societal, interpersonal, and individual barriers contribute to these disparities in the TNB population. However, there is a paucity of research examining the sexual and reproductive health experiences of TNB people living in Central Appalachia. Methods: An interpretive phenomenological analysis (IPA) research design was used to collect and analyze semi-structured interviews with TNB individuals living in central Appalachia regarding their sexual and reproductive health experiences. Transcribed interviews were analyzed repeatedly by the primary investigator and the study equity committee to identify emergent themes which focused on understanding an individual’s lived experiences through interpretation. Results: Twenty participants from eastern Tennessee and western North Carolina from four Appalachian states within three Appalachian sub-regions participated in this study. Four themes were identified: a) perceptions and/or experiences receiving SRH healthcare shaped the individual’s self-agency regarding accessing and receiving SRH care, along with influencing their sense of self regarding their gender; b) heteronormative expectations within the medical field influenced interactions with providers and the care system; c) community is integral to SRH experiences, and d) policy and laws impact willingness and ability to access SRH care. Discussion: TNB individuals living in central Appalachia experience individual, interpersonal, community, and social factors that impact their sexual and reproductive healthcare experiences. By addressing these factors based on recommendations from participants, public health leaders, policy makers, and providers can improve sexual and reproductive healthcare experiences of TNB people living in central Appalachia.

Because of the conceptual overlap between the two approaches, the Satir Model of experiential therapy can be effectively applied to existing frameworks for affirming couple therapy. The study reflects the efficacy of this proposed model of therapy through a phenomenological case study. The results, their implications, and the application of the model are discussed.</p>

Den här studien syftar till att undersöka vuxnas erfarenheter av sexuella problem under könsbekräftande behandling med testosteron, samt erfarenheter av samtal om sexuell hälsa inom den könsbekräftande vården. Tidigare forskning har visat att omfattningen av studier på området är bristfälligt. Studien har genomförts med kvalitativ metod och fenomenologisk ansats. Den består av berättelser från sex halvstrukturerade livsvärldsintervjuer som har analyserats med reflexiv tematisk analys. Fyra huvudteman presenteras i resultatanalysen: Nya sexuella kroppar, Nya sexuella känslor, Nya sexuella rum och relationer och Olika vård för lika problem. Resultaten visar att sexuella problem under könsbekräftande behandling är en flytande process där olika individuella, interpersonella och kontextuella faktorer spelar in. Sarah Ahmeds (2006) queera fenomenologi och Jack Halberstams (2005) begrepp queer tid har använts för att identifiera processer av orientering i tid och rum. Resultaten visar även på olika erfarenheter av den könsbekräftande vårdens förmåga att tillgodose behov för sexologiskt stöd samt ett fortsatt behov av forskning och insatser på området. / This study aims to explore adults’ experiences of sexual problems during gender affirming treatment with testosterone, and experiences of conversations about sexual health within gender affirming health care services. Previous research shows that there is a lack of studies on the subject. This study has been conducted with qualitative methods and phenomenological approach. It is constituted by stories from six semi structured interviews which has been analyzed through reflexive thematic analysis. Four themes are presented in the result analysis: New sexual bodies, New sexual feelings, New sexual spaces and relationships and Different care for equal needs. The results show that sexual problems during gender affirming treatment is a fluent process where different individual, interpersonal and contextual factors contribute. Sarah Ahmed’s (2006) queer phenomenology and Jack Halberstam’s (2005) queer time has been used to identify processes of orientation in time and space. The results also show different abilities for gender affirming health care services to meet the needs for support from a sexologist, and a need for future research on the subject.
