Methods: Forty-five completely edentulous individuals were treated using mandibular overdenture retained by a single implant (Titamax IT Cortical, Neodent, Curitiba, Brazil) opposed to conventional dentures. According to the primary stability obtained in the implant placement patients were treated with an immediate loading protocol when torque was at least 30 Ncm and implant stability quotient (ISQ) at least 60. In cases of values below cited conventional loading protocol were adopted. O’Ring attachment system from two different commercially available companies were used and allocated systematically between subjects. Follow-up evaluations were performed immediately and at 3 and 6 months after implant placement, when parameters related to implant stability, periimplant tissue aspects, implant survival rate and need for the maintenance of the overdenture were noted. Results: 38 (84.4%) of 45 patients received immediate loaded implants. Implant failure occurred in 3 cases, 2 of them in the conventional loading group (early loss before implant loading). There was an increase in ISQ values in subsequent periods after implant placement, evidenced between baseline and 6 months (p=0.004) and between 3 and 6 months (p<0.001). The mean marginal bone loss at follow-up was 0.50 mm (SD=0.66). It was observed gradual reduction of the gingival height up to 6 months (p=0.006), as well as progressive reduction in plaque index (p=0.028) and bleeding on probing (p=0.021). There was no difference in clinical condition concerning the attachments that were compared. However, Neodent attachments showed better clinical performance and lower replacement rates (p=0.014). Conclusion: This clinical study suggested that the single implant-retained mandibular overdenture is a feasible treatment for mandibular edentulousness, showing favorable clinical and radiographic periimplant outcomes as well as maintenance of the implant stability. / Objetivo: Realizar avaliação longitudinal da perda óssea marginal, aspectos periimplantares e da estabilidade em implante unitário utilizado para retenção de overdenturs mandibular, além de avaliar desfechos relacionados às próteses. Métodos: Quarenta e cinco indivíduos desdentados totais foram tratados com overdenture mandibular retida por implante unitário em oposição a prótese total superior convencional. De acordo com a estabilidade primária obtida no momento de instalação do implante, os indivíduos foram tratados com protocolo de carregamento do implante imediato quando o torque final de instalação era de no mínimo 30 Ncm e o quociente de estabilidade implantar (ISQ) de no mínimo 60. Em casos de valores abaixo dos citados, era adotado o protocolo de carga convencional. Utilizou-se o sistema de retenção O’Ring de duas marcas comerciais, alocados sistematicamente entre os sujeitos. Os desfechos avaliados foram: estabilidade do implante, aspectos teciduais periimplantares e necessidade de manutenção das próteses. O acompanhamento foi realizado nos períodos imediato, 3 e 6 meses após instalação dos implantes. Resultado: Dos 45 pacientes, 38 (84,4%) receberam implante com carga imediata. Houve perda de 3 implantes, sendo 2 no grupo de carga convencional (perda antes do carregamento do implante). O valor médio de ISQ aumentou nos períodos subsequentes à instalação dos implantes, entre o período inicial e de 6 meses (p=0,004) e entre os períodos de 3 e 6 meses (p<0,001). A perda óssea marginal média no período de acompanhamento foi de 0,50 mm (DP=0,66). Foi observada redução progressiva da altura gengival até o período de 6 meses (p=0,006), resultado da acomodação dos tecidos periimplantares, bem como diminuição do índice de placa (p=0,028) e sangramento à sondagem (p=0,021). Não houve diferença na condição clínica em relação às marcas comerciais do sistema de retenção, porém a marca Neodent apresentou desempenho clínico superior com menor ocorrência de substituição da matriz de retenção e maior tempo até necessidade de substituição (p=0,014). Conclusão: O presente estudo clínico mostrou resultados satisfatórios do tratamento com overdenture mandibular retida por implante unitário em relação aos parâmetros clínicos e radiográficos, além da manutenção da estabilidade do implante.

The aim of this in vitro study was to compare the photoelastic characteristics of stress transfer to the implant in a single-implant mandibular overdenture using different types of attachments. A photoelastic model of an edentulous mandible with a resilient edentulous ridge and a unique implant located at the symphyseal region was obtained to reproduce a single implant-retained mandibular overdenture. Six 2.0 mm height attachments were selected and inserted in the same photoelastic model in a random order. A universal material testing machine was used to induce axial vertical loads of 70 N applied in the central incisor and in the central region of the first right molar without contact on the contralateral side. The photoelastic analysis was performed using a polariscope integrated into the testing machine. Standard separate views were photographed and evaluated visually for stress-induced fringes, to comparatively rank the different attachment systems. All attachments showed a similar tension distribution concentrated in the apical third, and the highest stress concentration at the apical level. There was a low stress concentration at the coronal third of the implant, with no discernible stress in the first threads of the implants, except for Dalla Bona which showed low stress at the coronal part of the implant (one fringe). No identifiable fringes were observed when the load was applied in the molar region, which resulted from the low amount of stress transmitted by the implant for all attachments. It was concluded that the load transferred to a single mandibular implant is low and within the limits of functional loading, irrespective of the type of retention system. / Implantes osseointegrados vêm sendo utilizados com sucesso para restaurar a função e a estética de pacientes desdentados totais e parciais. No entanto há necessidade de tornar o tratamento de pessoas com necessidade de overdenture retida com implante menos oneroso e mais eficiente. O objetivo deste trabalho foi verificar pela técnica fotoelástica (Qualitativa) as tensões geradas ao redor do implante pela aplicação de carga sobre overdenture mandibular retida por um implante unitário na sínfise da mandibular. Foi confeccionado um modelo de resina fotoelástica Araldite, reproduzido de uma mandibular anatômica, onde foi fixado na região sínfise um implante 3.75x11mm (Titamax Ti Cortical, Neodent, Curitiba, Brasil). Com um modelo foi confeccionada uma prótese total e gingival artificial. Todos os pilares foram posicionados sobre o implante e apertado com 10 Ncm, a fêmea foi capturada de forma direta à prótese, com a réplica da mucosa em posição. Todos foram sucessivamente capturados e levados ‘à máquina de ensaio universal. Com uma carga axial de 70 Ncm na fossa central do 46 e incisal do 41. Foram fotografados em um polariscópio as franjas resultantes das tensões. Foram feitas análises qualitativa dos resultados das tensões sobre o attachment. De acordo com os resultados obtidos pode-se concluir que a carga transferida para o implante, é baixo, independente do sistema de retenção.

The aim of this study was to evaluate patient satisfaction and oral health-related quality of life of single-implant mandibular overdenture wearers and to assess the effectiveness of different loading protocols (conventional and immediate) and two different O’ring trademarks. After delivering new conventional dentures, a single external hexagon implant was inserted in the mandibular midline region to retain an overdenture opposing to a conventional maxillary denture. Forty-five patients were treated, 38 (84.4%) achieved sufficient primary stability (final insertion torque greater than 30N and implant stability quotient greater than 60) and immediate loading was adopted and 7 (15.6%) received conventional loading since not enough primary stability was achieved. There were two implant failures in the conventional loading group and one failure in the immediate loading group, resulting in a final sample of 42 patients. A questionnaire regarding patient satisfaction in relation to the upper and lower dentures was applied and the instrument OHIP-Edent was used in order to evaluate the oral health-related quality of life. Follow-up evaluations were performed before insertion of the implant and after 3 and 6 months postoperatively. Satisfaction with the lower denture was significantly increased at the 3 months follow-up (p<0.001) and remained unchanged at the 6 months follow-up. Impact on oral health-related quality of life decreased significantly after 3 months and 6 months (p<0.001). Comparison between groups of immediate and conventional loading was infeasible because only 5 cases (11.9%) adopted conventional loading. There was no significant difference between the twoattachment trademarks regarding patient outcomes, except for the time necessary to replacement of the retention matrix, which was significantly lower in the Conexão group. It was concluded the single implant-retained mandibular overdenture treatment showed satisfactory results during the study period, suggesting that this is a feasible treatment alternative for patients with limited adaptation to the conventional mandibular denture. / O uso da overdenture mandibular retida por implante unitário localizado na sínfise mandibular tem sido proposto como simplificação do tratamento com dois implantes, apresentando resultados satisfatórios com um menor custo do tratamento. Este trabalho objetivou avaliar a satisfação e o impacto na qualidade de vida relacionada à saúde bucal de usuários de overdenture mandibular retida por implante unitário, testar a efetividade de diferentes protocolos de carregamento (imediato e convencional) e de diferentes marcas comerciais do sistema de retenção O’ring. Após confecção de próteses totais convencionais superior e inferior, foram instalados implantes unitários do tipo hexágono externo com plataforma regular na região de sínfise mandibular. Foram tratados 45 pacientes, 38 (84,4%) atingiram parâmetros mínimos de estabilidade primária (torque acima de 30 N e ISQ acima de 60) e foi adotada carga imediata e 7 (15,6 %), por não terem atingido estabilidade primária suficiente, receberam carga convencional. Houve perda precoce de 2 implantes do grupo carga convencional e 1 de carregamento imediato, resultando em uma amostra final de 42 pacientes. Foram aplicados os questionários de satisfação em relação às próteses superior e inferior e também o instrumento OHIP-Edent, com a finalidade de avaliar o impacto na qualidade de vida relacionada à saúde bucal. Os desfechos foram avaliados nos períodos inicial (antes da instalação dos implantes) e após 3 e 6 meses da instalação (testes de Wilcoxon e Teste t pareado). A satisfação com a prótese inferior aumentou significativamente no período de 3 meses (p<0,001), mantendo-se no período de 6 meses. O impacto da condição bucal na qualidade de vida dos pacientes diminuiu significativamente após a instalação do implante e captura da prótese, no período de 3 meses (p<0,001) e 6 meses (p<0,001). A comparação entre os grupos de carga imediata e convencionalfoi inviável devido ao fato de que em apenas 5 casos (11,9%) instalou-se overdenture com carga convencional. Não houve diferença significativa entre as marcas comerciais dos sistemas de retenção O’ring utilizados em relação às medidas de satisfação com as próteses e os escores do OHIP-Edent, com exceção ao tempo de substituição da matriz de retenção que foi significativamente menor no grupo da marca comercial Conexão. Concluiu-se que o tratamento com overdenture mandibular retida por implante unitário apresentou resultados satisfatórios no período avaliado, o que sugere que esta é uma alternativa viável de tratamento para pacientes com dificuldade de adaptação à prótese total mandibular convencional.

Aucune étude n’a évalué les attentes des patients vis-à-vis d'un tel protocole. Objectifs : Évaluer les attentes, le niveau de satisfaction ainsi que la qualité de vie reliée à la santé bucco-dentaire des édentés complets suite à un protocole de mise en charge immédiate. Méthodologie : Cet essai clinique de phase 1 utilise un design pré-post afin d’évaluer les mesures centrées sur le patient. Dix-huit individus, complètement édentés et âgés en moyenne de 62,39 ± 7,65 ans, ont reçu une prothèse totale mandibulaire de recouvrement sur deux implants non jumelés suite à un protocole de mise en charge immédiate, conjointement à une prothèse totale conventionnelle maxillaire. Un instrument adapté pour mesurer leurs attentes à l’aide d’échelles visuelles analogues, le questionnaire « McGill Denture Satisfaction Instrument » ainsi que le questionnaire OHIP-20 ont été remis aux patients avant de procéder aux traitements (T0), ainsi qu’aux rendez-vous de suivi à 2 semaines (T1), 1 mois (T2) et 4 mois (T3). De plus, l’inventaire de personnalité révisé (NÉO PI-R) ainsi qu’un questionnaire sociodémographique ont été remplis par les participants. Les « change scores » ont été calculés puis des tests non paramétriques et des analyses de variances en mesures répétées suivies de comparaisons par paires ont été utilisés afin d’analyser les données recueillies. La taille d’effet a été estimée. Résultats : Les participants avaient différentes attentes par rapport à la mise en charge immédiate. Certains s’attendaient à un effet positif à court terme par rapport à leur apparence esthétique (83,3 %) et à leur vie sociale (55,7 %), alors que d’autres avaient des craintes envers leur confort (5,6 %), leur habileté à mastiquer (11,1 %) et à nettoyer leur prothèse inférieure (11,1 %). À 4 mois, le protocole de mise en charge immédiate avait rencontré la majorité des attentes des patients par rapport à l’esthétique (94.4 %), la mastication (83.3 %), la phonétique (61.1 %), le confort (94.4 %), l’hygiène (88.9 %) et leur vie sociale (88.9 %). Une amélioration statistiquement significative de la satisfaction en générale, du confort, de l’esthétique, de la stabilité de la prothèse inférieure et de l’habileté à mastiquer a été notée à 2 semaines (p<0,001). Également, les comparaisons par paires ont révélé une diminution statistiquement significative du score total de l’OHIP-20 (p < 0,001) de même que la majorité des domaines de l’OHIP (p < 0.01), sauf pour l’handicap social qui n’a diminué significativement qu’après 1 mois (p = 0.01). Ces changements (pour la satisfaction et la qualité de vie) sont restés stables au cours des suivis subséquents. Indépendamment des traits de personnalité et des variables sociodémographiques, le protocole immédiat a satisfait 94,4 % des participants et a amélioré leur qualité de vie avec une large magnitude d’effet (d = 1.9; p < 0.001). Bien que deux patients aient perdu des implants au cours du traitement, 100 % des participants étaient d’accord pour recommander cette procédure à leurs pairs. Conclusions: Le protocole de mise en charge immédiate semble satisfaire les patients quelles que soient leurs attentes. Le protocole MCI peut améliorer, à court terme, la qualité de vie des patients totalement édentés. Les résultats prometteurs de la phase 1 devraient être corroborés lors de la 2e phase de cette étude. / Statement of problem: Most published research on the immediate loading of a mandibular 2-implant overdenture has only reported on clinical outcomes. Few studies have evaluated patients centered-outcomes but with measurement errors. Consequently, there is a need to assess clinical and patient-reported outcomes in this regard. Objectives: To examine patients’ level of satisfaction, expectations and quality of life with regard to the implants’ immediate loading protocol in edentate individuals. Methods: This phase-I trial used a pre-post design to assess patient-centered outcomes in 18 edentate individuals (mean age 62.39 ± 7.65 years) who have received a 2-implant (unsplinted) mandibular overdenture through an immediate loading protocol. The McGill Denture Satisfaction Instrument, a VAS instrument assessing expectations and the OHIP-20 questionnaire were used to evaluate patients’ outcomes at baseline, 2 weeks, 1 and 4 months. Socio-demographic and personality trait data were obtained using the Revised NEO Personality Inventory and a self-administered questionnaire. Change scores were calculated and non-parametrical tests were used to analyse the data. Repeated measures ANOVAs and paired t-tests were also conducted, and effect size was estimated. Results: The participants had a wide range of expectations regarding the immediate loading protocol. Expectations included short-term positive impact on aesthetics (83.3 %) and social life (55.7 %). Other expectations included negative effects on comfort (5.6%), the ability to chew (11.1 %) and to clean their lower denture (11.1%). At 4 months, the immediate loading protocol had met most patients’ expectations regarding aesthetics (94.4%), the ability to chew (83.3%), ability to speak (61.1%), comfort (94.4%), the ability to clean their lower denture (88.9%) and their social life (88.9%). The statistical analysis showed an overall satisfaction increase with a median change score of 66 (range=7-98), 68 (range=7- 95), and 70 (range= 17- 99), at 2 weeks, 1 month and 4 months respectively. There was a statistically significant improvement in overall satisfaction, comfort, perceived aesthetics, stability of the overdenture, and ability to chew at 2 weeks (p < 0,001). Pairwise comparison (within-subjects comparisons) revealed significant decreases in total OHIP scores (P < 0.001), as well as fewer functional limitations, physical pain, psychological discomfort, physical disability, psychological disability, and handicap (P < 0.01) from baseline to 2 weeks, 1 month and 4 months follow-up. Social disability score decrease significantly only after 1 month follow-up (p = 0.01). These changes remained stable during subsequent follow-ups. The protocol satisfied 94.4% of the participants and improve their quality of life with an estimated effect size of 1.9 (p < 0.001), regardless of socio-demographic and personality profiles. There was 100% agreement among patients on recommending this procedure to their peers. Conclusions: Immediate loading of two unsplinted implants with a mandibular overdenture seemed to fulfill patients’ satisfaction regardless of their expectations and may improve oral health-related quality life of edentate individuals in short-term. Encouraging results from phase I should be confirmed and approved in Phase II clinical trials.

Certains s’attendaient à un effet positif à court terme par rapport à leur apparence esthétique (83,3 %) et à leur vie sociale (55,7 %), alors que d’autres avaient des craintes envers leur confort (5,6 %), leur habileté à mastiquer (11,1 %) et à nettoyer leur prothèse inférieure (11,1 %). À 4 mois, le protocole de mise en charge immédiate avait rencontré la majorité des attentes des patients par rapport à l’esthétique (94.4 %), la mastication (83.3 %), la phonétique (61.1 %), le confort (94.4 %), l’hygiène (88.9 %) et leur vie sociale (88.9 %). Une amélioration statistiquement significative de la satisfaction en générale, du confort, de l’esthétique, de la stabilité de la prothèse inférieure et de l’habileté à mastiquer a été notée à 2 semaines (p<0,001). Également, les comparaisons par paires ont révélé une diminution statistiquement significative du score total de l’OHIP-20 (p < 0,001) de même que la majorité des domaines de l’OHIP (p < 0.01), sauf pour l’handicap social qui n’a diminué significativement qu’après 1 mois (p = 0.01). Ces changements (pour la satisfaction et la qualité de vie) sont restés stables au cours des suivis subséquents. Indépendamment des traits de personnalité et des variables sociodémographiques, le protocole immédiat a satisfait 94,4 % des participants et a amélioré leur qualité de vie avec une large magnitude d’effet (d = 1.9; p < 0.001). Bien que deux patients aient perdu des implants au cours du traitement, 100 % des participants étaient d’accord pour recommander cette procédure à leurs pairs. Conclusions: Le protocole de mise en charge immédiate semble satisfaire les patients quelles que soient leurs attentes. Le protocole MCI peut améliorer, à court terme, la qualité de vie des patients totalement édentés. Les résultats prometteurs de la phase 1 devraient être corroborés lors de la 2e phase de cette étude. / Statement of problem: Most published research on the immediate loading of a mandibular 2-implant overdenture has only reported on clinical outcomes. Few studies have evaluated patients centered-outcomes but with measurement errors. Consequently, there is a need to assess clinical and patient-reported outcomes in this regard. Objectives: To examine patients’ level of satisfaction, expectations and quality of life with regard to the implants’ immediate loading protocol in edentate individuals. Methods: This phase-I trial used a pre-post design to assess patient-centered outcomes in 18 edentate individuals (mean age 62.39 ± 7.65 years) who have received a 2-implant (unsplinted) mandibular overdenture through an immediate loading protocol. The McGill Denture Satisfaction Instrument, a VAS instrument assessing expectations and the OHIP-20 questionnaire were used to evaluate patients’ outcomes at baseline, 2 weeks, 1 and 4 months. Socio-demographic and personality trait data were obtained using the Revised NEO Personality Inventory and a self-administered questionnaire. Change scores were calculated and non-parametrical tests were used to analyse the data. Repeated measures ANOVAs and paired t-tests were also conducted, and effect size was estimated. Results: The participants had a wide range of expectations regarding the immediate loading protocol. Expectations included short-term positive impact on aesthetics (83.3 %) and social life (55.7 %). Other expectations included negative effects on comfort (5.6%), the ability to chew (11.1 %) and to clean their lower denture (11.1%). At 4 months, the immediate loading protocol had met most patients’ expectations regarding aesthetics (94.4%), the ability to chew (83.3%), ability to speak (61.1%), comfort (94.4%), the ability to clean their lower denture (88.9%) and their social life (88.9%). The statistical analysis showed an overall satisfaction increase with a median change score of 66 (range=7-98), 68 (range=7- 95), and 70 (range= 17- 99), at 2 weeks, 1 month and 4 months respectively. There was a statistically significant improvement in overall satisfaction, comfort, perceived aesthetics, stability of the overdenture, and ability to chew at 2 weeks (p < 0,001). Pairwise comparison (within-subjects comparisons) revealed significant decreases in total OHIP scores (P < 0.001), as well as fewer functional limitations, physical pain, psychological discomfort, physical disability, psychological disability, and handicap (P < 0.01) from baseline to 2 weeks, 1 month and 4 months follow-up. Social disability score decrease significantly only after 1 month follow-up (p = 0.01). These changes remained stable during subsequent follow-ups. The protocol satisfied 94.4% of the participants and improve their quality of life with an estimated effect size of 1.9 (p < 0.001), regardless of socio-demographic and personality profiles. There was 100% agreement among patients on recommending this procedure to their peers. Conclusions: Immediate loading of two unsplinted implants with a mandibular overdenture seemed to fulfill patients’ satisfaction regardless of their expectations and may improve oral health-related quality life of edentate individuals in short-term. Encouraging results from phase I should be confirmed and approved in Phase II clinical trials.
