A study of the effects of sucralfate in the bile duct litigated pig peptic ulcer model with particular reference to the effects on the physico-chemical properties of gastric mucus and including comparisons with famotidine and misoprostol

Sucralfate is a drug that effectively heals duodenal, gastric and oesophageal ulcers. It is not absorbed systemically and it has been shown to act locally by coating the ulcer base. However when it was also shown to prevent stress ulcers and ethanolinduced gastric mucosa! lesions, it seemed likely that it acted in some way to improve the effectiveness of the gastric mucosa! barrier. Some investigators suggested that it did so by stimulating local prostaglandin release. The Slomiany group, on the basis of in vitro work on the effects of Sucralfate on pig gastric mucus, claimed that Sucralfate acted by altering the physico-chemical properties of mucus to increase the viscosity and retard the back diffusion of H+ ions. The work described in this dissertation set out to verify, in vivo, these claimed effects on mucus, using an experimental porcine model of peptic ulceration, the bile duct ligated pig. In addition, the effects of Sucralfate were compared with those of Famotidine and Misoprostol, and changes in mucous prostaglandins, gastric juice pepsin and gastric flora were sought. By way of introduction, the known and postulated actions of Sucralfate, current understanding of gastric mucus physiology and pathogenesis of peptic ulceration, have been reviewed, as have experimental animal models of peptic ulceration, in order to justify using the bile duct ligated pig model.

Psychosoziale Auswirkungen von Magenrestriktionen bei extremer Adipositas: Ergebnisse einer Zwei-Jahres Katamnese / Psychological and social effects after gastric restrictive surgery for extreme obesity: Results from a 2-year longitudinal study

Adipositas dritten Grades (BMI>40 kg/m2) geht mit erheblichen gesundheitlichen sowie psychosozialen Beeinträchtigungen einher. Da sich konservative Behandlungsversuche als langfristig wenig wirksam erwiesen haben, werden in der evidenzbasierten Leitlinie zur Therapie der Adipositas chirurgische Magenrestriktionen empfohlen. 39 Patienten, die sich einer Magenrestriktion unterzogen, wurden über einen Zeitraum von zwei Jahren nachuntersucht. Die Datenerhebungen fanden zu drei Zeitpunkten statt: vor der OP, 9 und 24 Monate postoperativ. Es wurden 46 Pat. in die Studie aufgenommen, von denen 39 (85%) über den gesamten Zeitraum nachbefragt werden konnten. Als Vergleichsgruppe wurden Daten einer konservativ stationär behandelten Gruppe herangezogen. Die operierten Patienten erzielten eine starke Gewichtsreduktion. Der BMI sank von durchschnittlich 50,4 kg/m2 auf 35,8 kg/m2 nach 24 Monaten. In der Vergleichgruppe war keine signifikante Gewichtsreduktion zu beobachten. Insgesamt zeigten sich bei den operierten Patienten deutliche positive Effekte auf psychosoziale Parameter, während in der Vergleichsgruppe kaum Verbesserungen zu beobachten waren. Die operierten Patienten zeigten folgende Verbesserungen: Die Depressivität war signifikant rückläufig. Das Essverhalten verbesserte sich signifikant. Die kognitive Kontrolle stieg an, während die Störbarkeit des Essverhaltens sowie Hungergefühle zurückgingen. Körperliche Beschwerden verringerten sich statistisch bedeutsam und die Lebenszufriedenheit stieg in zwei Bereichen signifikant an. Zwei Jahre nach der Operation wurde das Übergewicht in mehreren Lebensbereichen seltener als Problem empfunden. Nach zwei Jahren gaben 37 der 39 Patienten (95%) an, dass sie sich erneut für den Eingriff entscheiden würden. Aus den Ergebnissen der vorliegenden Studie kann gefolgert werden, dass grundsätzliche psychologische Bedenken gegenüber der Magenrestriktion zur Behandlung extremen Übergewichtes unbegründet sind.

Paving the Way: A Grounded Theory of Discovery and Decision-Making for Persons Diagnosed with the CDH1 Marker

Purpose: To understand the process of discovery and decision-making for adults with the CDH1 marker for hereditary diffuse gastric cancer (HDGC) and inherited breast cancer. Participants and Setting: Purposeful sampling included 20 participants; 17 adults (11 women and 6 men, ages 23–77) recruited through the No Stomach for Cancer organization; six participants were interviewed two times; with three healthcare providers also interviewed. Nineteen interviews were by telephone; one was in person. Methodological Approach: Grounded theory with constant comparison. Findings: The person diagnosed with the genetic marker CDH1 undergoes the decision-making process of Paving the way as they address this healthcare challenge. Paving the way explains the entry points for learning the risk, discerning testing for confirmation, choosing iterative individual cycles of surveillance, surgery, and ongoing adjustments postoperatively while normalizing to live longer. Implications for Nursing: Understanding the process of Paving the way explains and describes the nine key factors for decision-making and predicts the timing for nursing interventions for both post-genetic testing and pre- and postoperative assessment and planning. Knowledge Translation: Advocacy for the self and family is key to Paving the Way. Nursing has an opportunity to develop and expand the roles for navigator and counselor in the area of genetic testing. Patients undergoing PTG have chronic healthcare needs. Family implications for genetic testing require assessment beyond the individual.

Predictive and prognostic factors in gastric cancer Šmíd D. Surgical clinic of University Hospital in Pilsen and Faculty of Medicine in Pilsen, Charles University in Prague. Introduction: Gastric cancer is one of malignant diseases which have the worst prognosis. Unfortunately, there are most patients with advanced-stage disease who have to be treated in a palliative way. Patients suffered from the same type of tumor, being at the same stage of disease and treated with the same chemotherapy have various rates of survival, which can be caused by diverse expression of selected genes impacting on the mechanism of cytostatic effects. The determination of these genes or microRNAs which regulate these genes could be used as a predictive factor for prediction of effects of administered chemotherapy. The determination of some microRNAs, or in the combination with suitable plasmatic factors, could be used as a prognostic factor for patients with gastric cancer. It is also possible to use this combination for early diagnosis of cancerogenesis Object: The aim is to verify the possibility to use expression of selected genes and some microRNAs in tumor tissue as a prognostic factor or a predictor for therapeutic effects of chemotherapy in patients with gastric cancer.
