Sexual Anxiety Among Non-Monogamous Men Who Have Sex with Men: A Phenomenological Study

Sexual anxiety is a prevalent issue among men who have sex with men (MSM) that negatively affects sexual satisfaction and wellbeing. However, research investigating sexual anxiety among MSM has been scarce. This descriptive phenomenological study was conducted to address knowledge gaps and identify the predominant underlying causes of sexual anxiety among non-monogamous MSM. Using purposive sampling, eleven sexually active, non-monogamous MSM were recruited for this study (N=11). Data were collected using audio-recorded, semi-structured interviews, and analyzed using Moustakas’ (1994) structured approach to data analysis in phenomenological research. Participant descriptions of their lived experiences of sexual anxiety identified six prevalent causes among MSM individuals. These included performance anxiety, fear of HIV, fear of bacterial STIs, internalized homophobia, body image dissatisfaction, and history of sexual abuse. Not surprisingly, fear of HIV was the dominant cause of sexual anxiety among most participants. Recommendations to prevent and reduce sexual anxiety among MSM are provided.

Sexual Partnership-Level Correlates of Intimate Partner Violence Among Men Who Have Sex with Men and Transgender Women in Lima, Peru

To improve understanding of factors associated with intimate partner violence (IPV) and explore its role in sexually transmitted infection (STI) acquisition, we analyzed partnership-level correlates of IPV among men who have sex with men (MSM) and transgender women (TW) in Peru. In a 2017 cross-sectional study of rectal STI screening and HIV prevention, MSM/TW completed a sociobehavioral survey addressing demographic characteristics, sexual risk behaviors, and substance use, and were tested for rectal gonorrhea and chlamydia, syphilis, and HIV. Generalized estimating equations estimated individual- and partner-level correlates of IPV. Of 576 participants (median age, 27 years), 7.9% (36/456) of MSM and 15.0% (18/120) of TW reported IPV with ≥ 1 of their last three partners. MSM/TW reporting IPV were more likely to meet criteria for an alcohol use disorder (74.1%) than participants reporting no IPV (56.7%; p <.01). Physical violence (4.5% MSM; 9.2% TW) was associated with stable partnerships (aPR 3.79, 95% CI 1.79–8.04), partner concurrency (4.42, 1.19–16.40), and participant alcohol (4.71, 1.82–12.17) or drug use (5.38, 2.22–13.02) prior to sex. Psychological violence (4.5% MSM; 5.0% TW) was associated with stable partnerships (2.84, 1.01–7.99). Sexual IPV was reported by 1.1% of MSM and 5.0% of TW. Introduction: Men who have sex with men (MSM) are more likely to experience Intimate partner violence (IPV) during their lifetime than heterosexual couples. HIV-status and related HIV stigma may play an important role in IPV within intimate partnerships among MSM. HIV- and MSM-related stigmas are common among Chinese MSM, yet there is sparse literature on IPV and its relationship with stigma in this vulnerable population. We evaluate the prevalence of IPV and its relationship with HIV- and MSM-related stigma among newly diagnosed HIV-infected MSM in Beijing, China. Methods: Data were collected in the baseline survey among newly HIV-diagnosed Chinese MSM in a randomized clinical trial via face-to-face interviews. Simple and multiple logistic regression analyses were performed to assess the associations between IPV and HIV- and MSM-related stigma. Results: Of 367 newly HIV-diagnosed Chinese MSM, 23.7% experienced any IPV, including 16.6% physical, 7.4% psychological and 5.2% sexual IPV. Positive associations were found between HIV- and MSM-related stigma and IPV. Men with high HIV-related stigma (score ≥27) were 1.68 times as likely to experience any IPV as those with low stigma (adjusted odds ratio [AOR]: 1.68, 95% confidence interval [CI]: 1.03-2.76). Men with high MSM-related stigma (score ≥6) were 1.98 times as likely to experience any IPV as those with low stigma (AOR: 1.98, 95% CI: 1.19-3.31). Men with high HIV- and MSM-related stigma were 2.86 times as likely to experience any IPV as those with low stigma (AOR: 2.86, 95% CI: 1.44-5.69). Conclusion: HIV- and MSM-related stigma was positively associated with IPV experiences among newly diagnosed MSM in China. The manner in which stigma may exacerbate IPV, and/or the influence of IPV on worsening stigma should be further evaluated. The high prevalence of IPV and stigma in this population suggests that interventions should be taken to reduce stigma and prevent this risky behavior among MSM.

Effectiveness of Doxycycline as Post-exposure Prophylaxis (Doxy-PEP) for Bacterial Sexually Transmitted Infections (STIs) Among Men Who Have Sex with Men (MSM) in Central Florida

Bacterial sexually transmitted infections (STIs), notably syphilis, gonorrhea, and chlamydia, have been increasing among men who have sex with men (MSM) in the United States. The rise has been attributed to various factors, including increased condomless sexual acts due to the high uptake of pre-exposure prophylaxis (PrEP) for HIV prevention. To address this concerning trend, post-exposure prophylaxis with doxycycline (Doxy-PEP) has emerged as a potential biomedical intervention. This study aimed to assess the effectiveness of Doxy-PEP in preventing bacterial STIs among MSM in Central Florida, a region with rising STI rates. A de-identified dataset from Pineapple Health Clinic in Orlando, Florida, was analyzed (n = 73) participants who received Doxy-PEP for bacterial STIs between July 01, 2022, and June 30, 2023. Results revealed elevated rates of any bacterial STI and syphilis incidence among both HIV-negative and HIV-positive individuals, contrary to expectations from previous literature. Notably, the HIV-positive cohort showed significantly higher incidence rates of syphilis. The study also found discrepancies in gonorrhea incidence and resistance profiles, raising questions about the effectiveness of Doxy-PEP against gonorrhea in this population. The older age of the HIV-positive cohort and differences in medication regimens further complicated the analysis. These findings suggest the effectiveness of Doxy-PEP may vary in real-world settings compared to controlled trials. Limitations include the small sample size, single healthcare clinic setting, differences in medication regimens, and the investigational status of Doxy-PEP. However, the study highlights the need for interventions considering diverse demographics and local antibiotic resistance patterns. Future research should explore these factors to develop effective strategies for reducing STI incidence among MSM in Central Florida.

Intimate Partner Violence Victimization and the Increased Risk of HIV Among Young Black Men Who Have Sex With Other Men In Jackson, MS 6-Month Follow-Up

Introduction: Intimate Partner Violence (IPV), also called domestic violence, is defined as any physical, psychological, or sexual violence, and emotional violence perpetrated by an intimate partner (CDC, 2016). Sexual violence or rape is defined as a sexual act committed against someone without that person’s freely-given consent (CDC, 2017). IPV is experienced among heterosexual women globally, although this phenomenon is not unique to this population. IPV also disproportionately affects young gay, bisexual, and other men who have sex with men (MSM) (Stults et al., 2016). The purpose of this study is to examine the association between IPV victimization and the increased risk of HIV and other health outcomes among YBMSM. Methods: The 6-month follow-up was conducted in Jackson, MS. A total 600 YBMSM were eligible to participant in the study. The participants were recruited from two local clinics that were focused on diagnosing and treating STI’s. Also, recruiters promoted the opportunity to enroll in the study through social media, attending bars, and nightclubs. Inclusion criteria were: 1) assigned male at birth; 2) self-identification as Black/African American; 3) 15-29 years of age; 4) attending the clinic to be tested for HIV or other STIs, 5) having engaged in anal sex with a male partner at least once in the past 6 months, and 6) the ability to speak and comprehend English. Results: The overall study sample consisted of 600 YMSM. All participants were identified as African–American. The average age of the sample was 22.6 years (SD=±3.2). Bottoms were more likely to experience rape (AOR=1.7,CI 1.1,2.6; p=.02). YBMSM who experienced IPV since enrolling in the study, as compared to those who did not experience IPV, were more likely to use marijuana, alcohol, crack, and painkillers (AOR=7.7 ,CI 5.1,11.7; p= Conclusion: These findings suggest that IPV and Rape shows a significant association with decreased condom use, decreased condom negotiation, sexual position, drug use and HIV risk among YBMSM in Jackson, MS.

Com a presente dissertação de mestrado, tivemos o objetivo de analisar o essencial das políticas destinadas ao Ensino Superior no Brasil, principalmente as implantadas no século XX e início do XXI, tendo em vista a persistência dos dilemas e desafios que permeiam os comportamentos e as ações sociais no país. Partimos da perspectiva de que o sistema de ensino enfrenta, de um modo geral, uma série de entraves institucionais, repercutindo na falta de qualidade da educação, na contenção e evasão dos estudantes, no baixo reconhecimento e remuneração dos profissionais da área, no oportunismo político que nada de positivo acrescenta ao país etc. Na metodologia, utilizamos pesquisas bibliográficas e documentais. Em termos bibliográficos, consideramos as áreas de contato entre a sociologia, a ciência política, a filosofia, a economia e a física; no que diz respeito às políticas públicas, a importância da sociedade civil organizada, o liberalismo, a ciência etc. No aspecto documental, por sua vez, utilizamos leis, decretos, relatos, enfim, registros que, quando observados conjuntamente, resultaram em evidências e hipóteses que se entrelaçam e que foram organizados em três momentos do trabalho. No primeiro, a consideração da \"realidade\" histórica, social, política e econômica em que as políticas para o Ensino Superior estão sendo implantadas, corroborando uma hierarquia social autoritária. No segundo, a necessidade de se conceituar as políticas públicas num contexto de relativo avanço democrático, revelando estratégias destinadas a limitar a participação social no que diz respeito à deliberação sobre a destinação dos fundos públicos. No terceiro momento, a análise do conjunto das políticas adotadas, tendendo a reafirmar uma sociedade excludente e desigual. Concluindo, o enfretamento das questões prementes para o Ensino Superior a partir do conjunto das políticas, que poderiam ser pensadas com a perspectiva da sociedade na qual estão inseridas, indicou uma continuidade de princípios e diretrizes, de cunho liberal, a ser superada. Sem a perspectiva de uma ruptura política profunda, pode-se considerar que a atualização, em termos de políticas educacionais, tem sido totalmente contrária às demandas que pautam o país. Na ausência de uma transformação ampla desse quadro, a lógica que tem pautado as políticas no Brasil continuará respondendo a um projeto de país subordinado, contrário aos interesses da sociedade e, portanto, de nada adiantando tal ou qual política sem a consideração da \"força das coisas\" na sua totalidade e a sua aceitabilidade interna. / With the present dissertation, we had the goal of analyze the essential of the policies addressed to higher education in the Brazil, mainly those implanted at XX and earlier XXI century, considering the persistence of dilemmas and challenges that permeate the human behavior and social actions in the country. We started from the perspective that, in a general way, the educational system faces a range of institutional bridles, rebounding on the lack of educational quality, on the students hold and school evasion, on the low recognition to and payment for the professionals of this area, on the political opportunism that nothing of positive adds to the country, and so on. At the methodology, we utilized bibliographic and documental researches. In terms of bibliography, the study considered contact areas between the sociology, the politic science, the philosophy, the economy, and the physics; what was concerned about public policy, the importance of organized civil society, the liberalism, the science and so on. At the documental aspect, for your turn, we utilized laws, decrees, accounts, in short, records that, when observed jointly, resulted in evidences and hypothesis that interlace themselves and that were organized in three moments of the work. At the first, the consideration of the historical, social, political and economical reality in what the policies for the higher education are been implanted, that corroborates an authoritarian social hierarchy. At the second, the necessity of making conception of the public policies in a context of relative democratic advance, revealing strategies destined for limiting the social participation concern decision about the destination of public founds. At the third moment, the analyses of the policies set adopted, tending to reaffirm an excluding and unequal society. Concluding, the confront of pressing questions to the higher education starting from the policies set, that could be thought with the perspective of society in whose they are insert, indicated a continuity of principles and guidelines, with liberal matrix, to be overcome. Without the perspective of a deep politic rupture, we can consider that the update, in terms of educational policies, has been totally contrary to the demands that rule the country. In the lack of a broad transformation of this frame, the logic that has ruled the policies at Brazil will keep responding to the subordinated country project, opposed to the society interests and, therefore, its not worth this or that policy without the consideration of the things power in its totality and its intern acceptability.
