Burnout among young physicians and its association with physicians’ wishes to leave

Background: Concerns about burnout, and its consequences, among German physicians are rising. However, data on burnout among German physicians are scarce. Also, a suspected association between burnout and German physicians’ wishes to leave remains to be studied. Therefore, the extent of burnout, and the association between burnout and wishes to leave clinical practice or to go abroad for clinical work was studied in a sample of young physicians in Saxony. Methods: In a cross-sectional survey, all physicians ≤40 years and registered with the State Chamber of Physicians of Saxony, Germany (n = 5956) received a paper-pencil questionnaire inquiring about socio-demographics, job satisfaction, and wishes to leave clinical practice or to go abroad for clinical work. Response rate was 40 % (n = 2357). Burnout was measured with the German version of the Maslach Burnout Inventory - Human Services Survey (MBI) consisting of the subscales emotional exhaustion (feeling emotionally drained), depersonalization (feelings of cynicsm) and personal accomplishment (feelings of personal achievement in job). Variables associated with burnout, and the association between burnout and wishes to leave were assessed in multivariate logistic regression analyses. Results: For emotional exhaustion participants reached a mean of 21.3 [standard deviation = 9.74], for depersonalization a mean of 9.9 [5.92], and for personal accomplishment a mean of 36.3 [6.77]. Men exhibited significantly higher depersonalization than women (11.3 [6.11] versus 9 [5.62], p < 0.001). Eleven percent of participants showed a high degree of burnout on all subscales, while 35 % did not show a high degree of burnout on any subscale. Confirming that one would become a physician again, and higher satisfaction with the components \"work environment\" and \"humaneness\", were associated with a lower chance for a high degree of burnout on all subscales. Higher emotional exhaustion and lower personal accomplishment were associated with an increased chance of wishing to leave clinical practice. Higher emotional exhaustion and higher depersonalization were associated with an increased chance of wishing to go abroad for clinical work. Conclusions: Preventing physician burnout may not only benefit the affected individual. It may also benefit the health care system by potentially preventing physicians from leaving clinical practice or from going abroad for clinical work.

An Exploration of Burnout in Individuals with Type D Personality

There are numerous physical and mental health implications associated with burnout and Type D personality (TDP). TDP is defined by the presence of specific levels of both negative affectivity and social inhibition. The purpose of this research was to examine the severity and prevalence of burnout in working adults with TDP in comparison to those without TDP. Social cognitive theory was the theoretical foundation for this study. Online surveys were used to gather responses to the Type D Scale-14 (DS14), the standard for measure for assessing TDP, and the Burnout Measure, Short Version (BMS) from 333 participants. Quantitative analyses included the use of t tests, chi square tests, correlation, and regression analysis to determine (a) if there is a disparity in the severity and prevalence of burnout in individuals with and without TDP; (b) if levels of burnout correlate with levels of TDP; and (c) whether age, gender, or both moderate the relationship between burnout and TDP. According to study results, there was a difference in the prevalence of burnout between groups, as 25.5% of the 143 participants with TDP had burnout compared to 9.3% of the 190 participants without TDP. Mean scores on the BMS were also higher, indicating a significantly greater level of burnout severity for participants with TDP. A positive correlation was found between severity of TDP and severity of burnout. Age was found to moderate the relationship between burnout severity and TDP, but did not affect the relationship between burnout prevalence and TDP. Gender did not have any impact on burnout in individuals with TDP. Neither age nor gender affected the prevalence or severity of burnout in individuals without TDP. These results can be beneficial in healthcare environments for the development of treatments and preventative measures for patients, as well as used by businesses, which have increased expenditures associated with employee burnout.

One of the most valuable assets in any organisation is the employees, although a small percent of employees in organisations are truly motivated and energised. It is essential for organisations to look into the engagement of its employees. This mini-dissertation focuses on a comparative study of the occupational wellness between lecturers and the technical IT support staff of an academic institution of the North West province in South Africa. The objectives were to determine and compare the engagement of employees in the categories mentioned above. Thereafter a comparison was made with specific aspects of the work and work environment. Literary research was done in order to discuss and conceptualise terms such as wellness and health, occupational stress and occupational wellness. Thereafter the burnout and engagement concepts were discussed. Empirical research was done by using questionnaires that were disseminated among the two different employee groups identified for the study. Firstly, the Work and Well-being questionnaire was used to determine the work engagement levels of the different groups. This questionnaire focussed on the vigour, absorption and dedication dimensions of the study sample. In this survey the reliability indicated that all the dimensions of engagement were accepted and that employees of the IT support staff group have an overall higher level of work engagement than the lecturer group. Secondly, the Job Resources questionnaire was used to evaluate how specific aspects of the work and work environment are experienced by both IT personnel and lecturing personnel. Results gained from the Cronbach‟s alpha coefficient method indicated that the questionnaire regarding factors like emotional overload, achievement, collegiality, supervision, information regarding performance and satisfaction with pay were indicated as reliable and accurate. The results from the Job Resources questionnaire indicated that emotional overload and feelings of achievement in the work environment is higher in the IT group than for lecturers. The rest of the factors tested were significantly equal for both groups. The final chapter takes the information from the previous chapters to reach a conclusion. The key finding was that the IT sample has a higher level of absorption and dedication of engagement than the lecturer sample. Secondly, the findings were that the emotional overload and achievement for the sample population is higher for the IT staff than the lecturing staff. The collegiality, supervision, information regarding performance and satisfaction with pay were at the same level for both the IT group and lecturer group. El Síndrome de Burnout constituye la respuesta al estrés laboral crónico que se manifiesta en empleados o trabajadores que están en constante contacto con personas que tienen motivos de sufrimiento. Se caracteriza por la presencia se síntomas como el agotamiento emocional, despersonalización y realización personal. Ocasiona graves daños en la calidad de vida del profesional, así como importantes repercusiones en la calidad asistencial. Entre este tipo de trabajadores se encuentra el profesional de enfermería el cual esta expuesto frecuentemente a diversas situaciones de estrés además de estar en contacto el mayor número de horas con su paciente que por naturaleza presenta diversos tipos de sufrimiento, por tanto, es considerado vulnerable a padecer este tipo de síndrome. En este trabajo se determinó el nivel de Síndrome de Burnout que existe en Enfermeras del Hospital Nacional Especializado “Víctor Larco Herrera”, así como el nivel de Síndrome de Burnout en la dimensión de Agotamiento Emocional, la Despersonalización y Realización Personal en estas. El estudio es de nivel aplicativo, tipo cuantitativo, método descriptivo, de corte transversal. La población esta conformada por las Enfermeras de Hospital de Salud Mental y Psiquiatría “Víctor Larco Herrera”. La técnica usada una encuesta autoadministrada y el instrumento empleado es el Inventario de Maslach Burnout Inventory (MBI). Se aplicó la encuesta a un total de 64 enfermeras, al procesar y analizar los datos, se concluyó que un 53.1 % presentan nivel medio del Síndrome de Burnout y un 26.6 % un nivel bajo, sin embargo existe un 20.3 % con un nivel alto. Así mismo la mayoría de las enfermeras presentaban un nivel medio de agotamiento emocional y realización personal y un nivel bajo respecto a despersonalización. This Sindrome have different symptoms like the emotional exhaustion, despersonalization and low personal accomplishment. Cause grave damages in the quality life of the professional, as well as important repercussions in the helping quality. Between this workers' type are the nurses professionals which is exposed frequently to various stress situations and to be in constant contact with his patients, so la nurses profesionals is considered vulnerable to suffer this syndrome. In this study was determined the level Síndrome's Burnout in Hospital´s Nurses of the National Specialized Victor Larco Herrera and determined Burnout's Síndrome in the dimension of Emotional exhaustion, the Despersonalization and Personal Accomplishment. The study is from application level, quantitative type, descriptive, transverse- court method. The population was conformed by Hospital's Nurses of the National Specialized Victor Larco Herrera. The technique used was an auto-administered opinion poll and the instrument used was Maslach Burnout Inventory's Inventory (MBI). The results obtained were of 64 nurses' total, a 53,1 % presented a half level of Burnout's Syndrome and a 26,6 % presented a low level, however a 20,3 % present a tall level. So the majority of nurses presented a half a level of emotional exhaustion and Personal Accomplishment and a low level of Burnout´s Sindrome. Key words: Burnout's syndrome, nurses.

El síndrome de Burnout constituye un estado de fatiga o frustración, ocasionado por una falta de recompensa esperada, tras un esfuerzo dedicado a un proyecto de vida, precedido de una fase de falta de motivación, producción e ineficacia, constituyendo una de las principales causas de deterioro de las condiciones laborales. Este estudio de investigación estuvo dirigido a identificar el Nivel de Síndrome de Burnout y Estrategias de Afrontamiento en Enfermeros de los Servicios Críticos del Hospital Nacional Daniel Alcides Carrión (HNDAC), e identificar el agotamiento emocional, despersonalización y realización profesional, así como las estrategias de afrontamiento más aplicadas por los profesionales. La investigación es de nivel aplicativo, tipo cuantitativo, método descriptivo de corte transversal. El área de estudio fueron los Servicios Críticos del HNDAC. La población está conformada por 48 profesionales de enfermería. La exploración empírica se sustenta en la técnica de Maslach Burnout Inventory (MBI), a través de la encuesta, así como el Inventario sobre Estimación de Afrontamiento (COPE). Los principales resultados permiten afirmar que la mayoría 54.17% de los profesionales presentan un nivel medio de Síndrome de Burnout, a predominio de las dimensiones agotamiento emocional (54.17%) y despersonalización (54.17%) y realización personal (52.1%). Las estrategias de afrontamiento son aplicadas por la mayoría (79.20%) de los profesionales de enfermería. The investigation is of aplicativo level, quantitative type, descriptive method of cross section. The study area was the Services Critics of the HNDAC. The population is conformed by 48 professionals of infirmary. The empirical exploration is sustained in the technique of Maslach Burnout Inventory (MBI), through the survey, as well as the Inventory on Estimation of Facing (COPE). The main results allow to affirm that majority 54,17% of the professionals presents/displays a mean level of Syndrome of Burnout, to predominance of the dimensions emotional exhaustion (54,17%) and de-personalisation (54,17%) and personal accomplishment (52,1%). The facing strategies are applied by the majority (79,20%) of the infirmary professionals. These values indicate that the population is in a process of Burnout, that gradually it looks for to surpass reason why it is important to take part it.

El propósito de este estudio fue determinar la prevalencia del Síndrome de Burnout en los odontólogos docentes de la Facultad de Odontología de la UNMSM en el año 2013, según factores demográficos, sociales y laborales; teniendo en consideración los criterios y normas éticas establecidas; siendo un estudio de tipo descriptivo, transversal y retrospectivo. La muestra estuvo constituida por la totalidad de docentes odontólogos de la Facultad de Odontología de la UNMSM en el año 2013, de ambos géneros y con edades entre 25 y 75 años. Se aplicó dos encuestas la primera donde se recopilo los datos personales concernientes a los aspectos demográficos, sociales y laborales; i la segunda encuesta, proporcionándoles el IBM (Inventario de Maslach y Jacson); donde se observó una prevalencia baja (5.1%) del síndrome de Burnout en los odontólogos docentes de la Facultad de Odontología de la UNMSM en el 2013 de la Facultad de Odontología de la UNMSM en el año 2013, los resultados arroja. Con respecto a la prevalencia de las subescalas del Síndrome de Burnout en odontólogos docentes n en las dimensiones Cansancio Emocional y Despersonalización un nivel bajo en su gran mayoría (67.9% y 65.4% respectivamente). Sin embargo, con respecto a la dimensión Realización Personal, un nivel alto (73.1%). Por consiguiente este trabajo, tiene como propósito contribuir en la elaboración de un perfil de riesgo del Síndrome de Burnout en muestra profesión.

The relationship between burnout and job Satisfaction among registered nurses at a hospital in Johannesburg

ABSTRACT Introduction: Registered nurses (RNs) are becoming increasingly dissatisfied with factors in the workplace; monthly resignations are related to organizational management issues, diminishing resources, staff shortage, increasing responsibilities and lack of support The purpose of this study: This research sought to determine the relationship between burnout and job satisfaction among registered nurses at a hospital in the city of Johannesburg. The methodology: A quantitative descriptive survey was used as study design. The Job Satisfaction scale of Warr et al (1979) was used to measure overall satisfaction with both extrinsic and intrinsic factors. The Maslach Burnout Inventory-Human Service Survey (MBI-HSS) was used to measure emotional exhaustion; depersonalization and personal accomplishment. The sample comprised 165 RNs, who met the inclusion criteria and who agreed to participate. Results: The RNs reported dissatisfaction with selected extrinsic factors in their work environment, such as the physical work condition, the way their institution is managed and the rate of pay. The RNs at this hospital are satisfied with hours of work, their immediate boss, their fellow workers and job security; they are dissatisfied with intrinsic factors of their job such as promotion opportunities and recognition. The majority (75.76%) of RNs reported moderate to high emotional exhaustion. There is a significant relationship between emotional exhaustion and job satisfaction factors. The results show a significant relationship between emotional exhaustion and depersonalization; 68.48% of RNs reported moderate depersonalization. There is a significant relationship between depersonalization and extrinsic factors. The majority of RNs (76.97%) reported high levels of personal accomplishment.

No. of bitstreams: 0 Previous issue date: 2010-12-21 / The burnout syndrome is defined as a health impairment derived from work, more specifically, the result of a prolonged exposure to stress in the work situation. However, it has also been described among students, although not yet fully exercising a profession, the study itself, internships and other academic activities may become significant sources of stress. This study investigates burnout in a sample of medical students from the city of Fortaleza that were regularly enrolled in medical courses and regularly attending classes. The sample, for convenience, was 300 students, in average, 22.58 years old, with 48.3% men and 51.7% women. The instrument used for the detection and measurement of the syndrome was the MBI-SS (Maslach Burnout Inventory - Students Survey). The student version of the burnout inventory (MBI-SS) was developed to assess the syndrome in students and has fewer items than the original version of the instrument. The inventory measures three dimensions: Emotional Exhaustion (EE), Cynism (CY) and Professional Efficacy (PE). A questionnaire containing socio-demographic information on age, gender, satisfaction with the course, free time, leisure time, costs with the course etc.; and a third called the International Physical Activity Questionnaire, IPAQ (long version 6) were also applied in order to verify if these variables have some influence on the onset of burnout among the students surveyed. The results, analyzed by means of multidimensional regression and chi-square tests, showed that, according to the criteria used nationally, the graduate students of the sample show no symptoms of burnout and that 51.2% of the students sample can be considered sedentary and insufficiently active. Moreover, there was no relationship between the practice of physical activities and the syndrome scales EE and CY. In the scale EP the level of physical activity was found to be a predictor variable, but the relationship was not in the expected direction. It was concluded that students in the sample, although considered as not showing signs of burnout, have a low physical activity index, which allows us to assert that, at least in the students surveyed, the level of physical activity does not minimize the effects of the stress of the medical course. No entanto, tem sido descrita também entre estudantes, pois, embora ainda não estejam exercendo uma profissão plenamente, o próprio estudo, estágios e outras atividades acadêmicas podem se constituir em fontes de estresse. Este estudo investigou a síndrome de burnout em uma amostra de estudantes de medicina da cidade de Fortaleza, que se encontravam regulamente matriculados nos cursos de medicina da cidade e frequentando periodicamente as aulas. A amostra, por conveniência, foi de 300 alunos com idade média de 22,58 anos, sendo 48,3% homens e 51,7% mulheres. Os instrumentos utilizados para a detecção e mensuração da síndrome foram o MBI-SS (Maslach Burnout Inventory Students Survey). A versão para estudantes do inventário de burnout (MBI-SS) foi desenvolvida para avaliar a síndrome em estudantes e possui um número menor de itens do que a versão original do instrumento. O inventário mede três dimensões: Exaustão Emocional (EE), Descrença (DE) e Eficácia Profissional (EP). Um questionário com questões sociodemográficas, contendo informações sobre idade, gênero, satisfação com o curso, tempo livre, horas de lazer, ajuda de custo etc; e um terceiro denominado Questionário Internacional de Atividade Física, IPAQ (versão 6 longa) foram também aplicados com o objetivo de verificar se essas variáveis têm alguma influência no aparecimento do burnout nos estudantes investigados. Os resultados, analisados por meio de regressão multidimensional e testes de Qui-Quadrado mostraram que, segundo o critério utilizado nacionalmente, os alunos de graduação em medicina, da amostra pesquisada, não apresentam sintomas de burnout e que 51,2% dos estudantes da amostra podem ser considerados sedentários e pouco ativos. Além disso, não foram encontradas relações entre a prática de atividades físicas e a síndrome nas escalas EE e DE. A escala EP apresentou, como variável preditora, o nível de atividade física, mas as relações não foram no sentido esperado. Conclui-se que os estudantes da amostra, embora não possam ser considerados como apresentando burnout, possuem um índice de atividade física baixa, o que permite afirmar que, pelo menos nos estudantes investigados, o nível de atividade física não minimiza os efeitos do estresse do curso de medicina.
