The Story of fable: A narrative analysis of the experience of four registered nurses in their first year of practice.

This research explores the transition from student to registered nurse from the perspective of the new graduate. This interpretive study uses narrative analysis as the methodology. Individual stories were collected and processed using the method of core story creation and emplotment (Emden 1998). Four newly registered nurses were invited to share stories related to how they were experiencing their role. Participants were encouraged to tell their stories in response to the open question 'what is it like to be a registered nurse?' In the final step of the analysis one honest and critical story has been crafted (Barone 1992) using a process termed emplotment thus disclosing the themes that allow the stories to be grasped together as a single story (Polkinghorne 1988, Emden 1998). The final story of 'Fable' gives insight into the ways in which newly registered nurses experience their role. Becoming a registered nurse is not easy however, Fable finds that nursing is more than just a job and describes many rewarding experiences. It is hoped that the outcomes of this research will be valuable to students, graduates, nurse academics and the profession of nursing generally by enhancing understandings of the relationship between the graduate and the actual employment experience.

Perceiving and Coping with Exclusion: The Socialization Experiences of Ethnic Minority Nursing Students

This study focused on the experiences of ethnic minority nursing students at a predominately white institution, the Health Sciences University School of Nursing in an attempt to learn more about the stress, appraisal, and coping of this group of individuals. The University School of Nursing was selected as a comparison site as it offered a setting with no predominate ethnic group. Faculty•s perceptions of students stress, appraisal and coping were sought to provide a context for the students• experience. A review of the literature indicated that ethnic minority students in predominately white universities experience alienation. At some universities white and ethnic minority students and faculty differ in their perceptions of what should be offered as support to ethnic minority students. Studying the experiences of students at a health care science university, dedicated to the health and care of individuals provided useful insights. Of particular importance was the investigation of what constituted problematic and nonconstructive relationships and structures. Symbolic interactionism, socialization theory, stress, appraisal and coping theory and attribution theory offered sensitizing concepts from which 23 ethnic minority nursing student and 12 nursing faculty interviews were taped, and analyzed. A constant comparative method of qualitative analysis proposed by Glaser and Strauss offered a systematic approach in developing substantive concepts. Common to most nursing students was the phenomenon of making it in nursing school. Making it was characterized by two main categories, being out of control and gaining control. Being out-of-control was understood as the stress producing threats of conflicting demands of family, work and school and being sanctioned, the evaluation and approval by faculty of ones• professional performance. Gaining control included managing multiple demands, reaffirming ones• choice of nursing and disengaging from the student role to becoming a nurse. A set of experiences unique to ethnic minority students was identified when some aspect of ethnicity was central to the problematic experience. A core phenomenon of exclusion as a threat to identity emerged. Three forms of exclusion were identified: (1) linguistic difficulty; (2) interpersonal disregard; (3) ethno-cultural incongruity. Students 1 acceptance of responsibility for the problematic situation influenced their coping strategies. Holding back, keeping silent, disengaging and giving up were the usual coping responses. Only when the stakes were high, for example passing a course, would students speak out, negotiate or confront to in order to preserve their academic progression. Faculty accurately identified students 1 stresses as: staggering under the load, building a professional identity, experiencing isolation and facing cultural unresponsiveness. Faculty misidentified some of students 1 withdrawal coping behaviors as a cultural norm of being quiet or reserved. In addition, faculty offered descriptions of their own stress in teaching ethnic minority students with English as a second language such as trying to decide when to bend over backwards to help the students and when to draw the line. The most important conclusion reached was that ethnic minority students experienced a set of stressors linked to their perceptions of their ethnic status. A major stressor was exclusion, in that it interfered with the core task of becoming socialized as a nurse. Weak social bonds within the school of nursing and to the profession can hamper the recruitment of others from a particular ethnic group to the school and ultimately into the health care profession. A focus on the interpretation of interpersonal events in health care settings is crucial in surfacing the cultural nuances of understanding and meaning. Recommendations were made to: (1) develop an enriched grounded theory and promote mutual understanding through faculty, nursing staff and student group interviews and (2) increase the comprehensiveness of ethnic minority student retention data bases.

Telefonen har under de senaste årtiondena blivit ett viktigt redskap i hälso- och sjukvårdens arbete med människor som söker hjälp och råd för medicinska problem. Telefonlinjer till Sveriges vårdcentraler inrättades allmänt på 1970-talet. En betydande del av sjuksköterskornas arbete på vårdcentral består av telefonrådgivning. Den vårdsökande prioriteras och hänvisas till rätt vårdnivå utifrån den bedömning som görs vid dessa samtal. Syftet med studien är att få kunskap om vad sjuksköterskorna upplever när den vårdsökande inte kan beredas tid trots ett medicinskt behov, då läkartiderna inte räcker till på grund av brist på allmänläkare. Tio sjuksköterskor som arbetar med telefonrådgivning på vårdcentral intervjuades. Studien utfördes med en kvalitativ metod med fenomenologisk ansats. I resultatet framkommer att informanterna har flera olika känslomässiga upplevelser. Informanterna upplever att de inte alltid kan uppfylla det vårdgarantin lovar, men att problemet är organisationsrelaterat och ligger utanför deras egen kontroll. Telefonarbetet kan kännas betungande och tanken på hur det går för den vårdsökande finns ibland kvar efter arbetsdagens slut. Erfarenhet och kunskap anses viktigt för att bättre kunna hantera situationen och göra rätt bedömning. Det kollegiala stödet framhålls som betydelsefullt. Bakgrund: Vårdrelationen är ett fackbegrepp som används för att betona patientperspektivet i vårdvetenskapen. Sjuksköterskan har en hjälpande roll och är delaktig i skapandet av vårdrelationen. Patienten bör uppfatta sjuksköterskan som genuint vårdande med relevanta färdigheter samt ha en professionell klokhet. Vårdrelationen förutsätter en respekt för patientens livsvärld och en öppenhet för patienten som levd och subjektiv varelse. Det är i mötet människor bidrar till varandras livshistoria och förståelsen av sig själva och världen de lever i. Vårdrelationen är till sin natur asymmetrisk och sjuksköterskan bör ha vårdandets etiska värdegrund samt idén om värnandet av patientens värdighet som utgångspunkt. Syfte: Syftet var att undersöka psykiatrisjuksköterskans erfarenheter av vårdrelationen. Metod: En litteraturstudie valdes som metod och data analyserades med kvalitativ innehållsanalys. Artikelsökningen genomfördes i databaserna Cinahl och PubMed. Resultatet baserades på åtta artiklar med kvalitativ ansats. Resultat: Ur resultatet framkom två teman och fem subteman. Teman var Personliga förutsättningar samt I praktiken med tillhörande subteman tillit, tidsaspekter, allians samt i mötet och utförande. Diskussion: Diskussionen delas upp i metod samt resultatdiskussion. I metoddiskussionen diskuteras styrkor och svagheter med metoden. I resultatdiskussionen diskuteras resultatet utifrån Hildegaard Peplaus teori samt nya vetenskapliga artiklar. / Background: The nurse-patient relationship is a professional term used to emphasize the patient perspective in health care science. The nurse has a helping role and is involved in creating the relationship. The patient should perceive the nurse as genuinely caring with relevant skills as well as having a professional wisdom. The relationship requires a respect for the patient's lifeworld and openness for the patient as a lived and subjective being. It is in the meeting that people contribute to each other's life history and the understanding of themselves and the world in which they live. The relationship is by nature asymmetrical and the nurse should have the ethical value of care as well as the idea of safeguarding the patient's dignity as a starting point. Aim: The aim was to investigate the psychiatric nurse's experiences of the nurse-patient relationship. Method: A literature study with qualitative content analysis has been applied. The article search was conducted in the databases Cinahl and PubMed. The result is based on eight articles with a qualitative approach. Results: The result shows two themes and five sub themes. Personal prerequisites and In practice with subthemes trust, time aspects, alliance as well as in the meeting and execution. Discussion: The discussion is divided into method and result discussion. In the method discussion, strengths and weaknesses are discussed with the method. In the results discussion, the results are discussed based on Hildegaard Peplau's theory and new scientific articles.

Examining Predictors of Attitudes and Knowledge of Registered Nurses and Nursing Students in Tennessee toward Pregnant and Perinatal Women with a Substance Use Disorder

Substance use disorders (SUDs) among pregnant and perinatal women continue to be a national public health crisis. Furthermore, nursing students and perinatal nurses have historically negative and punitive attitudes toward this vulnerable population of women. As nurses are primary care providers for pregnant and perinatal women, this is troublesome as perinatal patients express feeling stigmatized by nurses whom they should be able to trust. This contributes to the reluctance of women to seek needed medical and prenatal care. Tennessee was the first state to criminalize drug use in pregnancy and has higher neonatal abstinence syndrome (NAS) rates, which were more than 2 times the national average in 2017. The purpose of this descriptive cross-sectional non-experimental study was to examine how formal SUD nursing education, personal experiences, and participant characteristics predict attitudes and knowledge of nursing students and practicing perinatal nurses in Tennessee toward pregnant and perinatal women with an SUD. The sample consisted of 262 nursing students and 99 perinatal nurses across the west, middle, and eastern regions of Tennessee. A linear multiple regression showed that having a personal experience with a close friend with an SUD was predictive of improved knowledge scores of pregnant and perinatal SUDs. Independent samples t-tests were non-significant between formal SUD nursing education and attitudes and knowledge. Additionally, non-significant findings were seen between having a personal experience with a family member with an SUD and attitudes and knowledge. The findings suggest that Tennessee nursing education efforts were not influential in positively affecting attitudes and knowledge scores toward pregnant and perinatal women with an SUD. Future studies focused on exploring various educational interventions to promote knowledge, improve attitudes, and empathy in nursing populations toward pregnant and perinatal women with an SUD are warranted.
