Männens delaktighet på BB: Ideologi eller verklighet? : En studie om förstagångsfäders upplevelser / Male Participation in Maternity Hospital: Ideology or Reality : A Study of First-time Fathers´ Experiences

BB som verksamhet har förändrats över tid och är idag en plats där familjen skall kunna vila ut efter förlossningen, i en lugn och trygg miljö. Att fadern på olika sätt är delaktig kring sitt barn främjar faderskapet. Syftet med studien var att beskriva förstagångsfäders upplevelser av delaktighet på BB efter en normal förlossning. För att undersöka fenomenet valdes en deskriptiv, fenomenologisk metod enligt Giorgi. Halvstrukturerade intervjuer genomfördes med tio förstagångsfäder i åldrarna tjugotre till fyrtioett år. Utifrån det insamlade materialet identifierades fyra teman i analysen. En inre kärna, den så kallade essensen, fastställdes och den benämndes Dubbelhet - Familjeinriktad eller mor- och barninriktad? När familjen anländer till BB efter en normal förlossningen uppträder en tvetydighet och förutsättningarna för faderns möjlighet till delaktighet förändras. Att få vara närvarande med sin familj på BB upplevdes som något positivt av fäderna och flera ansåg att det borde vara en självklarhet att få den möjligheten. Organisatoriska faktorer orsakade en känsla av utanförskap hos fäderna. Personalens bemötande och mannens eget intresse hade betydelse för hur fäderna upplevde graden av delaktighet.

No. of bitstreams: 0 Previous issue date: 2009 / OBJETIVOS: Avaliar o nível de conhecimento e as práticas de aleitamento materno e de aleitamento cruzado de parturientes internadas em um Hospital e Maternidade da região nordeste brasileira, com vistas ao desenvolvimento futuro de projetos educacionais de encorajamento à amamentação em Horizonte, Ceará. SUJEITOS E MÉTODO: Foi realizado um estudo transversal em uma amostra de 120 mães internadas em alojamento conjunto na Unidade Hospitalar e Maternidade Venâncio Raimundo de Sousa, Horizonte, Ceará, no período de janeiro a abril de 2008. Foi aplicado um questionário estruturado às mães nas primeiras 24 horas após o parto contendo perguntas sobre características sócio econômicas, assistência pré-natal, conhecimentos e práticas em aleitamento materno e em aleitamento cruzado. RESULTADOS: A maior parte das mães (86 por cento) vivia com o companheiro, 25 por cento eram adolescentes e 20 por cento exerciam trabalho remunerado, a renda mediana encontrando-se entre um e dois salários mínimos. Embora todas tenham realizado consultas de pré-natal, 56 por cento não tiveram as mamas examinadas e 30 por cento não receberam qualquer orientação sobre aleitamento materno. Quanto aos conhecimentos das puérperas sobre a amamentação exclusiva, 89 por cento das mulheres informaram que sua duração deveria ser até os seis meses, enquanto 14 por cento achavam que deveria ser administrada água e chá e 4 por cento que deveria ser oferecido suco. A maioria (81 por cento) afirmou ser necessário oferecer os dois seios a cada mamada ao lactente. Dentre as mulheres multíparas, 88 por cento já tiveram a vivência da amamentação, sendo a duração mediana de aleitamento materno do último filho de nove meses. A maior parte dos companheiros (75 por cento) e dos familiares (70 por cento) considerou que amamentar é importante. O aleitamento cruzado foi praticado por 32 por cento das multíparas, predominantemente com parentes. CONCLUSÕES: O estudo mostrou a necessidade de instrumentalização da rede básica de saúde em procedimentos de promoção, proteção e apoio ao aleitamento materno nos serviços de pré-natal com o intuito de melhorar o conhecimento e as práticas maternas em amamentação. / OBJECTIVE: The purpose of this study was to assess the level of knowledge and the practice on breastfeeding and cross-nursing among parturient women at a maternity-hospital in Northeast Brazil. The goal was to show the importance of developing educational projects to encourage breastfeeding in the city of Horizonte, Ceará. METHODS: A cross-sectional study was conducted at the Venâncio Raimundo de Sousa Maternity Hospital, located in Horizonte, Ceará, Brazil. A sample of 120 rooming-in mothers were interviewed between January and April, 2008. A structured questionnaire was applied during the first day after labor. Mother’s social economic conditions, reproductive history, prenatal care, level of knowledge and practice on breastfeeding and cross-nursing were investigated. RESULTS: Most mothers (86%) lived with their partners and 25% were adolescents. Only 20% were working mothers, the median payment being from one to two minimum wages. All of them had prenatal care, but 56% did not have their breasts examined and 30% did not receive any advice on breastfeeding during prenatal care. Most mothers (89%) affirmed that exclusive breastfeeding should last for 6 months, 14% considered that babies should be offered water or tea and 4% juices. Most mothers (81%) considered that both breasts should be offered each time to the baby. Among the multiparous women, 88% had already breastfed. The median duration of previous breastfeeding was of 9 months. Most part of mother’s partners (75%) and relatives (70%) considered that breastfeeding is important. Cross-nursing had been practiced by 32% of them, mainly with a kin. CONCLUSIONS: The research has shown the need of implementing breastfeeding promotion, protection and support in prenatal units, in order to improve mothers knowledge and practices on breastfeeding.

Zkušenosti žen s poporodní péčí v českých porodnicích / Women's Experiences of Postnatal Care in Czech Maternity Hospitals

This diploma thesis focuses on the issue of postnatal care in maternity hospitals in the Czech Republic as it is perceived by women who went through the medicalised obstetric birth in the past year. It examines in particular women's expectations concerning the care on the postnatal ward, their experiences of various aspects of care provided, the moments women highlight and the strategies they adopt in case of possible clashes of their own "non-expert" opinions with the regime of particular maternity hospital or "authoritative knowledge" of the staff. The aim of this thesis is to present the authentic experiences of postnatal care and to implant them into broader conceptual framework, especially considering the medical discourse and possible disciplinary techniques which are implemented to strengthen the authority of medical staff and to provide the opportunity for physical and social control of women.

Reliability of the COntext Assessment for Community Health (COACH) tool when administered on mobile phones versus pen-paper: A comparative study among healthcare staff in Nairobi, Kenya.

Aim: To investigate the reliability of the COntext Assessment for Community Health (COACH) tool on mobile phone versus pen-paper in Nairobi, Kenya. Background: One of the barriers to the progress of the MDGs has been the failure of health systems in many LMICs to effectively implement evidence-based interventions As a result of the “know-do” gap, patients do not benefit from advances in healthcare and are exposed to unnecessary risks. Better mapping of context improves implementation by allowing tailoring of strategies and interpretation of knowledge translation. COACH investigates healthcare contexts for LMICs and has only been used on pen-paper. With 5 billion mobile phone users globally, mobile technologies is being recognized as able to play a formal role in health services. Methods: Comparative study with 140 nurses/midwives and doctors in four hospitals in Nairobi. 70 were randomly assigned to mobile phone and pen-paper each. The tool was administered twice with a two week interval and test-retest reliability, internal consistency and interrater reliability were assessed. Findings: Excellent test-retest reliability for both pen-paper and mobile phone (ICC >0.81). 45% (pen-paper) and 34% (mobile phone) moderate agreement between individual questions in round 1 and 2. Acceptable average Cronbach’s alpha (>0.70). Conclusion: Both mobile phone and pen-paper were reliable and feasible for data collection. The findings are a good first step towards using COACH in Kenya. Additional research is needed for individual settings. Using mobile phones could increase healthcare facilities’ accessibility in implementation research, helping to close the “know-do” gap and reach the SDGs.
