Predição de acidemia ao nascimento em gestações com dopplervelocimetria anormal e fluxo diastólico positivo das artérias umbilicais / Predicting acidemia at birth in pregnancies with abnormal but present end-diastolic flow velocity in umbilical artery

No setor de Vitalidade Fetal na Clínica Obstétrica do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo foi conduzido estudo caso-controle com seguimento prospectivo com quarenta e seis pacientes. O objetivo desse estudo foi identificar a associação dos testes de avaliação de vitalidade fetal e acidemia ao nascimento em gestações acometidas com dopplervelocimetria anormal e fluxo diastólico positivo das artérias umbilicais. Da mesma maneira, objetivou-se avaliar a relação entre os resultados antenatais, do parto, e neonatais com a ocorrência de pH fetal inferior a 7,20. A evolução para fluxo diastólico ausente ou reverso durante o seguimento foi critério de exclusão. Os casos foram classificados em dois grupos de acordo com a presença (grupo 1), ou ausência (grupo 2) de acidemia ao nascimento (pH inferior a 7,20), por meio da gasometria arterial de cordão imediatamente após o parto. As seguintes variáveis foram submetidas à análise univariada, comparando-se os grupos 1 e 2: idade materna, síndrome hipertensiva, uso de corticóde antenatal para maturação pulmonar, idade gestacional no momento do parto e testes de avaliação de vitalidade fetal do dia do parto. As modalidades dopplervelocimétricas analisadas foram: índice de pulsatilidade do território arterial (representado pela artéria umbilical e artéria cerebral média) e venoso (índice de pulsatilidade para veias do ducto venoso). Os seguintes parâmetros das atividades biofísicas fetais foram estudados: cardiotocografia (normal e anormal), presença de desaceleração tardia, presença de desaceleração (desaceleração tardia, ou desaceleração variável de mau prognóstico, ou desaceleração prolongada), ausência ou não de movimentos respiratórios, presença ou não de oligoâmnio e classificação do escore do perfil biofísico fetal (normal quando maior que 6, e anormal quando menor ou igual a 6). As variáveis clinicamente relevantes e estatisticamente significativas foram analisadas pela regressão logística. A série compôs-se de 46 pacientes classificadas nos grupos 1 (24 casos) e 2 (22 casos) e as seguintes variáveis foram estatisticamente relacionadas à ocorrência de acidemia no nascimento (p0,05): idade materna, síndrome hipertensiva, cardiotocografia anormal, presença de desaceleração, e ausência de movimentos respiratórios. Na regressão logística, a presença de desaceleração foi preditora de acidemia (p=0,024; OR=8,2; IC95%=1,2-52). Os presentes achados sustentam que a presença de acidemia ao nascimento em fetos com dopplevelocimetria anormal e fluxo diastólico positivo das AU está associada principalmente à anormalidade nos parâmetros agudos da avaliação da vitalidade fetal. A presença de desaceleração no traçado cardiotocográfico foi preditora de acidemia ao nascimento. / A longitudinal prospective study was conducted on 46 pregnancies with umbilical artery abnormal pulsatility index and positive diastolic flow velocity. Patients were evaluated at the Fetal Surveillance Unit/ Obstetrics Clinics of the University of São Paulo Medical School General Hospital (HCFMUSP) between February 2007 and March 2009. The objective of this study was to identify a potential association between fetal antenatal surveillance tests and acidemia at birth, in pregnancies with umbilical artery abnormal Doppler velocimetry and positive diastolic flow. In the same context, this research aimed to establish the correlation of data of birth, and neonatal outcome with umbilical cord pH below 7,20. The development of reversed or absent end- diastolic flow velovity in the umbilical artery was an excluision criteria. The cases were divided in two groups, according to the presence (Group 1) or absence (Group 2) of acidemia at birth (pH below 7.20), based on umbilical cord blood gas measurement immediately after birth. The following variables were submitted to univariate analysis comparing Groups 1 and 2: maternal age, hypertensive syndrome, corticosteroid use (for lung maturation), gestational age at birth, and fetal vitality assessments on the day of delivery. The Doppler parameters analysed were: pulsatility index of the umbilical artery, middle cerebral artery and the pulsatility index for veins for the ductus venosus. The following variables of the fetal biophysical profile were obtained: cardiotocography or nonstress test ( normal/ abnormal), presence of late deceleration, presence of deceleration (late or severe variable decelerations, or prolonged bradycardia), absence of fetal breathing movements, presence of oligohydramnios and biophysical profile score (considered normal when the score was higher than 6). Clinically relevant and statistically significant variables were analyzed by logistic regression. The series included 46 patients divided in Group 1 (n=24 with acidosis) and Group 2 (n=22 without acidosis), and the following variables were statistically correlated to acidemia at birth (p0.05): maternal age, hypertensive syndrome, abnormal cardiotocographic findings, deceleration (late deceleration, or variable deceleration with poor prognosis, or prolonged deceleration), and absence of respiratory movements. On logistic regression, the presence of deceleration was predictive of acidemia (p=0,024; OR=8,2; CI95%=1,2-52). In conclusion, we believe that the presence of acidemia at birth in fetuses with umbilical artery positive diastolic flow and abnormal Doppler velocimetry is mainly associated with abnormality of acute parameters of fetal well-being. The finding of deceleration on cardiotocographic assessments was predictive of acidemia at birth. ____________________

Comparison of Discrimination between Logistic Model with Distance Indicator and Regularized Function for Cardiology Ultrasound in Left Ventricle

Most of the cardiac structural abnormalities will be examined by echocardiography. With more understanding of heart diseases, it is commonly recognized that heart failures are closely related to left ventricular systolic and diastolic functions. This work discusses the association between gray-scale differences and the risk of heart disease from the changes in left ventricular systole and diastole of ultrasound image. Owing to the large dimension of data matrix, following Chen (2011), we also simplify the influence factors by factor analysis and calculate factor scores to present the characteristics of subjects. Two kinds of classification criteria are used in this work, namely logistic model with distance indicator and discriminant function. According to Guo et al. (2001), we calculate the Mahalanobis distance from each subject to the center of normal and abnormal group, then use logistic model to fit the distances for classification later. This is called logistic model with distance indicator. For the discriminant analysis, the regularized method by Friedman (1989) for estimation of covariance matrix is used, which is more flexible and can improve the covariance matrix estimates when the sample size is small. As far as the cut-point of ROC curve, following the approach as in Hanley et al. (1982), we find the most appropriate cut-point which has good performances for both sensitivity and specificity under the same classification criteria. Then the regularized method and the cut-point of ROC curve are combined to be a new classification criterion. The results under the new classification criterion are presented to classify normal and abnormal groups.

Left Ventricular Diastolic (Dys)Function in Sepsis

BACKGROUND: Sepsis is a clinical syndrome characterised by the systemic response to infection. It is a common problem in modern intensive care units and is associated with significant morbidity and mortality. Though the underlying cause of death is often multifactorial, refractory hypotension and cardiovascular collapse are frequently observed in the terminal phases of the condition. The aetiology of these cardiovascular abnormalities is complex but appears to be mediated by a circulating factor(s). The impact of sepsis upon left ventricular systolic function has been studied extensively. This may be because it is more readily assessed than diastolic function. Despite being increasingly appreciated as a contributor to morbidity and mortality in other clinical settings, there are scant data regarding the evaluation of left ventricular diastolic function in sepsis. Review of the haemodynamic monitoring literature reveals that many conventional measures of left ventricular filling, intravascular volume status and fluid responsiveness are influenced by ventricular diastolic (dys)function, such that interpretation can be challenging in critical care settings. In addition, many available techniques, such as pulmonary artery catheterisation, are invasive and potentially associated with risk to the patient. More robust and less invasive measures of left ventricular diastolic function and filling that can be applied within the intensive care unit (ICU) must be developed. The use of cardiac biomarkers, such as B-type natriuretic peptide (BNP), might represent a novel approach to evaluating left ventricular diastolic function and filling. BNP is released by the myocardium in response to wall stretch/tension. It has demonstrated value in the emergency department diagnosis of heart failure but interpretation of plasma BNP concentrations in critical care remains problematic. At least in part, this appears to relate to the significant number of potential confounders in patients with critical illness. Associations between BNP concentration and diastolic function have not previously been evaluated in severe sepsis and septic shock. The overall aim of this thesis is to investigate the usefulness of plasma BNP concentration in the evaluation of left ventricular diastolic function (including ventricular filling) in severe sepsis and septic shock. DIASTOLIC (DYS)FUNCTION IN SEPSIS: Review of the literature reveals that sepsis is associated with a spectrum of diastolic dysfunction. Characterisation of diastolic function in sepsis is challenging. In this regard, tissue Doppler imaging (TDI), offers promise. TDI is an echocardiographic technique that measures myocardial velocities, which are low frequency, high-amplitude signals filtered from conventional Doppler imaging. TDI has gained acceptance amongst cardiologists in the evaluation of diastolic function, particularly as a measure of ventricular relaxation and ventricular filling pressure; however, there are scant data regarding its use in critical care. We analysed echocardiographs from a large heterogeneous cohort of consecutive ICU patients (n=94) who had TDI as part of their clinically requested echocardiography. As well as supporting the feasibility of TDI in critically ill and mechanically ventilated patients, we demonstrated a wide range of TDI variables and a high prevalence of diastolic dysfunction using this modality. RODENT MODELS OF SEPSIS: We also sought to adapt, refine and evaluate rodent models of sepsis. Such models would allow control for a multitude of potential confounders commonly encountered in clinical sepsis. Two commonly employed rodent models of sepsis include caecal ligation and perforation (CLP) and endotoxin infusion. Comparison between CLP, sham and control groups demonstrated no difference in TDI or BNP. The observed changes in echocardiographic diastolic variables did not reflect those expected in sepsis and may be best explained by increases in heart rate rather than diastolic dysfunction per se. Endotoxaemia was associated with changes consistent with impaired myocardial relaxation (TDI) and reversible myocardial injury (histopathology), as expected in sepsis. BNP did not change significantly from baseline. This might be explained by the potential influence of fluid management upon BNP secretion. CLINICAL RESEARCH: The prediction of fluid responsiveness potentially prevents ineffective, excessive or deleterious intravenous fluid administration. Prospective evaluation of plasma BNP concentration in patients with septic shock found that it was not a predictor of a fluid responsive state. Furthermore, elevated BNP did not rule out a favourable response and therefore does not contraindicate a fluid challenge. Both impaired diastolic dysfunction, especially E/e’, and elevated BNP, have been associated with excess mortality in a range of cardiovascular diseases. These have not previously been compared in septic shock. In a cohort of patients with septic shock, E/e’ was a stronger predictor of mortality than cardiac biomarkers, including BNP. Fluid balance was an independent predictor of BNP in septic shock. OVERALL CONCLUSION: BNP appears not to be clinically useful in the evaluation of ventricular filling or diastolic function in sepsis. The association with fluid balance is a new finding and should be evaluated in a wider range of critically ill patients. In contrast to BNP, TDI appears to be a promising bedside tool in the evaluation of diastolic function and should be further evaluated in critical care.

The assessment of echocardiographic and tissue Doppler profiles of asymptomatic follow-up patients in cardiology practice

Tech.) -- Central University of Technology, Free State, 2010 / This main aim of this study was to assess patients in a general cardiology practice in order to determine the systolic and diastolic profiles of these patients. The aim was also to determine what effect life style and risk factors may have on the echocardiographic variables measured during such an examination. The specific aim of this study was the importance of not only examining the systolic function but the necessity to also examine the diastolic profile of patients. Life-style plays an important role, with the main culprit being obesity. Obesity was the single most important factor that affected the diastolic profile of patients seen in this study. With obesity a combination of other risk factors related to obesity was observed. Most abnormalities found due to these risk factors were associated with diastolic changes in the left ventricle. Echocardiography is routinely used in daily practice, but the diagnostic value of this tool can be enhanced if proper analyses of the systolic as well as the diastolic profiles are determined. Many cardiologists only measure the systolic function of the heart as an indication of the well- being of the left ventricle, although in this study it was proven that systolic function did not alter with ageing or with changes in the risk profile. Hundred-and-twelve patients, divided into three age groups, were evaluated in this study. Both systolic and diastolic variables were measured and analysed for abnormalities. None of these patients had systolic function abnormalities, although they had detectable anatomic changes due to ageing, obesity and hypertension. Several abnormalities were found on the diastolic profile of these patients. Muscle thickness increased due to obesity and hypertension and even with ageing, but with no significant abnormalities in the systolic function of the heart. There was a slight increase in the circumferential shortening of the left ventricle and that both the septal and longitudinal functions decreased with ageing. It is noteworthy that even where the systolic function remained normal in ageing subjects, their diastolic profiles changed significantly. Assessment of left ventricular function required a meticulous and systematic approach. In this study forty- one percent of patients visiting this general practice had abnormalities of their diastolic function although their systolic function was normal. It was found that with ageing, especially in the older age group, important abnormalities occur in their diastolic profile. The most common changes were that the E- peak velocity decreased and that the Apeak velocity of the trans-mitral flow increased. It seemed that passive filling decreased with ageing but that active filling increased simultaneously, causing the cardiac output to remain constant in older subjects. This is important to know because diseases affecting the atrium may have a profound effect on the cardiac output of older patients, even if they have normal systolic function, (due to the decreased passive filling they need their active filling or atrial contraction to support a normal cardiac output). An important marker will be to look at the ratio of the E/A- velocities in older patients to determine the ratio of active against passive filling. Other than that, a relatively new tool in echocardiography called tissue Doppler was used to determine what happened to the muscle with ageing. Here it was demonstrated that the different layers of the left ventricle acted differently with ageing. Results showed that the longitudinal fibres weakened with ageing although the circumferential fibres remained unchanged or even strengthened with ageing. It was apparent in this study that the traditional use of only systolic function may not be adequate when evaluating relative asymptomatic patients presenting at a general cardiology practice. It is important to also evaluate the diastolic profiles of these patients in order to scientifically quantify their heart health, even in asymptomatic patients. It is important to routinely evaluate the diastolic profile of patients so that early detection of these diastolic variables can be detected and timely consideration for its treatment can be given by their cardiologist. It is also important to take note of the significance of the obesity problem and the effect it has on the heart’s health. In conclusion, this study emphasizes the importance of the echocardiographic evaluation of diastolic cardiac function in addition to routine systolic evaluation in asymptomatic patients. This will enable the clinician to detect abnormalities early and tailor therapy accordingly. Lifestyle related risk factors, especially obesity, also have significant effects on diastolic cardiac function.

Introdução: Disfunção diastólica (DD) e diminuição da reserva contrátil do ventrículo esquerdo aumentam o risco cardiovascular de pacientes com hipertensão arterial sistêmica. As estatinas, pelos seus benefícios sobre a fibrose miocárdica, podem melhorar a função diastólica ou reserva contrátil de forma mais eficaz que inibidores da enzima de conversão da angiotensina (I-ECA) nesses pacientes. Objetivos: Investigar o efeito aditivo da estatina ao I-ECA na função diastólica e reserva contrátil de hipertensos com níveis de colesterol limítrofe. Métodos: Pacientes hipertensos com DD e LDL-colesterol < 160mg/dl submeteram-se a uma fase experimental para atingir pressão arterial sistólica (PAS) < 135mmHg e pressão arterial diastólica (PAD) < 85mmHg com enalapril ou enalapril e hidroclorotiazida. Quatro semanas após atingir o objetivo terapêutico, 55 pacientes foram aleatorizados para receber 80mg de sinvastatina (n = 27) ou placebo (n = 28) por um período de 20 semanas. Ecocardiograma de repouso e de estresse com dobutamina foram realizados antes e após o tratamento. O volume máximo do átrio esquerdo (VAE) foi medido pelo método biplanar de Simpson. Foram obtidas as velocidades de Doppler convencional e tecidual (DT) na diástole precoce (E, e) e diástole tardia (A, a) em repouso e durante estresse. As velocidades de DT foram a média dos 4 anéis mitrais basais. A reserva contrátil e a reserva diastólica do VE foram calculadas. A PA foi aferida mensalmente em consultório e o perfil lipídico foi dosado a cada 2 meses. Resultados: Após 20 semanas, a sinvastatina reduziu significativamente a PAS (-4±2mmHg; p=0,02), os níveis de colesterol total (-47±6 para estatina versus 6,2±5mg/dl para placebo; p<0,0001), LDL-colesterol (-41±5 para estatina versus 9,6±4mg/dl para placebo; p<0,0001) e triglicérides (-22,8±11,1 para estatina versus 15,3±8,3mg/dl para placebo; p<0,01). A razão E/A aumentou significativamente no grupo estatina (1,00±0,05 para 1,18±0,06 para estatina versus 1,06±0,05 para 1,06±0,04 para placebo; p=0,03), ao mesmo tempo em que o VAE reduziu significativamente neste grupo quando comparado ao placebo (24,5±0,9 para 21,1±0,8ml/m² para estatina versus 23,5±1,0 para 23,2±1,1ml/m² para placebo; p=0,048). A velocidade de e aumentou marginalmente no grupo estatina (9,6±0,6 para 10,2±0,5cm/s; p=0,05), mas sem diferença entre os grupos. A reserva contrátil aumentou significativamente em ambos os grupos (0,53±0,03 para 0,66± 0,05, p=0,009 para placebo versus 0,58±0,05 para 0,70±0,05, p=0,02 para estatina). Não houve correlação entre razão E/A, VAE e mudanças na pressão arterial ou níveis de colesterol. Houve uma moderada correlação positiva entre pressão arterial e LDL-colesterol (r=0,54; p=0,004). Conclusões: 1) A adição da sinvastatina ao enalapril melhora parâmetros de função diastólica em pacientes hipertensos com níveis de colesterol limítrofe, sendo este efeito independente da redução da pressão arterial ou do colesterol. 2) A PAS reduz com a sinvastatina, sendo esta redução correlacionada à redução do LDL-colesterol. 3) A reserva contrátil melhora com o tratamento com enalapril independente do uso da sinvastatina / Background: Diastolic dysfunction (DD) and decreased contractile reserve associated with hypertension are a surrogate for increased cardiovascular risk. Statins have experimental benefits on myocardial fibrosis, and could improve diastolic function or contractile reserve to a greater extend than ACE-inhibitors in hypertension. Objectives: Test in a double-blinded, placebo-controlled randomized study the effects of simvastatin added to enalapril treatment on DD and contractile reserve in hypertensive patients with average cholesterol levels. Methods: Hypertensive patients with DD and LDL-cholesterol < 160mg/dl underwent a run-in phase to achieve a systolic blood pressure (SBP) < 135mmHg and diastolic blood pressure (DBP) < 85mmHg with enalapril. Hydrochlorothiazide was added when need to achieve SBP or DBP control. Four weeks after reaching the optimum anti-hypertensive regimen, 55 patients were randomized to receive 80mg simvastatin (n = 27) or placebo (n = 28) for a period of 20 weeks. Transthoracic echocardiograms at rest and with dobutamine stress were performed before and after treatment. Left atrial volume (LAV) was measured by biplane modified Simpsons rule. Conventional mitral Doppler velocities were obtained at early diastole (E), late diastole (A) and E/A ratio was calculated, also Tissue Doppler velocities from mitral annulus (average from 4 basal walls) were measured at early diastole (e), late diastole (a) and systole (s); both at rest and during stress. The contractile and diastolic reserves were calculated at low dose of dobutamine stress. Blood pressure was measured monthly and lipid profile was analyzed every two months. Results: After 20 weeks, statin group showed a significant decrease in SBP (-4±2mmHg; p=0.02), total cholesterol (-47±6 for statin and 6.2±5mg/dl for placebo; p<0,0001), LDL-cholesterol (-41±5 for statin and 9.6±4mg/dl for placebo; p<0,0001) and tryglicerides levels (-22.8±11 for statin and 15.3±8mg/dl for placebo; p<0,01). E/A ratio increased significantly in statin group (1±0.05 to 1.18±0.06 for statin and 1,06±0,05 to 1,06±0,04 for placebo; p=0.03) at the same time that left atrial volume decreased (24.5±0.9 to 21.1±0.8ml/m² for statin and 23.5±1.0 to 23.2±1.1ml/m² for placebo; p=0.048). Moreover, e velocity had a trend to increase in statin (9.6±0.6 to 10.2±0.5cm/s; p=0.05) but there was no difference from placebo. Contractile reserve increased equally in both groups at lower dose of dobutamine (0.53±0.03 to 0.66±0.05, p=0.009 for placebo; 0.58±0.05 to 0.70±0.05, p=0.02 for statin). There was no correlation between E/A ratio, LAV and changes in blood pressure or cholesterol levels. There was a positive moderate correlation of blood pressure and LDL-cholesterol changes (r=0.54; p=0.004). Conclusions: 1) Simvastatin added to enalapril treatment in hypertensive patients with average cholesterol levels improves parameters of diastolic function independent of blood pressure or cholesterol changes. 2) Simvastatin decrease in SBP is correlated with LDL-cholesterol decrease. 3) Contractile reserve improves with hypertensive treatment irrespective to treatment with simvastatin

Avaliação ecoDopplercardiográfica da função diastólica do ventrículo esquerdo em indivíduos obesos pré e pós cirurgia bariátrica / EchoDopplercardiographic analysis of left ventricular diastolic function in severe and morbid obesity before and after bariatric surgery

A obesidade tem sido associada com a insuficiência cardíaca, e indivíduos obesos apresentam uma forma de cardiomiopatia caracterizada por dilatação ventricular esquerda, e hipertrofia ecêntrica, atribuída à sobrecarga volêmica. A disfunção ventricular tem sido descrita, sendo que a maioria dos estudos descrevem anormalidades da função diastólica do ventrículo esquerdo. O presente estudo foi desenhado para se determinar o efeito da obesidade sobre a função diastólica do ventrículo esquerdo e o impacto da perda de peso induzida pela cirurgia bariátrica sobre esta função.104 pacientes agendados para cirurgia bariátrica foram incluídos, 74 mulheres e 30 homens. Idade média de 38,8 ± 11,44 anos. Oitenta e um pacientes eram obesos mórbidos ( IMC maior ou igual 40 kg/m²) e 23 pacientes eram obesos graves ( IMC entre 35 e 39,99 kg/m²) com pelo menos duas co- morbidades associadas. A análise ecoDopplercardiográfica estrutural e funcional foi realizada previamente e após 6 meses do procedimento cirúrgico. Encontramos que indivíduos obesos tinham aumento da espessura septal do ventrículo esquerdo, do diâmetro atrial esquerdo e da massa ventricular esquerda. Além disso, apresentavam uma incidência maior de disfunção diastólica no pré-operatório. Observamos que a perda de peso induzida pela cirurgia bariátrica causou impacto nas velocidades do fluxo mitral, nas velocidades do fluxo de veia pulmonar e no Doppler tissular, mas não causou impacto nos outros parâmetros ecoDopplercardiográficos de análise da função diastólica. Como conclusões, obtemos que a perda de peso induzida pela cirurgia bariátrica melhora a função diastólica do ventrículo esquerdo em indivíduos obesos. Os parâmetros ecoDopplercardiograficos de análise da função diastólica do ventrículo esquerdo devem ser utilizados em associação para uma melhor avaliação desta função. / Obesity has been associated with heart failure, and individuals with severe obesity have been recognized to have a form of cardiomiopathy attributed to chronic volume overload, characterized by left ventricular dilation, and eccentric left ventricular hypertrophy. Impairment of cardiac function has been reported, with most studies reporting abnormal diastolic function.The present study was designed to determinate the effect of obesity on left ventricular diastolic function and the impact of weight loss after bariatric surgery on this function. One hundred and four patients scheduled for bariatric surgery were enrolled, 74 women and 30 men. Baseline mean age was 38.8 ± 11.44 years. Eighty- one patients were morbid obese (B.M.I. maior ou igual 40 kg/m²) and 23 were severe obese (B.M.I from 35 to 39.99 kg/m²), with at least two associated co-morbidities. Structural and functional echoDopplercardiographic analysis were performed before and 6 months after the bariatric surgery. We found that obese individuals had higher septal wall thickness, left atrial diameter, left ventricular mass and a higher incidence of diastolic dysfunction before the surgery than after. It was observed that weight loss had an impact on mitral flow velocities, on pulmonary vein flow velocities, and on tissue Doppler imaging, but had no impact on the other diastolic echocardiographic parameters. Weight loss after bariatric surgery had improved cardiac diastolic function. Being hypertensive, diabetic or having another associated co-morbidity was not related to a higher incidence of diastolic dysfunction. The diastolic echoDopplercardiographic parameters should be used in combination for a better diastolic evaluation.

Objetivo: Avaliar o comportamento da dopplervelocimetria de artérias umbilicais, artéria cerebral média e ducto venoso após utilização de corticoterapia antenatal para maturação pulmonar fetal em casos de diástole zero à dopplervelocimetria das artérias umbilicais. Métodos: Analisaram-se de forma prospectiva 22 gestações com diagnóstico de diástole zero à dopplervelocimetria das artérias umbilicais, no período de maio de 2004 a fevereiro de 2006. Foram incluídas gestações únicas com fetos sem malformações, idade gestacional de 26 a 34 semanas e com indicação para utilização de corticóide antenatal para maturação pulmonar fetal (betametasona - 12mg/dia por dois dias) e posterior resolução da gestação. Para uso do referido medicamento foi necessária a presença de ao menos uma das seguintes condições: valores do ducto venoso (índice de pulsatilidade para veias - IPV) de 1 a 1,5 ou oligoâmnio (índice de líquido amniótico inferior a 5,0 cm). Foram analisados os índices de pulsatilidade das artérias umbilicais, artéria cerebral média e ducto venoso antes da primeira dose do corticóide (D0), 24 horas (D1) e 48 horas (D2) após a primeira dose de betametasona. Para análise estatística foi utilizado o teste de análise de variância com medidas repetidas e adotado nível de significância de 5%. Resultados: Em todos os casos realizaram-se cesarianas em intervalo de 24 a 36 horas após a segunda dose de corticóide. A média de idade gestacional no parto foi de 29,09 (±1,94) semanas e de peso no nascimento de 800,90 (±246,49) gramas. Em 19 (86,3%) casos verificou-se diminuição dos valores do índice de pulsatilidade no sonograma das artérias umbilicais, sendo que em 15 (68,1%) houve retorno do fluxo diastólico final. A média dos índices de pulsatilidade das artérias umbilicais no D0 foi 2,91 (±0,59); no D1: 2,16 (±0,63) e no D2: 2,54 (±0,74), com diferença significativa do D0 para D1 (p<0,001). Não houve modificações significativas na dopplervelocimetria da artéria cerebral média nas 48 horas subseqüentes ao uso de corticóide (p=0,686). Na dopplervelocimetria do ducto venoso notou-se diminuição dos valores do índice de pulsatilidade para veias, com as seguintes médias: D0: 1,02 (±0,26), D1: 0,71 (±0,16), D2: 0,69 (±0,13). Nota-se diminuição significativa dos valores de IPV do ducto venoso à comparação do D0 com D1 (p<0,001) e do D0 com D2 (p<0,001). Conclusão: Houve diminuição significativa dos índices de pulsatilidade da artéria umbilical e de pulsatilidade para veias do ducto venoso nas primeiras 24 horas após o uso de betametasona. The mean umbilical artery pulsatility index was 2.91 (±0.59) at D0, 2.16 (±0.63) at D1, and 2.54 (±0.74) at D2, with a significant difference between D0 and D1 (p<0.001). No significant changes in the Doppler velocimetry findings of the middle cerebral artery was observed over the 48 h after corticosteroid administration (p=0.686). Correlação entre medidas ecocardiográfica e invasiva da pressão diastólica final do ventrículo esquerdo em coronariopatas com fração de ejeção preservada / Correlation between echocardiographic and invasive measurements of left ventricular end-diastolic pressure in patients with coronary artery disease and preserved ejection fraction

A doença arterial coronária é importante e prevalente manifestação da aterosclerose. A avaliação da função diastólica pelos parâmetros mitrais obtidos com Doppler ecocardiográfico possui limitações nos coronariopatas com fração de ejeção do ventrículo esquerdo (FEVE) preservada. Nestes, a disfunção diastólica, independentemente da FEVE, associa-se a maior incidência de desfechos desfavoráveis. A elevação da pressão diastólica final (PD2) do ventrículo esquerdo (VE) é a principal consequência fisiológica da disfunção diastólica. A pesquisa por melhores formas de determinação da PD2 do VE estendeu-se às modernas técnicas ecocardiográficas de quantificação da mecânica cardíaca. O objetivo deste estudo é correlacionar as medidas de deformação miocárdica, obtidas pelo speckle-tracking ecocardiográfico bidimensional, com a medida invasiva da PD2 do VE em pacientes com insuficiência coronariana e FEVE preservada. Métodos: foram avaliados 81 coronariopatas (idade: 61 ±8 anos) com FEVE >50%, encaminhados para cineangiocoronariografia eletiva, 40 destes com PD2 elevada (>16 mm Hg). Todos os indivíduos foram submetidos à avaliação ecocardiográfica convencional imediatamente antes do cateterismo e subsequente avaliação offline, com ecocardiografia speckle tracking (EST) para obtenção de medidas sistólicas e diastólicas de strain e strain rate circunferenciais e longitudinais, e estudo rotacional do VE. Foram analisadas as variáveis diastólicas da EST, tanto de forma isolada, quanto combinada com a velocidade da onda E ao Doppler. Resultados: Comparativamente, os pacientes do grupo com PD2 do VE elevada (n=40) mostraram aumento do volume indexado do átrio esquerdo (22 ±6 mL vs 26 ±8,26 mL p=0,04), velocidade da onda E (65 ±15 cm/s vs 78 ±20 cm/s p=0,02), relação E/e\' médio (8,14 ±2,0 vs 11,54 ±2,7 p=0,03) e relação E/strain rate global circunferencial (SRGC) pico E (39 cm vs 46 cm p <0,01). Nos 81 pacientes a correlação de Spearman com a medida invasiva da PD2 do VE foi de 0,56 para a relação E/e\' (p=0,03) e de 0,43 para a relação E/ESRGC pico E (p<0,01). A área sob a curva ROC foi significativa em ambas, sendo 0,83 e 0,73 respectivamente (p<0,05). Conclusão: A relação E/SRGC pico E é capaz de identificar elevação da PD2 do VE em coronariopatas com FEVE preservada, com menor desempenho que a relação E/e\'. / Introduction: Coronary artery disease (CAD) is important and prevalent manifestation of atherosclerosis. The assessment of diastolic function by mitral Doppler echocardiographic parameters has limitations in patients with CAD and preserved left ventricular ejection fraction (LVEF). Diastolic dysfunction is associated with higher incidence of unfavorable outcomes in these patients, regardless of LVEF. The increase in left ventricle end-diastolic pressure (LVEDP) is the main physiological consequence of diastolic dysfunction. The search for better ways of determining the LVEDP extended to the quantitative evaluation of cardiac mechanics with the modern echocardiographic techniques. The aim of this study is to correlate the invasive mesures of LVEDP and myocardial deformation measurements obtained by the two-dimensional speckle-tracking echocardiography in patients with coronary artery disease and preserved LVEF. Methods: 81 CAD patients (age: 61 ± 8 years) with LVEF >50%, scheduled for elective coronary angiography were evaluated, 40 of these with high LVEDP (>16 mm Hg). All subjects underwent conventional echocardiography immediately before catheterization and subsequent offline assessment with speckletracking echocardiography (STE) to obtain systolic and diastolic values of circumferential and longitudinal strain and strain rate, and rotational LV study. Diastolic variables of EST were analyzed both isolated and combined with the speed of the transmitral Doppler E wave. Results: Patients in the group with the high LVEDP (n =40) showed increased left atrial volume index (22 ± 6 mL vs 26 ± 8.26 mL p =0.04), E wave velocity (65 ± 15 cm/s vs 78 ±20 cm/s p = 0.02), E/e\' (average) ratio ( 8.14 ± 2.0 vs 11.54 ± 2.7 p = 0.03) and E/global circumferential strain rate (GCSR) peak E (39 cm vs 46 cm p <0.01). In 81 patients, Spearman\'s correlation with the invasive measurement of LVEDP was 0,56 (p =0.03) for the E/e\' ratio and 0.43 for the E/GCSR peak E (p <0.01). The area under the ROC curve was significant for both (p < 0.05): 0.83 and 0.73 respectively. Conclusion: The E/GCSR peak E ratio is able to identify elevated LVEDP in CAD patients with preserved LVEF, with less performance than the E/e\' ratio.
