Perda clínica de inserção periodontal em uma população brasileira com deficiência isolada do hormônio do crescimento / Periodontal Clinical Attachment Loss in an Isolated Growth Hormone Deficiency Brazilian populationIsabella Maria Porto de Araujo Britto 01 December 2010 (has links)
Não existe relato da condição periodontal em indivíduos com Deficiência Isolada do Hormônio do Crescimento (IGHD). O objetivo deste estudo foi investigar possíveis associações entre IGHD e a perda clínica de inserção periodontal (PCI) em uma população com IGHD congênita, presente no Nordeste do Brasil. Material e Métodos: Todos os indivíduos previamente identificados com IGHD e com idade 12 anos foram elegíveis para participar do estudo (n=46). A amostra final ficou composta por 33 casos (IGHD) e 33 controles (não - IGHD) após a exclusão dos edêntulos (n=5) e dos que haviam recebido previamente tratamento com reposição do Hormônio do Crescimento (n=8). Eles foram pareados por idade, gênero, condição sócio-econômica, hábito de fumo e diabetes. Todos foram submetidos a exame periodontal completo em 6 sítios por dente, e entrevistados por meio de um questionário estruturado. Resultados: Indivíduos com IGHD apresentaram quantidade semelhante de biofilme (p=0,32), menos cálculo supragengival (p=0,01), e mais sangramento à sondagem (p<0,01) em comparação com os controles. Após uma série de análises de regressão logística múltipla condicional, ajustada para cálculo supragengival, indivíduos com IGHD mostraram maior chance de apresentar Perda Clínica de Inserção Periodontal 7mm (OR= 18,1; IC 95% = 2,4 - 137,2). Conclusão: Indivíduos com IGHD severa e congênita possuem maior chance de apresentar Perda Clínica de Inserção Periodontal. / There are no reports of periodontal status in subjects with Isolated Growth Hormone Deficiency (IGHD). The aim of this study was to investigate possible associations between IGHD and periodontal clinical attachment loss (CAL) in a population affected by congenital IGHD, residing in Northeastern Brazil. Material and Methods: All previously identified IGHD subjects age 12 years were eligible for this study (n=46). The final study sample comprised 33 cases (IGHD) and 33 controls (non - IGHD) after excluding edentulous (n = 5) and subjects previously treated with GH replacement (n = 8). They were matched by age, gender, socioeconomic status, smoking and diabetes status. Subjects were submitted to a full-mouth clinical examination of six sites per tooth and were interviewed using a structured, written questionnaire. Results: IGHD subjects had same amount visible plaque (p=0.32), less supragingival calculus (p = 0.01), and more bleeding on probing (p < 0.01) than controls. After performance of conditional multiple regression analyses adjusted by supragingival calculus, IGHD subjects had a higher likelihood of having CAL 7 mm (OR = 18.1; 95% Cl = 2.4 - 137.2). Conclusion: Severe congenital IGHD subjects have a greater chance of having Periodontal Clinical Attachment Loss.

O efeito do consumo de álcool sobre progressão da perda de inserção periodontal : estudo de Porto AlegreWagner, Marcius Comparsi January 2008 (has links)
Background: Conflicting evidence of an association between alcohol consumption and periodontal disease has been suggested in the literature. The aim of the present study was to investigate the impact of alcohol consumption on periodontal attachment loss (PAL) over a period of 5-years. Methods: A multistage probability sampling strategy was used to draw a representative sample of the metropolitan area of Porto Alegre, Brazil. Five hundred sixty one individuals (238 males and 323 females) that were 19-65 years-old, had no medical history of diabetes and at least 6 teeth were included. Participants were clinically examined and interviewed in 2001 and 2006. Alcohol consumption was assessed by asking participants about the usual number of drinks consumed in a week. Standard formulas were used to calculate the amount of pure alcohol consumed per day in grams. Drinkers were categorized into occasional (<3g/day) and regular drinkers (≥3g/day). Individuals showing ≥4 teeth with proximal PAL ≥3 mm over the 5-years follow-up period were classified as having disease progression. Linear models were used to estimate the relative risk. Results: Male regular drinkers had higher risk of having PAL progression than never-drinkers. After adjusting for important co-factors regular drinkers had 1% increased risk per grams/day of pure alcohol consumption (RR: 1.01, 95%CI 1.00-1.02), which means between 5 to 7% increased risk per drink per day. Male occasional drinkers were not at higher risk of having disease progression. No association between alcohol consumption and periodontitis was observed for females. Conclusion: Alcohol consumption increased the risk of PAL progression in male regular drinkers. The impact of alcohol cessation initiatives on the periodontal health should be evaluated.

Os objetivos deste estudo são avaliar a prevalência, extensão e severidade de perda clínica de inserção (NCI); e investigar as possíveis associações entre variáveis demográficas, socioeconômicas e comportamentais com NCI em uma população isolada brasileira. MATERIAL E MÉTODO: Todos os indivíduos com mais de 12 anos de idade foram identificados por um censo. Eles foram entrevistados por meio de um questionário estruturado e submetidos a um exame clínico completo que consistiu na avaliação de 6 sítios por dente em toda a boca. RESULTADOS: Dentre os 214 indivíduos que foram entrevistados e receberam exame clínico completo, NCI >= 5mm foi observado em 8%, 37%, 70%, 83% e 100% dos indivíduos dentados nas faixas etárias de 12-19, 20-29, 30- 39, 40-49 e 50 ou mais anos de idade; enquanto que a prevalência de NCI >= 7mm foi de 5%, 8%, 20%, 67% e 83% nas faixas anteriormente descritas, respectivamente. Análises de regressão logística multivariável identificaram quantidade de placa visível (OR = 2,8), quantidade de cálculo supra-gengival (20-50%, OR = 2,9; e > 50%, OR= 10,6), idade (OR = 11,4) e tabagismo (OR = 2,4) como indicadores de risco para NCI >= 5mm; e tabagismo (OR = 8,2) para NCI >= 7mm. CONCLUSÃO: Os resultados demonstraram que perda clínica de inserção é altamente prevalente nesta população isolada. A alta prevalência de NCI em faixas etárias jovens e a identificação de indicadores de risco tradicionais para NCI nesta população sugerem que fatores microbiológicos ou a susceptibilidade para a exposição de fatores ambientais possam ser considerados fatores-chave para a alta ocorrência encontrada nesta população. / The aim of this study was to assess the prevalence, extent and severity of clinical attachment loss (CAL); and to investigate the associations between demographic, socioeconomic and behavioral risk indicators with CAL in an untreated isolated population in Brazil. METHODS: All subjects aged 12 years forward were selected by a census. They were submitted to a full-mouth clinical examination of six sites per tooth and were interviewed using a structured written questionnaire. RESULTS: Among the 214 subjects who were interviewed and clinical examined, CAL >= 5mm was observed in 8%, 37%, 70%, 83%, and 100% of the dentate subjects, depending on age; while the age dependent prevalence of CAL >= 7mm was 5%, 8% 20%, 67% and 83%, respectively. Multivariate analysis identified amount of plaque (OR=2.8), thresholds of supragingival calculus (OR=2.9-10.6), age (OR=11.4) and smoking (OR=2.4) as risk indicators for CAL >= 5mm, and smoking (OR=8.2) for CAL >= 7mm. CONCLUSION: The results demonstrate that CAL is highly prevalent in this isolated population. The high prevalence of CAL in young age groups and the observation of traditional risk indicators for CAL in this population suggest microbiological factors or host susceptibility to be key factors explaining the high levels of CAL found in this population.

Exotoxins of Aggregatibacter actinomycetemcomitans and periodontal attachment loss in adolescentsHöglund Åberg, Carola January 2013 (has links)
Aggregatibacter actinomycetemcomitans is an oral bacterium that is mainly associated with aggressive forms of periodontitis, which most often starts at an early age. Amongst the virulence factors of A. actinomycetemcomitans, two exotoxins, the leukotoxin (LtxA) and the cytolethal distending toxin (Cdt), are suggested to play an important role in the pathogenicity of aggressive periodontitis. There is also a genetic diversity of the different strains of A. actinomycetemcomitans, and a variation in the ability of different strains to express and release exotoxins has been suggested. Of the different genotypes of A. actinomycetemcomitans, the highly leukotoxic JP2 genotype, which is prevalent in individuals of African origin, seems to be the genotype that is most strongly associated with localized aggressive periodontitis. This thesis is built upon studies of a West African adolescent population. The aim was to study the virulence characteristics of A. actinomycetemcomitans genotypes with a specific focus on the LtxA and the Cdt in relation to the progression of attachment loss (AL). The specific aim was first, to investigate cross-sectionally the presence of the JP2 and non-JP2 genotypes of A. actinomycetemcomitans in relation to the prevalence of AL and then prospectively to assess the progression of AL in a Ghanaian adolescent population. Second, in clinical isolates of A. actinomycetemcomitans obtained from the participants of the study, the serotypes and the virulence characteristics related to the two exotoxins were studied and associated with the progression of AL at the individual level. In Paper I, based on the study population consisting of 500 adolescents (mean age 13.2 years; SD ±1.5), it was shown that the overall carrier rate of A. actinomycetemcomitans was high (54.4%) and that the presence of this bacterium was associated with AL ≥ 3 mm. The JP2 genotype was prevalent (8.8%) in this population. In Paper II, 397 (79.4%) of the study participants were periodontally examined again at a 2-year follow-up. The presence of the JP2 genotype of A. actinomycetemcomitans in subgingival plaque was strongly associated with the progression of AL. This study also provided support for an enhanced estimated risk (odds ratio, OR=3.4), though less pronounced, for the progression of AL in individuals positive for the non-JP2 genotypes of A. actinomycetemcomitans. In Paper III, all three cdt genes (a, b and c) were detected in 79% of the examined A. actinomycetemcomitans isolates, all of which expressed an active toxin. The distribution of the cdt genes showed a serotype-dependent pattern. In particular, the presence of the b serotypes (both JP2 and non-JP2 genotypes) was associated with the disease progression, whereas the expression of Cdt was not particularly related to the disease progression. In Paper IV, it was shown that the presence of of A. actinomycetemcomitans isolates with high leukotoxicity, also those of the non-JP2 genotypes of A. actinomycetemcomitans, were associated with an increased risk of the progression of AL in relation to the reference group. The main proportion of the serotype b isolates was distributed in the category of highly leukotoxic isolates. The analyses of the non-JP2 genotypes of serotype b indicated a diversity linked to the level of leukotoxicity. In conclusion, A. actinomycetemcomitans in general was associated with the progression of AL. Individuals with an increased risk of developing progression of AL mainly harboured isolates of A. actinomycetemcomitans with a high leukotoxicity, which suggests that the LtxA is an important virulence factor. Of the two exotoxins, the pathogenic potential was mainly associated with the LtxA, while the role of the Cdt is unclear.

Periodontal diseases in a representative urban population in south BrazilSusin, Cristiano January 2004 (has links)
Background and Aims: There is little information about the epidemiology and risk factors of periodontal diseases in Latin America in general, and Brazil in particular. The principal aims of this study were to: 1) describe the prevalence and severity of periodontal attachment loss and gingival recession, and to assess the contribution of demographic, behavioral, and environmental exposures to the occurrence of periodontal disease outcomes in a sample representative of the urban population in the state of Rio Grande do Sul in south Brazil; and 2) report the epidemiology and risk indicators of aggressive periodontitis in this population. Methods: A representative sample consisting of 1,586 subjects 14-103 years of age (mean 38 y) and comprising 45.3% males and 54.7% females was selected using a multi-stage, probability, cluster sampling strategy. The subjects were interviewed using a structured questionnaire and underwent a full-mouth, six sites per tooth clinical examination in a mobile examination center. Results: Moderate and severe clinical attachment loss and gingival recession were widespread among adults in this population. The prevalence and extent of attachment loss ³5 and ³7 mm were 79% and 52% subjects, and 36% and 16% teeth; and for gingival recession ³3 mm and ³5 mm were 52% and 22% subjects, and 17% and 6% teeth, respectively. Aggressive periodontitis was diagnosed in 5.5% of subjects, which is significantly higher than the reported prevalence in most other populations. Among the main risk indicators for chronic as well as aggressive destructive periodontal diseases were: older age, low socioeconomic status, dental calculus, and smoking. Cigarette smoking accounted for an important part of periodontal disease burden, particularly in adults, and should be considered an important target in any prevention strategy aimed at reducing the burden of periodontal diseases. Partial recording methods consistently underestimated the prevalence of attachment loss in the population, and the extent of underestimation was dependent on the type of system used and the threshold of attachment loss. Conclusions: Destructive periodontal diseases are prevalent in this Brazilian population. Suitable disease prevention and health promotion programs should be established to improve the periodontal health in this population.

Objetivo: Comparar dois métodos de aferição da perda de inserção clínica (PI) em pacientes periodontais. Materiais e métodos: Setenta e cinco exames (50,9 ± 8,02 anos, 72,2% mulheres e 49,4% não-fumantes), realizados por examinadores calibrados, com registro do Índice de placa visível (IPV), Índice de Sangramento gengival (ISG), Profundidade de Sondagem (PS), Sangramento à Sondagem (SS), Perda de Inserção Clínica aferida pelo método direto (PID) e Recessão Gengival (RG), em 6 sítios/dente, foram utilizados. Valores para PII (método indireto) resultaram do somatório da PS e RG dos sítios RG+ (sítios com recessão gengival). A PID foi considerada como referência. Os dados foram analisados por “clusters”. A dependência dos dados foi observada em níveis hierárquicos: indivíduo/exame, dente e sítio. Para a comparação entre PID e PII gerou-se o valor de d (diferença entre os métodos). Modelos multivariados foram aplicados para investigar o comportamento das demais variáveis em relação à estimação da PI. O Gráfico de Bland-Altman foi gerado para verificar a distribuição da concordância entre os métodos. Resultados: A média de PID (3,96±2,07) foi significativamente menor que a observada para PII (4,47±2,03). Portanto, o valor de d foi de 0,513±1,23. A análise multivariada revelou que ISG, SS, PS e o fato de ser sítio interproximal exerceram influência significativa nas aferições de PII em relação à PID (p≤0,05). A cada milímetro a mais de PS, a PII aumenta em até 0,38mm. Conclusão: Considerando-se a superestimação do dano periodontal determinada pelo método indireto, sugere-se que a PID seja preferível para a realização de exames periodontais, como uma forma de minimizar vieses de aferição. / Objective: This observational study sought to compare two methods for determination of clinical attachment loss (CAL) in periodontal evaluation. Materials and methods: The sample comprised 75 exams from periodontal patients (50.9±8.02 years; 72,2% female; 49.4% nonsmokers). Visible plaque index (VPI), gingival bleeding index (GBI), probing depth (PD), bleeding on probing (BOP), clinical attachment loss (CAL), and gingival recession (GR) were assessed at six sites per tooth. Direct clinical attachment level (CALD) measurements were obtained by measuring the distance between the CEJ and the most apical of probe penetration in the gingival sulcus/periodontal pocket. Indirect clinical attachment level (CALI) was calculated by adding the PD and GR measurements of sites with gingival recession (GR+). These measurements were obtained by three calibrated raters. Means were calculated for each indicator. Data were assessed by cluster analysis. Data dependence was observed at the following hierarchical levels: patient/examination, tooth, and site. The methods were compared by means of the difference (d) between CALD and CALI values. Univariate and multivariate mixed models were then applied. A Bland–Altman plot was generated to ascertain the distribution of agreement between the two methods for CAL determination. Results: The mean CALD and CALI values were 3.96±2.07and 4.47±2.03, respectively. Therefore, the indirect method overestimated CAL (d=0.513±1.23). On multivariate analysis, GBI, BOP, PD, and interproximal site location had a significant influence on CALI measurements as compared with CALD (p ≤ 0.05). With each additional 1 mm in PD, CALI increased by up to 0.38 mm. Conclusion: Given the extent to which periodontal damage is overestimated by the indirect method, we suggest that the directly measured clinical attachment level is preferable for periodontal examination, so as to minimize measurement bias.
