Assessing the Effectiveness of a School-Based Dental Clinic on the Oral Health of Children Who Lack Access to Dental Care: A Program Evaluation

This program evaluation examines the effectiveness of a school-based dental clinic. A repeated-measures design was used to longitudinally examine secondary data from participants (N = 293). Encounter intensity was developed to normalize data. Multivariate analysis of variance and Kruskal–Wallis test were used to investigate the effect of encounter intensity on the change in decay, restorations, and treatment urgency. A Pearson’s correlation was used to measure the strengths of association. Encounter intensity had a statistically significant effect on change in decay (p =.005), restorations (p =.000), and treatment urgency (p =.001). As encounter intensity increased, there was a significant association with the decrease in decay (−.167), increase in restorations (.221), and reduction in referral urgency (−.188). Incorporating dental care into a school-based health center resulted in improved oral health in underserved children while overcoming barriers that typically restrict access. The collaboration of school nurses with the school-based dental clinic was an important element for maximizing student access to dental care.

Bakgrund: Tobaksanvändande är kraftigt associerat med ohälsa i munnen. Socioekonomiskt svaga grupper har högre tobaksanvändning, vilket resulterar i att de drabbas av mer tobaksrelaterad ohälsa. Denna ojämlikhet i hälsa minskar möjligheten till en hållbar utveckling. I synnerhet är rökare överrepresenterade bland patienter med tandlossningssjukdom. Tidigare forskning har fokuserat på tandvårdens upplevelser av att tillhandahålla tobaksrådgivning och tobaksavvänjning, mycket få studier har fokuserat på patienternas perspektiv. Sveriges åtta folkhälsopolitiska mål strävar åt en mer jämlik hälsa och betonar vikten av att agera mot tobaksanvändning. För att nå framgång i detta arbete måste organisationer samarbeta och samverka tvärsektoriellt. Detta har givit tandvården incitament att ge rökande patienter tobaksrådgivning och stöd i tobaksavvänjning. Syfte: Syftet är att utforska patienters upplevelse av råd och stöd för tobaksavvänjning inom tandvård genom en systematisk litteraturöversikt. Metod: Systematisk litteraturstudie har genomförts på 15 artiklar publicerade mellan 1999–2021. Samtliga artiklar hämtades från PubMed och behandlade patienters upplevelser kring tobaksrådgivning och stöd för tobaksavvänjning. Resultat: Majoriteten av patienterna upplever tandvården som en god arena för tobaksrådgivning och tobaksavvänjning. Tandläkare har möjlighet att påverka patienterna, då resultatet visar att yrkesrollen erhåller ett högt förtroendekapital. Tobakens påverkan på munhälsan betonas vara en viktig faktor för att inse att en beteendeförändring måste ske. Slutsats: Efter analys av granskade artiklar visar resultatet att en majoritet av patienterna förväntar sig råd om tobaksanvändning och även stöd i tobaksavvänjning av sin tandläkare. Genom att använda samtalsmetoder för att stärka empowerment kan tandläkare och tandvårdspersonal bidra till folkhälsofrämjande interventioner.

The role of behavioral technology in the promotion of oral health behavior

This study implemented two behavior management strategies, self-monitoring and monetary incentives, in a dental clinic and a private periodontal practice to explore the effects of these intervention strategies on subjects' dental flossing frequencies and to compare these strategies to standard educational procedures. Group analysis of four dependent variables generally showed minimal impact of the intervention strategies on flossing frequency. However, when the percentages of subjects within groups who improved on the dependent measures were evaluated, differential effects for some dependent variables were noted between settings and among intervention strategies. Based on those findings, the behavioral strategies of self-monitoring and monetary incentives did appear to enhance the effectiveness of education. Multimodal measures were used to assess changes in the target behavior. The general lack of covariance found among the dependent measures used in this study demonstrated that the interpretation varied with the choice of dependent variable. This finding suggested that past researchers, who used only physiological dependent measures to assess changes in the frequencies of dental flossing and brushing behaviors, should have selected more direct measures of the targeted behaviors (e.g. unobtrusive measures or direct observations). / M.S.

In my thesis I focused on the relationship between the influence of risk factors and the dental health status. The evaluation of the results was made on the basis of testing and verification of the set hypotheses, using my own graphs and statistical evaluation (in the program Statistica 6.0). For the research I used the data of 613 dental patients. I could draw these data from the provided file of their medical cards. The study is divided into the theoretical and practical part. In the theoretical part I focused on the anatomical structure of teeth, on dental caries, dental stone, and their creation and prevention. A large part of my work is dedicated to disease of the oral cavity and its hygiene. In this section I deal with various types of periodontal diseases (chronica of gingivitis, periodontitis) and their prevention. The practical part deals with the hypotheses themselves. I set seven hypotheses. The set hypotheses focus on risk factors, namely sex, residence, age, oral hygiene, frequency of preventive medical examinations, and illnesses which the patients suffer from. I evaluated the influence of these risk factors with regard to the cariosity and overall dental health of the dental patients. An Investigation of the Relationship Between Child, Family, and Community Factors and Early Childhood Oral Health and the Utilization of Dental Health Services

Background / Objective: Children under the age of 5 years bear a disproportionate burden of oral disease. The aim of this study is to investigate how child, family, and community determinants impact dental care utilization, and parental report of child’s oral health. Methods: Data for this study came from the 2011/2012 National Survey of Children’s Health for children aged 1 to 5 years old. Dependent variables evaluated were if the child had an oral health problem, been to a dentist in the past year, and parents description of the child’s teeth. Independent variables were selected from child, family, and community levels. Binary logistic methods were applied to each outcome and predictor variable. Stepwise logistic regression models were constructed for child, family, and community variables. Additionally the mediating effect of oral health services utilization in the association between child, family and community factors and parental perception of child’s oral health was evaluated. National results and Health Resource Service Area (HRSA) region IV results were compared. Results: In the national (n=24,875) and HRSA region IV sample (n=4,017) 9.7% and 10.2% of caregivers, reported that the child had an oral health problem in the past 12 months. Fewer than half (46.7%) of caregivers reported that their child had visited a dentist in the past 12 months. Absence of neighborhood cohesion, neighborhood amenities, and residence in metropolitan statistical area all had positive significant effects on children seeing a dentist. There was a mediating effect by utilization of oral health services between child with special health care needs (p=0.005), number of children (p=0.045) and adults (p=0.046) in the household, and tobacco use (p=0.018) and parents perception of oral health in the HRSA region IV population. Conclusion: This study identified several factors as correlates of poor oral health outcomes. Our results expand our knowledge of early childhood oral health by studying how oral health is impacted not only by child factors but also the family and community at large. Our results begin identifying the unique constellation of risk factors that contribute to early childhood oral health.

Tandhygienister i södra Sverige som var registrerade hos SRAT (n=313) tillfrågades att delta i studien. Enkäten skickades ut elektroniskt, vilket gav en svarsfrekvens på 48% (n=151). Inom folktandvården svarade 59% (n=101) och inom den privata tandvården svarade 35% (n=50). Enkätfrågorna omfattade arbete, arbetsklimat, arbetssituation, profession, hälsa, inflytande och stöd i yrkesrollen samt bakgrundsfrågor om ålder, kön, anställningsform och arbetstid. Resultaten visade statistiskt signifikanta skillnader (p<0,05) mellan tandhygienister inom folktandvården och tandhygienister inom den privata tandvården gällande i vilken utsträckning de styrde över sina tidsbokningar, upplevde trötthet, orolighet, besvär från rygg, nacke eller axlar samt besvär från andra delar av kroppen. En statistisk signifikant skillnad (p<0,01) mellan tandhygienister inom folktandvården och tandhygienister inom den privata tandvården syntes även i vilken utsträckning respondenterna styrde över sin arbetstakt. Kunskap och uppfattning om tandhälsa samt munhygienvanor hos ungdomar med invandrarbakgrund / Knowledge of dental health and oral hygiene habits among adolescents of immigrant origin

Introduktion: En god tandhälsa innebär att tänderna och munhålan är fria från smärtaeller obehag, samt att det finns en god tuggfunktion. Tandhälsa är en del av denallmänna hälsan och bidrar till välbefinnande hos människor. Syftet med studien var att undersöka kunskaper om tandhälsa och munhygienvanorhos 16-19- åringar med invandrarbakgrund. Ett ytterligare syfte var att undersökaderas uppfattningar om munhälsa. Material och metod: Studien är empirisk och kvantitativ och utfördes medanvändning av en enkät. Datainsamlingen gjordes vid tre gymnasieskolor i södraSverige. Två klasser från varje gymnasieskola valdes ut som hade högt antal elevermed invandrarbakgrund. Av 90 elever med invandrarbakgrund inkluderades 87 (97%) elever i studien. Resultat: Eleverna hade goda kunskaper om munhygienvanor. De flesta användetandborste och tandkräm två gånger dagligen. De hade de goda kunskaper om karies.Däremot visade de brist på kunskap om fluor och gingivit. De flesta av elevernatyckte att tandhälsan var viktig för dem. Slutsats: Eleverna uppgav ganska goda munhygienvanor. När det gäller kunskaperom tandhälsa fanns goda kunskaper vad som orsakar karies men inte gingivit. / Introduction: A good dental health can be defined as the teeth and oral cavity is free from pain and discomfort. Dental health is a part of the general and is related to the quality of life. Aim: The aim of this study was to assess knowledge in dental health and oral hygiene among 16-19-year-old immigrant-students. The student’s perception of oral health is also assessed. Materials and methods: This study is quantitative and is conducted with a questionnaire. Data was collected at three high schools in the South of Sweden. Two classes with students with immigrant origins were selected. Out of 90 participated 87 (97 %) were included in the study. Result: Results showed that the students had a good knowledge in oral hygiene habits. Most of them used a tooth brush and tooth paste twice a day. They also had a good knowledge in dental caries. However, there was a lack of knowledge about fluoride and gingivitis. Most of the students thought that the dental health was of importance. Conclusion: The participated had good oral hygiene habits. They had good knowledge about dental caries, but not about gingivitis.
