A biodisponibilidade de ferro de diferentes fontes foi avaliada pelo método de recuperação de hemoglobina em suínos anêmicos. O ensaio teve duração de 13 dias e os grupos foram homogeneizados de acordo com o produto do peso (kg) x hemoglobina (g/dL). As fontes de ferro estudadas foram: ferro microencapsulado com polímero de carboximetilcelulose sódica (NaCMC), ferro microencapsulado com polímero de alginato, ferro quelado com metionina e ferro reduzido eletrolíticamente. Com a finalidade de corrigir respostas devidas à variação na ingestão de ferro foram acrescentados três grupos chamados de grupos-padrão, cuja fonte dietética de ferro foi FeSO4.7H2O. A porcentagem média de absorção de ferro para os grupos experimentais variou de 12,8 (4,3) % (ferro eletrolítico) a 15,1 (3,8) % (ferro alginato), não sendo significativamente diferentes (P>0,05). A porcentagem média de absorção relativa ao FeSO4.7H2O variou de 89,5 (25,9)% (ferro NaCMC) a 105,9 (60,5)% (ferro quelado com metionina), sem diferenças significativas (p>0,05). Também não houve diferença significativa nos índices de conversão alimentar, eficiência alimentar e coeficiente de eficácia proteica (CEP). Não houve diferenças significativas (p>0,05) nas concentrações de ferro hepático entre as diferentes fontes estudadas. Nas condições deste ensaio, houve similaridade entre as fontes de ferro testadas quanto à biodisponibilidade. A microencapsulação proporcionou rneltlOr distribuição do produto na mistura quando comparada ao FeSO4.7H2O. / The iron availability from different sources was evaluated by the hemoglobin regeneration method in anemic pigs. The assay took 13 days and the animais were selected according to the weight (kg) x hemoglobina (g/dL) product. The iron sources studied were: microencapsulated iron with sodium carboximetilcelulose polymer (NaCMC), microencapsulated iron with alginato polymer, metionin-quelated iron and electrolytically reduced iron. To COITect answers due to iron ingestion variation three control groups were added, whose dietary iron source was FeSO4.7H2O. The average percentage of iron absorption for the experimental groups ranged from 12,8 (4,3) % (electrolytic iron) to 15,1 (3,8) % (iron alginato), without significant differences (p>0,05). The average percentage of FeSO4.7H2O relative absorption ranged from 89,5 (25.9)% (NaCMC iron) to 105,9 (60,5)% (metionin quelated-iron), without significant differences (p>O,OS). Also it did not have significant difference in the alimentary conversion and efficiency indexes and proteic effectiveness coefficient (CEP). No significant differences (p>0,05) were found in the hepatic iron concentrations among the different studied sources. Based on this assay\'s conditions, there were similarities among the tested iron sources regarding to their bioavailability. Microencapsulation provided better distribution of the product in the mixture when compared to the FeSO4.7H2O.

Avaliação dos níveis de hemoglobina de gestantes brasileiras antes e após a fortificação de farinhas com ferro / Evaluation of hemoglobin levels of Brazilian pregnant women before and after the fortification of flours with iron

Objetivo: Avaliar os níveis de hemoglobina de gestantes brasileiras antes e após a fortificação das farinhas de trigo e milho com ferro e investigar as variáveis associadas. Método: Estudo transversal que integra um projeto matricial desenvolvido com dados retrospectivos obtidos de prontuários de 12.119 gestantes atendidas em serviços públicos de pré-natal localizados em 13 municípios das cinco regiões geográficas do Brasil, divididas em dois grupos: Antes-fortificação das farinhas com ferro (gestantes com parto realizado antes de junho de 2004); e Após-fortificação (gestantes com data da última menstruação posterior a junho de 2005). A coleta de dados ocorreu em 2006-2008 e incluiu apenas gestantes de baixo risco, cujos prontuários continham pelo menos a data da primeira consulta de pré-natal e da última menstruação e a dosagem de hemoglobina (Hb). A variável dependente foi o nível de Hb (g/dL) e as independentes foram: grupo de fortificação, região geográfica, características sociais e demográficas, antecedentes obstétricos e características do pré-natal. Realizou-se análise descritiva, univariada e múltipla do nível de Hb para o total das gestantes, por região geográfica e por trimestre de gestação, por meio de modelos de regressão linear. Contruíram-se modelos de regressão polinomial para o ajuste das curvas de Hb por mês de gestação, que foram comparadas com referências nacional e internacional. Curvas de níveis médios e críticos (-2 desvios-padrão) de Hb de gestantes não anêmicas do grupo Após-fortificação também foram comparados às referências. O nível de significância de todos os testes foi de 5%. O estudo foi aprovado por Comitê de Ética em Pesquisa. Resultados: Não houve aumento significativo do nível de Hb para o total da amostra (p=0,325) após a fortificação, exceto em gestantes da região nordeste (p<0,001; =0,214) e em gestantes no segundo trimestre de gestação (p=0,025; =0,093). O nível de Hb foi menor entre aquelas que tinham menor idade, viviam sem companheiro, tinham menor Índice de Massa Corporal-IMC, maior idade gestacional e/ou duas ou mais gestações anteriores. Apesar da curva Após-fortificação apresentar níveis superiores em todos os meses de gestação, a regressão polinomial não mostrou efeito significativo da fortificação de farinhas (p=0,316). As curvas de Hb de ambos os grupos de fortificação mostraram-se acima dos níveis críticos das referências nacional e internacional no primeiro trimestre, com queda a seguir e estabilização no final da gestação. A curva de níveis críticos, construída com dados de gestantes não anêmicas do grupo Após-fortificação, ficou abaixo da curva de níveis críticos da referência nacional e do ponto de corte da OMS, mas semelhante à referência internacional, exceto no final da gestação. Conclusões: A fortificação de farinhas com ferro aumentou significativamente o nível de Hb apenas em gestantes da região nordeste do Brasil e no segundo trimestre de gestação. Idade, situação conjugal, IMC, idade gestacional e número de gestações anteriores mantêm-se como características importantes que devem ser consideradas na avaliação da anemia na gestação. As curvas construídas seguem os padrões das referências nacional e internacional. Propõe-se uma curva de Hb de gestantes não anêmicas para ser utilizada na avaliação da anemia em gestantes brasileiras. / Objective: To evaluate the hemoglobin levels of Brazilian pregnant women before and after fortification of wheat and corn flours with iron and to investigate the associated variables. Methods: This collaborative cross-sectional study was developed with retrospective data obtained from medical records of 12,119 pregnant women who attended public prenatal care services in 13 municipalities of five Brazilians geographical regions. They were divided into two groups: Before-fortification (women who delivered before June/2004), and After-fortification (women with date of last period after June/2005). Data collection occurred between 2006-2008 and included only low risk pregnant women, whose medical records contained at least the date of the first prenatal visit, date of the last menstrual period and measurement of Hemoglobin (Hb). The dependent variable was the Hb level (g/dL) and the independent variables were: group of fortification, geography region, social and demographic characteristics, obstetric history and characteristics of prenatal care. We conducted descriptive analysis, univariate and multiple (linear regression) of the Hb level for the total of pregnant women, by geographic region and trimester of pregnancy. Polynomial regression models were used to fit the curves of Hb by month of pregnancy, which were compared with national and international references. Curve of Hb mean and critical levels (-2 standard deviations) constructed with data of non-anemic pregnant of After-fortification group were also compared to references. The significance level for all tests was 5%. This study was approved by the Research Ethics Committee. Results: There was no significant increase in Hb level for the total sample after the fortification (p=0.325), except pregnant women in the northeast region (p <0.001, =0.214) and pregnant women in the second trimester of pregnancy (p=0.025; =0.093). The Hb levels were lower on those who were younger, lived without partner, had lower body mass index-BMI, had higher gestational age and/or had two or more previous pregnancies. Although the curve of After-fortification group had presented higher levels in all months of pregnancy, the polynomial regression showed no significant effect of fortification of flour (p=0.316). The curves of Hb in both groups of fortification were higher than the national and international references critical levels in the first trimester, followed by a drop and stabilization in late pregnancy. The curve of critical levels constructed with data of non-anemic pregnant women of After-fortification group were below the curve of critical levels of national reference and the WHO cut-off point, but similar to the international reference, except in late pregnancy. Conclusions: Fortification of flour with iron significantly increased the Hb levels of pregnant women only in northeast region of Brazil and in the second trimester of pregnancy. Age, marital status, BMI, gestational age and number of previous pregnancies remain as important characteristics that should be considered in the evaluation of anemia in pregnancy. The constructed curves follow the national and international references. We propose a curve of Hb non-anemic pregnant women to be used in the evaluation of anemia in pregnant Brazilian women.

Effects of iron and omega–3 fatty acid supplementation on physical activity of iron deficient primary school children residing in KwaZulu–Natal / Greeff J.

Background: Iron deficiency (ID) is the most prevalent nutritional deficiency in the world. In children, both inadequate iron and fatty acid (FA) status have been found to have an effect on cognitive and behavioural function, including physical activity behaviour and attention deficit hyperactivity disorder (ADHD)–related behaviour. Aim: To investigate the effects of supplementation with iron and omega–3 fatty acids (n–3 FAs), alone and in combination, on spontaneous motor activity and ADHD–related behaviour in iron deficient primary school children in KwaZulu–Natal. An additional aim was to evaluate the use of the Actical accelerometer as a tool to assess physical activity behaviour. Methods: The study design was a 2x2 factorial, randomized, double–blind and placebo–controlled trial. Iron deficient school children aged six to ten years with or without mild anaemia were included in the study (n = 321). Subjects were randomly assigned to receive one of the following supplement combinations: (1) 420mg docosahexaenoic acid (DHA)/80 mg eicosapentaenoic acid (EPA) + 50mg of iron as ferrous sulphate (Fe); (2) 420mg DHA/80mg EPA + placebo; (3) 50mg of Fe + placebo; (4) placebo + placebo. Supplements were provided four times a week for a duration of 8.5 months (excluding school holidays). Physical activity of a subgroup of subjects (n=98) was recorded on four random school days at baseline, midpoint and endpoint (12 days in total) during three different time periods namely class time 1 (08h00-10h30), break time (10h30–11h00) and class time 2 (11h00–12h00). Classroom behaviour of study subjects was assessed by teachers at baseline and endpoint using the Conners’ Teacher Rating Scale–Revised: Short Forms (CTRS). Iron status indicators and red blood cell (RBC) FA composition were measured at baseline and endpoint. Treatment effects were assessed for activity and CTRS scores. Furthermore, the relationship between activity, CTRS scores and iron/FA status indicators was determined using bivariate correlation and multivariate linear regression analysis. Results: Overall activity of all subjects varied over time from baseline and midpoint to endpoint. A significant cycle x age interaction (P = 0.005) as well as a significant cycle x time period x gender interaction (P = 0.036) was observed on overall activity. There were no significant interactions of cycle or time period with treatment. However, there was a significant main effect of DHA/EPA supplementation for lower class time 1 activity at endpoint (P = 0.014). Biological markers indicating better or poorer iron status were positively and negatively associated with activity at break time, respectively. Subjects in the group receiving both iron and DHA/EPA supplements showed a significant improvement from baseline to endpoint on the cognitive problems/inattention subscale (P = 0.005) of the CTRS. Hyperactivity scores increased iv significantly from baseline to endpoint in all groups (P = 0.006). DHA (r = –.203; P = 0.040) and EPA (r = –.199; P = 0.044) content of RBC were negatively associated with activity at class time 1. No significant associations were observed between activity and CTRS scores at baseline. At endpoint, class time 1 activity was positively associated with all CTRS subscale scores except for the cognitive problems subscale, which only bordered significance (correlation, P = 0.051; regression, P = 0.073). Conclusions: These findings suggest that n–3 FA supplementation may have an influence on ADHD–related behaviour during class time. During school break time when subjects were allowed to move around freely, iron status was positively associated with spontaneous motor activity. Introdução: Anemia é um importante problema de saúde pública no Brasil, ainda muito associado às condições sociais. A fortificação de alimentos com ferro é uma alternativa de grande alcance no combate à deficiência de ferro e controle da anemia, pois representa uma fonte complementar que contribui para a formação de reservas do mineral para os períodos de maior vulnerabilidade. As gestantes compõem um dos grupos mais vulneráveis à anemia, com consequências deletérias à sua saúde e do feto. Considerando que a fortificação compulsória das farinhas de trigo e milho com ferro foi efetivamente implantada em junho de 2004, supõe-se que mulheres em idade reprodutiva tenham aumentado suas reservas, tornando as gestações de menor risco. Justifica-se, assim, avaliar o efeito da fortificação das farinhas no controle da anemia em gestantes. Objetivo: Avaliar o efeito da fortificação das farinhas de trigo e milho com ferro, no controle da anemia em gestantes atendidas em serviços públicos de pré-natal, localizados em municípios das cinco regiões do Brasil. Métodos: Estudo de avaliação de intervenção por meio de painéis repetidos, com amostras transversais independentes, realizado em serviços públicos de saúde localizados em municípios das cinco regiões do Brasil. Foram coletados dados retrospectivos de 12.119 prontuários de gestantes distribuídas em dois grupos: Antes-fortificação (parto antes de Jun/2004) e Após-fortificação (última menstruação após Jun/2005). Anemia foi definida como Hb<11g/dL. Níveis de hemoglobina-Hb por idade gestacional foram avaliados segundo critérios do Center for Disease Control-CDC (1989) e Szarfarc et al. (1983). Utilizou-se teste qui-quadrado, t de Student e regressão logística, com nível de significância de 5%. Resultados: Na amostra total, a anemia diminuiu de 25% para 20% após a fortificação (p<0,001), com aumento das médias de Hb (p<0,001). Verificaram-se, entretanto, diferenças regionais importantes. Nas regiões Nordeste e Norte, onde as prevalências eram elevadas, houve queda significativa Após-fortificação: de 37% para 29% e de 32% para 25%, respectivamente. Nas regiões Sudeste e Sul, cujas prevalências eram baixas mesmo antes da fortificação, também houve redução: de 18% para 15% e de 7% para 6%, respectivamente. Nas regiões Sudeste e Sul, as gestantes tinham, respectivamente, razão de chance 30% e 75% menor de serem anêmicas, em relação às da região Nordeste. Em comparação com os parâmetros do CDC (1989), os níveis de Hb segundo idade gestacional de ambos os grupos se mostraram discretamente mais elevados nos primeiros meses, porém bem mais baixos após o 4º mês. O mesmo se verificou em relação à curva de Szarfarc et al (1983), porém após o 3º mês, as médias de Hb das gestantes estudadas acompanharam essa curva. Mesmo após o controle das variáveis que se mostraram diferentes entre os grupos (p<0,20), o grupo Após-fortificação apresentou menor razão de chance de ter anemia. Análise de regressão logística mostrou que grupo, região geográfica, situação conjugal, trimestre gestacional, estado nutricional inicial e gestação anterior associaram-se com anemia (p<0,05). Conclusões: Apesar de a prevalência da anemia em gestantes continuar elevada nas regiões Nordeste, Norte e Centro-Oeste, a diminuição no total da amostra e aumento das médias de Hb sugerem efeito positivo da fortificação das farinhas no controle da deficiência do mineral, embora outras variáveis não estudadas possam ter contribuído para tal resultado. A distribuição desigual da anemia nas diferentes regiões geográficas do Brasil, entretanto, reitera sua determinação social. Considerando as dificuldades na obtenção dos dados de Hb a partir de prontuários, sugere-se a implantação de um sistema de monitoramento permanente que possibilite acompanhar a evolução da anemia gestacional em resposta às estratégias implementadas. / Introduction: Anemia is an important public health problem in Brazil, and it has been quite associated to social conditions. Food fortification with iron is a far-reaching alternative to combat iron deficiency and anemia control, as it represents an additional source that contributes to the formation of the mineral stores for the periods of greatest vulnerability. Pregnant women are one of the most vulnerable to anemia, with deleterious consequences to their health and the fetus. Whereas the mandatory fortification of flour and corn flour with iron was effectively implemented in June 2004, it is assumed that women of reproductive age have increased their reserves, making lower-risk pregnancies. Therefore, it justified to evaluate the effect of flour fortification in the control of anemia in pregnant women. Objective: To evaluate the effect of fortification of wheat and corn flours with iron in the control of anemia in pregnant women attending public prenatal care, located in cities of the five regions of Brazil. Methods: A repeated cross-sectional panel evaluation study of intervention was carried out in public health care services located in cities of the five Brazilian regions. We collected backward data in 12.119 medical records. Pregnant women were divided into two groups: Before-fortification (delivery before Jun/2004) and After-fortification (last menstrual period after Jun/2005). Anemia was defined as Hb<11g/dL. Hb level by gestational age was evaluated according to criteria of Centers for Disease Control-CDC(1989) and Szarfarc et al(1983). We used chi-square, Students t test and logistic regression, with significance level of 5%. Results: In the total sample, anemia was dropped from 25% to 20% after fortification (p<0.001). Hb level also was significantly higher after fortification (p <0.001). However, the findings showed great differences among the regions. In the Northeast and North, where the prevalence of anemia were high, significant drop After-fortification was found: from 37% to 29% and 32% to 25%, respectively. In the Southeast and South, whose prevalences were low Before-fortification, also decreased: from 18% to 15% and 7% to 6%, respectively. In the Southeast and South, the women had, respectively, odds ratio 30% and 75% less to be anemic compared to the Northeast. Hb levels according to gestational age, comparing to CDC parameters, were better in the first trimester but worse from the 4th month in both groups. The same was verified comparing to curve of Szarfarc et al, but Hb levels of pregnant women studied followed this curve after 3rd month. Even after controlling for variables that were different between groups (p <0.20), the group After-fortification had lower odds ratio of having anemia. Logistic regression showed that group, geographic region, marital status, gestational trimester, initial nutritional status and previous pregnancy were associated with anemia (p<0.05). Conclusions: Despite the prevalence of anemia in pregnant women is still high in the Northeast, North and Midwest, the decrease in the total sample and increase in Hb levels suggest positive effect of fortification of flour to the control of iron deficiency, although other variables not studied may have contributed to this result. The unequal distribution of anemia in different geographical regions of Brazil, however, reiterates its social determination. Considering the difficulties in obtaining data of Hb from medical records suggest the establishment of a permanent monitoring system allows monitoring the evolution of pregnancy anemia in response to the strategies implemented.

The etiology of anemia is characterized by abnormal hemoglobin synthesis. Iron deficiency is characterized by microcytic and hipochromic red cells and low serum ferritin, being the most prevalent nutritional deficiency worldwide, responsible for iron deficiency anemia (FA). Anemia of chronic disease (ACD) is considered a clinical syndrome associated with chronic inflammation, infectious disease, neoplastic or traumatic, being the second most frequent cause of anemia. The severity of anemia correlates with the degree of pathology. Both have functional iron deficiency. The objective of this study was to evaluate hematological and inflammatory, as well as the presence of oxidative stress in patients with anemia. The blood analyzer was done by the CBC, automated hematology analyzer processed, Sysmex® (Automated Hematology Analyzer). The quantitative determination of ferritin is serum was done in IMMULITE analyzer. Levels of CRP and AOPP were performed in serum by automated Cobas MIRA® (Roche Diagnostics). Statistical analysis was performed using GraphPad Prism 5. We analyzed 70 patients with microcytic and hypochromic anemia. Of these, 29 (41.43%) were diagnosed as iron deficiency anemia and 41 (58.57%) with anemia of chronic disease. As a control group, we used samples from 44 patients with hematological parameters, serum ferritin, CRP and AOPP normal. The values of MCV, MCH and MCHC significantly lower in iron deficiency anemia. Ferritin levels showed that it can be considered both a measure of iron store as an inflammatory marker. In ACD there is increased production of inflammatory cytokines, which, in turn, increases the concentration of C-reactive protein (CRP). The results indicate that AOPP in both groups with anemia showed increased levels of this marker, which indicates the presence of oxidative stress, probably caused by increased production of free radicals and decreases in enzyme activities of the antioxidant defense system of erythrocytes. / A etiologia das anemias caracteriza-se pela síntese anormal de hemoglobina. A deficiência de ferro é caracterizada por eritrócitos microcíticos e hipocrômicos e por ferritina sérica baixa, sendo a carência nutricional mais prevalente em todo o mundo, responsável pela Anemia Ferropriva (AF). A Anemia de Doença Crônica (ADC) é considerada uma síndrome clínica, associada à inflamação crônica, doença infecciosa, traumática ou neoplásica, sendo a segunda causa mais freqüente de anemia. Ambas apresentam deficiência funcional de ferro. O objetivo deste trabalho foi avaliar parâmetros hematológicos e inflamatórios, bem como a presença de estresse oxidativo em pacientes com anemia. A análise hematológica foi feita através do hemograma, processado em analisador hematológico automatizado, Sysmex® (Automated Hematology Analyzer). O doseamento quantitativo da ferritina no soro foi feito em analisador IMMULITE. A dosagem de Proteína C-Reativa (PCR) e de Produtos da Oxidação Avançada de Proteínas (AOPP) foram realizadas no soro através do sistema automatizado Cobas MIRA® (Roche Diagnostics). A análise estatística foi realizada através do programa GraphPad Prism 5. Foram analisados 70 pacientes portadores de anemia microcítica e hipocrômica. Destes, 29 (41,43%) foram diagnosticados como anemia ferropriva e 41 (58,57%) com anemia de doença crônica. Como grupo controle, foram utilizadas amostras de 44 indivíduos com parâmetros hematológicos, níveis de ferritina, PCR e AOPP dentro da normalidade. Os valores de VCM, HCM e CHCM foram significativamente menores na anemia ferropriva. Os níveis de ferritina revelaram que ela pode ser considerada tanto uma medida das reservas de ferro quanto um marcador inflamatório. Na ADC há aumento da produção de citocinas inflamatórias, que, por sua vez, aumenta também a concentração de PCR. Os resultados do AOPP indicam que ambos os grupos com anemia apresentaram níveis aumentados deste marcador, o que indica a presença de estresse oxidativo, provavelmente causado por aumento na produção de radicais livres e declínio das atividades das enzimas do sistema de defesa antioxidante dos eritrócitos.

Determinantes da anemia em mães e filhos no Brasil / Determinants of anemia in mothers and children in Brazil

Introdução: Anemia por carência alimentar de ferro é a deficiência nutricional mais freqüente e preocupante do ponto de vista da saúde coletiva. Afeta principalmente crianças, gestantes e mulheres em idade fértil. Apesar das medidas de intervenção para prevenção e controle da anemia no Brasil, estudos mostram que as prevalências ainda continuam elevadas. Objetivo: Investigar a situação da anemia e seus determinantes em mães e filhos no Brasil, considerando o contexto familiar. Método: Pesquisa transversal de abordagem quantitativa, que utilizou o banco de dados da Pesquisa Nacional de Demografia e Saúde da Mulher e da Criança (PNDS) 2006. Foram utilizados dois questionários com informações básicas sobre o domicílio e seus moradores e informações detalhadas sobre o público-alvo, mulheres de 15 a 49 anos e seus filhos menores de 5 anos. Aproximadamente 40% dos domicílios foram selecionados para coleta de sangue das mulheres, porém todas as crianças nascidas a partir de janeiro de 2001 tiveram o sangue colhido, desde que filhos biológicos da entrevistada. Análise de hemoglobina foi realizada em 5.915 mulheres e 4.558 crianças. Este estudo analisou 1.476 pares, considerando a mãe e apenas uma criança. O programa Statistical Package for Social Science (SPSS versão 18.0) foi utilizado para análise dos dados, que analisou anemia em mães e/ou filhos, mães e filhos isoladamente e constou das etapas univariada e múltipla. Resultados: A ocorrência de anemia foi de 36,8% em mães e/ou filhos, 23,4% nas mães e 17,0% nos filhos. Mães apresentaram chance 1,49 vezes maior de ter anemia do que os filhos, e não se verificou associação entre anemia nas mães e nos filhos (p=0,478). Na análise múltipla, mães e/ou filhos que residiam nas regiões Nordeste e Centro-Oeste apresentaram chance 2,24 e 1,70 vezes maior, respectivamente, em relação às da região Sul; a chance também foi estatisticamente maior (1,78 vezes) para o par com insegurança alimentar (sentiu fome) e em que a mãe realizou consulta de puerpério (1,39 vezes); primiparidade mostrou-se como fator de proteção. Anemia nas mães também se associou com macrorregião de residência, com chance 2,39 vezes maior para as da região Nordeste em relação às da região Sul; mães com insegurança alimentar, que realizaram consulta de puerpério e tiveram pelo menos uma doença também apresentaram chance 50% maior para anemia. Nas crianças, anemia associou-se com insegurança e consumo alimentar, sendo que aquelas que ingeriram multimistura e leite fresco com água nas últimas 24 horas apresentaram chance 2,49 e 1,69 vezes maior para anemia, respectivamente, assim como crianças que não ingeriram lanche da tarde no dia anterior e que ingeriram arroz menos de 4 vezes na semana (chance 1,57 e 2,38 vezes maior, respectivamente); primiparidade materna revelou-se como fator de proteção. Conclusões: A ocorrência de anemia em mães e/ou filhos é maior nas macrorregiões menos desenvolvidas e em famílias com insegurança alimentar, o que evidencia a determinação social dessa carência nutricional. É mais freqüente nas mães e não se associa à ocorrência de anemia nos filhos. Nas mães, anemia associa-se com variáveis socioeconômicas, demográficas e insegurança alimentar. Nas crianças, anemia se associa apenas com insegurança e consumo alimentar. Os resultados indicam que embora mães e filhos estejam expostos aos mesmos determinantes sociais e ambientais que aumentam a suscetibilidade para anemia, as restrições alimentares ocasionadas por condições socioeconômicas e demográficas desfavoráveis têm maior impacto nas mães, em virtude da maior necessidade orgânica de ferro da mulher em idade fértil e provável proteção e cuidado das mães para com os filhos. Indica também que independente da condição socioeconômica, a alimentação inadequada da criança torna-a mais susceptível à anemia. Tais resultados podem estar atrelados também a um efeito positivo do Programa Nacional de Suplementação de Ferro, embora apenas um quarto das crianças de 6 a 24 meses tivesse recebido ferro nos últimos seis meses, o que indica a necessidade de melhorias na operacionalização do programa. / Introduction: Anemia an iron deficiency, is the most frequent and worrisome nutritional deficiency from the standpoint of public health. It mainly affects children, pregnant women and women of childbearing age. Despite the intervention measures for prevention and control of anemia in Brazil, studies shows that prevalence rates are high. Objective: Investigate the situation of anemia and its determinants in mothers and children in Brazil, considering the family context. Method: Quantitative cross-sectional survey, that used the database of the national survey of demography and health of women and children (PNDS) 2006. This information was collected through two questionnaires, with basic information about the home and its residents and also detailed information about the target audience women between 15 to 49 years and children under 5 years. The study evaluated approximately 15.000 women and 5.000 children, with a representative sample of five brazilian regions in urban and rural context. Approximately 40% of households had been selected for blood sampling of women, and all children born from January 2001 had the blood sampling taken provided they were biological children of the interviewees. Hemoglobin analysis was performed in 5.915 women and 4.558 children. This study looked at 1.476 pairs, considering the mother and only one children. The Statistical Package Program for Social Science (SPSS version 18.0) was used for data analysis, which examined anemia in mothers and/or children, mothers and children separately and consisted of univariate and multiple steps. Results: The occurrence of anemia was 36.8% in mothers and/or children, 23.4% of mothers and 17.0% in children. Mothers were 1.49 times more likely to have anemia than children, there was no association between anemia in mothers and children (p=0.478 ). In multiple regression analysis, mothers and/or children residing in the Northeast and Midwest had odds 2,24 and 1,70 times higher, respectively, in relation to the south; the chance was also statistically higher (1,78 times) for those with food insecurity and those in which the mother held a puerperal consultation (1,39 times); primiparity shown as protective factor. Anemia in mothers was also associated with the region of residence, with 2,39 times more chances for the Northeast than in mothers in the South; mothers with food insecurity, who performed a puerperal consultation and had at least one disease, also were 50% more likely to have anemia. In children, anemia was associated with insecurity and food consumption, and those who ate a mixture of water and fresh milk in the last 24 hours had a chance (2,49 and 1,69 times) to be more likely to develop anemia, respectively, as well as children who did not eat an afternoon snack on the last day and ate rice 4 times less in the week (odds 1,57 and 2,38 times higher for anemia, respectively), maternal primiparity proved to be a protector factor. Conclusions: The occurrence of anemia in mothers and/or children is higher in less develop macro-regions and among families with food insecurity, highlights the social causes of nutritional deficiency. It is more frequent in mothers and not associated with anemia in children. In mothers, anemia is associated with socioeconomic, demographic and food insecurity. In children, anemia is associated only with insecurity and food consumption. This result indicates that although there is evidence of mothers and children are exposed to the same social and environmental causes that increase susceptibility to anemia, dietary restrictions and their nutritional consequences caused by unfavorable demographic and socioeconomic conditions will have a greater impact on mothers, owing to the greater need for organic iron in women of childbearing age and protection and care of their mothers to the children. It also indicates that regardless of socioeconomic status, whether the childs diet is inadequate, it will be more susceptible to anemia. The results can be linked also to a positive effect of the National Iron Supplementation, although only one quarter of children aged 6 to 24 months had received iron in the last six months, which indicates the need for improvements in the operation of the program.

Avaliação dos níveis de hemoglobina de gestantes brasileiras antes e após a fortificação de farinhas com ferro / Evaluation of hemoglobin levels of Brazilian pregnant women before and after the fortification of flours with iron

Objetivo: Avaliar os níveis de hemoglobina de gestantes brasileiras antes e após a fortificação das farinhas de trigo e milho com ferro e investigar as variáveis associadas. Método: Estudo transversal que integra um projeto matricial desenvolvido com dados retrospectivos obtidos de prontuários de 12.119 gestantes atendidas em serviços públicos de pré-natal localizados em 13 municípios das cinco regiões geográficas do Brasil, divididas em dois grupos: Antes-fortificação das farinhas com ferro (gestantes com parto realizado antes de junho de 2004); e Após-fortificação (gestantes com data da última menstruação posterior a junho de 2005). A coleta de dados ocorreu em 2006-2008 e incluiu apenas gestantes de baixo risco, cujos prontuários continham pelo menos a data da primeira consulta de pré-natal e da última menstruação e a dosagem de hemoglobina (Hb). A variável dependente foi o nível de Hb (g/dL) e as independentes foram: grupo de fortificação, região geográfica, características sociais e demográficas, antecedentes obstétricos e características do pré-natal. Realizou-se análise descritiva, univariada e múltipla do nível de Hb para o total das gestantes, por região geográfica e por trimestre de gestação, por meio de modelos de regressão linear. Contruíram-se modelos de regressão polinomial para o ajuste das curvas de Hb por mês de gestação, que foram comparadas com referências nacional e internacional. Curvas de níveis médios e críticos (-2 desvios-padrão) de Hb de gestantes não anêmicas do grupo Após-fortificação também foram comparados às referências. O nível de significância de todos os testes foi de 5%. O estudo foi aprovado por Comitê de Ética em Pesquisa. Resultados: Não houve aumento significativo do nível de Hb para o total da amostra (p=0,325) após a fortificação, exceto em gestantes da região nordeste (p<0,001; =0,214) e em gestantes no segundo trimestre de gestação (p=0,025; =0,093). O nível de Hb foi menor entre aquelas que tinham menor idade, viviam sem companheiro, tinham menor Índice de Massa Corporal-IMC, maior idade gestacional e/ou duas ou mais gestações anteriores. Apesar da curva Após-fortificação apresentar níveis superiores em todos os meses de gestação, a regressão polinomial não mostrou efeito significativo da fortificação de farinhas (p=0,316). As curvas de Hb de ambos os grupos de fortificação mostraram-se acima dos níveis críticos das referências nacional e internacional no primeiro trimestre, com queda a seguir e estabilização no final da gestação. A curva de níveis críticos, construída com dados de gestantes não anêmicas do grupo Após-fortificação, ficou abaixo da curva de níveis críticos da referência nacional e do ponto de corte da OMS, mas semelhante à referência internacional, exceto no final da gestação. Conclusões: A fortificação de farinhas com ferro aumentou significativamente o nível de Hb apenas em gestantes da região nordeste do Brasil e no segundo trimestre de gestação. Idade, situação conjugal, IMC, idade gestacional e número de gestações anteriores mantêm-se como características importantes que devem ser consideradas na avaliação da anemia na gestação. As curvas construídas seguem os padrões das referências nacional e internacional. Propõe-se uma curva de Hb de gestantes não anêmicas para ser utilizada na avaliação da anemia em gestantes brasileiras. / Objective: To evaluate the hemoglobin levels of Brazilian pregnant women before and after fortification of wheat and corn flours with iron and to investigate the associated variables. Methods: This collaborative cross-sectional study was developed with retrospective data obtained from medical records of 12,119 pregnant women who attended public prenatal care services in 13 municipalities of five Brazilians geographical regions. They were divided into two groups: Before-fortification (women who delivered before June/2004), and After-fortification (women with date of last period after June/2005). Data collection occurred between 2006-2008 and included only low risk pregnant women, whose medical records contained at least the date of the first prenatal visit, date of the last menstrual period and measurement of Hemoglobin (Hb). The dependent variable was the Hb level (g/dL) and the independent variables were: group of fortification, geography region, social and demographic characteristics, obstetric history and characteristics of prenatal care. We conducted descriptive analysis, univariate and multiple (linear regression) of the Hb level for the total of pregnant women, by geographic region and trimester of pregnancy. Polynomial regression models were used to fit the curves of Hb by month of pregnancy, which were compared with national and international references. Curve of Hb mean and critical levels (-2 standard deviations) constructed with data of non-anemic pregnant of After-fortification group were also compared to references. The significance level for all tests was 5%. This study was approved by the Research Ethics Committee. Results: There was no significant increase in Hb level for the total sample after the fortification (p=0.325), except pregnant women in the northeast region (p <0.001, =0.214) and pregnant women in the second trimester of pregnancy (p=0.025; =0.093). The Hb levels were lower on those who were younger, lived without partner, had lower body mass index-BMI, had higher gestational age and/or had two or more previous pregnancies. Although the curve of After-fortification group had presented higher levels in all months of pregnancy, the polynomial regression showed no significant effect of fortification of flour (p=0.316). The curves of Hb in both groups of fortification were higher than the national and international references critical levels in the first trimester, followed by a drop and stabilization in late pregnancy. The curve of critical levels constructed with data of non-anemic pregnant women of After-fortification group were below the curve of critical levels of national reference and the WHO cut-off point, but similar to the international reference, except in late pregnancy. Conclusions: Fortification of flour with iron significantly increased the Hb levels of pregnant women only in northeast region of Brazil and in the second trimester of pregnancy. Age, marital status, BMI, gestational age and number of previous pregnancies remain as important characteristics that should be considered in the evaluation of anemia in pregnancy. The constructed curves follow the national and international references. We propose a curve of Hb non-anemic pregnant women to be used in the evaluation of anemia in pregnant Brazilian women.
