The effectiveness of the referral system in primary health care in the West Rand region : a normative-ethical study with special emphasis on traditional healers

Thesis (M.Phil.)--Stellenbosch University, 2000. / ENGLISH ABSTRACT: The aim of this research is to identify the various levels of health care units, their relationships and the problems hindering an effective referral system. To achieve this goal, use is made of a case study of the West Rand area in Gauteng. The standpoint is that, to achieve Primary Health for all South Africans referral systems within health care units and levels must be reciprocal. It is argued that for Primary Health Care to be successful, it must satisfy the goal of affordability appropriateness and accessibility. Results from the research revealed that four health care levels, namely traditional healers, health NGOs, Clinics, and Hospitals. Though there is some degree of referral in the study area, it was observed that referrals in the study area were not reciprocal. Amongst the major problems identified as hindering an effective referral system in the study area, include lack of cooperation between health institutions, poor health infrastructure and communication network as well as lack of other health paraphernalia. The research also found that government policy towards some of the health institutions (Traditional healers) contributes to the inefficiency of proper referrals in the study area. / AFRIKAANSE OPSOMMING: Die oogmerk van hierdie ondersoek is om die verskillende vlakke van gesondheidsorgeenhede, hulonderlinge verbande en die probleme wat doeltreffende verwysings in die wiele ry, te identifiseer. Dit word gedoen aan die hand van 'n gevallestudie van die Wes-Randarea in Gauteng. Die uitgangspunt is dat doeltreffende Primêre Gesondheid vir alle Suid- Afrikaners afhang van resiprokale verwysingsisteme tussen gesondheidsorgeenhede en -vlakke. Suksesvolle Primêre Gesondheidsorg vereis bekostigbaarheid, toepaslikheid en toeganklikheid. Die ondersoek het vier gesondheidsorgvlakke aan die lig gebring: tradisionele genesers, gesondheids-nie-regerings-organisasies, klinieke en hospitale. Hoewel daar 'n mate van onderfinge verwysing in die studie-area bestaan, was dit nie wederkerig nie. Onder die vernaamste struikelblokke vir 'n doeltreffende verwysingsisteem tel swak samewerking tussen gesondheidsinstellings, gebrekkige gesondheidsinfrastruktuur en kommunikasienetwerk, en 'n skaarste aan ander gesondheidsmiddelle. Die ondersoek het ook bevind dat regeringsbeleid aangaande sommige van die gesondheidsinstellings (tradisionele genesers) bydra tot die ondoeltreffendheid van verwysings in die studie-area.

Researching sensitive issues in education in the Limpopo Province of South Africa

Thesis (DPhil)--Stellenbosch University, 2004 / ENGLISH ABSTRACT: Researchers in Social Sciences have generally encountered problems in ensuring data quality when dealing with topics that are regarded as sensitive. This thesis reports on an investigation into the methodology used for research projects around sensitive issues in education in the Limpopo Province of South Africa. Data consists of twelve interviews with individual principals (each interview schedule containing semi-structured question items from ten categories) and a thirty-three item self-report questionnaire survey administered to one hundred and fifty principals drawn from two hundred and seventy two secondary schools over the course of an academic year (2002) and conducted in the Limpopo Province of South Africa .. Results reveal nine major categories of sensitive issues in school management on which principals have difficulty in providing information to researchers: school policy, school financial issues, moral or social relations issues, learner and educator disciplinary issues, working conditions, absenteeism, developmental appraisal, educator unions, and religious matters policy issues. Principals have also reported on the various reasons why they regard each of the aspects as sensitive, thereby making it difficult for them to provide information around. Of these, moral issues were considered the most sensitive. The major reasons given were: confidentiality, intrusion of privacy, fear oflegal sanction, threat to work and violation of the rights of the individual. Analysis of the different sensitive issues also show that certain biographical characteristics - age of the principal and years of experience as principal - are significant mediators in principals' perceptions of sensitive issues in school management. That is to say, these contribute to principals' assessment of their emotional, physical and psychological well-being. Sensitivity is a problem when collecting data for research purposes. It warrants the attention of all those involved in social science research. The findings in this study point to the issues in school management that are highly sensitive to provide information suggesting that data collected would therefore not be of a high quality. / AFRIKAANSE OPSOMMING: Menige navorsers in sosiale wetenskappe het oor die algemeen probleme teëgekom met die versekering van data kwaliteit in sensitiewe onderwerpe. Hierdie tesis raporteer oor 'n ondersoeke in die metodologie gebruik vir navorsing projekte rondom sensitiewe onderwerpe in onderwys in die Limpopo Provinsie in Suid-Afrika. Data bestaan uit twaalf onderhoude met individuele prinsipale (elke onderhoud se skedule bevat se gestruktureerde items van tien katagorieë) en 'n drie-en-dertig item (self-report) vraelys wat onder 150 prinsipale uitgedeel is, waarvan 272 sekondêre skole oor 'n tydperk van 'n akademiese jaar (2002) gebruik was in die Limpopo Provinsie van Suid Afrika. Die resultate toon nege hoof onderwerpe in skoolbestuur waar prinsipale dit moeilik vind om informasie aan navorsers te verskaf: skoolbeleid, finansiële onderwerpe, moraliteite of sosiale verhoudinge, leerder en onderwyser dissiplinêre onderwerpe, werkverhoudinge, afwesigheid, personeelontwikkeling, onderwysunies, en godsdienstige sake. A survey of disclosure of compliance with King II by top listed South African companies : an investigative study of the companies listed on the FTSE/JSE top 40 index

Thesis (MBA (Business Management))--University of Stellenbosch, 2008. / ENGLISH SUMMARY: During the period of change in South Africa in the year of the first democratic elections, 1994, the first King Report on Corporate Governance appeared. For the first time in South Africa, companies had a good reference for corporate practice and conduct. A second King report, commonly referred to as King II, appeared in March 2002 and expanded on the earlier Code to produce, what was considered at the time to be, a world-class code of corporate governance. In addition to the acceptance and incorporation of King II into corporate governance practice, the JSE included compliance with the King Code as part of its listing requirements. The code itself is not an enforceable set of rules, but rather guidelines to assist companies in implementing principles of good governance. In similar vein, the JSE has given companies the flexibility to justify non-compliance. This underscores the socalled comply or explain philosophy to which corporate governance in South Africa subscribes. Studies have found the level of corporate governance in South Africa to be high, especially for an emerging market. Compliance with corporate governance principles and transparency go hand in hand. In a comply or explain regime, where the regulation is considered to be done by the market, disclosure of compliance is especially important. The goal of this research project was to conduct a survey of the top listed South African companies to ascertain what the level of disclosure of corporate governance was, with specific reference to the recommendations contained in the King Code. For the purposes of this study, the top companies were defined as the FTSE/JSE Top 40 Index companies. The supposition was that most companies, having been confronted with the King Code for almost six years, would be compliant to a large degree and will disclose their compliance. This was expected to be more evident amongst top companies who have the resources and influence to effect changes sooner and take the lead on high profile issues. The study confirmed the supposition that companies, at least in the sample, were highly compliant. There were however some areas where improvements can be made to be more in line with global best practices. / AFRIKAANSE OPSOMMING: Gedurende die periode van vernuwing in Suid-Afrika en die jaar van die eerste demokratiese verkiesing, 1994, het die eerste King verslag op korporatiewe bestuur verskyn. Vir die eerste keer het maatskappye in Suid-Afrika ‘n goeie verwysing gehad in die bepaling van hul korporatiewe bestuurspraktyke en gedrag. ‘n Tweede King verslag, alombekend as King II, het in Maart 2002 verskyn en op die eerste verslag uitgebou om ‘n kode te voorskyn te bring wat ten tyde van publisering as wêreldklas bestempel is. Bo en behalwe die aanvaarding en toepassing van King II in die korporatiewe bestuurspraktyk, het die JSE ook nakoming van die Kode as deel van die noterings vereistes ingesluit. How to utilise marketing opportunities in a highly regulated environment : the veterinary industry

Up till now these regulations prevented veterinarians to freely market veterinary services, especially professional services. Every business person in the biggest part of the world is subject to free-market economic pressures. However in the case of the professional, who is also a business person, these economic market-pressures could become very intense and unbalanced, and lead to what is called the “professional dilemma” (Du Preez, 2003: 8). Professional persons are governed by their respective codes of conduct. Those professionals who practice as entrepreneurs therefore need to be more business skilled and creative to successfully manage their businesses as they do not have the luxury of utilising the normal business strategies and options. The state of professionalism within the veterinary profession in South Africa in terms of entrepreneurship has been addressed in the past, but there is still a major gap in terms of providing business tools to managers in the veterinary industry. The biggest challenge in terms of the general business functions faced by veterinarians is the marketing function, thus the reason for this study. It is necessary that the highly regulated environments of professionals being understood. Therefore the research has been conducted by highlighting these environments. The researcher then continues to analyse the internal and external environment, client needs and customer profile in order to identify ways of optimising marketing opportunities. / AFRIKAANSE OPSOMMING: Die veeartseny industrie word bedryf in ‘n hoogs gereguleerde omgewing. Hierdie regulasies het veeartse tot nou toe verhoed om dienste, veral professionele dienste, vrylik te bemark. Besigheidspersone regoor die wêreld verkeer onder vryemark ekonomiese druk. In die geval van die professionele persoon, wat ook ‘n besigheidspersoon is, kan die ekonomiese druk baie intens en ongebalansseerd word. Daar word verwys na die “professionele dilemma” (Du Preez, 2003: 8). Professionele persone word gereguleer deur hul onderskeie kodes. Daardie professionele persone wat praktiseer as entrepreneurs moet dus oor meer besigheids- en kreatiewe vaardighede beskik om hul besigheid suksesvol te bestuur. Hulle het nie die luuksheid om die algemene strategiee te benut nie. Die stand van professionalisme in die veeartsenyprofessie in Suid- Afrika in terme van entrepeneurskap is al voorheen aangespreek, maar daar is nog steeds gapings ten opsigte van die beskikbaarstelling van besigheidsvaardighede aan bestuurders in die bedryf. Die grootste uitdaging in terme van besigheidsfunksies wat veeartse in die gesig staar, is die bemarkingsfunksie. Dit is dus die rede vir hierdie studieprojek. Dit is belangrik dat die hoogs gereguleerde omgewing van professionele persone verstaan word. Die studie begin dus deur hierdie verskillende omgewings te skets. Die studie skenk verder aandag aan die ontleding van die interne en eksterne omgewings, klientebehoeftes en –profiel om maniere te vind om bemarkingsgeleenthede binne regulasies ten volle te kan benut.

Theoretical approaches underpinning educational psychologists’ practice in district-based support teams

Thesis (MEdPsych)--Stellenbosch University, 2012. / Includes bibliography / ENGLISH ABSTRACT: There has been a paradigm shift occurring in the field of educational psychology over the last few decades from a predominantly medical approach towards a more inclusive and systemic approach. This development has called for a change in the way educational psychologists conceptualise problems, as well as an expansion in their practices in order to provide effective support services. However, the question arose which theoretical approach educational psychologists currently espouse and implement. This study therefore endeavoured to explore the theoretical approaches underpinning educational psychologists’ practice, with a focus on those professionals working within District Based Support Teams (DBSTs) in the Western Cape.Within this broad aim, the research aimed to identify which theoretical approach(es) the educational psychologists personally espouse and which theoretical approach(es) is/are espoused within the DBSTs. A further objective was to determine what the practice of their theory-in-use entailed. This information would ascertain whether the educational psychologists’ espoused theories and theory-in-use correspond. Qualitative research within the interpretive/constructivist paradigm was employed for the research design. The participants included eight educational psychologists practising within District-Based Support Teams (DBSTs) in the Western Cape. Data was collected by means of an extensive literature review, self-administered questionnaires and individual interviews and analysed using qualitative thematic analysis and interpretation. The key findings of this research revealed that the educational psychologists in this study personally espouse a systemic approach to their practice of educational psychology. However, it was revealed that this approach is not necessarily adopted by all members of the DBSTs. Furthermore, the educational psychologists themselves experience many challenges in implementing this theory. Their theory-in-use at the Education District Offices incorporates both medical and systemic approaches. Assessment of learners is largely based on a medical model, whilst the interventions and support they provide appeared to be focused on a more systemic level. It was concluded that the theory that the educational psychologists personally espouse; the theory espoused within DBSTs; and the theory-in-use within the DBSTs, do not entirely correspond. / AFRIKAANSE OPSOMMING: ’n Paradigma skuif wat die laaste paar dekades plaasgevind het in die veld van opvoedkundige sielkunde, het meegebring dat die benadering verander het vanaf ‘n hoofsaaklik mediese model na ‘n meer inklusiewe en sisteem gebaseerde model. Hierdie ontwikkeling vra dat opvoedkundige sielkundiges probleme op nuwe maniere konseptualiseer, asook hul praktyk uitbrei om effektiewe ondersteuningsdienste te kan verskaf. Die vraag watter teoretiese benaderings opvoedkundige sielkundiges tans onderskryf en implementeer, het onstaan. Hierdie studie poog dus om te ondersoek watter teoretiese raamwerke opvoedkundige sielkundiges se praktyke onderlê, en fokus op die opvoedkundige sielkundiges wat werk binne die Distriksgebaseerde Ondersteuningsspanne (DBSTs) in die Wes-Kaap. Binne hierdie breë doelwit, word deur die navorsing gepoog om die teoretiese benadering(s) wat opvoedkundige sielkundiges binne die DBSTs persoonlik aanhang sowel as die teoretiese benadering(s) wat binne hierdie DBSTs gepropageer word, te identifiseeer. ‘n Verdere mikpunt was om vas te stel wat die deelnemers se praktyk (teorie-in-gebruik) behels. Hierdie inligting sou dit moontlik maak om vas te stel of die teorie voorgestaan en die teorie-in-gebruik ooreenstem. Kwalitatiewe navorsingsmetodologie vanuit ‘n interpretatiewe/konstruktivistiese paradigma is gebruik in die ontwerp van die navorsing. Die deelnemers was ag opvoedkundige sielkundiges wat binne die DBSTs van die Wes-Kaap praktiseer. Data is versamel deur ‘n uitgebreide literatuur oorsig, self ingevulde oop vraelyste en individuele onderhoude. Die data is geanaliseer deur van kwalitatiewe tematiese analise en interpretasie gebruik te maak. Die kernbevindinge van die studie het gedui op ‘n sistemiese benadering tot hul praktyk, wat deur die opvoedkundige sielkundiges persoonlik onderskryf word. Die bevindige het ook gedui daarop dat nie al die lede van die DBSTs dit aangeneem het nie. Verder ervaar die opvoedkundige sielkundiges self verskeie uitdagings ten opsigte van die implementering van sodanige teoretiese benadering. Die teorie-in-gebruik binne die Onderwys Distrikskantore bevat beide mediese model benaderings sowel as meer sistemiese benaderings. Die assessering van leerders is grootliks gebaseer op ‘n mediese model, terwyl die intervensies en ondersteuning wat gebied word blyk meer sistemiese onderlê te wees. Dit kom dus vooras of die teorie wat opvoedkundige sielkundiges persoonlik aanhang, die teorie wat binne die DBSTs onderskryf word, en die teorie-in-gebruik binne die DBSTs nie noodwendig ooreenkom nie.

The church as a credible contributor to moral regeneration in Democratic South Africa today : a theological-ethical approach to current challenges

Text in English / With the start of democracy in South Africa and the end of the legal Apartheid rule, it became apparent to those who identified the church with the liberation struggle that Christian communities had lost their significance in society today. This feeling, although not popular among theologians, became widely held by those who thought that we had arrived politically. This feeling was also supported by views which felt that democracy was the ultimate human form of governance. In this light, it is conceivable for churches to abandon the struggle because of the irrelevance of agitating a form of government that is affirmed and celebrated globally. Also, it became justified to abandon the struggle, if indeed the struggle was against Apartheid, because the enemy had been defeated. Consequently, this meant a withdrawal from siding with those whose rights were marginalised. Such an abrupt withdrawal from championing the rights of those who are marginalised (especially as displayed by the church during the Apartheid era) also raises many concerns pertaining to the role that the churches ought to be playing in the current democratic dispensation. It is also conceivable that the initiative to struggle against Apartheid was motivated only by the fact that the Apartheid regime was unjust and that it discriminated against certain groups of South African citizens. The attainment of liberation in South Africa has plunged churches into an illusion that democratic states are capable of good governance and that when such a government exists, churches can safely concern themselves with their primary ecclesiastical duties—of preaching the Word of God and administering the sacraments and leave the socio-economic and political affairs to those who are governing. It seems that this view is especially prevalent among the constituency of the church that brought forward a theological confession against the theological legitimacy granted to the Apartheid ideology. To unapologetically declare its association with those on the margins, and to defy Apartheid for the evil that it was, this church brought about the Belhar Confession as proof of its allegiance to those on the margins. This study shall confine itself to the realm of the Christian community. "It is not only the guilty who suffer" : exploring gender, power and moral politics through the contagious diseases acts in the Cape Colony, c1868-1885

Thesis (MA)--Stellenbosch University, 2014. / ENGLISH ABSTRACT: This study deals with the build-up to, and resultant reactions against, regulating sexual practices in the Cape Colony, especially the Contagious Diseases Acts in 1868 and 1885. The focus will be on the existence of venereal disease as a colonial epidemic. The wider context in terms of Britain, India, Hong Kong, Singapore and Australia will also be taken into account. The research is based on a theoretical framework made up of three components; gender, power and moral politics. The role of gender will be looked at through the existence of the double standard and the prostitute. Power and the existing relations between the colonies and the colonisers will be looked at by addressing the issue of race, superiority and the exportation of the colonial mindset. Moral politics will be analysed through the discussion of purity campaigns, women’s role in society and the medical aspect of politics. Within this thematic framework, the focus of the study will then move to the Cape Colony and the existence of regularity practices there. This study seeks to establish the ways in which regulation developed at the Cape and in doing so hopes to contribute to the existing historiography. / AFRIKAANSE OPSOMMING: Hierdie studie fokus op die aanloop tot en die daaropvolgende reaksies ten opsigte van regulatoriese sekspraktyke, waarvan die bekendste die Aansteeklike Siektes Wette van 1869 en 1885 was. Daar word na die bestaan van veneriese siekte as ‘n koloniale epidemie gekyk. Die breër konteks van Brittanje, Indië, Hong Kong, Singapoer en Australië word ook in ag geneem. Nursing student's perspectives on Spiritual care in clinical nursing practice in a selected school of nursing at Umkhanyakude District in KZN Province

A qualitative, non-experimental, explorative and descriptive research design based on the phenomenological philosophical tradition by Heidegger to broaden hermeneutics was conducted. The study was conducted at Umkhanyakude District to investigate the perspectives of eligible nursing students relating to the provision of spiritual care to patients. A purposive sample of 9 participants was recruited and consent form obtained. An unstructured interview guide, with a grand tour question, was used to conduct face to face individual interviews. The Thematic analysis and interpretative phenomenological method of analysis were employed until three themes, six categories and eleven subcategories emerged from the data. Data analysis revealed that nurses had difficulty to differentiate spiritual care from religious care. Commonly cited methods of providing spiritual care were prayer, reading sacred text and singing spiritual songs. Nurses still felt inadequately prepared educationally on how to provide spiritual care in nursing practice. Most of the participants provided spiritual care out of their own interest and not as part of their professional responsibility. Recommendations proposed that the matter be taken up by nurse managers to conduct related in-service education and mentoring programs and nurse educators to guide curriculum planning which evidently include spiritual care. / Health Studies / M.A. (Health Studies)

The therapeutic privilege is a defence in terms of which a doctor may withhold information from a patient if disclosure of such information could harm the patient. This study explores the defence of therapeutic privilege and provides a critical evaluation. A comparative investigation is undertaken, while arguments springing from a variety of disciplines are also incorporated. A number of submissions are made for limiting the ambit of the defence. The main submission is that the therapeutic privilege should comply with all the requirements of the defence of necessity. In addition, it should contain some of the safeguards afforded to the patient by the requirements of the defence of negotiorum gestio so that therapeutic privilege is out of the question if medical treatment is administered against the patient's will, or the doctor has reason to believe (or knows) that the patient will refuse to undergo an intended intervention once properly informed. / Jurisprudence / L.L.M.
