Title: Evaluation and determination of standard ratio of extensors and flexors of the knee in sporting youth. Objectives: The aim of the thesis is to establish the standards of the force ratio H: Q for the youth football categories U16, U17, U18, U19, U21 and adult players of the Czech Republic and based on the prescription type of study to establish a percentage critical disbalance of dynamic knee stabilizers which may be the most common cause of injuries to dynamic or static knee joint stabilizers in soccer players. Methods: The monitored group of players consisted of football players playing in elite Czech football clubs, both in youth and senior categories, who meet the predetermined selection criteria. The isokinetic strength parameters were monitored using a Cybex Humac Norm isokinetic dynamometer (Cybex NORM ®, Humac, CA, USA). We investigated maximum strength moment (PT) of extensors and knee flexors of dominant and non-dominant limb at concentric contraction at angular velocities of 60, 180 and 300ř.s-1 . Bakgrund: Fotboll är en av dom största sporterna i världen och en av de främsta i Sverige. Skador är relativt vanligt inom sporten där knäskador är en av de mest förekommande. Det finns mycket forskning som visar på att skadeförebyggande träning minskar skadeincidensen men få studier är gjorda på svenska fysioterapeuters syn gällande skadeförebyggande träning inom fotboll. Syfte: Syftet var att undersöka fysioterapeuters syn och erfarenheter av skadeförebyggande träning för knäskador inom fotboll hos fysioterapeuter anställda av svenska fotbollslag. Metod: En kvalitativ design användes. Fem semistrukturerade intervjuer genomfördes. Kvalitativ innehållsanalys användes vid bearbetning av data. Resultat:  Studiens resultat visar att deltagarna har en varierad syn på skadeförebyggande träning.  Fysioterapeuterna beskriver vikten av en god träningsplanering, träning av stabilitet och kontroll, neuromuskulär påkoppling samt tid till återhämtning. Vidare bör kunskap förmedlas till spelare och tränare kring skadeförebyggande träning för att öka implementeringen.  Mental förberedelse ses också som en viktig faktor för att minska risken för skador. Fysioterapeuterna i studien kände alla till knäkontroll programmet och ansåg att det är ett väl utformat program med bra och lätta övningar men att det dock kan förkortas. Konklusion: Knäkontrollprogrammet, neuromuskulär uppvärmning och att spelarna har en god knäkontroll ses som viktiga aspekter inom skadeförebyggande träning. Fysioterapeuterna belyste mental förberedelse som en viktig faktor, vilket inte belysts i tidigare studier. Vidare studier behövs för att ytterligare undersöka betydelsen av mental förberedelse. / Background: Soccer is one of the largest sports in the world and one of the foremost in Sweden. Injuries are common in the sport where knee injuries are one of the most common. Research shows that injury prevention reduces the injury incidence, but few studies have explored Swedish physiotherapists views on injury prevention in soccer. Purpose: The purpose was to investigate physiotherapists views and experiences of injury prevention for knee injuries, among physiotherapists employed by Swedish soccer teams. Method: A qualitative design was used. Five semi-structured interviews were conducted. Qualitative content analysis was used in data processing. Results: The result shows that the participants have a varied view of injury prevention. The physiotherapists describe the importance of good training planning, training of stability and control, neuromuscular control and time for recovery. Furthermore, knowledge should be communicated to players and coaches about injury prevention to increase implementation. Mental preparation is also seen as an important factor in reducing the risk of injury. The physiotherapists in the study all knew about the knee control program and felt that it is a well-designed program with good and simple exercises, but the program can be shortened. Conclusion: The knee control program, neuromuscular warm up and that players have good knee control are an important aspects of injury prevention. The physiotherapists emphasized mental preparation as an important factor, which was not emphasized in previous studies. Further studies are needed to investigate the importance of mental preparation.

Avaliação do controle postural de indivíduos com indicação de reconstrução combinada do ligamento cruzado anterior e do ligamento anterolateral do joelho / Postural control evaluation of individuals with indication for combined reconstruction of the anterior cruciate ligament and the anterolateral ligament of the knee

INTRODUÇÃO: Estudos têm apontado para a manutenção do déficit do controle postural após a reconstrução do Ligamento Cruzado Anterior (LCA), além de uma eventual instabilidade rotatória. Acredita-se que a lesão do Ligamento Antero Lateral (LAL) possa ter influência na manutenção dessa instabilidade após a reconstrução isolada do LCA. Não há esclarecimento acerca de como indivíduos com indicação da reconstrução combinada desses dois ligamentos se comportam do ponto de vista do controle postural. OBJETIVO: Verificar se há diferença no controle postural de pacientes com indicação cirúrgica de reconstrução combinada desses dois ligamentos (Grupo LCA+LAL), quando comparados aos indivíduos com indicação de reconstrução isolada do LCA (Grupo LCA). O objetivo secundário foi o de realizar uma avaliação clínica e da funcionalidade. MÉTODOS: Foi avaliado o controle postural estático por meio de plataformas de força. Foram realizados testes bipodais e unipodais com olhos abertos e olhos fechados. Para avaliação da funcionalidade foram realizados testes funcionais no aparelho Balance Master, aplicada a escala de Lysholm, além do Single Hop Test e Crossover Hop Test. A avaliação clínica se deu por meio da aplicação da Escala Visual Analógica e a translação anterior da tíbia por meio do artrômetro KT- 1000. Os achados foram comparados ao de um grupo controle. RESULTADOS: Não houve diferença estatística significativa entre os grupos para os testes de controle postural de olhos abertos e fechados. Uma maior porcentagem do Grupo do LCA+LAL (48%) quando comparado ao Grupo LCA (27%) e ao Grupo Controle (0%) não conseguiu realizar o teste unipodal de olhos fechados. O Grupo LCA+LAL apresentou maior translação anterior da tíbia que o Grupo LCA. Quanto aos Hop Tests, mais indivíduos do grupo LCA+LAL (67% versus 33% do Grupo LCA) apresentaram dor no Single Hop Test. CONCLUSÕES: Um número maior de indivíduos do grupo LCA+LAL não conseguiu realizar o teste unipodal com olhos fechados, indicando possivelmente uma instabilidade postural nessa condição. Os indivíduos desse grupo também apresentaram maiores valores de translação anterior da tíbia indicando uma pior instabilidade anteroposterior / INTRODUCTION: Studies have shown a postural control deficit after anterior cruciate ligament (ACL) reconstruction and a persistent rotational instability in some cases. It has been argued that an anterolateral ligament (ALL) injury could be responsible for such instability after an isolated ACL reconstruction. There are no further evidences in order to understand how patients in the preoperative period of a combined surgery of these two ligaments behave regarding postural control. PURPOSE: To verify the postural control of patients in ACL + ALL Group, when compared to the isolated ACL reconstruction indication (ACL Group). As a secondary purpose, the research aims to perform a clinical and functional evaluation. METHODS: An assessment of static postural control was performed using the force plane. Doubleleg and single-leg tests were performed with eyes open and closed. Functionality was assessed by perfoming tests in the Balance Master equipment. The Lysholm scale was applied, and the Single Hop Test and Crossover Hop Test were performed. The clinical evaluation consisted of the Visual Analogue Scale for pain and the anterior tibial translation was measures with a KT-1000 arthrometer. Both groups were compared to a control group. RESULTS: There was no statistical difference between groups regarding postural control in any of the tests. A higher percentage of the ACL + ALL Group (48%), when compared to ACL Group (27%) and Control Group (0%), was not able to perform the single-leg test with eyes closed. The ACL + ALL Group had greater anterior tibial translation than the ACL Group. Regarding the hop tests, a greater number of patients in the ACL + ALL Group (67% versus 33% of the ACL Group) reported pain during the Single Hop test. CONCLUSION: Most patients in the ACL + ALL Group were not able to perform the single-leg test with eyes closed, showing a possible postural instability in this condition. The higher values in anterior tibial translation may also indicate greater anteroposterior instability

Thus, the aim of this study was to evaluate the correlation among trunk and hip strength with three-dimensional trunk and lower-limb kinematics during a jump-landing task in female recreational athletes. Methods: Twenty-three healthy women recreational athletes aged between 18 and 35 years underwent evaluation of hip abductor, hip extensor, and lateral trunk muscle strength by manual dynamometry and three-dimensional trunk and lower-limb kinematics during the landing phase of a single-leg drop vertical jump. Pearson's correlation coefficients (r) were calculated to establish the relationship between hip and trunk strength and trunk, hip, and knee movements. Results: Greater hip extensor strength was found to be significantly associated with greater trunk flexion at initial contact. No significant correlations were found among the strength data and the peak values of kinematics during the landing phase. Conclusion: Considering that the greater trunk flexion is related to lower anterior cruciate ligament (ACL) load, patellofemoral stress and patellar tendon force, hip extensor muscle strengthening may be important to prevent and rehabilitate ACL injury, patellofemoral pain, and patellar tendinopathy. / Objetivo: Alterações da cinemática têm sido correlacionadas com diferentes lesões nos membros inferiores, sendo algumas dessas lesões mais comuns em mulheres atletas, e um provável fator predisponente para essas alterações é a fraqueza dos músculos do quadril e do tronco. Assim, o objetivo deste estudo foi avaliar a correlação entre a força do tronco e do quadril com a cinemática tridimensional de tronco e membros inferiores durante uma tarefa de aterrissagem de um salto em mulheres atletas recreacionais. Métodos: Vinte e três mulheres sadias atletas recreacionais com idade entre 18 e 35 anos foram submetidos a avaliação da força dos músculos abdutores de quadril, extensores do quadril e inclinadores laterais do tronco através de um dinamômetro manual e da cinemática tridimensional de tronco e membros inferiores durante a fase de aterrissagem de uma queda e salto vertical unipodal (single-leg drop vertical jump). Os coeficientes de correlação de Pearson (r) foram calculados para estabelecer a relação entre a força do quadril e do tronco e os movimentos do tronco, quadril e joelho. Resultados: Maior força extensora do quadril foi encontrada significativamente associada com maior flexão do tronco no contato inicial. Não foram encontradas correlações significativas entre os dados de força e os valores de pico da cinemática durante a fase de aterrissagem. Conclusão: Considerando que a maior flexão do tronco está relacionada à carga do ligamento cruzado anterior (ACL), ao estresse patelofemoral e a força do tendão patelar, o fortalecimento dos músculos extensores do quadril pode ser importante para prevenir e reabilitar a lesão do LCA, dor femoropatelar e tendinopatia patelar.

Is There a Relationship Between Landing, Cutting, and Pivoting Tasks in Terms of the Characteristics of Dynamic Valgus?

Anterior cruciate ligament (ACL) injuries are a major problem among female athletes. Screening for the risk of ACL injuries tends to focus on landing tasks, which may be limited in sports where changing direction is the main action involved in noncontact ACL injuries such as soccer. To investigate whether there is a relationship between single-legged landing (SLL), cutting (90° cuts), and pivoting (180° turns) in terms of the characteristics of dynamic valgus. Study Design: Controlled laboratory study. Methods: Twenty female soccer players (mean ± SD: age, 21.0 ± 3.9 years; height, 1.65 ± 0.08 m; mass, 58.4 ± 6.4 kg) performed a minimum of 6 trials of SLL from a 0.3-m drop height and cutting and pivoting all on the right leg. Kinematics and kinetics were calculated from 3-dimensional motion analysis. Results: Strong correlations were found for peak knee abduction angles between tasks (R = 0.63-0.86, P < .01), whereas only moderate correlations between SLL and cutting (R = 0.46, P < .05), cutting and pivoting (R = 0.56, P < .05), and SLL and pivoting (R = 0.43, P > .05) were found between tasks for peak knee abduction moments. Conclusion: The results suggest that female athletes who exhibit poor SLL mechanics perform the same during various changing direction tasks. Clinical Relevance: The results support the use of existing screening tests that involve landing tasks to identify at-risk athletes for noncontact ACL injuries.
