The problems arising from the use of alcohol and other drugs, major concern from a public health point of view, generate issues and special needs for the individuals and, when undetected, can harm the individualized and comprehensive care. Considering the need for measurement based on evidence in the area, which ranges the different needs of the individual, this study aimed to evaluate the psychometric properties of the instruments \"Global Appraisal of Individual Needs - Initial and Short Screener\" for use in Brazil. Objective: To evaluate the psychometric properties of the instruments \"Global Appraisal of Individual Needs Initial and Short Screener\". Methods: The study was designed as a methodological study of validation of the instruments \"Global Appraisal of Individual Needs\" in the versions Short Screener and Initial. Data with Short Screener version were collected with 128 individuals in the initial phase of treatment in a Psychosocial Care Center for Alcohol and Other Drugs, and the Initial version with 40 patients being treated in an inpatient service and 70 individuals in the embracement stage of treatment in a Psychosocial Care Center for Alcohol and Other Drugs in Sao Paulo. Internal consistency analysis was performed for both versions of the instruments and its subscales. Analysis of the Rasch model was performed for the subscales of the Short Screener version and for the subscale Substance Problem Scale. The association of the Substance Disorder Screener with the other subscales of the Short Screener was verified. We also performed the analysis of convergent validity between the subscales of the Short Screener version and its wider versions of the Initial. Results: Both versions of the instrument presented internal consistency indices greater than 0.7 Cronbachs alpha for their most comprehensive subscales. The Substance Problem Scale and subscales of the Short Screener fit the Rasch model, and none of their items distort the measurements. The subscale Substance Problem Scale has two items of its sixteen with different performance between men and women and seems to have redundancy for measuring severity. The subscales of Short Screener show moderate to strong correlation with the subscales of the Initial, and their most comprehensive scales have convergent validity (rho = 0.801). The Short Screener analysis shows that there is an association between the severity of symptoms related to alcohol and other drugs and mental health symptoms and problems related to crime and violence. Conclusions: Both instrument versions have internal consistency and convergent validity with each other and are valid according to the Rasch model. They also show to be compatible with results from American and Canadian studies with the instrument

Estudo de validação de instrumentos de rastreamento para transtornos depressivos, abuso e dependência de álcool e tabaco / Validation of screening instruments for depressive disorders and alcohol and tobacco dependence and abuse.Carolina de Meneses Gaya 19 August 2011 (has links)
A menor versão, o AUDIT-3 obteve excelentes resultados. / Depressive disorders, tobacco dependence, and alcohol-related disorders (ARD) are highly prevalent and are associated with several comorbidities. Nonetheless, they are still under-diagnosed and under-treated in many health settings. Therefore, this study was aimed at (1) assessing reliability and validity parameters of screening instruments for depressive disorders (Patient Health Questionnaire 2 [PHQ-2] and Well-Being Index 5 [WHO-5]), tobacco dependence (Fagerström Test for Nicotine Dependence [FTND] and Heaviness of Smoking Index [HSI]), and alcohol dependence (Fast Alcohol Screening Test [FAST], CAGE, Alcohol Use Disorders Identification Test [AUDIT] and their brief versions); (2) performing the factorial analysis of the WHO-5, FTND, and AUDIT; and (3) assessing the comorbidity between the above-mentioned disorders. The test-retest reliability study involved 429 university students and the inter-rater reliability study included 41 patients of a Center for Psychosocial Attention Alcohol and Drugs (CAPS-AD, in the Portuguese acronym). The assessment of the predictive validity, internal consistency, comorbidity, and factorial analysis involved a sample of 530 patients of an emergency unit and the CAPS-AD. Reliability was estimated by means of kappa and interclass correlation (ICC) coefficients. In the predictive validity studies, the SCID-CV was used as the comparison diagnostic measure, with calculation of the areas under the ROC curve (AUC), sensitivity, specificity, accuracy, and positive and negative predictive values of the instruments. Concurrent validity was assessed using Spearmans coefficient. Cronbachs alpha coefficient was used to assess internal consistency. The exploratory factorial analysis was conducted according to Kaisers criteria. Comorbidities were analyzed by logistic regression raw and adjusted odds ratio. The level of statistical significance was set at p < 0.05, with a 95% confidence interval. The PHQ-2 and WHO-5 had high levels of reliability (> 0.98) and internal consistency (0.76 and 0.78, respectively). The WHO-5 had an AUC of 0.89; sensitivity of 0.85; and specificity of 0.81 for a cut-off of 9. The PHQ-2 had an AUC of 0.86; sensitivity of 0.74; and specificity of 0.87 for a cut-off of 3. The WHO-5 yielded one single factor, which explained 51% of the data variance. The reliability study of the FTND provided high ICC coefficients (0.92 for test-retest and 0.99 for inter-rater). In the factorial analysis, the FTND yielded two factors. The internal consistency of the FTND was high (0.83), but that of the HIS was below recommended levels (0.56). The FTND and the HIS had high indices of sensitivity, specificity, and accuracy. The AUDIT and its brief versions had internal consistency values between 0.83 and 0.94. In the study of predictive validity, these instruments had AUC between 0.92 and 0.96, with sensitivity levels between 0.84 and 0.93 and specificity levels ranging from 0.83 and 0.94 for the screening of ARD. The shortest version of the AUDIT (AUDIT-3) had excellent results. In the assessment of concurrent validity, expressive correlations were found between the AUDIT and its brief versions (0.91-0.99). In all analyses, the CAGE had satisfactory results, although inferior to those of the brief versions of the AUDIT. The results confirm the validity and reliability of the Brazilian versions of the WHO-5, PHQ-2, FTND, and HIS, as well as the efficacy of all the brief versions of the AUDIT, demonstrating that their psychometric qualities are as satisfactory as those of the full version of the instrument and superior to those of the CAGE. Therefore, the use of these instruments is supported by their viability and validity, which encourage their use in routine clinical practice and research settings. The results also showed that smoking and alcohol abuse and dependence are important predictors of depressive disorders and that alcoholics had a fourfold higher chance of being smokers.

The relationship between personality and alcohol consumption and alcohol-related problemsLindeberg, Sofie January 2012 (has links)
The purpose of the study was to examine the relationship between the five personality dimensions; neuroticism, extraversion, openness, agreeableness and conscientiousness, and if they can predict adults alcohol consumption and alcohol-related problems in a Swedish sample. A webbased-survey was sent to a sample of municipality employees in Stockholms county. The results indicate a negative association between openness and the frequency of binge drinking and alcohol-related problems. They further imply a positive association between extraversion and the quantity of alcohol consumption at a typical instance when alcohol is consumed and the frequency of binge drinking.

Pharmacological Versus Social Alcohol Expectancies: Making an Important Distinction between the Anticipated Rewarding Effects of AlcoholMcmurray, Megan Victoria 01 January 2013 (has links)
Despite over 30 years of research investigating alcohol expectancies, they have never been examined in terms of the anticipated pharmacological versus social rewards resulting from alcohol consumption, and both appear to play a central role in drinking motivation and behavior. The purpose of this study was to develop a two-dimensional instrument designed to assess both the pharmacological alcohol expectancies of pleasurable, internal states that result from alcohol consumption, as well as the social expectancies that drinking alcohol will result in higher social status and increased effectiveness in social situations. This measure, called the Pharmacological and Social Alcohol Expectancy Scale (PSAES), was developed and validated in a college sample using a two-phase design with three separate samples. Phase I results demonstrated that a respecified model of the PSAES adequately fit the proposed two-dimensional factor structure and provided justification for the items representing two distinguishable domains: social and pharmacological. The measure was then used to 1) assess patterns of drinking expectancies at various drinker levels and 2) investigate whether known risk factors for alcohol use disorders differentiate scores on the two factors. Phase II results indicated that pharmacological and social expectancies are both significantly positively associated with drinking behavior, and that sensation-seeking is significantly associated with pharmacological expectancies. The PSAES represents the first alcohol expectancy instrument to provide adequate coverage of pharmacological expectancies. Implications and limitations are discussed.

The nature and extent of policing alcohol related crime and reducing violence in and around late night entertainment areasPalk, Gavan Roger Mark January 2008 (has links)
The misuse of alcohol is well documented in Australia and has been associated with disorders and harms that often require police attention. The extent of alcohol-related incidents requiring police attention has been recorded as substantial in some Australian cities (Arro, Crook, & Fenton, 1992; Davey & French, 1995; Ireland & Thommeny, 1993). A significant proportion of harmful drinking occurs in and around licensed premises (Jochelson, 1997; Stockwell, Masters, Phillips, Daly, Gahegan, Midford, & Philp, 1998; Borges, Cherpitel, & Rosovsky, 1998) and most of these incidents are not reported to police (Bryant & Williams, 2000; Lister, Hobbs, Hall, & Winlow, 2000). Alcohol-related incidents have also been found to be concentrated in certain places at certain times (Jochelson, 1997) and therefore manipulating the context in which these incidents occur may provide a means to prevent and reduce the harm associated with alcohol misuse.
One of the major objectives of the present program of research was to investigate the occurrence and resource impact of alcohol-related incidents on operational (general duties) policing across a large geographical area. A second objective of the thesis was to examine the characteristics and temporal/spatial dynamics of police attended alcohol incidents in the context of Place Based theories of crime. It was envisaged that this approach would reveal the patterns of the most prevalent offences and demonstrate the relevance of Place Based theories of crime to understanding these patterns. In addition, the role of alcohol, time and place were also explored in order to examine the association between non criminal traffic offences and other types of criminal offences. A final objective of the thesis was to examine the impact of a situational crime prevention strategy that had been initiated to reduce the violence and disorder associated with late-night liquor trading premises.
The program of research in this doctorate thesis has been undertaken through the presentation of published papers. The research was conducted in three stages which produced six manuscripts, five of which were submitted to peer reviewed journals and one that was published in a peer reviewed conference proceedings. Stage One included two studies (Studies 1 & 2) both of which involved a cross sectional approach to examine the prevalence and characteristics of alcohol-related incidents requiring police attendance across three large geographical areas that included metropolitan cities, provincial regions and rural areas. Stage Two of the program of research also comprised two cross sectional quantitative studies (Studies 3 & 4) that investigated the temporal and spatial dynamics of the major offence categories attended by operational police in a specific Police District (Gold Coast). Stage Three of the program of research involved two studies (Studies 5 & 6) that assessed the effectiveness of a situational crime prevention strategy. The studies employed a pre-post design to assess the impact on crime, disorder and violence by preventing patrons from entering late-night liquor trading premises between 3 a.m. and 5 a.m. (lockout policy). Although Study Five was solely quantitative in nature, Study Six included both quantitative and qualitative aspects. The approach adopted in Study Six, therefore facilitated not only a quantative comparison of the impact of the lockout policy on different policing areas, but also enabled the processes related to the implementation of the lockout policy to be examined.
The thesis reports a program of research involving a common data collection method which then involved a series of studies being conducted to explore different aspects of the data. The data was collected from three sources. Firstly a pilot phase was undertaken to provide participants with training. Secondly a main study period was undertaken immediately following the pilot phase. The first and second sources of data were collected between 29th March 2004 and 2nd May 2004. Thirdly, additional data was collected between the 1st April 2005 and 31st May 2005.
Participants in the current program of research were first response operational police officers who completed a modified activity log over a 9 week period (4 week pilot phase & 5 week survey study phase), identifying the type, prevalence and characteristics of alcohol-related incidents that were attended. During the study period police officers attended 31,090 alcohol-related incidents. Studies One and Two revealed that a substantial proportion of current police work involves attendance at alcohol-related incidents (i.e., 25% largely involving young males aged between 17 and 24 years). The most common incidents police attended were vehicle and/or traffic matters, disturbances and offences against property. The major category of offences most likely to involve alcohol included vehicle/traffic matters, disturbances and offences against the person (e.g., common & serious assaults). These events were most likely to occur in the late evenings and early hours of the morning on the weekends, and importantly, usually took longer for police to complete than non alcohol-related incidents.
The findings in Studies Three and Four suggest that serious traffic offences, disturbances and offences against the person share similar characteristics and occur in concentrated places at similar times. In addition, it was found that time, place and incident type all have an influence on whether an incident attended by a police officer is alcohol-related. Alcohol-related incidents are more likely to occur in particular locations in the late evenings and early mornings on the weekends. In particular, there was a strong association between the occurrence of alcohol-related disturbances and alcohol-related serious traffic offences in regards to place and time. In general, stealing and property offences were not alcohol-related and occurred in daylight hours during weekdays.
The results of Studies Five and Six were mixed. A number of alcohol-related offences requiring police attention were significantly reduced for some policing areas and for some types of offences following the implementation of the lockout policy. However, in some locations the lockout policy appeared to have a negative or minimal impact. Interviews with licensees revealed that although all were initially opposed to the lockout policy as they believed it would have a negative impact on business, most perceived some benefits from its introduction. Some of the benefits included, improved patron safety and the development of better business strategies to increase patron numbers.
In conclusion, the overall findings of the six studies highlight the pervasive nature of alcohol across a range of criminal incidents, demonstrating the tremendous impact alcohol-related incidents have on police. The findings also demonstrate the importance of time and place in predicting the occurrence of alcohol-related offences. Although this program of research did not set out to test Place Based theories of crime, these theories were used to inform the interpretation of findings. The findings in the current research program provide evidence for the relevance of Place Based theories of crime to understanding the factors contributing to violence and disorder, and designing relevant crime prevention strategies. For instance, the results in Studies Five and Six provide supportive evidence that this novel lockout initiative can be beneficial for public safety by reducing some types of offences in particular areas in and around late-night liquor trading premises. Finally, intelligent-led policing initiatives based on problem oriented policing, such as the lockout policy examined in this thesis, have potential as a major crime prevention technique to reduce specific types of alcohol-related offences.

Tal situação não deflagra a necessidade de busca de ajuda, porém, implica risco à saúde física e mental / Abstract: Objective: To study the prevalence of self-assessed mental disorders, the prevalence of alcohol and illicit drugs use and associated risk behavior amongst undergraduate students from UNICAMP. In addition we investigated associations between students¿ demographics, cultural and educational aspects and self-assessed mental disorders, alcohol and illicit drugs use and risk behavior. Methods: A cross-sectional study was performed, from October 2005 to November 2006. A sample proportional to study fields was obtained involving 1,290 students, from both genders, regularly enrolled in either day-time and night-time courses from the following study fields: Natural Sciences, Exact Sciences and Technology, Humanities, Arts and Health Sciences, at the following campuses: Barão Geraldo (Campinas) and CESET (Tertiary Technological Education Center of Limeira) of the State University of Campinas (UNICAMP). A self-reported questionnaire was designed to collect socio-demographic, cultural and educational data. Quantitative instruments were used to assess mental health (M.I.N.I. Mini International Neuropsychiatric Interview); alcohol use (AUDIT The Alcohol Use Disorder Identification Test); illicit drugs use
(questionnaire based on the CEBRID - Brazilian Information Center on Psychotropic Drugs - method). The self-report questionnaire and the quantitative instruments were anonymously filled out by students in the classroom. The qui-squared test and logistic regression analysis (univariate and multivariate) were adopted for the variables association study. The significance level adopted for statistical analysis was 5%. Results: A total of 1,290 questionnaires, completed by students from both sexes, 55.5% women and 44.5% men, were analyzed. 92% of the students were single, 82% were aged between 17 and 23
years old, 67% had studied in private high schools, 75% lived in Campinas, 56% shared houses with other students or rented rooms in student houses and 33% were still living with their parents. Also, 77% were Caucasian, 13% were of African-descent or mixed-race and 9% were of Asiatic-descent. Regarding religious beliefs, 47% were Catholic, 24% had no religion, 15% were Spiritualist and 14% were Evangelic. The following prevalences were found: 58% of the students had some kind of self-assessed mental disorder; 24% presented hazardous drinking; 27% used illicit drugs; and 25% presented risk behavior. Conclusion: Women presented more often some kind of subjective mental distress and more interpersonal and environmental self-assessed difficulties. Men presented more often hazardous drinking and illicit drugs use, as well as risk behavior related to such use. Men had better social interactions and better social-economic status. Problems resulting from hazardous drinking and illicit drugs use seem to be regarded as ¿natural¿ in the student ¿ethos¿, specially when involving male students. Such behaviors and resulting problems does not entail help-seeking behavior among the students, however they implicates risks to mental and physical health / Mestrado / Saude Mental / Mestre em Ciências Médicas
