Attitudes among Swedish medical personnel towarduniversal varicella vaccination and other new vaccines for children

Background: Thea ttitudes, knowledge, and experience of health personnel regarding vaccines and preventable diseases contribute importantlyto the success of vaccination programs. Aim: This study aimed to valuate the opinions of healthpersonnel involved in the care of children on the introduction of various new and older vaccines to the Swedish childhood vaccination. We particularly examined the knowledge of varicella diseaseas chickenpox and shingles and attitudes toward the varicellavaccine. Method: We created and administered aquestionnaire on vaccineprioritization forseveral vaccines, including hepatitis A and B,BCG(BacilleCalmette-Guérin) vaccine to preventtuberculosis, pneumococcal, meningococcal, HPV (human papilloma virus), rotavirus, influenza,respiratory syncytial virus,andTBE(tick bornencephalitis virus),and also explored health personnel’s knowledge about the VZV (varicella zoster virus) vaccine and its diseases. In 2006, the study targeted 600 nurses and physicians in Gothenburg, Sweden, whereas the current study in 2012 followed up with 160 school healthcare personnel. Results: The 2006 questionnaire generated 191/600 responses (32%), compared withthe 2012 follow-up questionnaire, which generated 40/160 (25%) responses from school health care personnel. Medical personnel ranked vaccination against hepatitis B highestin both studies. However, our data showed an important shift in attitude regarding HPV and rotavirus vaccination, which ranked lowestin 2006 but higher priority in 2012. Respondents also gave high priority to BCG. In 2006,only 34 of 138 respondents (25%) knew that a varicella vaccine was available, and universal varicella vaccination was generally ranked lower compared with other various vaccines. Additionally, pediatricians and personnel from infectious diseases department in the hospital having direct experience with these verity of varicella and zoster diseases were more likely to support universal varicella vaccination. Interestingly, in 2012 only one third of school healthcarepersonnel favored universal varicella vaccination.The health professionals xpressed a general demand for information and in-depth nowledge about the newer vaccines. Conclusion: If Swedish authorities decide to implement universal varicella vaccine into the current successful vaccination program for children, relevant healthcare personnel will require further education about VZV vaccineand disease / <p>ISBN 978-91-982282-6-7</p>

Atitudes do pessoal de enfermagem relacionadas ao processo de enfermagem / Attitudes of nursing staff towards nursing process

INTRODUÇÃO: A legislação em enfermagem prevê o uso da Sistematização da Assistência de Enfermagem (SAE). As atitudes dos profissionais de enfermagem sobre o processo de enfermagem (PE) e o poder clínico que percebem ter para realizar mudanças são fatores potencialmente associados ao processo de implantação e manutenção da SAE. OBJETIVOS: Descrever as atitudes relacionadas ao PE (disposição sobre o PE e percepção de poder clínico) de auxiliares de enfermagem e enfermeiros; analisar associações entre atitudes relacionadas ao processo de enfermagem e variáveis selecionadas. MÉTODO: Estudo descritivo-exploratório de delineamento transversal, com amostra não probabilística de 973 auxiliares de enfermagem e 632 enfermeiros de 35 hospitais e ambulatórios vinculados à Secretaria de Estado da Saúde de São Paulo (86,9% do sexo feminino, idade média de 44,12 anos (DP= 9,55), 52,3% trabalhavam em hospitais gerais, cerca de 44 horas semanais, estavam nas instituições há 10 anos em média). Os dados foram coletados por meio de resposta ao instrumento de caracterização sociodemográfica e aos questionários padronizados Posições sobre o Processo de Enfermagem (PPE) adaptado para esse estudo e o Power as Knowing Participation in Change Tool- versão brasileira (PKPCT), que apresentaram boas estimativas de validade e confiabilidade. Testes não paramétricos foram usados para analisar associação entre as atitudes relacionadas ao PE e as variáveis selecionadas. RESULTADOS: O escore total médio no PPE foi de 112,37 (DP=22,28) e o escore total médio no PKPCT foi de 281,12 (DP= 38,72) para os 1605 respondentes. Houve associação entre as duas atitudes relacionadas ao PE e categoria profissional, a atitude dos enfermeiros sobre o PE foi mais favorável que a dos auxiliares de enfermagem (p=0,024) e a percepção de poder dos enfermeiros foi mais elevada que a dos auxiliares de enfermagem (p=0,004). Houve correlação positiva moderada entre os escores do PPE e do PKPCT para os auxiliares (r=0,480; p=0,000) e para os enfermeiros (r=0,460; p=0,000). Os auxiliares do sexo feminino tiveram disposição sobre o PE (111,93) mais favorável que os do sexo masculino (106,11) (p=0,018) e maior percepção de poder (279,75 vs 270,84) (p=0,020); na amostra de enfermeiros não houve associação entre essas variáveis. Apenas para os auxiliares de enfermagem ter pós-graduação associou-se significativamente a maiores médias de atitude sobre o PE (119,0, p=0,011). As enfermeiras que relataram ter cargo de chefia apresentaram escores de percepção de poder mais elevados (293,52) que as enfermeiras sem cargo de chefia (282,61 p=0,010), mas não houve associação entre cargo de chefia e atitude sobre o PE. As associações entre os escores do PPE e PKPCT e as variáveis idade, tempo de formado, atividade principal, satisfação com a carreira, satisfação com o local de trabalho, conhecimento e contato com a SAE foram de intensidade desprezível no caso das correlações ou não significantes ao nível de 5%. / INTRODUCTION: The Brazilian nursing legislation requires that the nursing process (NP) is documented in all health institution. Attitudes of nursing staff towards the nursing process (NP) and their perception on their own clinical power to make changes are factors potentially associated with implementation and maintenance of the NP. OBJECTIVES: To describe the attitudes towards NP (attitudes on NP and perception of clinical power) of auxiliary nurses and baccalaureate nurses; and to analyze associations between attitudes towards the NP and selected variables. METHODS: An exploratory descriptive study with cross-sectional design, with a non-probabilistic sample of 973 auxiliary nurses and 632 baccalaureate nurses from 35 health agencies linked to the Ministry of Health of São Paulo, Brazil (86.9% female, mean age = 44.12 years (SD = 9.55), 52.3% worked in general hospitals, about 44 hours per week, were in institutions for 10 years on average). Data were collected by self-report using a form for personal, educational and other social data, and the Positions on Nursing Process (PNP) tool, adapted for this study, and the Power as Knowing Participation in Change Tool-Brazilian version (PKPCT), which showed good estimates of validity and reliability. Nonparametric tests were used to test associations between attitudes towards the NP and the selected variables. RESULTS: The mean PNP total score was 112.37 (SD = 22.28) (possible range = 20-140) and the mean PKPCT total score was 281.12 (SD = 38.72) (possible range = 48-336) for 1605 subjects. There was an association between the two PNP-related attitudes and professional category; the attitude of baccalaureate nurses on the NP was more favorable than that of auxiliary nursing (p = 0.024) and perceived clinical power of the baccalaureate nurses was higher than that of the auxiliary nurses (p = 0.004). There was moderate positive correlation between the scores of the PNP and PKPCT for auxiliary nurses (r = 0,480, p = 0.000) and baccalaureate nurses (r = 0.460, p = 0.000). The female auxiliary nurses had higher scores on PNP (111.93vs 106.11; p = 0.018) and PKPCT (279.75vs. 270.84; p = 0.020) than the male ones. There were no associations between sex an attitudes towards NP among baccalaureate nurses. Only for auxiliary nurses sample attending a continuing education program was associated with higher NP scores (119.0, p = 0.011). Baccalaureate nurses serving in leadership positions had higher mean total score on PKPCT (293.52) than nurses serving in direct care (282.61 p = 0.010), but there was no association between leadership position and PNP scores. Association tests for scores of the PNP and PKPCT and the variables age, time since graduation, main activity, satisfaction with the career, satisfaction with the workplace, knowledge on NP and contact with the NP resulted in negligible correlation coefficients or non-significant coefficients at 5%.

Detta ledde till att personalen försökte undvika konflikter med dem och de tyckte även att det var svårt att kommunicera med dem. Attityderna varierade beroende på vilken sjukdom personen led av. Sjukvårdspersonals attityder till patienter med borderline personlighetsstörning med eller utan självskadebeteende : En litteraturstudie

BAKGRUND: Litteraturen antyder att sjuksköterskor stigmatiserar och diskriminerar patienter med vissa diagnoser, i synnerhet schizofreni och borderline personlighetsstörning (BPS). BPS beskrivs som den psykiatriska diagnos med störst laddning och kring vilken det florerar flest stereotypa fördomar. SYFTE: Genom granskning av befintlig forskning i form av originalartiklar publicerade i vetenskapliga tidskrifter avser denna studie att sammanställa befintlig kunskap om sjukvårdspersonals attityder till patienter med borderline personlighetsstörning med eller utan självskadebeteende. METOD: Systematisk litteraturstudie. Litteratursökningar utfördes i databaserna PubMed, CINAHL och SCOPUS. RESULTAT: 21 studier inkluderades i resultatet. En rad olika studier påvisade en allmänt negativ attityd gentemot patienter med BPS med eller utan självskadebeteende. Vårdpersonal skattade mer negativa erfarenheter av att vårda patienter med BPS i jämförelse med schizofreni och depression. Vårdpersonal tenderade att skatta patienter med BPS som mer farliga, mindre trovärdiga samt ansåg att de hade mindre interpersonella färdigheter. SLUTSATS: Vårdpersonal, inte minst sjuksköterskor uppvisar negativa attityder i en icke försumbar utsträckning till patienter med BPS med eller utan självskadande beteenden. Detta kan leda till en inadekvat vård och behandling för den berörda patientgruppen. / BACKGROUND: The literature shows that nurses stigmatize and discriminate against patients with certain diagnoses, particularly schizophrenia and borderline personality disorder. Borderline personality disorder is generally described as being among the more heavy psychiatric conditions, and therefore very mired in stereotypes and prejudices. AIM: To summarize the current knowledge regarding attitudes of health professionals relating to borderline personality disorder and self-harm. METHOD: Systematic literature review. Literature search was made in the databases PubMed, CINAHL and SCOPUS. RESULTS: 21 studies were included in the results. A variety of studies demonstrated a general attitude of negativity toward patients with BPD with or without tendencies of self-harm. Clinicians reported more negative experiences working with BPD patients than with schizophrenia or depression patients. Caregivers tended to assess BPD patients as being more dangerous, less credible and having less interpersonal skills. CONCLUSION: Healthcare professionals, especially nurses, exhibit negative attitudes to a significant extent toward BPD patients as well as patients with self-harm tendencies. This may lead to inadequate care.

Sjuksköterskans bemötande av patienter med substansbrukssyndrom ur patienters perspektiv.

Bakgrund: Ett bra bemötande karaktäriseras av ett respektfullt, kompetent och empatiskt samspel som är individanpassat. Patienter med substansbrukssyndrom är en grupp som uppfattas som vårdkrävande och är föremål för stigmatisering, därmed kan bemötandet ses som en utmaning. Patientcentrerad vård innebär att sträva efter individens välmående. Med hjälp av ett motiverande förhållningssätt kan sjuksköterskan öka patienternas empowerment. Syfte: Att undersöka hur patienter med substansbrukssyndrom upplever bemötandet från vårdpersonal, med fokus på sjuksköterskor. Metod: Litteraturöversikt där både kvalitativa och kvantitativa artiklar ingick. Resultat: Tre kategorier angående bemötande identifierades: att se hela individen, vårdande möte och stigma. Under varje kategori urskildes både positiva och negativa upplevelser av bemötande. Diskussion: Ett patientcentrerat förhållningssätt kan förstås som ett empatiskt, omtänksamt, engagerat och hjälpsamt bemötande. Många patienter hade upplevelser av diskriminering och stigmatisering inom vården, detta kunde delvis förklaras som en maktobalans i relationen mellan vårdgivare och patient.  Slutsats: Sjuksköterskans bemötande kan både öka och minska vårdlidande. Ett gott bemötande är betydelsefullt för att främja hälsa och optimera resultatet av vården. Omvårdnaden av personer med substansbrukssyndrom kan förbättras. / Introduction: The responsibility of the nurse is to have competence and to treat every patient with respect and empathy. Patients with substance use disorders are a group that can be viewed as a group with a large demand of care. They are exposed to stigmatization and therefor they need specialized care. The meaning of patient centered care is to strive to optimize every individual’s well-being. Nurses may increase patient’s empowerment through a motivating approach. Aim: To explore patients’ perceptions of the nurse-patient relation of patients with substance use disorder. Method: A literature review, including both qualitative and quantitative studies. Result: Three categories were identified: To see the whole person, caring encounter and stigma. Both positive and negative aspects of every category were discovered. Discussion: A patient centered approach can be understood as an emphatic, caring, committed and helpful encounter. Many patients had experiences of discrimination and stigma during care. This could partly be understood as an unbalanced power-control relationship between caregiver and patient. Conclusion: The nurses actions can increase as well as decrease patients suffering. A caring encounter is important to promote health and to optimize the results of care. There is room for development of nursing skills in care of patients with substance use disorders.

The overall aim of the present thesis was to investigate experiences of living with multidrug-resistant bacteria (MDRB), using methicillin-resistant Staphylococcus aureus (MRSA) colonization as an illustration, and to develop and validate a tool to describe healthcare personnel’s attitudes towards patients with MDRB. A further objective was to study MRSA-colonized persons’ and healthcare personnel’s experiences of patient-professional interactions and responsibilities for infection prevention. Four empirical studies were conducted. A total of 18 MRSA-colonized persons and 20 healthcare personnel were interviewed regarding their experiences, and a total of 726 RNs responded the MDRB Attitude Questionnaire. The findings revealed the difficulties associated with living with MRSA colonization, which was described as something uncertain, and as an indefinable threat that has to be managed in both everyday life and in contacts with healthcare. Interactions with healthcare personnel were described as unprofessional owing to personnel’s inappropriate behaviour and insufficient information provision. According to the personnel, achieving adequate patient-professional interactions required having knowledge and experiences of MRSA. They also experienced difficulties in providing tailored information to patients. The MRSA-colonized persons described their unwanted responsibility to inform healthcare personnel about the colonization, but also felt responsible for limiting the spread of infection to others. Furthermore, responsibility for infection control was regarded as shared between healthcare personnel and patients. The personnel described such responsibility as a natural part of their daily work, although it was not always easy to adhere to hygiene precautions. The MRSA-colonized persons felt that healthcare personnel have insufficient knowledge of the bacteria and of hygiene precautions. The MDRB Attitude Questionnaire showed that registered nurses do have knowledge deficiencies. The MDRB Attitude Questionnaire has adequate psychometric properties. In conclusion, MRSA colonization constitutes a psychological strain for carriers, and interactions with healthcare personnel resulted in feelings of stigmatization. The present thesis indicates that there is a need to improve healthcare personnel’s knowledge, behaviour and emotional response in relation to patients with MDRB, in order to ensure patient safety and address patients’ needs. The heads of department is responsible for such improvements, and the MDRB Attitude Questionnaire is useful in identifying areas in need for improvement.

Vårdpersonalens känslor i mötet med den våldsutsatta kvinnan inom akutsjukvården

Syfte: Att undersöka vårdpersonalens känslor i mötet med våldsutsatta kvinnor inom akutsjukvården. Metod: Kvalitativ studie med halvstrukturerad intervjumetod. Tretton sjuksköterskor och undersköterskor inom akutsjukvården blev intervjuade om mötet med våldsutsatta kvinnor genom tio halvstrukturerade frågor. Resultat: Vårdpersonalen saknade kunskap om våld i nära relationer och kände sig inte beredda att möta en kvinna utsatt för våld trots framtagna riktlinjer och handlingsplaner. Mötet med våldsutsatta kvinnor väckte många starka känslor som ilska och frustration, men också medkänsla och empati. Frågan om våldsutsatthet upplevdes som komplicerad att ställa av vårdpersonalen. Faktorer som tidsbrist och bristen på avskildhet försvårade arbetet. Vårdpersonalen ansåg att det var akutsjukvårdens ansvar att ställa frågan, men det var oklart vem som bar det yttersta ansvaret. Slutsats: För att kvinnor utsatta för våld ska få ett gott omhändertagande behöver vårdpersonalen inom akutsjukvården mer utbildning och tid. Det är av betydelse att arbetsledningen eftersträvar ett öppet klimat för diskussion kring dessa frågor och markerar att våld i nära relationer är en lika viktig fråga som andra medicinska tillstånd inom akutsjukvården. / Objective: To investigate health care professionals’ emotions in the meeting with women who experience intimate partner violence within emergency care. Methods: Qualitative study with semi-structured interview methodology. Thirteen nurses and nursing assistants within the emergency care shared their thoughts in interviews about the meeting with abused women through 10 semi-structured questions. Results: Health care professionals lacked knowledge about intimate partner violence and did not feel prepared to meet an abused woman despite developed guidelines and instructions. The meeting induced many strong emotions such as anger and frustration, but also compassion and empathy. The issue was perceived as complicated by the health care professionals. Factors such as lack of time and privacy hampered their work. They felt that it was the emergency care’s responsibility to address the issue, but with different views about whose obligation it was to raise the question. Conclusion: To provide good care for abused women, the health care professionals in the emergency care need more training and time. It is important that management encourages discussion and clarifies that intimate partner violence is just as essential as any other medical issues in the emergency care.
