Avaliação comparativa da morbidade e funcionalidade do membro superior em mulheres submetidas à linfadenectomia axilar total e biópsia de linfonodo sentinela por câncer de mama / Evaluation of the morbidity and function in women underwent axillary lymph node dissection or sentinel lymph node biopsy for breast cancer

Objetivos: Este trabalho objetivou avaliar e comparar a incidência de linfedema, dor, restrição na amplitude de movimento e déficit sensorial 1 dia, 15 dias, 3 meses, 6 meses e 18 meses no membro superior homolateral, após linfadenectomia axilar total (LAT) e biópsia de linfonodo sentinela (BLS) para tratamento do câncer de mama. MÉTODOS: Neste estudo incluiu-se pacientes que realizaram ressecção segmentar de mama e LAT ou BLS. As pacientes foram avaliadas entre Dezembro de 2003 e Dezembro de 2004 e foram constituídos por dois grupos, o grupo A (n=100) foi formado por casos submetidos à LAT e o Grupo B, por casos submetidos à BLS (n=50). As pacientes foram analisadas através de questionário com respostas afirmativas e negativas, onde foram incluídas questões avaliando os sintomas de linfedema, dor, restrição da amplitude de movimento e déficit sensorial 1 dia (n=30), 15 dias (n=30), 6 meses (n=30) e 18 meses (n=30), após o procedimento cirúrgico. Além disto efetuou-se avaliação fisioterapêutica na qual foi realizada perimetria de membros superiores, mensuração da amplitude de movimento através da goniometria, os testes de sensibilidade cutânea através dos monofilamentos de Semmes-Weinstein e teste eletrodiagnóstico, utilizando-se gerador universal de correntes. RESULTADOS: No grupo A houve diferenças significantes (p < 0,05) em todos os sintomas subjetivamente avaliados, existindo maior queixa de edema (31%), dor (58%), restrição do movimento (52%) e dormência (57%). No grupo B houve menor prevalência da sensação de edema (2%), dor (26%), restrição do movimento (24%) e dormência (20%), em todos os tempos estudados. No exame clínico, o exame sensorial com monofilamentos apresentou 82% de diminuição da sensibilidade tátil na região axilar e 73% na região braquial proximal no grupo de mulheres submetidas à LAT e no grupo de mulheres submetidas à BLS houve diminuição da sensibilidade tátil nas duas regiões avaliadas em 40% (p<0.05). Nas provas cronaximétricas, a sensibilidade estava diminuída no grupo de mulheres submetidas à LAT em 56% na região axilar e 48% na região braquial proximal medial e no grupo BLS a sensibilidade estava diminuída em 39% e 14% respectivamente. In group B there were less symptoms of subjective swelling (2%), pain (58%), motion restriction (24%), arm numbness (20%). In clinical examination sensory deficit using monofilaments the occurrence of numbness was 82% on axillary fold and 73% on medial part of upper arm after ALND and in 40% on axillary fold and medial part of upper arm after SLNB, with significantly different between the groups (p < 0.05). At cronaximetric proofs the sensibility was decreased in 56% in the axillary fold and 48% in the medial part of upper arm for ALND and 39% in the axillary fold and 14% at medial part of upper arm on patients who underwent SLNB. CONCLUSIONS: Arm morbidity is higher and function is lower after ALND compared to SLNB, in women submitted to breast cancer surgery

Its prevalence varies from 7% to more than 50% of patients submitted to surgery depending on the surgical approach and associated treatment employed. Main: objectives of this study were to evaluate the efficacy of mechanical lymphatic drainage in reducing the volume of lymphedematous limbs after breast cancer treatment and identify the optimal time of treatment. Patients and Method: From July 2007 to July 2008, twenty-five patients with lymphedema resulting from breast cancer treatment were enrolled in this study by order of arrival at the Godoy Clinic. The ages of the patients ranged from 42 to 86 years old (mean: 55.6 years). Diagnosis of lymphoedema was made clinically and confirmed by volumetry. Lymphedema was confirmed with a difference in volume between the patient s arms of more than 200 ml. Exclusion criteria were the presence of active infection, joint problems including immobility due to neurological involvement or any clinical condition in which physical exercises are contraindicated, such as heart failure and advanced active cancer. This study was divided into two stages with all patients being evaluated by water-displacement volumetry before and after activities. In phase I (n = 25) the patients were submitted to one hour of activities and phase II (n = 13) patients were submitted to three hours of activities with volumetry being carried out every hour. The RAGodoy® electromechanical device, which makes 15 to 25 passive flexion and extension movements of the elbow joint per minute, was used for mechanical lymphatic drainage. The t-test with a Bonferroni alpha correction of 0.008 was used for statistical analysis and an alpha error of 5% (p-value &#8804; 0.05) was considered significant. The study was approved by the Research Ethics Committee of the Medical School in Sao Jose do Rio Preto. Results: The reduction in volume was significant in Phase I (one hour treatment: p < 0.001), with the initial mean weight being 2026.4 and final weight being 1967.2 (mean loss of 59.2 grams). In Phase II (three hours activities), the reductions were significant in the first and second hours (p <0.0005 and 0.002, respectively). However, no statically significant difference was seen using a Bonferroni alpha correction of 0.008 (p-value = 0.01) between the second and third hour. Conclusion: In conclusion the use of the device reduces the volume of lymphedematous limbs; it is most effective in the first hour, but provides significant reductions for a maximum of two hours. / O lindedema pós-câncer de mama enquadra-se no tipo secundário de linfedema e segue os mesmos padrões diagnósticos e terapêuticos dos demais tipos de linfedema. Sua prevalência varia entre 7% a mais de 50% das pacientes submetidas ao tratamento cirúrgico, dependendo da abordagem cirúrgica realizada e dos tratamentos complementares empregados. Objetivo: O objetivo deste estudo foi avaliar a eficácia da drenagem linfática mecânica na redução de volume dos membros linfedematosos pós-tratamento câncer de mama e a identificação do tempo ideal de tratamento. Pacientes e Método: De julho 2007 a julho de 2008, 25 pacientes com linfedema pós-tratamento de câncer de mama foram avaliadas neste estudo, selecionadas por ordem de chegada, na Clínica Godoy. A idade das pacientes variou entre 42 a 86 anos e a idade média foi de 55,6 anos. O diagnóstico do linfedema foi clínico e confirmado por volumetria. O linfedema foi confirmado quando a diferença de volume era maior que 200 ml entre os membros. Considerou-se como critério de exclusão a presença de infecção ativa, problemas articulares envolvendo a imobilidade devido a comprometimento neurológico ou qualquer condição clínica com contraindicação para atividade física como a insuficiência cardíaca e o câncer em atividade. O estudo foi dividido em duas fases sendo que todas pacientes realizaram a volumetria por técnica de deslocamento de água antes a após a atividade. Na fase I (n = 25) as pacientes realizaram uma hora de atividade e na fase II (n = 13) as pacientes realizaram três horas de atividade, sendo feito a volumetria ao final de cada hora. O dispositivo eletromecânico RAGodoy®, que faz de 15 a 25 movimentos de flexão e extensão passivas da articulação do cotovelo, foi usado para a drenagem mecânica do membro. Para análise estatística foi usado o teste-t com correção alfa de Bonferroni = 0,008, considerando erro alfa de 5% (valor p&#8804; 0,05) como significativo. O estudo foi aprovado pelo Comitê de Ética em Pesquisa da Faculdade de Medicina de São Jose do Rio Preto-SP. Resultados: A redução do volume foi significativa na Fase I (uma hora de tratamento: p < 0,001), sendo a média do peso inicial de 2026,4g e final de 1967,2g (perda média de 59,2 g). Na Fase II (três horas de atividade) as reduções foram significativas na primeira e segunda horas (p < 0,0005 e p < 0,002, respectivamente). Entretanto, entre a segunda e terceira hora, a diferença não foi estatisticamente significativa. Conclusões: Conclui-se que o dispositivo mecânico reduz o volume de membro linfedematoso durante sua aplicação, sendo mais eficaz na primeira hora mas proporciona redução significativa quando usado por até duas horas.

The aim of this study was a prospective detection of postoperative lymphedema of the lower limbs in patients after the surgery for cervical and vulvar cancer using different methods of examination and their comparison and monitoring of postoperative complications and quality of life. Totally 78 women were followed after the surgery for cervical cancer and 36 for carcinoma of the vulva. Due to the radicality of the surgery the patients were divided into the conservative and radical groups. Lower limbs lymphedema were evaluated preoperatively and 3, 6 and 12 months after the surgery by the measurement of the lower limbs circumference, multifrequency bioelectrical impedance analysis (MFBIA) and subjective feeling. Quality of life using the European Organisation for Research and Treatment of Cancer (EORTC) questionnaires was evaluated before and 6 and 12 month after the surgery. 12 months after the cervical cancer surgery 35.9 % of patients reported subjective lymphedema, 37.18 % lymphedema were objectively diagnosed by the measurement of lower limb circuits and in 52.56 % of cases the increase of amount of extracellular fluid was detected by the MFBIA Ri/R0 method. Subjektivní hodnocení celkového zdravotního stavu pacienta na konci redukční fáze léčby lymfedému / The Subjective Evaluation of a Patient´s Overall Health State at the End of the Reduction Phase of Lymphedema Treatment

The graduation thesis is focused on Lymphedema patientś problems. It addresses how the patient values his or her own health state. The differences between a patientś health state before therapy and after finishing its reduction phase have been compared. The following aspect have been valued: limited movement, pain and mental stress. All of them were improved after the reduction phase. Thus Hypothesis 1 has been confirmed: "At the end of the Reduction Phase of Lymphedema therapy the overal health state of the patient is improved." Out of above meditioned three aspect, the mental stress improved at a lowest rate. Differences between patients with and without good social background were demonstrated. Patients with bad social background valued mental stress caused by Lymphedema before and after the therapy as significantly higher then patients with good social background. Thus Hypothesis 2 was also confirmed: "Patients with good social background cope with their disease in a better way." In order to test the hypotheses the quantitative research was done. The technique was standardised integration.

METHODS: A cross-sectional study was performed on women with breast cancer who had been submitted to quadrantectomy or to MRM+IBR with a transverse rectus abdominis myocutaneous (TRAM) flap, who had completed treatment at least six months previously. A total of 44 women submitted to quadrantectomy and 26 submitted to MRM+IBR were included in the study. RESULTS: Prevalence of lymphedema was 12% in the MRM+IBR group and 18% in the quadrantectomy group (OR: 0.51; 95%CI: 0.02-10.1; p=0.66). There was a greater prevalence of restricted inner shoulder rotation in the women submitted to quadrantectomy (OR: 7.23; 95%CI: 1.28-17.1; p=0.03). There was no difference in QOL scores or satisfaction with surgery. When the participants were questioned whether they would have opted for a different surgical technique, 25% of women in the MRM+IBR group and 12% in the quadrantectomy group reported that they would have made a different choice (adjusted analysis, OR: 7.4; 95%CI: 0.7- 73.3; p=0.09). CONCLUSIONS: The present findings suggest that the type of surgery does not affect the occurrence of lymphedema. Quadrantectomy increased the likelihood of restricted shoulder movement. In addition, the two surgical techniques evaluated were found to exert a similar effect on QOL and on the woman s satisfaction with surgery. / OBJETIVOS: Comparar a prevalência da morbidade no complexo ombro-braço, satisfação com o tratamento cirúrgico e a qualidade de vida (QV) de mulheres submetidas à quadrantectomia às daquelas submetidas à mastectomia radical modificada (MRM) com reconstrução imediata (RI) da mama. MÉTODOS: Conduziu-se um estudo de corte transversal incluindo mulheres com câncer de mama, que haviam se submetido à quadrantectomia ou à MRM+RI com retalho miocutâneo transverso do músculo reto abdominal, que apresentavam pelo menos seis meses de término do tratamento. Foram incluídas 44 mulheres submetidas à quadrantectomia e 26 à MRM+RI. RESULTADOS: A prevalência de linfedema foi de 12% no grupo de MRM+RI e 18% na quadrantectomia (OR: 0,51; IC 95%: 0,02-10,1; p=0,66). As mulheres submetidas à quadrantectomia apresentaram maior prevalência de restrição do movimento de rotação interna do ombro (OR: 7,23; IC 95%; 1,28-17,1; p=0,03). Não houve diferença nos escores de QV e da satisfação com a cirurgia. Quando as participantes foram questionadas se optariam por outra técnica cirúrgica, 25% no grupo de MRM+RI e 12% no grupo de quadrantectomia referiram que fariam uma escolha diferente (análise ajustada, OR: 7,4; IC 95%: 0,7-73,3; p=0,09). CONCLUSÕES: Nossos dados sugerem que o tipo de cirurgia não influenciou a ocorrência de linfedema. A quadrantectomia aumentou a chance de restrição do movimento do ombro. Observou-se ainda que a técnica cirúrgica apresentou impacto semelhante na QV e na satisfação da mulher com a cirurgia.
