Protection contre les effets d’une exposition prénatale à l’alcool durant la préimplantation chez l’embryon par une diète maternelle enrichie en donneurs de méthyles

La consommation d’alcool pendant la grossesse peut entraîner des conséquences néfastes sur le développement de l’enfant et mener aux troubles du spectre de l’alcoolisation fœtale (TSAF). Cependant on en connaît peu sur son effet durant la période de préimplantation. Cette période est reconnue comme étant sensible à l’environnement, majoritairement dû au fait qu’elle est caractérisée par la reprogrammation dynamique des profils de méthylation de l’ADN. L’alcool est reconnu pour altérer les mécanismes impliqués dans les processus de méthylation de l’ADN (e.g., cycle du folate, actions des DNMTs). Récemment, nous avons démontré qu’une exposition prénatale à l’alcool éthylique durant la préimplantation altère les futurs profils de méthylation de l’ADN du cerveau et augmente le nombre de défauts morphologiques à la mi-gestation. Il n’y a présentement aucun traitement pour les TSAF, mais certaines études suggèrent qu’une diète enrichie en donneurs de groupement méthyles (e.g., folate, choline), nécessaires aux réactions de méthylation, pourrait atténuer les effets d’une exposition prénatale à l’alcool. C’est pourquoi nous avons voulu déterminer si une diète enrichie en donneurs de groupements méthyles pouvait apporter une protection aux embryons ayant subi une exposition prénatale à l’alcool durant la préimplantation. Pour ce faire, une diète standard ou enrichie a été donné à des souris 4 semaines avant la gestation et pendant celle-ci. Les femelles gestantes ont reçu une dose d’alcool au jour E2.5, puis nous avons récolté les embryons à E10.5 et E18.5 pour évaluer les anomalies physiques et faire l’analyse de la méthylation des cerveaux antérieurs. Nos résultats démontrent que la diète enrichie prévient ou élimine un certain nombre de défauts morphologiques à E10.5 et E18.5. Nous avons aussi observé que la diète enrichie n’affectait pas la mise en place et le maintien de la méthylation et l’expression des gènes à empreintes durant le développement. La diète enrichie en donneurs de groupements méthyles aide donc à prévenir certains effets nuisibles de l’exposition prénatale à l’alcool survenant durant la préimplantation. / Alcohol consumption during pregnancy can have a significant impact on the development of the child and lead to fetal alcohol spectrum disorders (FASD). Nonetheless, little is known about its effect during the pre-implantation period. However, we know that this period is very sensitive to the environment, mainly because of major reprogramming of DNA methylation patterns. Moreover, alcohol can alter the mechanisms involved in DNA methylation processes (e.g., folate cycle, actions of DNMTs). Recently, we have shown that prenatal alcohol exposure (PAE) during preimplantation alters future DNA methylation patterns of the young embryo brain and increases the number of morphological defects. There is currently no treatment for FASD, but studies suggest that a diet enriched in methyl donors (e.g., folate, choline), which are necessary for methylation reactions, may mitigate the effects of PAE. Therefore, we wanted to determine if a diet enriched in methyl group donors could provide protection to embryos against a PAE during preimplantation. To do so, a standard or enriched diet was given to mice 4 weeks before and during gestation. Pregnant females were exposed to alcohol at day E2.5, then we harvested embryos (E10.5, E18.5) to assess physical abnormalities and analyse forebrain methylation. Our results demonstrate that the enriched diet reduces or eliminate defects at E10.5 and E18.5. We also observed that the fortified diet did not affect the establishment and maintenance of methylation and expression of imprinted genes during development. Thus, our results show that a methyl enriched diet can prevent some of the adverse effects of prenatal alcohol exposure occurring during preimplantation.

Sleep and developmental risks: The roles of extra-axial cerebrospinal fluid

<p>The manifestations of early sleep disturbances on cerebrospinal fluid and their relations with early developmental competencies are understudied. Recent studies highlight cerebrospinal fluid disbursement as a potential factor associated with dysfunctions in brain development. With two studies, we explored sleep and extra-axial cerebrospinal fluid (EA-CSF) connection as a potential mechanistic pathway by which sleep dysregulation influences brain and behavior development. Specifically, we evaluated associations between (1) EA-CSF to total cerebral volume (EA-CSF/TCV) ratios, (2) parent-report of child sleep problems, and (3) social communication development in typical (Study 1) and atypical populations (Study 2). In typical infants, early sleep problems did not precede later elevated EA-CSF/TCV ratios or social-communicative competence. Elevated EA-CSF/TCV ratios were associated with impaired social communication skills, suggesting that a relationship between elevated EA-CSF/TCV ratios and social communication impairments exists regardless of neurological or sleep problems. In an atypical population with autism spectrum disorder (ASD), older children with ASD had similar EA-CSF/TCV ratios to a group of their typically developing peers. Sleep problems were negatively associated with EA-CSF/TCV ratios but positively associated with social-communicative impairments for children with ASD, highlighting the influence of sleep problems on both brain and behavioral outcomes in an atypical population. In both studies, EA-CSF volumes continue to increase during early development in the typically developing populations (but not later in the atypical sample), underlining its relevance as a marker of atypical processing. Recognizing the potential roles of EA-CSF in influencing several biosocial and behavioral aspects of development, we encourage researchers to continue to explore EA-CSF growth, especially during developmental periods of flux and transition. Future work with longitudinal data can also serve to explore sleep-related developmental changes in EA-CSF, in association with behavioral and phenotypic changes. </p>

Phénotypage computationnel de l’interaction sociale dans le développement psychopathologique et neurotypique

Le Trouble du Spectre de l’Autisme (TSA) est un trouble neurodéveloppemental défini par deux caractéristiques principales : un comportement social atypique et des intérêts restreints et répétitifs incluant une rigidité comportementale. Le comportement social atypique est typiquement mesuré et quantifié par des déficits de Théorie de l’Esprit (ToM). Les outils classiques pour évaluer la ToM comportent plusieurs limites dont les biais liés à la subjectivité, la pauvre validité écologique et le manque d’échange réciproque. Ainsi, une tâche dyadique et intégrative pourrait pallier ces limites. En population adulte autiste, une telle tâche a été testée. Concrètement, le jeu du Penny-Hiding Game (PHG), où un joueur doit deviner dans laquelle de ses mains un adversaire a caché une pièce de monnaie, a été administré à un groupe de personnes autistes et un groupe de personnes non-autistes. La tâche comportait deux contextes : l’un social, où le participant croyait qu’il jouait contre un être humain en ligne, et l’un non-social, où le participant était informé qu’il jouait contre un ordinateur seulement. Cette étude a montré que les personnes autistes se montraient insensibles au contexte dans leur comportement modélisé par un phénotype computationnel comportant deux indices : la sophistication et la flexibilité. De plus, l’étude a identifié une stratégie de jeu qui semblait propre au groupe autiste. Le phénotype computationnel permettait aussi de classer les participants avec un taux de spécificité équivalent aux outils diagnostiques standards. La présente étude vise donc à étendre cette compréhension de la réciprocité sociale des personnes autistes à une population pédiatrique. L’enjeu des comorbidités en autisme constitue également un frein important aux avancées scientifiques. Ainsi, nous avons décidé d’inclure deux groupes dans notre échantillon qui présentent les comorbidités les plus prévalentes en autisme : le Trouble de l’Attention avec et sans Hyperactivité (TDAH) et les Troubles Anxieux (TA). À la suite d’une analyse de la performance avec ces trois groupes et une inspection visuelle des séquences de choix au cours de la tâche, un paramètre d’alternance/persévération et un groupe d’enfants neurotypiques (NT) ont été ajoutés. Ces deux ajouts font de ce mémoire une étude partiellement exploratoire. Le but était donc de valider la tâche informatique du PHG auprès de populations pédiatriques cliniques, d’identifier et d’explorer des différences de groupes au niveau des stratégies de jeu, de la performance et du phénotype computationnel et d’explorer le rôle potentiel des associations entre le phénotype computationnel, la performance et les symptômes associées aux trois conditions cliniques à l’étude : les symptômes autistiques, attentionnels/exécutifs et anxieux. Les analyses ont permis de révéler que le groupe NT avait une performance significativement supérieure aux groupes TSA, TDAH et TA et que la quantité et la sévérité des symptômes étaient négativement associées au score de performance en contexte social. La stratégie la plus utilisée à travers tous les groupes et indépendamment du contexte était une stratégie d’apprentissage par renforcement et aucune différence au niveau de la sophistication ou de la flexibilité n’a été détectée. Ces deux résultats constituent une non-réplication de ce qui a été observé chez les adultes. La persévérance dans la tâche était plus élevée chez les personnes autistes spécifiquement, ce qui constitue un marqueur potentiel pour dépister, et éventuellement identifier le TSA, bien que d’autres études soient nécessaires pour le confirmer. Cette étude novatrice a permis d’illustrer la complexité de la réalité clinique et de mettre la table pour des études subséquentes en psychiatrie computationnelle auprès des enfants. / Autism Spectrum Disorder (ASD) is a neurodevelopmental disorder defined by two main diagnostic criteria: atypical social communication and restricted, repetitive interests including behavioral rigidity. Atypical social communication is typically measured and quantified by deficits in Theory of Mind (ToM). Conventional tools for assessing ToM have several limitations, including subjectivity bias, poor ecological validity and lack of reciprocal exchange. A dyadic, integrative task could be used to overcome these limitations. Such a task was tested in an adult autistic population. Specifically, the Penny-Hiding Game (PHG), in which a player must guess in which of his or her hands an opponent has hidden a coin, was administered to a group of autistic and a group of non-autistic individuals. The task was comprised of two framings: one social, where the participant believed he or she was playing online against a human being, and one non-social, where the participant was informed that he or she was playing against a computer. This study showed that autistic adults were insensitive to the framing in their behavior, which was modeled by a computational phenotype made up of two indices: sophistication and flexibility. In addition, the study identified an adaptation strategy that appeared to be specific to the autistic group. The computational phenotype also made it possible to classify participants with a specificity equivalent to standard diagnostic tools. The present study therefore aims to extend this understanding of social reciprocity in people with autism to a pediatric population. The issue of comorbidities in autism is also a major obstacle to scientific progress. We therefore decided to include in our sample two groups with the most prevalent comorbidities in autism: Attention-Deficit with or without Hyperactivity Disorder (ADHD) and Anxiety Disorder (AD). Following an analysis of performance with these three groups and visual inspection of choice sequences during the task, an alternation/perseveration parameter and a neurotypical (NT) group were added. These two additions make this master’s thesis partially exploratory. The aims were therefore to validate the PHG computerized task with clinical pediatric populations, to identify and explore group differences in adaptation strategies, performance and computational phenotype, and to explore the potential role of associations between computational phenotype, performance and symptoms associated with the three clinical conditions included in the study: autistic, attentional/executive and anxious symptoms. Analyses revealed that the NT group performed significantly better than the ASD, ADHD and AD groups, and that symptom quantity and severity were negatively associated with task performance score in the social framing. The most frequently used strategy across all groups and regardless of framing was reinforcement learning, and no differences in sophistication or flexibility were detected. These two results represent a lack of replication of previous work with autistic and NT adults. Perseverance in the task was higher in autistic individuals, which could be a potential marker for screening - possibly identifying - ASD, although further studies are needed to confirm this. This novel study helped illustrate the complexity of clinical contexts and set the table for subsequent computational psychiatry studies with children.

A data driven machine learning approach to differentiate between autism spectrum disorder and attention-deficit/hyperactivity disorder based on the best-practice diagnostic instruments for autism

Autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD) are two frequently co-occurring neurodevelopmental conditions that share certain symptomatology, including social difficulties. This presents practitioners with challenging (differential) diagnostic considerations, particularly in clinically more complex cases with co-occurring ASD and ADHD. Therefore, the primary aim of the current study was to apply a data-driven machine learning approach (support vector machine) to determine whether and which items from the best-practice clinical instruments for diagnosing ASD (ADOS, ADI-R) would best differentiate between four groups of individuals referred to specialized ASD clinics (i.e., ASD, ADHD, ASD + ADHD, ND = no diagnosis). We found that a subset of five features from both ADOS (clinical observation) and ADI-R (parental interview) reliably differentiated between ASD groups (ASD & ASD + ADHD) and non-ASD groups (ADHD & ND), and these features corresponded to the social-communication but also restrictive and repetitive behavior domains. In conclusion, the results of the current study support the idea that detecting ASD in individuals with suspected signs of the diagnosis, including those with co-occurring ADHD, is possible with considerably fewer items relative to the original ADOS/2 and ADI-R algorithms (i.e., 92% item reduction) while preserving relatively high diagnostic accuracy. Clinical implications and study limitations are discussed.

Introduction: Developmental dysphasia (DD) is a developmental language disorder with high prevalence, but also with low publication index and confusion of the terminology (including the Czech version of ICD - "speech" disorder instead of "language" disorder). The scientific view has passed the development from a symptom, through the specific nozologic unit to the current term Developmental language disorder (DLD) and its classification among a new category of diseases in ICD-11 (existing in DSM-5) called Neurodevelopmental disorders. Another disorder with the characteristic pathology in communication is the autism spectrum disorder (ASD) which is classified as a disease of the same category as DD. Compared to DD, it has a higher publication index, lower prevalence, and even more serious consequences. Three different domains that cause confusion, both in ASD and DD, are "speech", "language", and "communication". The same confusion applies to the current subtypes in ICD-11 with the term "functional language". In the Czech Republic there are very few studies dealing with the patients with DD, their profile of a clinic picture has not been processed so far. A transdisciplinary approach to dealing with child offenders with psychiatric disorders

This study aimed to develop a framework for improved legislation, methods of practice and services used to meet the best interest of child offenders with psychiatric disorders within the South African child justice system. This aim was achieved via a qualitative study, which evaluated child justice and mental health legislation, methods of practice, role-players, and services in South Africa, and compared same to those in Namibia, Botswana, and Nigeria; to establish if the best interest of child offenders with psychiatric disorders are currently met from a South African perspective. Data collection occurred in two phases: a document analysis of legislation, policies, and procedures in the selected comparative jurisdictions; followed by semi-structured interviews with child justice and mental health experts. During phase one the researcher analysed child justice and mental health legislation, human rights protections, and policy related to child justice in Nigeria, Botswana, Namibia, and South Africa. In addition, she interrogated literature relating to child justice and mental health, in comparative jurisdictions. This phase formed the foundation for the semi-structured interview schedule. Purposive and theoretical sampling was used to conduct 24 semi-structured interviews. Data was analysed and interpreted using pure qualitative document and thematic analysis. The overarching findings indicate that the best interest of child offenders with psychiatric disorders, in the South African child justice system, are unprotected. This vulnerable group is not dealt with from an individualised, case-specific, multi-disciplinary perspective; informed by legislation, practice-policy, and a service-orientated approach. Further, certain domestic legislation and methods of practice are inadequate in holistically assessing a child who comes into conflict with the law in the jurisdictions of comparison. Further findings identified that biological, psychological, environmental, cultural, and social factors influence the development of psychiatric disorders in children, which may bring them into conflict with the law. This study further confirmed that Attention Deficit Hyperactivity Disorder, Intellectual Development Disorder, Learning Disorder, Oppositional Defiant Disorder, and Conduct Disorder are prevalent in children who iv come into conflict with the law. Further, substance abuse disorder, depression and attachment disorder were identified as predominant factors affecting children who come into conflict with the law. The influence of these factors, in conjunction with biological, psychological, environmental, cultural, and social factors, were found to predispose children to psychiatric disorders linked to criminal behaviour. Thus, to meet the best interest standard; children in conflict with the law must be dealt with using a multi-factorial approach which considers biological, environmental, social, cultural, and psychological factors. In this way, the behaviour of child offenders with psychiatric disorders will be addressed holistically in a manner that considers all factors influencing behaviour.
