Computer aided characterization of degenerative disk disease employing digital image texture analysis and pattern recognition algorithms

Introduction: A computer-based classification system is proposed for the characterization of cervical intervertebral disc degeneration from saggital magnetic resonance images. Materials and methods: Cervical intervertebral discs from saggital magnetic resonance images where assessed by an experienced orthopaedist as normal or degenerated (narrowed) employing Matsumoto’s classification scheme. The digital images where enhanced and the intervertebral discs which comprised the regions of interest were segmented. First and second order statistics textural features extracted from thirty-four discs (16 normal and 16 degenerated) were used in order to design and test the classification system. In addition textural features were calculated employing Laws TEM images. The existence of statistically significant differences between the textural features values that were generated from normal and degenerated discs was verified employing the Student’s paired t-test. A subset with the most discriminating features (p<0.01) was selected and the Exhaustive Search and Leave-One-Out methods were used to find the best features combination and validate the classification accuracy of the system. The proposed system used the Least Squares Minimum Distance Classifier in combination with four textural features with comprised the best features combination in order to classify the discs as normal or degenerated. Results: The overall classification accuracy was 93.8% misdiagnosing 2 discs. In addition the system’s sensitivity in detecting a narrow disc was 93.8% and its specificity was also 93.8%. Conclusion: Further investigation and the use of a larger sample for validation could make the proposed system a trustworthy and useful tool to the physicians for the evaluation of degenerative disc disease in the cervical spine. / Σκοπός: Η στένωση των μεσοσπονδύλιων δίσκων της αυχενικής μοίρας, ως κύρια έκφραση εκφυλιστικής νόσου, είναι μια από τις σημαντικότερες αιτίες πρόκλησης πόνου στην περιοχή του αυχένα. Στην κλινική πράξη η αξιολόγηση της στένωσης γίνεται μέσω μέτρησης του μεσοσπονδύλιου διαστήματος, σε διάφορες απεικονίσεις της αυχενικής μοίρας του ασθενούς. Στην παρούσα εργασία προτείνεται μια υπολογιστική μέθοδος ανάλυσης εικόνας, για την αυτοματοποιημένη εκτίμηση της στένωσης από εικόνες μαγνητικής τομογραφίας. Υλικό και Μέθοδος: Μελετήθηκαν 34 μεσοσπονδύλιοι δίσκοι από οβελιαίες τομές μαγνητικής τομογραφίας της αυχενικής μοίρας, οι οποίες ελήφθησαν με χρήση Τ2 ακολουθίας. Η στένωση των μεσοσπονδύλιων δίσκων αξιολογήθηκε από έμπειρο ορθοπαιδικό βάσει της κλίμακας Matsumoto. Οι δίσκοι χωρίστηκαν σε δύο κατηγορίες: (α) 16 φυσιολογικοί και (β) 16 δίσκοι που παρουσίαζαν στένωση. Με χρήση διαδραστικού περιβάλλοντος επεξεργασίας εικάνας καθορίστηκε το περίγραμμα των μεσοσπονδύλιων δίσκων οι οποίοι αποτελούν τις προς ανάλυση περιοχές ενδιαφέροντος (Π.Ε.). Σε κάθε Π.Ε. εφαρμόστηκαν αλγόριθμοι εξαγωγής χαρακτηριστικών υφής. Συγκεκριμένα υπολογίστικαν χαρακτηριστικά υφής από στατιστικά πρώτης και δεύτερης τάξης καθώς και χαρακτηριστικά από τα μέτρα ενέργειας υφλης κατλα Laws. Τα παραπάνω χαρακτηριστικά, ποσοτικοποιούν διαγνωστικές πληροφορίες της έντασης του σήματος της Π.Ε. και συσχετίζονται με τη βιοχημική σύσταση των απεικονιζόμενων δομών. Τα εξαχθέντα χαρακτηριστικά υφής αξιοποιήθηκαν για τη σχεδίαση του ταξινομητή ελάχιστης απόστασης ελαχίστων τετραγώνων, ο οποίος χρησιμοποιήθηκε για το διαχωρισμό μεταξύ φυσιολογικών δίσκων και δίσκων που παρουσίαζαν στένωση (εκφυλισμένων). Αποτελέσματα: Η ακρίβεια της ταξινόμησης φυσιολογικών και εκφυλισμένων μεσοσπονδύλιων δίσκων ανήλθε σε 93.8%. Η ευαισθησία καθώς και η ειδικότητα της μεθόδου, σε ότι αφορά την ανίχνευση εκφυλισμένων δίσκων, είναι επίσης 93.8%. Συμπέρασμα: Με δεδομένο το μικρό μέγεθος του δείγματος που χρησιμοποιήθηκε για το σχεδιασμό της μεθόδου, απαιτούνται περετέρω εργασίες πιστοποίησης της ακρίβειας ταξινόμησης, προκειμένου η μέθοδος αυτή να αξιοποιηθεί από ακτινολόγους και ορθοπαιδικους, ως βοηθητικό διαγνωστικό εργαλείο.

Characterization of a Biodegradable Electrospun Polyurethane Nanofiber Scaffold Suitable for Annulus Fibrosus Tissue Engineering

The current study characterizes the mechanical and biodegradation properties of a polycarbonate polyurethane (PU) electrospun nanofiber scaffold intended for use in the growth of a tissue engineered annulus fibrosus (AF) intervertebral disc component. Both the tensile strength and initial modulus of aligned scaffolds were higher than those of random scaffolds and remained unaffected during a 4 week biodegradation study, suggesting a surface-mediated degradation mechanism. The resulting degradation products were non-toxic. Confined compressive mechanical force of 1kPa, was applied at 1Hz to in vitro bovine AF tissue grown on the scaffolds to investigate the influence of mechanical force on AF tissue production, which was found to decrease significantly at 72 hours relative to 24 hours, independent of any effects from mechanical forces. Overall, the consistent rate of PU degradation, along with mechanical properties comparable to those of native AF tissue, and the absence of cytotoxic effects, make this polymer suitable for further investigation for use in tissue-engineering the AF.

Intoxicação por monensina em búfalos. / Monensin toxicosis in water buffaloes

O primeiro artigo desse estudo apresenta a ocorrência de um surto de intoxicação por monensina em búfalos de um rebanho misto com bovinos, os quais não foram afetados. Tal fato sugeriu a possibilidade de que búfalos fossem menos tolerantes à monensina que bovinos. Embora com número reduzido de animais experimentais, dados preliminares foram compatíveis com essa hipótese. O segundo artigo desse trabalho descreve detalhadamente achados clínicopatológicos da intoxicação por monensina em búfalos, confirma a maior susceptibilidade dos búfalos (em comparação com bovinos) à monensina e determina a mínima dose tóxica de monensina para búfalos. Sinais clínicos e lesões característicos de intoxicação por monensina foram induzidos em búfalos dosados (1 dia) com 15, 10, 7,5 e 5 mg/kg de monensina. Apenas os búfalos dosados com 2,5 (1 dia) e 1 mg/kg (7 dias) de monensina não morreram. Os sinais clínicos iniciaram aproximadamente 6 h após dosagem com monensina e incluíram apatia, anorexia, diarréia, sialorréia, fraqueza muscular, taquicardia, dificuldade locomotora, dispnéia, distensão da jugular, decúbito e morte. As dosagens de creatinina quinase (CK) dos búfalos aumentaram acentuadamente após dosagem com monensina. As alterações macroscópicas foram ascite, hidrotórax, hidropericárdio, cardiomegalia, hepatomegalia e áreas pálidas focais no miocárdio e nos músculos esqueléticos. Degeneração e necrose de miofibras foram os principais achados histopatológicos. Os búfalos intoxicados naturalmente no surto desenvolveram predominantemente lesões nos músculos esqueléticos e os búfalos experimentais tiveram lesões cardíacas mais pronunciadas. Por outro lado, nenhuma evidência de doença, nem mesmo alteração nos níveis de CK, foram observados nos bovinos dosados com as mesmas dosagens de monensina, confirmando observações preliminares que esses animais são mais resistentes à monensina que os búfalos. / The first article of this study reports the occurrence of an outbreak of monensin toxicosis in water buffaloes from a feedlot in which buffaloes and cattle were kept together but only the former were affected. This suggested that buffaloes were less tolerant to monensin than cattle. Although tested with small number of experimental animals, preliminary data were consistent with this hypothesis. The second manuscript describes the clinicopathological findings in monensin toxicosis in water buffaloes, confirms that buffaloes are more susceptible to monensin than cattle, and presents the minimal toxic dosage of monensin to buffaloes. Typical clinical signs and lesions of monensin intoxication were induced in water buffaloes dosed with single doses of 15, 10, 7.5, and 5 mg/kg of monensin. Only buffaloes dosed with 2.5 mg/kg (1 d) and 1 mg/kg (7 d) survived. Clinical signs initiated approximately 6 h postdosing and included apathy, anorexia, diarrhea, drooling, muscular weakness, locomotion disorders, dyspnea, tachycardia, jugular distension and pulse, recumbency and death. The creatine kinase (CK) levels were highly augmented in blood samples of buffaloes dosed with monensin. Most prominent gross changes were ascites, hydrothorax, hydropericardium, cardiomegaly, hepatomegaly, and focal pale areas in the myocardium and in skeletal muscles. Degeneration and necrosis of myofibers were the main histopathological findings. Conversely, no evidence of disease, neither change in CK levels were observed in the beef cattle steers dosed with same doses, confirming preliminary findings that buffaloes are more susceptible to monensin than cattle. Buffaloes in field cases predominantly developed lesions in skeletal muscles, and those from the trial had cardiac lesions as the most pronounced changes. In addition, this report presents the minimal toxic dosage of monensin to buffaloes and suggests that CK tests may serve as monitoring tools in the management of buffalo herds supplemented with monensin.

Observou-se a exeqüibilidade e eficácia desta técnica cirúrgica e o comportamento do enxerto e dos parafusos durante o período de 120 dias, baseandose na capacidade dos exames clínicos, radiográficos e artroscópicos, em detectar o possível início e a progressão de alterações degenerativas secundárias na articulação do joelho. Para tanto foram utilizados oito animais da espécie canina pesando entre 17,2 e 26,4Kg. O enxerto utilizado correspondeu ao segmento composto pelo terço intermédio da patela, ligamento patelar e porção cranial da tuberosidade da tíbia, coletado de cadáveres de cães e conservado em glicerina 98%. Os oito cães foram separados em dois grupos, A e B e cada um contendo quatro animais para avaliação artroscópica (segunda observação) aos 60 e 120 dias do pós-operatório. Todos os cães foram submetidos ao procedimento cirúrgico no joelho direito. Clinicamente, avaliou-se o grau de claudicação, o perímetro muscular da coxa e o movimento de gaveta cranial nos oito cães no pré-operatório e semanalmente, até completarem-se 120 dias. Foi observada diminuição da circunferência da coxa significativa (p<0,05) após o período de dez dias em que os cães permaneceram com a bandagem compressiva, sendo recuperada de forma significativa até 13° semana. As pontuações obtidas na avaliação do movimento de gaveta (extensão e flexão) tornaram-se significativamente diferentes da pré-operatória entre a 5° e 15° semanas (p<0,05). Quatro cães apresentaram ao final do experimento ausência de instabilidade articular como na avaliação pré-operatória e os outros... / The aim of this study was to evaluate the use of patelar ligament allograft (BLB) conserved in 98% glycerin and fixated with interferemce screws as a substitute of the cranial cruciate ligament in dogs (CrCL). It was observed the feasibility and efficiency of this surgical technique and the graft and screws conduct along 120 days, basing on clinical, radiographic and arthroscopic capacity in detect the beginning and progress of secondary degenerative stifle joint changes. For this purpose, eight mongrel dogs weighting from 17,2 e 26,4 Kg were used. The graft consisted of the intermediate third of the pattela, patellar ligament and cranial portion of the tibial crest, harvested from dog cadavers and preserved in 98% glycerin.The eight dogs were randomly allocated in two groups, A and B, each one with four dogs for arthroscopic evaluation (second-look) at 60 and 120 days postoperative. All the dogs were submitted to the surgical procedure in the right knee. Clinically, lameness degree, thigh girth measurement and cranial-drawer test were evaluated in the eight dogs in preoperative, and weekly, until complete 120 days. A significant decrease in thigh girth (p<0,05) observed after ten days period that the dogs were using compressive bandage was recovered until the 13th week. The cranial-drawer test scores (flexion and extension) became significantly different between the 5th and 15th weeks postoperative (p>0,05). Four dogs presented at the final study period with no articular instability just like the preoperative evaluation, and the other four with just a slight craniocaudal instability. Diversas formas de tratamento, cirúrgicas ou conservativas, são descritas, entretanto a terapêutica da osteoartrite continua sendo grande desafio ao médico veterinário. Neste contexto, a pesquisa envolvendo células-tronco mesenquimais destaca-se na busca de melhorias e avanços na reparação da cartilagem articular. Objetivou-se, no presente projeto, comparar a regeneração cartilaginosa da articulação coxofemoral de coelhos, com e sem o transplante de células-tronco mesenquimais autólogas, por meio de exames radiográficos e histopatológicos. Dois grupos, com 15 animais da espécie leporina cada, foram submetidos à indução química de osteoartrite com solução de colagenase 2% na articulação coxofemoral direita. No Grupo 1 (Células-tronco) realizou-se a aplicação intra-articular de células-tronco mesenquimais autólogas, enquanto que, o Grupo 2 (Controle) foi constituído por animais submetidos à aplicação intra-articular de solução salina estéril. Foram realizadas avaliações radiográficas e histopatológicas aos 30, 60 e 90 dias após a aplicação. Os resultados histológicos deste ensaio indicam que células-tronco mesenquimais (Grupo 1) melhoraram discretamente a qualidade do tecido de reparo, de acordo com os critérios da escala semi-quantitativa ICRS 1 (“International Cartilage Repair Society”). O Grupo 1 (Células-Tronco) demonstrou superioridade em relação ao Grupo 2 principalmente nos parâmetros: Superfície articular, matriz extracelular e distribuição celular. / The articular cartilage has limited repair capacity, leading to an increased risk for degenerative changes, potentially irreversible. Several treatments, surgical or not, are described, however osteoarthritis remains a major challenge for the veterinarian. In this context, research involving mesenchymal stem cells stands out. The aim of this study was to compare cartilage regeneration of the hip in rabbits, with and without the transplantation of autologous mesenchymal stem cells. Radiographic and histopathological evaluation were used. Thirty rabbits were submitted to chemical induction of osteoarthritis with a 2% colagenase in the right hip. They were divided into 2 groups of 15 animals each: Group 1 (intra-articular application of autologous mesenchymal stem cells) and Group 2 (control - intra-articular application of sterile saline solution). Radiographic and histopathological evaluations were performed at 30, 60 and 90 days after application. The mesenchymal stem cells group (Group 1) showed slight improvement of the quality of the repair tissue, according to the semi-quantitative scale criteria ICRS 1 (International Cartilage Repair Society). The Group 1 (Stem Cells) showed superiority in relation to Group 2, specially in the parameters joint surface, extracellular matrix and cellular distribution.

O segundo artigo desse trabalho descreve detalhadamente achados clínicopatológicos da intoxicação por monensina em búfalos, confirma a maior susceptibilidade dos búfalos (em comparação com bovinos) à monensina e determina a mínima dose tóxica de monensina para búfalos. Sinais clínicos e lesões característicos de intoxicação por monensina foram induzidos em búfalos dosados (1 dia) com 15, 10, 7,5 e 5 mg/kg de monensina. Apenas os búfalos dosados com 2,5 (1 dia) e 1 mg/kg (7 dias) de monensina não morreram. Os sinais clínicos iniciaram aproximadamente 6 h após dosagem com monensina e incluíram apatia, anorexia, diarréia, sialorréia, fraqueza muscular, taquicardia, dificuldade locomotora, dispnéia, distensão da jugular, decúbito e morte. As dosagens de creatinina quinase (CK) dos búfalos aumentaram acentuadamente após dosagem com monensina. As alterações macroscópicas foram ascite, hidrotórax, hidropericárdio, cardiomegalia, hepatomegalia e áreas pálidas focais no miocárdio e nos músculos esqueléticos. Degeneração e necrose de miofibras foram os principais achados histopatológicos. Os búfalos intoxicados naturalmente no surto desenvolveram predominantemente lesões nos músculos esqueléticos e os búfalos experimentais tiveram lesões cardíacas mais pronunciadas. Por outro lado, nenhuma evidência de doença, nem mesmo alteração nos níveis de CK, foram observados nos bovinos dosados com as mesmas dosagens de monensina, confirmando observações preliminares que esses animais são mais resistentes à monensina que os búfalos. / The first article of this study reports the occurrence of an outbreak of monensin toxicosis in water buffaloes from a feedlot in which buffaloes and cattle were kept together but only the former were affected. This suggested that buffaloes were less tolerant to monensin than cattle. Although tested with small number of experimental animals, preliminary data were consistent with this hypothesis. The second manuscript describes the clinicopathological findings in monensin toxicosis in water buffaloes, confirms that buffaloes are more susceptible to monensin than cattle, and presents the minimal toxic dosage of monensin to buffaloes. Typical clinical signs and lesions of monensin intoxication were induced in water buffaloes dosed with single doses of 15, 10, 7.5, and 5 mg/kg of monensin. Only buffaloes dosed with 2.5 mg/kg (1 d) and 1 mg/kg (7 d) survived. Clinical signs initiated approximately 6 h postdosing and included apathy, anorexia, diarrhea, drooling, muscular weakness, locomotion disorders, dyspnea, tachycardia, jugular distension and pulse, recumbency and death. The creatine kinase (CK) levels were highly augmented in blood samples of buffaloes dosed with monensin. Most prominent gross changes were ascites, hydrothorax, hydropericardium, cardiomegaly, hepatomegaly, and focal pale areas in the myocardium and in skeletal muscles. Degeneration and necrosis of myofibers were the main histopathological findings. Conversely, no evidence of disease, neither change in CK levels were observed in the beef cattle steers dosed with same doses, confirming preliminary findings that buffaloes are more susceptible to monensin than cattle. Buffaloes in field cases predominantly developed lesions in skeletal muscles, and those from the trial had cardiac lesions as the most pronounced changes. In addition, this report presents the minimal toxic dosage of monensin to buffaloes and suggests that CK tests may serve as monitoring tools in the management of buffalo herds supplemented with monensin.

Estudo prospectivo randomizado entre a carpectomia proximal e a artrodese dos quatro cantos para tratamento da osteoatrose pós-traumática do carpo / Comparison of proximal row carpectomy and four corner arthrodesis: a prospective study

Introdução: Ao analisar a osteoartrose pós-traumática do punho a literatura demonstra que sua etiologia é, na maioria das vezes, secundária a lesões ligamentares ou a fraturas do carpo que, por sua vez, seguem um padrão definido e evolutivo de acometimento articular do punho. Várias abordagens cirúrgicas são descritas para o tratamento da osteoartrose pós-traumática do carpo. Considerando a carpectomia proximal e a artrodese dos quatro cantos, a literatura apresenta apenas estudos retrospectivos comparando os resultados funcionais entre as técnicas. O objetivo desta tese é comparar os resultados funcionais entre a carpectomia proximal e a artrodese dos quatro cantos no tratamento da osteoartrose pós-traumática do punho, sem acometimento da articulação mediocárpica. Método: Neste estudo prospectivo e randomizado 20 pacientes portadores de osteoartrose pós-traumática sem acometimento da articulação mediocárpica do punho, submetidos à carpectomia proximal ou a artrodese dos quatro cantos tiveram seus dados pós-operatórios analisados e comparados. Resultados: Todos os pacientes deste estudo referiram melhora da dor e da sua capacidade funcional. Em todos os casos houve diminuição dos valores pré e pós-operatórios de goniometria em ambos os procedimentos. Não houve diferença estatisticamente significante entre os procedimentos. Conclusão: Com relação aos resultados funcionais, a carpectomia proximal e a artrodese dos quatro cantos são procedimentos efetivos e semelhantes no tratamento dos pacientes portadores de SLAC/SNAC WRIST sem acometimento da articulação mediocárpica. / Introduction: Wrist arthritis results most of the times from ligaments tears or carpal bones fractures. Many surgical procedures are described in literature. Regarding proximal row carpectomy and four corner arthrodesis, there are only retrospective studies available in literature. The objective of this study is to compare the functional results between these two surgical procedures. Method: In this prospective and randomized study 20 patients underwent proximal row carpectomy or four corner arthrodesis for the treatment of wrist arthritis. In all patients the midcarpal joints were free of lesions. Functional results were compared. Results: Both proximal row carpectomy and four corner arthrodesis improved pain. All cases showed decreased range of motion after surgery. The differences between the groups were not statistically significant. Conclusion: Regarding functional results both procedures were similar and improved pain in patients with SLAC/SNAC WRIST without degenerative changes in midcarpal joint.
