Na primeira etapa utilizou-se de questionário semiestruturado com questões qualitativas e quantitativas, aplicado na forma de entrevista com todos os alunos, das referidas séries, que estavam presentes nos dias da pesquisa; na segunda etapa também realizada por meio de entrevistas, mas com outro questionário, aplicado somente aos alunos que residiam em assentamentos rurais. A pesquisa abrangeu também entrevistas com familiares de 11 jovens, no próprio lote, abordando questões pertinentes à vida familiar, à visão que tinham da Escola e do processo de sucessão do estabelecimento familiar e, por fim, foram ouvidos ainda três professores visitadores do referido Curso. Evidenciou-se que a Etec conseguiu conquistar os jovens de tal forma que todos gostavam de frequentar a escola e que a modalidade alternância foi importante referencial para os alunos filhos de assentados rurais, aproximando e reforçando a interação entre o jovem, a família e a Escola. Com a pedagogia da alternância, principalmente por meio do papel do professor visitador, ocorreu a valorização dos conhecimentos adquiridos na Instituição e aumentou a participação do jovem nas decisões familiares, assim como houve uma revalorização do modo de vida dos pais. Factors influencing the choice of mathematics as a subject at senior secondary level

The study was undertaken to identify the factors that influence standard seven pupils when they choose whether to continue with mathematics at senior secondary level or not. The relative importance of the factors was also determined. The literature study identified the following factors: attitude towards mathematics, utility of mathematics, family members' influence, mathematics teacher's influence, peer group influence, achievement and gender. The empirical study dealt with the following: * A 77 item questionnaire was completed by 201 standard seven pupils. * The statistical analysis revealed significant differences between pupils who chose mathematics and those who did not, with regard to all variables except gender. * A regression analysis identified the most influential factors as achievement, family members' influence, attitude and the mathematics teacher. * The overall implications were: - Pupils be made to experience success so that their attitudes may change. - Parents must be involved in their children's education. / Psychology of Education / M. Le récepteur Gb3/CD77 : analyse de l’apoptose induite par la vérotoxine-1 dans les cellules de lymphome de Burkitt et recherche de ligands endogènes / Gb3/CD77 receptor : VT-1 apoptotic signaling pathway analysis in Burkitt lymphoma cells and research of endogens ligands

Le glycolipide Gb3/CD77 qui est fortement exprimé en surface des cellules de lymphome de Burkitt (LB) est le récepteur d’une toxine bactérienne la Vérotoxine-1 (VT-1). Notre équipe a montré précédemment que, dans les cellules de LB, la VT-1 induit une cascade apoptotique mettant en jeu les caspases et la mitochondrie. Mon travail a consisté à poursuivre l’analyse des bases moléculaires de ce processus, notamment en m’intéressant au rôle de la protéine pro-apoptotique Bid. Bid est un membre de la famille Bcl-2 qui est clivé par la caspase-8 au cours de l’apoptose et dont la forme tronquée (t-Bid) se relocalise à la mitochondrie. Grâce à l’utilisation de clones cellulaires de LB où Bid a été inhibée puis réexprimée sous forme non clivable et d’un inhibiteur de la caspase-8, nous avons montré que lors de l’apoptose induite par la VT1 : 1) la protéine entière Bid (full-length Bid ou FL-Bid) contrôle l’activation de protéines pro-apoptotiques Bax et de Bak ; 2) t-Bid et FL-Bid sont, toutes les deux, impliquées dans la libération des protéines pro-apoptotiques (cytochrome c et Smac/DIABLO) de l’espace intermembranaire de la mitochondrie vers le cytosol ; 3) FL-Bid contrôle l’homodimérisation de Bax et de Bak qui contribuerait à la libération initiale du cytochrome c et de Smac/DIABLO alors que t-Bid est nécessaire à l’hétérodimérisation de Bax et Bak qui permettrait l’amplification de cette libération. L’ensemble de ces résultats montre donc une coopération fonctionnelle entre Bax et Bak au cours de l’apoptose induite par la VT-1 et surtout met en évidence que l’activation de la voie caspase-8/t-Bid n’est absolument pas requise pour initier la mort cellulaire.Gb3/CD77 est aussi exprimé à la surface de certains lymphocytes B normaux, où il constitue un marqueur de différenciation mais sa fonction endogène reste encore indéterminée. Une deuxième partie de mon travail a consisté à essayer d’identifier le ligand physiologique de Gb3/CD77 pour comprendre son rôle biologique. Grâce à une analyse en spectrométrie de masse, nous avons identifié deux protéines potentiellement partenaires de Gb3/CD77 : la galectine-7 et la protéine S100A11. / The Gb3/CD77 glycolipid, which is strongly expressed in Burkitt's lymphoma (BL) cells, is a receptor for the bacterial toxin Verotoxin-1 (VT-1). Previously, our group has shown that VT-1 induces an apoptotic pathway in BL cells which is dependent on caspases and mitochondria. Here, we provide new insights into this pathway. A pro-apoptotic member in the Bcl-2 family, Bid is cleaved by caspase-8 and its truncated form t-Bid is translocated to mitochondria. Using LB cell clones where Bid was inhibited prior to being reexpressed as a non-cleavable mutated form (BID D59A) and a caspase-8 inhibitor to explore VT-1-induced apoptosis, we showed that 1) the full length Bid (FL-Bid) controls the activation of pro-apoptotic proteins Bax and Bak; 2) Both t-Bid and FL-Bid are involved in the release of pro-apoptotic proteins (cytochrome c and Smac/DIABLO) from the mitochondrial intermembrane space to the cytosol; 3) FL-Bid controls the homo-oligomerization of both Bax and Bak, likely contributing to the initial release of cytochrome c and Smac/DIABLO while t-Bid is needed for their hetero-oligomerization followed by amplification of the release. Together, these results reveal a functional cooperation between Bax and Bak during VT-1-induced apoptosis and, most importantly, that activation of caspase-8 and t-Bid is not required to induce the onset of cell death. Gb3/CD77 is also expressed in a proportion of normal B-lymphocytes where it constitutes a differentiation marker but whose function remains uncharacterized. In an effort to look for physiological ligands, we have used a biochemical approach followed by mass spectrometry analysis. Two proteins have been identified as potentially Gb3/CD77 partners, namely galectin-7 and protein S100A11.

Traumatic brain injury (TBI) patients’ post-acute rehabilitation : the experience of family members

A traumatic brain injury (TBI) is what its name suggests: an extremely traumatic event that affects the most supremely powerful but fragile organ that controls all our bodily functions and holds the essence that makes individuals unique. Some patients with a traumatic brain injury will recover without realising what the effect could have been and others will be left with effects that will last throughout their lives. “Many sufferers will remain severely incapacitated and a lamentably large number will become part of the statistics on the mortality after TBI” (Burns, 2008a:76). The impairments that an individual faces after a TBI will be dealt with in an acute rehabilitation setting. Significant emotions are experienced by patients with a traumatic brain injury. The researcher believes that this emotional reaction is also experienced by the family members. The event may be even more devastating to the family members, because of the impaired cognitive functions of the injured person. Ross and Deverell (2004:36) state that when individuals are diagnosed with disabling conditions, they experience strong emotional reactions. “Feelings of grief, anxiety, inadequacy, anger, guilt, vulnerability and confusion are some of the more common emotions that clients and their families experience when they encounter a disability in themselves or a family member” (Ross and Deverell, 2004:41). After a traumatic brain injury the patient is usually admitted to an intensive care unit (ICU). Once medically stable and able to participate in an active rehabilitation programme, provided this is authorised by the patient’s medical aid, the patient is transferred to a rehabilitation unit. While in ICU, the outcome is very uncertain and the the family often receives very limited support. Patients are often in a coma and unable to communicate. In working with TBI patients, the researcher found that family members receive very limited support, education on the condition and feedback on the patient’s medical state from team members, and therefore become very anxious when the patient is eventually transferred to a rehabilitation unit. In this study, the researcher focused on the family’s experience from the time of the patient’s admission into ICU until discharge from rehabilitation. During the rehabilitation process, family members are encouraged to support their family member in the rehabilitation unit. This is often very difficult for them, as it entails being confronted with reality and the often devastating impact of the injury. The focus of this study is on the experiences of family members while the patient with a TBI is in hospital and in the rehabilitation unit and the challenges they face. The researcher strove to understand the experiences of family members of patients with TBI, from ICU through until discharge from acute rehabilitation. The goal was to explore the experiences of adult family members of the traumatic brain-injured person in post-acute rehabilitation. The research question was: What were the experiences of adult family members of the person with a traumatic brain injury (TBI) during post-acute rehabilitation? A qualitative approach was used in this study, with a collective case study research design. The population for this study was all the family members of patients who underwent rehabilitation as a result of traumatic brain injury in the Life Eugene Marais Hospital, Pretoria, Gauteng. In this study purposive sampling was used to choose participants who were family members of TBI patients. The criteria for sampling of family members as participants were as follows. The patient <ul> <li> had suffered a traumatic brain injury;</li> <li> had undergone acute rehabilitation at Life Eugene Marais Hospital for the last four years;</li> <li> had completed rehabilitation, in other words had to be post discharge;</li> <li> lived in the province of Gauteng; and</li> <li> was able to speak and understand English or Afrikaans, irrespective of gender, race, religion, culture or age.</li> </ul> Eight participants who were family members of eight TBI patients were chosen for this study. Semi–structured individual interviews were conducted with participants. Interviews were voice recorded with the permission of the participants and were transcribed by the researcher. The data were analysed by the researcher and the themes and sub-themes generated from the data. The research findings were presented by providing a profile of the research participants and then presenting the themes and sub-themes, including literature control and verbatim quotes from the transcriptions. The themes included the following: Theme One – Understanding of TBI; Theme Two – Period of hospitalisation; Theme Three – Family members’ emotional experience of TBI; Theme Four – Period of rehabilitation; Theme Five – Period post discharge; Theme Six – Support systems; Theme Seven – Effects of TBI and Theme Eight – Future. The conclusions of this study are that traumatic brain injury (TBI) is experienced as severe trauma by family members, who struggle to cope not only initially but especially once the person is discharged and has to be cared for at home. In the long term this experience leads to changes in the family regarding structure, roles, functioning, relationships, communication, finances and social life. (English) This diploma thesis deals with the issue of mental disorders and services provided to family members of people with mental disorder within a non-profit sector. The first part includes theoretical chapters which are processed by analysing available literature. The chapters inform about mental disorder, quality of life, and describe the care of people with mental disorders - particularly social skills and communication, housing, education, employment and leisure time. The theoretical part also focuses on social work with the mentally ill and includes a role of a social worker. In conclusion, the theoretical part describes the possibilities of supporting family members within the system of the care of persons with mental disorder, especially educational and psychoeducational programmes. The empirical part maps the current range of services for family members of people with mental disorder, and in connection with that, it determines what workers consider to be their benefit. Then, the views of family members on the psychoeducational programme and the current system of taking care of persons with mental illness were examined in a parent organization. 69 Abstract This diploma thesis deals with manifestations of a borderline type of emotionally unstable personality disorder and its influence on family members of people with this type of disease. The aim of this thesis was to present the experience of living with a person with BPD. Qualitative research, using the semi-structured type of interview, was conducted with three respondents, one man and two women. The data was evaluated using interpretative phenomenological analysis. The results show the negative effects of contact with the individual with the BPD and the psychological burden is also evident. The thesis could be beneficial for social and health workers who come into contact with people with the BPD and their loved ones within their practice. At the end of the thesis step are mentioned through which each of us can contribute to the improvement of the situation.

Organdonationens storm : Insikter från familj och närstående / The Storm of Organ Donation : Insights from Family and Loved Ones

Bakgrunden betonar vikten av organdonation för att rädda liv och förbättra livskvaliteten för patienter med terminal organsvikt. Trots ökningen av transplantationer globalt finns det fortfarande en betydande brist på tillgängliga organ, vilket leder till hög dödlighet bland väntande patienter. Närstående spelar en central roll i processen genom att ofta fatta beslut om organdonation när den potentiella donatorn inte själv har uttryckt en vilja. Intensivvårdssjuksköterskor har en nyckelroll i att stödja och kommunicera med närstående under denna känsloladdade process. Familjecentrerad vård är avgörande för att ge stöd och vägledning till familjer genom svåra beslut och sorgearbete.  Syftet var att beskriva upplevelsen av donationsprocessen ur ett närståendeperspektiv. Metoden omfattande en strukturerad litteraturöversikt med en kvalitativ ansats. Genom systematiska sökningar i CINAHL och PubMed har relevant litteratur inom området identifieras och analyserats i teman, för att få en djupare förståelse för ämnet. Totalt identifierades 17 artiklar. Dataanalysen genomfördes med hjälp av Braun och Clares metod för tematisk analys.  Resultatet avslöjar tre teman som belyser närståendes upplevelse av donationsprocessen. Det första temat stormens förståelse, symboliserar en tid av kaos och utmaningar där närstående upplevde olika aspekter av information och kommunikation. Samtidigt som de präglades av svårigheter att acceptera den juridiska dödförklaringen. Det andra temat stormens öga avslöjade närståendes erfarenheter av bemötande, stöd och beslutsfattande i samband med donation. Det tredje temat brisen efter stormen, belyser närståendes upplevelser av generositet och känslomässig kontakt med mottagaren. Dessa teman ger insikt i den emotionella resa och de utmaningar som närstående möter under donationsprocessen.  Slutsatsen är att närståendes upplevelse i donationsprocessen har en direkt koppling till familjecentrerad vård. Där en familjecentrerade vård på intensivvårdsavdelningar kan bidra till utvecklingen av framtida vårdstrategier och riktlinjer. / The background emphasises the importance of organ donation to save lives and improve the quality of life for patients with terminal organ failure. Despite the increase in transplants globally, there is a significant shortage of available organs, which results in high mortality among waiting patients. Family members play a central role in the process by often making decisions about organ donation when the potential donor has not expressed their wishes. Intensive care nurses have a pivotal role in providing support and communicating with families and loved ones during this emotionally intense process. Family centred care is crucial in order to support and guide families through difficult decisions and grief work. The aim was to describe the experience of the donation process through the perspective of family members and loved ones.  The method comprised a structured literature review utilising a qualitative approach. Systematic searches were conducted in CINAHL and PubMed to identify and analyse relevant literature within the field, in order to achieve a deeper comprehension of the topic. A total of 17 articles were selected for inclusion. Data analysis was performed using Braun and Clare’s method of thematic analysis.  The results reveal three central themes that highlight the experiences of families in the organ donation process, providing insight into the emotional journey and challenges faced by loved ones. The first theme, ‘Understanding the Storm’, symbolises a time of chaos and challenges where family members experienced different aspects of information and communication, whilst they faced difficulties in accepting the legal declaration of death. The second theme, ‘The Eye of the Storm’, featured the families experiences of treatment, support and decision making in the context of organ donation. The third theme, ‘The Breeze After the Storm’, highlights the families experiences of generosity and emotional connection with the recipient.  The conclusion determines that the experiences of next of kin in the donation process are intricately connected to family centred care. Implementing such care in intensive care units could significantly contribute to the development of future care strategies and guidelines.

Il coinvolgimento di utenti e familiari nella progettazione, gestione e valutazione dei servizi di salute mentale consente di migliorare i servizi, facendo valere gli interessi e la prospettiva di stakeholders che spesso vengono visti esclusivamente come ricettori passivi di prestazioni. La tesi prende il via da queste considerazioni, legate ad approcci recovery oriented, con l’obiettivo di analizzare come le organizzazioni sociosanitarie possono favorire la partecipazione e accogliere il punto di vista degli utenti e dei familiari attraverso l’introduzione di pratiche innovative. Per raggiungere questo obiettivo si è scelto di analizzare due casi: il Fareassieme del Servizio di salute mentale di Trento e il Social Point del Dipartimento di salute mentale di Modena. Attraverso osservazione e interviste, sono stati raccolti dati qualitativi che hanno portato a una interpretazione complessa e sfaccettata. In particolare, sono emersi due versanti: (1) quello del cambiamento culturale necessario per introdurre nell’organizzazione un nuovo approccio centrato sulla partecipazione e (2) quello della formalizzazione delle nuove pratiche partecipative, con i risvolti positivi e negati dell’incardinamento nell’organizzazione. Nelle conclusioni, si ipotizzano possibili sviluppi per la ricerca futura e si elencano i suggerimenti operativi emergenti dalla ricerca per i manager dei servizi sociosanitari. / The participation of users and family members in the design, management and evaluation of mental health services can improve services, relying on the interests and perspective of the stakeholders that are often seen only as passive recipients of services. The thesis takes away from these considerations, related to recovery-oriented approaches. The aim is to analyze how organizations can promote the health and social participation and accept the point of view of users and their families, through the introduction of innovative practices. I analyze two Italian cases: “Doing Together” (a project of the Mental Health Service of Trento) and “Social Point” (a service of the Department of Mental Health of Modena). Two issues emerged from qualitative data: (1) the organizational cultural change related to the introduction of a new approach, centered on participation and (2) the formalization of the participatory practices, with positive and negative consequences. In conclusion, I propose possible development for future research and suggestions to managers of health services.
