Kineziterapijos efektyvumas, gydant stuburo juosmeninės dalies skausmus Druskininkų UAB "Lietuvos" sanatorijoje / The physical management of a patient with low back pain in sanatorium "Lietuva"

The topic of this work – „The physical therapy management of a patient with low back pain in sanatorium „Lietuva“. The study was caried out in sanatorium „Lietuva“ Druskininkai from 2003 to 2004. 60 participants took part in the study. The age of participants from 18 to 74. First group (9 men and 21 women) with low back pain to continue exercing after treatment at home and second control group (5 men and 25 women) noexercing at home. The course of treatment consisted 14 day 30 – 45 min two time by day 6 day by weak. The purpose of study carried out by us was determine the change of the pain and the functioal state of the back muscles in the course of these procedures. Pain intensity (visual analog skale) in the active rehabilitation group (p<0,05) than in the passive control treatmen group after treatment was 3,07 ± 0,14 points. The abdominal muscle strangeht and abdominal and lumbar muscle endurance change the exercise time in both groups but in the active exercise group significanty more (p<0,05) after 14 day treatment. The spine muscle flexibility was still greater in both groups after programmed exercise. The study show that the patient must to do actine exercise at home because the pain did not disappear after therapy.

Pratimų individualizacijos reikšmė gydant lėtinį nugaros skausmą / The importance of exercise individualization in choronic low back pain treatment

The purpose of the study was to find out the importance of exercise individualization treating chronic low back pain. Methods: 32 chronic low back pain pacients, 32.12±4.83 years of age, were divided into two groups under research. The first group took part in 14 weeks general trunk muscles strengthening program. The second group patients obtained individual 14 weeks trunk muscles strengthening programs. Individual programs were made by physiotherapist in accordance with trunk muscles static and dynamic endurance characteristics which were measured and evaluated with trunk muscles capability tests. The capability of trunk muscles of all patients were measured and evaluated before and after exercise program using tests suggested by D. J Magee in year 2000. At the same time the intensity of experienced pain was evaluated using Phoenix recommendations (1999). Results: Both exercise programs significantly reduced chronic low back pain and also amplified trunk muscles static and dynamic endurance. The effect of individualized exercise treatment programs for reducing pain was significantly higher. The average intensity of pain dropped from 5.69 to 2.25 in the general exercise group, and from 5.63 to 0.94 in the individualized exercise programs group. The improvement in the individualized group was significantly higher than that in the general group, i.e. 4.69 ± 1.78 and 3.44±1.71 respectively. Trunk postural demands of physical occupational activities for women in Benin

Women in Benin commonly participate in physically demanding activities that involve the carriage of heavy loads on the head and back. These strenuous tasks combined with pregnancy can result in back pain that may persist after delivery in some cases. The objective of this study was to examine how the trunk postures of pregnant women in Benin were affected by their occupational activities. This study also examined trunk postures, as well as postures of the head relative to the trunk, in the specific task of carrying loads on the head. Finally, the instrument used in this study to measure trunk postures, the Virtual CorsetTM (VC) (Microstrain, Williston, VT, USA), was validated against a system of potentiometers. Questionnaires completed by 26 pregnant and 25 non-pregnant subjects revealed that 58% of pregnant women suffered from back pain since the start of pregnancy. An average of 328 instances of trunk flexion at angles larger than 60° were recorded during the workdays of 17 pregnant women, while 66 of those flexions events were held for more than four seconds. Furthermore, an average of 36% of the recorded workday was spent in trunk flexion at angles exceeding 20°. Trunk postural data, at C7 and S1, as well as sagittal positions of the head relative to the trunk were compared between pregnant and non-pregnant subjects and between unloaded and loaded walking conditions for the specific task of head load carriage. These comparisons showed that load on the head significantly increased upper trunk extension and lateral bending of the upper trunk towards the left during walking. Motion of the head relative to the trunk and motion of the upper trunk significantly decreased in the loaded condition and was compensated by increased motion at the sacrum level. In the validation study, the VC was moved at different speeds to observe the effects of accelerations on the angle measurements. USE OF ACCEPTABILITY AND USABILITY TRIALS TO EVALUATE VARIOUS DESIGN ITERATIONS OF THE PERSONAL LIFT ASSISTIVE DEVICE (PLAD)

The purpose of this research is twofold: 1) to apply a user-centred approach to examine the usability and acceptability of an on-body ergonomic aid called the Personal Lift Assistive Device (PLAD) in a variety of industrial work environments and 2) to utilize subjective user acceptability data on discomfort, pain and overall PLAD assessment by participants to identify design features that need improvement in the next PLAD iteration. Case studies took place in four different industrial work environments: two distribution centre environments, one retail store environment and one automotive assembly plant environment. These environments were selected to represent different industrial work environments. In total, 20 industrial workers were selected to wear the PLAD and provide their feedback. Results were obtained using subjective questionnaires through verbal and written comments as well as through direct observation of the participants. Three major design changes occurred to the original PLAD iteration tested at the first industrial location. These included: 1) using different stiffness levels of springs for different sizes of individuals in an attempt to decrease discomfort and optimize the effectiveness of the PLAD, 2) an increase in pelvic spacer size and surface area contacting the user’s body to reduce discomfort and 3) modification to the shoulder harness including; shape, dimensions and amount of cushioning to reduce discomfort. Currently, three major design changes were identified and still need to be addressed. These include: 1) reduction of thermal discomfort caused by wearing the PLAD, 2) validation of selecting the appropriate spring stiffness for small, medium and large users of the PLAD (male and female) under various working postures and 3) simplification of the donning and doffing process. Additionally, a simple PLAD spring stiffness sizing chart was created to aid potential manufacturers of the PLAD to select the appropriate spring stiffness for various users of the PLAD. <p>Low back pain (LBP) is a growing health and socio-economic problem worldwide, affecting humans from adolescent to adult age. In developed countries, more than 80% of adults are at risk of suffering a disabling episode of LBP at one point during their life time. In developing countries, particularly in Africa, the life time prevalence of LBP varies in population groups, but the disability due to LBP is increasing. The aetiology of LBP is multifactorial, and there is still no consensus on the exact cause and contributing factors to LBP. In addition, little is known about patients&rsquo / knowledge and beliefs on the contributing factors to their LBP. The current study therefore, aimed to identify patients&rsquo / knowledge, attitudes and beliefs on the contributing factors to LBP, among patients attending physiotherapy outpatient departments in Malawi.</p>

Temos aktualumas: Nugaros skausmas – dažna vidutinio amžiaus žmonių problema, kuri pastebimai didėja. Pagrindinis gydymo tikslas – stiprinti liemens raumenis. Tyrimo objektas: 22 – 45 metų amžiaus moterų bei vyrų nugaros skausmas. Tyrimo tikslas: Nustatyti kineziterapijos poveikį suaugusiųjų nugaros skausmui. Tyrimo uždaviniai: 1. Nustatyti tiriamųjų nugaros skausmo intensyvumą prieš kineziterapiją ir po jos. 2. Palyginti tiriamųjų vyrų bei moterų nugaros skausmą prieš ir po kineziterapijos užsiėmimų. 3. Įvertinti tiriamųjų gyvenseną. Tyrime dalyvavo 10 tiriamųjų, iš jų 5 vyrai ir 5 moterys. Tris mėnesius keturis kartus per savaitę buvo pravedami kineziterapijos užsiėmimai žmonėms, jaučiantiems nugaros skausmą. Taikant specialias skausmo vertinimo skales buvo vertintas tiriamųjų nugaros skausmas prieš ir po kineziterapijos užsiėmimų. Išvados: 1. Po kineziterapijos užsiėmimų tiriamųjų nugaros skausmas reikšmingai sumažėjo (p<0,05). 2. Prieš ir po kineziterapijos užsiėmimų nugaros skausmas tarp vyrų ir moterų nereikšmingai skyrėsi. 3. Visi tiriamieji (100%) dirbo sėdimą darbą, laisvalaikį pasyviai leido 50% tiriamųjų, 70% tiriamųjų manė, kad per pastaruosius metus patiria streso daugiau nei kiti, 60% tiriamųjų miegui skyrė pakankamai laiko, tiek pat tiriamųjų ryte atsikeldavo nežvalūs. / Problem: Back pain – is the most popular problem in the middle age people, which is growing significantly. The main goal of treatment – to strengthen the trunk muscles. Research object: Low back pain in 22 − 45 years of age men and women. Aim: to evaluate the impact of physical therapy on back pain in adults. Tasks: 1. To evaluate intensity of back pain before and after physical therapy. 2. To compare intensity of back pain between men and women before and after physical therapy. 3. To evaluate life style of subjects. The study involved 10 subjects, including 5 men and 5 women. Physical therapy was applied for three months, four times a week for people experiencing back pain. Back pain intensity was evaluated by specific pain scales before and after the physical therapy sessions. Conclusions: 1. Intensity of back pain decreased significantly after physical therapy. 2. Before and after physical therapy intensity of back pain differed insignificantly between men and women. 3. All subjects (100%) worked a sedentary job, leisure time spent passively 50% of subjects, 70% of them felt that over the past year they experienced stress more than others, 60% of subjects had enough time to sleep, and the same amount of subjects woke up in the mornings feeling not well.

Vis dažniau yra diskutuojama apie minkštojo įtvaro poveikį nėščiųjų juosmeninės stuburo dalies skausmo mažinimui. Yra skausmo sindromų, būdingų tik moterims. Vienas jų – nėščiųjų juosmens skausmas. Apie 50–80 proc. nėščiųjų vargina juosmens skausmai, kurie dažniausiai prasideda 5–7 nėštumo mėnesį, tačiau skausmai gali prasidėti 8—12 nėštumo savaitę (Samėnienė, 2005). Tikslas. Nustatyti minkštojo įtvaro poveikį nėščiosioms mažinant juosmeninės stuburo dalies skausmą. Objektas. Minkštojo įtvaro poveikis mažinant nėščiųjų juosmeninės stuburo dalies skausmui. Hipotezė. Manome, kad minkštasis įtvaras turės teigiamą poveikį nėščiosioms mažinant juosmeninės stuburo dalies skausmą, kadangi minkštojo įtvaro dėka yra suteikiama atrama ir stabilumas sąnariams ir raumenims netrikdant jų kraujotakos ir nevaržant judesių. Tyrimo metu buvo spėjama, kad minkštasis įtvaras turės teigiamą poveikį nėščiosioms mažinant juosmeninės stuburo dalies skausmą Tyrimo uždaviniai: 1. Įvertinti nėščiųjų juosmeninės stuburo dalies skausmą ir negalią prieš minkštojo įtvaro taikymą ir po. 2.
