Social cognition deficits and violence in people with a diagnosis of schizophrenia

Introduction It is widely reported that people with schizophrenia have social cognition deficits. In addition to their negative impact on functioning and quality of life, these deficits may also contribute to the use of violence. It has recently been established that social cognitive interventions (SCIs) can ameliorate deficits in facial affect recognition (FAR). This project aimed to systematically review whether SCIs can also improve theory of mind (ToM) abilities in people with schizophrenia. The empirical study aimed to explore whether the extent of the deficits in FAR and ToM in people with schizophrenia differed between those with and without a substantial history of violence. Method A systematic review was undertaken to identify studies where SCIs were provided to adults with schizophrenia or schizoaffective disorder. Key findings were highlighted with the quality of the studies’ methodology and reporting assessed. A quantitative research study was also undertaken involving 22 men aged 18-64 with a diagnosis of schizophrenia or schizoaffective disorder, comparing those with and without a substantial history of violence (SHV) on measures of FAR and ToM. Results The majority of the 13 studies included in the systematic review found that the provision of SCIs led to significant improvements in ToM. However, all studies demonstrated a potential for bias and were limited by inadequate sample size. In the empirical study, less than half of participants scored within the normal range for overall FAR ability, with no difference identified between the SHV and no-SHV group. However, the SHV group were poorer at recognising sadness and showed a tendency to perform better at the detection of faux pas, compared to the no-SHV group. Conclusions The systematic review identified that a wide range of SCIs can improve ToM abilities in people with schizophrenia. Its findings highlight that stringent, adequately powered studies should be undertaken, utilising standardised assessments of a range of levels of ToM ability, to enable identification of the most effective intervention. The findings of the empirical study are limited by a small and imbalanced sample size between groups and so must be interpreted with caution. However, patterns observed in the results highlight areas for further exploration. The strengths of this study’s design and recruitment challenges are discussed.

Making heat visible : improving household heat efficiency through thermal images

Energy is largely invisible to users. It has been argued that employing technologies to visualise energy will assist people in conserving energy. Energy visibility interventions have largely focussed on appliance use and electricity consumption. This thesis aims to firstly explore whether making heat visible, using thermal images, promotes heat (and thereby energy) conservation. Secondly using a multiple method approach, it explores how. Five studies were employed. Study One and Three investigated whether using thermal images as a tailored antecedent intervention would promote energy conservation behaviours. The results confirmed that the images led to a reduction in Kg CO2 emissions attributed to domestic energy use. Study One and Three indicated that householders undertook more energy saving behaviours in relation to those aspects that were visible in the images. These actions were attributed to simple, energy saving behaviours such as proofing draughts. Study Two investigated how people make sense of the images and how behaviours are promoted by the images. Study two suggested that the images provide a unique medium through which factors which contribute to energy saving can be combined and reasoned by the viewer. It suggests the psychological factors in a pathway from prompt to behaviour. Study Four established that showing the images in an information presentation was not as effective when influencing participants’ ideas about energy conservation. Finally, Study Five explored participants gaze and demonstrated how features of the images, can attract the viewer. The novel contribution of this thesis is in establishing that ‘making heat visible’ through a tailored thermal imaging prompt can increase the likelihood of a householder taking simple energy saving actions, by providing a novel medium through which householders attend to heat and energy use.

The Remediation of Students in Counseling Graduate Programs: Behavioral Indicators, Terminology, and Interventions

This study explored the concept of student remediation in counseling graduate programs by examining the behavioral indicators associated with student remediation, the terminology used to discuss student remediation, and remedial interventions and their effectiveness. Members of the Association for Counselor Education and Supervision (ACES) were electronically surveyed using a researcher-designed instrument, the Counseling Graduate Student Remediation Questionnaire. A total of 607 individuals participated in the study for a response rate of 28.8%. The results of this study indicated that the overall five behavioral indicators considered by participants as needing remediation were the following: receptivity to feedback; basic counseling skills; boundaries with clients, supervisors, and/or colleagues; openness to self-examination; and advanced counseling skills. Five factors were identified within the behavioral indicators based on which can be remediated: Factor I, Personal Competencies; Factor II, Professionalism; Factor III, Personal Challenges; Factor IV, Honesty; and Factor V, Clinical Competencies. Significant group differences were found between counselor educators' and doctoral students' perceptions of what needs remediation with counseling graduate students, as well as what can be remediated with students. Significant group differences also were present between administrative faculty and non-administrative faculty on perceptions of what needs remediation with students. The overall three most preferred terms by participants used to discuss students in remediation included: challenging, problems with professional competence, and problematic. For remedial interventions, participants perceived that increased supervision was often effective as an intervention and that referring or recommending to personal counseling was occasionally effective. The results of this study seemed indicative of a broadening perspective regarding the topic of remediation, with perceptions shifting toward a positive framework rather than the more pejorative historical approaches. Overall, the conclusion from this study was that student remediation is currently developing within the field as a distinct concept with many specific associated behavioral indicators, terminology, and interventions.

Exploring an Monolingual English - Speaking Teacher's Perceptions of Classroom Interventions to Foster Hispanic English Language Learners' Primary Language

Although awareness of cultural diversity is slowly occurring, the need for multicultural curricula is increasing more each day as even more ethnically and linguistically diverse students enter into United States classrooms. The education of English Language Learners is a controversial topic due to people's mixed beliefs on the amount of English and primary language instruction needed to aid students' English language development. Due to the shortage of bilingual teachers many English Language Learners in the Unites States are learning English through the regular education classroom instructed by monolingual English-speaking teachers. This case study implemented qualitative research methods in order to extend understanding of how a monolingual English speaking teacher can strategically incorporate Hispanic English Language Learner's primary language into the classroom setting and the teacher's experiences throughout this process. This case study will introduce a monolingual English speaking teacher to interventions that can foster the use of Hispanic English Language Learners' primary language in the classroom setting. The findings of this study include the experiences the participating teacher encounters throughout the intervention process. These findings include: heightened awareness, challenges, changes to interventions and enjoyment.

Private Diplomats, Mediation Professionals, and Peace Activists: Can Non-governmental Actors Bring Peace to Civil Wars?

This dissertation investigates how actors without the means of state power can affect the behavior of warring parties in order to end civil conflicts. Drawing on the intervention and mediation literature, I propose a theoretical framework that presents causal mechanisms for various forms of non-state conflict management to contribute to conflict resolution. The research distinguishes between direct mediation, capacity-building, and problem-solving approaches, and analyzes the approaches’ potential contributions to shorter wars and more sustainable peace. On the one hand, non-state actors can be substitutes for governmental or inter-governmental mediators. They derive legitimacy from long-standing relations with the conflict parties, and their claims to neutrality are more believable than those of powerful states with strong national interests. Further, a confidential and deliberate process can lead to more stable agreements. On the other hand, NGOs and others can prepare or enhance ongoing high-level negotiations by giving parties the tools they need to engage with each other constructively, and by improving attitudes and changing perceptions. The data collected for this dissertation allows me to test hypotheses for the sample of African internal conflicts (1990-2010) with econometric means. Results confirm that non-state conflict management is a significant precursor to high-level mediation. I find further that conflict dyads that experience non-state conflict management in one year are significantly more likely to end in the following year. Unofficial diplomacy is significantly related to lower conflict severity, as well as to a more stable post-conflict peace. The findings challenge the common assumption that governments are the only actors in international relations that matter. In fact, non-state actors make important contributions to conflict resolution, and conflict parties as well as governmental mediators should consider cooperating with them in their search for peace.

Un modèle pour les algorithmes avec retour sur trace dans les tuteurs par traçage de modèles

La présente thèse décrit des travaux de recherches effectués sur des systèmes tutoriels intelligents (STI) et plus précisément sur les tuteurs par traçage de modèle (MTT). Les travaux de recherche présentés ici s’intéressent à la conception de MTT pour des domaines dans lesquels les étudiants peuvent résoudre la tâche qui leur est assignée de plusieurs façons. Ces domaines comportent parfois des algorithmes avec retour sur trace lorsque l’étudiant ne sait pas forcément quelles sont les alternatives qui feront progresser correctement l’état de la tâche.Cette thèse présente dans un premier temps un système de représentation de connaissances pour les algorithmes avec retour sur trace qui rend les connaissances de cet algorithme exploitables par des agents logiciels. Elle présente dans un second temps un ensemble de processus qui exploitent ces connaissances dans le cadre de MTT pour assurer automatiquement le suivi de l’étudiant et ainsi que la production d’interventions pédagogiques. En premier, ces interventions consistent à fournir à l'étudiant de l’aide pour la prochaine étape qui explique quelles sont les possibilités dont dispose l'étudiant et comment déterminer laquelle est la meilleure. En deuxième, elles fournissent à l'étudiant des rétroactions stratégiques qui lui confirment que son action est valide tout en l’informant de l’existence d’une meilleure alternative le cas échéant. Enfin, elles fournissent à l'étudiant des rétroactions négatives qui lui apprennent dans quelles situations les actions invalides qu’il vient d’effectuer s’appliquent.Une expérimentation a été réalisée avec des étudiants de biologie de l’Université de Sherbrooke pour évaluer les effets de ces interventions sur les choix des étudiants au cours de la résolution de la tâche. Les résultats de cette expérience montrent que les étudiants bénéficiant de ces interventions effectuent plus souvent des choix optimaux, et démontrent ainsi une plus grande maîtrise du domaine.

Weight bias internalization, core self-evaluation, and health in overweight and obese persons

Objective: Weight bias has strong associations with psychopathology in overweight and obese individuals. However, self-evaluative processes, as conceptualized in the process model of self-stigma, and implications for other health-related outcomes, remain to be clarified. Design and Methods: In a representative general population sample of N = 1158 overweight and obese individuals, the impact of core self-evaluation as a mediator between weight bias internalization and mental and global health outcomes as well as between weight bias internalization and health care utilization, was examined using structural equation modeling. Results: In overweight and obese individuals, greater weight bias internalization predicted lower core self-evaluation, which in turn predicted greater depression and anxiety, lower global health, and greater health care utilization. These mediational associations were largely stable in subsample analyses and after controlling for sociodemographic variables. Conclusions: The results show that overweight and obese individuals with internalized weight bias are at risk for impaired health, especially if they experience low core self-evaluation, making them a group with which to target for interventions to reduce self-stigma. Weight bias internalization did not represent a barrier to health care utilization, but predicted greater health care utilization in association with greater health impairments.

Policy approaches to prevent chronic non-communicable diseases: The role of population-based data

Philosophiae Doctor - PhD / Background: Non-communicable diseases (NCDs) continue to rise in South Africa, accounting for 43% of total deaths in 2012. Smoking and a diet high in salt are among the major modifiable risk factors for NCDs that can be addressed through cost-effective policy interventions in the form of regulation or legislation and active multisectoral engagement. Population-based prevalence and mortality data are necessary for monitoring and evaluation such interventions. South Africa has developed a National Strategic Plan for NCDs but there is limited evaluation of NCD policies. Furthermore, there is a need to explore the availability of population-based data and the role that it can play to monitor interventions. Aim: The overall aim of the thesis is to assess the implementation of policies for reducing risk factors for chronic NCDs in South Africa, and to explore the role of population-based data in supporting environmental and policy approaches to prevent NCDs. The thesis will also examine whether there are differences in urban and rural settings in the implementation of tobacco control and salt reduction regulation as well as the barriers to implement the National Strategic Plan for prevention of NCDs. Methods: Multiple methods of data collection were used. A desk review of policies to address NCDs in South Africa was undertaken and semi-structured interviews with the NCD policymakers and managers in two provinces (the Eastern Cape and Western Cape) were undertaken, to explore challenges and successes of implementation of the NSP. The Cross-sectional baseline questionnaire and quantified food frequency data from the PURE study were used to determine the prevalence of smoking and the intakes of sodium and potassium in a selected urban and a rural community. Data collected using a validated community audit tool was used to assess the physical environment related to tobacco as well as questionnaire data from face-to-face interviews about perceptions about tobacco use in the urban and rural communities. Trends in mortality from tobacco related and high salt consumption related conditions together with prevalence data from national health surveys were reviewed to assess the health impact.

The effect of a youth development programme combatting engagement in health risk behaviours amongst grade 8 learners in a selected high school in the Paarl area

Magister Scientiae (Physiotherapy) - MSc(Physio) / Background: Health risk behaviours can directly affect health outcomes. Healthy behaviours such as exercising and eating sensibly can lower the risk of conditions like heart disease and diabetes, while unhealthy behaviours such as smoking and excessive drinking raise the risk of conditions like lung cancer and liver disease. In a study using two large national data sets, the Youth Risk Behaviour Survey (YRBS) and Add Health, the Center on Addiction and Substance Abuse (CASA) found that teenagers who consume alcohol or take illicit drugs are more likely to engage in sex, to do so at a younger age, and to have several partners. For adolescents who are 14 years old and younger, consuming alcohol or using drugs doubles and quadruples, respectively, the likelihood that sexual intercourse has ever been experienced compared to adolescents who have never used these substances. A study reported that early onset of alcohol, tobacco and other drugs, school problems, delinquency, and physical aggression are significantly associated with early onset of sexual behaviour. Alcohol use in adolescence has also been found to be related to more frequent sexual activity and less frequent use of condoms. Aim: To investigate and explore the effect of a youth development programme (YDP) of combatting health risk behaviours amongst 250 Grade 8 learners in a selected high school in the Paarl area.
