The aim of this research was to measure the central subfield thickness (CST), the best-corrected visual acuity (BCVA) and the macular volume, and compare these data to those of diabetic patients without DME, using spectral-domain optical coherence tomography (SD-OCT), for quantitative analysis of DME.Two hundred patients with nonproliferative diabetic retinopathy (NPDR) were enrolled in a transversal study, from which 55 eyes with macular edema were detected by fundus biomicroscopy. Twenty seven diabetic patients without DME and with normal ophthalmological exam served as control group. Measurements of CST (main variable), BCVA and macular volume were taken, considering the variables age, gender, NPDR classification, external limitant membrane (ELM) integrity and type of DME. Biomicroscopy with noncontact lens, fluorescein angiography and spectral optical coherence tomography were performed using Cirrus SD-OCT. Besides descriptive statistics, additional tests were applied to analyze the results, such as variance analysis, t-Student, chisquare, Fischer s exact test, Mann-Whitney and Kruskall-Wallis. The relation between CST and BCVA was studied by linear regression, with logarithmic scale of minimal angle of resolution (logMAR). It was found that there is no significant difference between case and control groups for the variables age and gender. However, there was significant difference for variables CST, BCVA and macular volume. It was found significant correlation between retinopathy severity according to NPDR classification and the mean value of CST,which is the greater the retinopathy severity, the greater the CST. Most of the patients presented moderate NPDR. A strong correlation between macular volume and CST was found. Patients with serous retinal detachment presented higher mean value of CST, and those with CME presented worse BCVA. Patients with disrupted ELM showed greater mean value of CST and worse BCVA. It was concluded that SD-OCT allows quantitative measurement of CST in DME and, for identifying the retinal layers individually, also detects retinal structural changes and correlates them to visual functions. / O edema macular diabético é a principal causa de perda visual em diabéticos e é caracterizado pelo espessamento retiniano, cuja avaliação é importante para o diagnóstico e conduta da retinopatia diabética. O objetivo deste trabalho foi comparar as medidas da espessura do subcampo central (ESCC), a acuidade visual e o volume macular de pacientes com edema macular diabético (EMD) com diabéticos sem EMD, usando tomografia de coerência óptica de domínio espectral (SD-OCT) para análise quantitativa do EMD. Foi realizado um estudo transversal em que foram avaliados 55 pacientes com EMD, selecionados pela biomicroscopia de fundo, de um grupo de 200 pacientes com retinopatia diabética não proliferativa (RDNP). Um grupo de 27 pacientes diabéticos sem edema macular e com exame oftalmológico normal serviu como controle. Mediu-se a espessura do subcampo central (principal variável), a acuidade visual , o volume macular e avaliou-se a idade, o sexo, a classificação RDNP, a integridade da membrana limitante externa (MLE) e a morfologia do edema macular. Realizou-se biomicroscopia com lente de não contato, angiofluoresceinografia e tomografia de coerência óptica de domínio espectral com Cirrus SD-OCT. Além da estatística descritiva, utilizou-se testes para análise dos resultados: análise de variância, t-Student, qui-quadrado, teste exato de Fisher, Mann-Whitney e Kruskall-Wallis. A relação entre a ESCC e a acuidade visual foi estuda da através de análise de regressão linear com escala logarítmica de mínimo ângulo de resolução (logMAR). Registrou-se não haver diferença significativa entre casos e controles nas variáveis idade e sexo. Entretanto, houve diferença significativa para as variáveis ESCC, acuidade visual e volume macular. Foi encontrada correlação significativa entre a gravidade da retinopatia na classificação RDNP e a média da ESCC, sendo que quanto maior a gravidade da retinopatia, maior a ESCC. A maior parte dos pacientes apresentou RDNP moderada. Foi encontrada correlação significativa entre o volume e a ESCC. Pacientes com descolamento seroso apresentaram maior média da ESCC; e aqueles com EMC apresentaram pior acuidade visual. Pacientes com MLE descontínua apresentaram maior média da ESCC e pior acuidade visual. Concluiu-se que o SD-OCT mede quantitativamente a ESCC no EMD e, por identificar individualmente as camadas retinianas, detecta suas mudanças estruturais. Estas medidas podem ser correlacionadas com a função visual e auxiliar na conduta de pacientes com retinopatia diabética.

Estudo das alterações retinianas em olhos de coelhos após injeções intravítreas seriadas de infliximabe / Study of retinal alterations in eyes of rabbits after serial intravitreous injections of infliximab

Twelve New Zealand albino rabbits (24 eyes) were used in the study. Each eye was given two (n=10) or three (n=10) serial intravitreous 2 mg injections of infliximab dissolved in 0.06 ml of saline, at monthly intervals. A separate group of rabbits (n=4 eyes) served as a control group. Ninety days after the first injection, the rabbits underwent electroretinographic and clinical ophthalmological tests. After being enucleated, the eyes underwent histological examination. No clinical ophthalmologic abnormalities were detected in the 24 eyes studied. The histological change noted was the presence of rare lymphocytes and eosinophiles in the posterior vitreous of four eyes subjected to two injections and six eyes subjected to three injections of infliximab, but it was not considered clinically significant. One clinically significant abnormality was found, a severe inflammatory reaction with vitreous exudates and ganglion cell edema in both eyes of a single rabbit, subjected to two to three injections of infliximab. The electroretinographic tests showed amplitudes that were on the average 12% smaller than those obtained before the treatment. However, there were no statistically significant differences when comparing amplitude or the implicit time between the pre and post-treatment electroretinographic findings, in all groups examined. Then, two and three intravitreous 2 mg injections of infliximab in eyes of rabbits at monthly intervals did not cause any changes after a 90-day follow-up, according to histological, electroretinographic tests and clinical ophthalmological evaluation. It was concluded that serial intravitreous infliximab doses to rabbits is a safe procedure. / O objetivo deste trabalho foi determinar os níveis de toxicidade de duas e três aplicações intravítreas de infliximabe na retina e coroide de coelhos albinos, por meio de exames clínicos oftalmológicos, eletrorretinográficos e histológicos. Foram utilizados doze coelhos albinos (24 olhos) da raça New Zealand. Cada olho recebeu duas (n=10 olhos) ou três (n=10 olhos) injeções intravítreas seriadas de 2 mg de infliximabe dissolvidos em 0,06 ml de solução salina, em intervalos mensais. Um grupo separado de olhos (n=4 olhos) serviu como controle. Noventa dias após a primeira injeção, os coelhos foram novamente submetidos a exames clínicos oftalmológicos e eletrorretinográfico e, após enucleados, os olhos foram submetidos a exame histológico. Nos 24 olhos estudados, não foram detectadas alterações clínicas oftalmológicas. A alteração histológica notada foi a presença de raros linfócitos e eosinófilos na região posterior do vítreo de quatro olhos submetidos a duas aplicações e de seis olhos que receberam três aplicações de infliximabe, mas sem significado clínico. Foi encontrada uma única alteração clinicamente significante, caracterizada como reação inflamatória grave, com presença de exsudatos vítreos nos dois olhos de um coelho, que foi submetido a duas e três aplicações de infliximabe. Os exames eletrorretinográficos mostraram amplitudes em média 12% menores do que aquelas obtidas antes do tratamento, porém sem diferenças estatisticamente significantes, comparando-se a amplitude ou o tempo implícito entre os achados eletrorretinográficos pré e pós-tratamento em todos os grupos examinados. Assim, duas e três aplicações intravítreas de infliximabe em olhos de coelhos em intervalos mensais, na dosagem de 2 mg, não provocaram alterações após seguimento de noventa dias, quer no exame histológico, na eletrorretinografia ou na avaliação clínica oftalmológica. Conclui-se que doses seriadas de infliximabe por via intravítrea em coelhos é um procedimento seguro.

In this work I present conclusions of clinical-laboratory research focused on the patients with diabetic macular edema (DME). We performed biochemical and immunochemical analyses of vitreous samples that were collected during the pars plana vitrectomy. Moreover, at patients with non-proliferative diabetic retinopathy (NPDR) we assessed morphological characteristics of DME using optical coherence tomography (OCT). According to our findings, the vitreous and serum concentrations of uric acid and glucose were significantly higher in patients with diabetic retinopathy and DME compared to controls. Also total ratio (serum/ vitreous concentration) of uric acid and glucose was in diabetics significantly higher than in controls. The most important determinant of increasing concentration of both uric acid and glucose in the vitreous was the grade of diabetic retinopathy. Moreover, we demonstrated significant correlation between vitreous concentration of uric acid and concentration of the vascular endothelial growth factor (VEGF) in patients with DME and NPDR. We found further, that the volume of the macula (cube volume - CV) computed with the software of Cirrus HD-OCT correlates in diabetics significantly with the vitreous VEGF concentration, but not with uric acid. Machine learning assisted decision support system for image analysis of OCT

Optical Coherence Tomography (OCT) has been around for more than 30 years and is still being continuously improved. The department of ophthalmology is a part of Sahlgrenska Hospital that heavily uses OCT for helping people with the treatment of eye diseases. They are currently facing a problem where the time to go from an OCT scan to treatment is being increased due to having an overload of patient visits every day. Since it requires a trained expert to analyze each OCT scan, the increase of patients is too overwhelming for the few experts that the department has. It is believed that the next phase of this medical field will be through the adoption of machine learning technology. This thesis has been issued by Sahlgrenska University Hospital (SUH), and they want to address the problem that ophthalmology has by introducing the use of machine learning into their workflow. This thesis aims to determine the best suited CNN through training and testing of pre-trained models and to build a tool that a model can be integrated into for use in ophthalmology. Transfer learning was used to compare three different types of pre-trained models offered by Keras, namely VGG16, InceptionResNet50V2 and ResNet50V2. They were all trained on an open dataset containing 84495 OCT images categorized into four different classes. These include the three diseases Choroidal Neovascularization (CNV), Diabetic Macular Edema (DME), drusen and normal eyes. To further improve the accuracy of the models, oversampling, undersampling, and data augmentation were applied to the training set and then tested in different variations. A web application was built using Tensorflow.js and Node.js that the best-performed model later was integrated into. The VGG16 model performed the best with only oversampling applied out of the three. It yielded an average of 95% precision, 95% recall and got a 95% F1-score. The second was the Inception model with only oversampling applied that got an average of 93% precision, 93% recall and a 93% F1-score. Last came the ResNet model with an average of 93% precision, 92% recall and a 92% F1-score. The results suggest that oversampling is the overall best technique for this given dataset. The chosen data augmentation techniques only lead to models performing marginally worse in all cases. It also suggests that pre-trained models with more parameters, such as the VGG16 model, have more feature mappings and, therefore, achieve higher accuracy. On this basis, parameters and better mappings of features should be taken into account when using pre-trained models.
