The role of food gardens in mitigating the vulnerability to HIV-AIDS of rural women in Limpopo, South Africa

The purpose of this study was to investigate the role of food gardens in influencing the vulnerability of women in rural communities to HIV-infections. The study was conducted at the Makotse Women's Club in rural Limpopo. Qualitative field research was used and five participants were purposefully selected for participation in face-to-face interviews. The study found that the women's involvement in the food gardens enabled them to take responsibility for their own lives. Participation in food gardens gave these women an opportunity for income generation, the chance to access healthy food sources to improve their and their families' diets and a sense of meaning and purpose in their lives. Financial independence from husbands and male partners freed them from poverty and male domination. Through exposure to HIV and AIDS education programmes offered at the food gardens the women were empowered with knowledge about sexual health, hence reducing their vulnerability to HIV-AIDS. / Sociology / M. Diet and exercise intervention adherence and health-related outcomes among older long-term breast, prostate, and colorectal cancer survivors

Indiana University-Purdue University Indianapolis (IUPUI) / Given the numerous benefits of a healthy diet and exercise for cancer survivors, there has been an increase in the number of lifestyle intervention trials for this population in recent years. However, the extent to which adherence to a diet and exercise intervention predicts health-related outcomes among cancer survivors is currently unknown. To address this question, data from the Reach out to ENhancE Wellness in Older Cancer Survivors (RENEW) diet and exercise intervention trial were analyzed. RENEW was a yearlong telephone and mailed print intervention for 641 older (>65 years of age), overweight (body mass index: 25.0-39.9), long-term (>5 years post-diagnosis) survivors of colorectal, breast, and prostate cancer. Participants were randomized to the diet and exercise intervention or a delayed-intervention control condition. The RENEW telephone counseling sessions were based on determinants of behavior derived from Social Cognitive Theory (SCT) (e.g., building social support, enhancing self-efficacy). These factors have been hypothesized to improve health behaviors, which in turn should improve health outcomes. Thus, drawing on SCT and prior diet and exercise research with cancer survivors, I hypothesized that telephone counseling session attendance would be indirectly related to health-related outcomes (i.e., physical function, basic and advanced lower extremity function, mental health, and body mass index) through intervention-period strength and endurance exercise and dietary behavior (i.e., fruit and vegetable intake, saturated fat intake). The proposed model showed good fit to the data; however, not all of the hypothesized relationships were supported. Specifically, increased telephone counseling session attendance was related to engagement in all of the health behaviors over the intervention period. In turn, (a) increased endurance exercise was related to improvement in all of the health-related outcomes with the exception of mental health; (b) increased strength exercise was solely related to improved mental health; (c) increased fruit and vegetable intake was only related to improved basic lower extremity function; and (d) saturated fat intake was not related to any of the health-related outcomes. Taken together, these findings suggest that SCT determinants of behavior and the importance of session attendance should continue to be emphasized in diet and exercise interventions. Continued exploration of the relationship between adherence to a diet and exercise intervention and health-related outcomes will inform the development of more cost-effective and efficacious interventions for cancer and other medical populations.

Dietary intake and urinary excretion of phytoestrogens in relation to cancer and cardiovascular disease

Indiana University-Purdue University Indianapolis (IUPUI) / Phytoestrogens that abound in soy products, legumes, and chickpeas can induce biologic responses in animals and humans due to structural similarity to 17β-estradiol. Although experimental studies suggest that phytoestrogen intake may alter the risk of cancer and cardiovascular disease, few epidemiologic studies have investigated this research question. This dissertation investigated the associations of intake of total and individual phytoestrogens and their urinary biomarkers with these chronic conditions using data previously collected from two US national cohort studies (NHANES and PLCO). Utilizing NHANES data with urinary phytoestrogen concentrations and follow-up mortality, Cox proportional hazards regression (HR; 95% CI) were performed to evaluate the association between total cancer, cardiovascular disease, and all-cause mortality and urinary phytoestrogens. After adjustment for confounders, it was found that higher concentrations of lignans were associated with a reduced risk of death from cardiovascular disease (0.48; 0.24-0.97), whereas higher concentrations of isoflavones (2.14; 1.03-4.47) and daidzein (2.05; 1.02-4.11) were associated with an increased risk. A reduction in all-cause mortality was observed for elevated concentrations of lignans (0.65; 0.43-0.96) and enterolactone (0.65; 0.44-0.97). Utilizing PLCO data and dietary phytoestrogens, Cox proportional hazards regression examined the associations between dietary phytoestrogens and the risk of prostate cancer incidence. After adjustment for confounders, a positive association was found between dietary intake of isoflavones (1.58; 1.11-2.24), genistein (1.42; 1.02-1.98), daidzein (1.62; 1.13-2.32), and glycitein (1.53; 1.09-2.15) and the risk of advanced prostate cancer. Conversely, an inverse association existed between dietary intake of genistein and the risk of non-advanced prostate cancer (0.88; 0.78-0.99) and total prostate cancer (0.90; 0.81-1.00). C-reactive protein (CRP) concentration levels rise in response to inflammation and higher levels are a risk factor for some cancers and cardiovascular disease reported in epidemiologic studies. Logistic regression performed on NHANES data evaluated the association between CRP and urinary phytoestrogen concentrations. Higher concentrations of total and individual phytoestrogens were associated with lower concentrations of CRP. In summary, dietary intake of some phytoestrogens significantly modulates prostate cancer risk and cardiovascular disease mortality. It is possible that these associations may be in part mediated through the influence of phytoestrogen intake on circulating levels of C-reactive protein.

Cultural practices and diet adherence of patients living on haemodialysis

These patients must change their diet when receiving dialysis treatment, but cultural beliefs and practices can affect their adherence to the prescribed diet. The purpose of this qualitative, descriptive, exploratory study was to improve health education on prescribed diet adherence to patients living on haemodialysis at a haemodialysis unit in eThekwini Municipality. Data from a sample of 20 patients was collected using semi-structured interviews and analysed using qualitative content analysis. The findings revealed that haemodialysis patients’ prescribed diet adherence was influenced by cultural and religious views, and by family support. Food availability, patients’ geographical location and patients’ financial means hindered their adherence to their prescribed diet. Ecological Influences on Weight Status in Urban African-American Adolescent Females: A Structural Equation Analysis

The present study employed a quantitative, non-experimental, multivariate correlational research design to test a hypothesized model examining associative paths of influence between ecological factors and weight status of urban, African-American adolescent females. Anthropometric and self-report survey data of 182 urban, African- American adolescent females were collected during after-school programs, health and physical education classes, and community events in an urban area in northeast Florida. Descriptive analyses were conducted to characterize the study participants based upon their age, study setting, and weight status. A scale reliability analysis was conducted to assess the internal consistency reliability of the sample data using selected measures within the context of the study’s specific population and subsequently guided the structural equation model (SEM) analyses. The SEM path analysis was used to develop two measurement models to control for observed error variance for variables demonstrating poor internal consistency reliability (diet behaviors and nutrition selfefficacy) and a final structural model to test the associative paths of influence between latent (diet behaviors and nutrition self-efficacy) and manifest variables (teacher social support and friend social support) on weight status. The results of the path analysis indicated that both teacher social support and friend social support demonstrated a positive, indirect influence on child weight status through nutrition self-efficacy and diet behaviors following two different and specific paths of influence. Diet behaviors, in turn, demonstrated a positive, direct effect on child weight status. These findings provide clear implications for educational leaders that call for the integration of health behavior change theory into traditional education and leadership practice and actively addressing the childhood obesity epidemic in the school environment by implementing health behavior change strategies at various ecological environmental levels.

The association between levels of fish consumption early in pregnancy and birth outcomes of pregnant women in Johannesburg, South Africa

Background: Neonates born with low birth weight or preterm are at an increased risk of long-term adverse health outcomes. Research studies on the association of fish consumption during pregnancy and birth outcomes, have led to inconsistent conclusions. Maternal dietary intakes during pregnancy have a significant impact on foetal development and growth. The aim of this project is to determine levels of maternal fish intake at <18 weeks during pregnancy and to determine the association with birth outcomes in pregnant women from Johannesburg, South Africa. Methods: This Master‘s study is nested in a larger study with a longitudinal observational research design was conducted on 250 pregnant women in Johannesburg, South Africa. For this Master‘s study, data from the first 102 participants were used. Data for this study were collected early in pregnancy (<18 week‘s gestation) and at birth. The birth data were collected by the study mid-wife. Maternal fish consumption during early pregnancy was measured using a Quantitative Food Frequency Questionnaire (QFFQ). Correlation analysis was used to examine the association between maternal fish consumption during early pregnancy and neonatal anthropometry (birth weight, crown heel length and head circumference) and gestational age at birth. Results: Majority (88.1%) of the mothers were black South Africans between the ages of 18 and 39 with a mean age of 28 ± 5 years. At enrolment, the mean BMI of the women was 27.8±5.8kg/m2 having a mean height of 158.8±6.7cm and a mean weight of 70.4±15.2kg. Most of them were unmarried (45.4%), living in households of 2 – 5 members (86.3%), wage-earning (44.6%) and had Grade 11 or 12 schooling (58.4%). Most (76.5%) of the pregnant women consumed fish rarely (once a month) and the overall median fish intake was 4.8g/day (0; 25). In the study sample 12.5% of new-borns had a low birth weight (<2500g), the percentages of preterm births were 1.0% - extremely preterm (<28 weeks), 2.0% - very preterm (28 – <32 weeks) and 10.0% - moderate to late preterm (32 – 37 weeks). The mean birth weight was 2999.2±624.4g with boys having a mean birth weight of 3157.3±571g and girls at 2819±671g. The new-borns‘ mean gestational age at birth was 38.8±2.4weeks (271.6days). The percentage of new-born head circumference ≤ 31.49cm was 9.2% and the mean head circumference was 34.3±3.6cm with the boys having a mean head circumference of 34.5±2.4cm and the girls 34.1±4.3cm. In this study sample, 3.7% of new-borns were born with crown heel length of 31 – 40cm and the mean crown heel length mean was 49.5±4.6cm with the boys having a mean crown heel length of 49.8±4.9cm and the girls having mean crown heel length of 49.3±4.3cm. In this study, there were no statistically significant associations between fish consumption at early pregnancy and birth outcomes such as gestational age at birth (r=0.051; p=0.625), birth weight (r=-0.043; p=0.695) and crown heel length (r=0.008; p=0.943). There was a positive association between maternal fish consumption in early pregnancy and head circumference of the new-born which tended towards statistical significance (r=0.193; p=0.079). Conclusions: In this study of pregnant women living in Johannesburg, a few women consumed fish at early pregnancy compared with women who did not consume fish during pregnancy. The interrelationship of food, culture, and diabetes among Mexican American women

Diabetes and related complications are one of the leading causes of death for Mexican American women. Although dietary behavior is a critical component of diabetes management, research is lacking in relation to the interplay of food habits, culture, and diabetes among this population. The specific aims of this ethnographic study were to: (1) explore the relationships between culture, food habits, and type 2 diabetes as experienced by low income Mexican American women; (2) examine the role of food in the cultural identity of low-income Mexican American women; (3) examine how trying to adhere to the ADA dietary guidelines for people with diabetes impacts the relationship between food and culture of low income Mexican American women with type 2 diabetes. This sample consisted of 16 Mexican American women between the ages of 39 to 60 years. Twelve of the sixteen had experienced diabetes for at least 10 years, and were considered experts in the management of their illness. The remaining four had experienced diabetes for a shorter period of time and were considered novices. Individual interviews were conducted with each informant. All interviews were audiotaped with the exception of one. The interviews, observations, and field notes were analyzed for data. The analysis of data rendered 6 themes: (1) “la dieta,” (2) the location and fluidity of food (3) confidence-defiance self-management connection, (4) negotiating sociocultural and biomedical expectations, (5) eating for diabetes is a family affair, and (6) strategies for self-care. In addition, preliminary comparisons were conducted between experienced and more novice individuals with diabetes. / text

An evaluation of food parcel interventions by Societas Socialis (SOS) Children's Villages in the context of HIV and AIDS : a case study of Ennerdale informal settlement in Johannesburg, South Africa

This study investigated and evaluated the success and impact of the foodstuff package intervention that is offered by SOS Children’s Villages to households affected by HIV and AIDS. The study population comprised households who were involved in the food parcel distribution intervention, those affected by HIV and AIDS, those individuals who were able to talk freely about their lives as full participants and recipients of food parcels, and those aged 18 years and older. This study determined how food parcel intervention promotes self-sufficiency post-intervention. This was determined by interviewing recipients of food parcels from 2010 to 2018. The food parcel dissemination has been a core intervention to families affected by HIV and AIDS, but yielded very little progress towards assisting people to reach self-reliance. The aim of the study was to gain an understanding of the food parcel intervention using a qualitative research design. This study focused only on families affected by HIV and AIDS, including young people aged eighteen years and above. The study was based on a purposive sampling method with in-depth and focus group interviews as a means of collecting data. The results and implications of this study are discussed in depth in this research report. Essentially, the findings of this study could inform the development of policies and strategies to be considered for possible implementation by NGO’s and government in supporting family units affected by HIV and AIDS. / Sociology / M.A. (Social and Behavioural Studies in HIV and AIDS)
