Avaliação do papel da matriz extracelular na musculatura da parede lateral da faringe na fisiopatologia da síndrome da apnéia obstrutiva do sono / Extracellular matrix of the lateral pharyngeal wall in obstructive sleep apnea syndrome

A Parede lateral da faringe parece ser a estrutura central envolvida no colapso da faringe nos pacientes com síndrome da apnéia obstrutiva do sono (SAOS). Os pacientes com roncos e apnéia têm uma faringe mais colapsável associada a um espessamento da musculatura da parede lateral da faringe, em grande parte formada pelo músculo constritor superior da faringe. O endomísio da matriz extracelular do músculo esquelético tem uma íntima relação com as células musculares, influenciando suas propriedades mecânicas e biológicas, podendo modificar seu comportamento e função. As fibras colágenas e elásticas formam o arcabouço do tecido conectivo, enquanto os proteoglicanos e as glicoproteínas estruturais (fibronectina) têm importante papel na propriedade de adesão e hidratação dos tecidos e as metaloproteínas são responsáveis pela degradação dos componentes da matriz extracelular. Os fatores determinantes da complacência excessiva da musculatura da parede lateral são desconhecidos. É possível que a MEC tenha um papel relevante neste aspecto. O objetivo deste estudo é descrever e comparar a densidade dos componentes da matriz extracelular do endomísio da musculatura da parede lateral da faringe em controles, roncadores e apnéicos. Neste estudo prospectivo foram avaliados 61 pacientes maiores de 18 anos e não obesos do Departamento de Otorrinolaringologia do Hospital das Clínicas da Faculdade de medicina da Universidade de São Paulo, com indicação de cirurgias faríngeas no período de 2005 e 2006. Os pacientes foram divididos em três grupos: controles (14 pacientes sem roncos ou SAOS), 18 roncadores primários e 29 pacientes com SAOS. Os espécimes foram obtidos da musculatura da parede lateral da faringe (constritor superior da faringe) durante cirurgias faríngeas. Por meio de análise histoquímica e imunohistoquímica foi determinada a área proprocional do colágeno total, fibras elásticas, colágeno tipos I e III, metaloproteínas 1 e 2, versican e fibronectina, no endomísio destas amostras e comparada entre os grupos. Os resultados foram ainda correlacionados com dados antropométricos. O colágeno tipo I foi o componente mais abundante da matriz extracelular no constritor superior da faringe. Observamos uma correlação positiva entre o colágeno I e a idade (r=0,42, p=0,01) e inversa com o colágeno III (r=-0,28, p=0.027). O índice de massa corpórea também mostrou uma correlação inversa com o colágeno III (r=-0,311, p=0,017). Não houve diferenças estatisticamente significantes para os componentes da MEC entre os grupos. Em resume, a análise histológica da musculatura da parede lateral da faringe de um largo grupo de pacientes com distúrbios do sono, revelaram que a composição da matriz extracelular é relacionada à idade e o peso, e não pode ser envolvida nos mecanismos que levam a colapsibilidade e espessamento da parede lateral da faringe. / The lateral muscular pharyngeal wall appears to be the predominant structure involved in the collapse of the pharynx in obstructive sleep apnea (OSA). OSA patients and snorers have a narrower pharynx which is more susceptible to collapse and is associated with muscle thickening in the lateral pharyngeal wall, largely formed by superior pharyngeal constrictor. The endomysial extracellular matrix of skeletal muscle has an intimate relationship with the muscle cells and has an important influence on its mechanical and biological properties and can modify their behavior and function. Collagen and elastic fibers form the scaffolding of the connective tissue and proteoglycans and structural glycoproteins (fibronectin) have important roles in hydration, resiliency and adhesive properties of the tissues and the matrix metalloproteinases are involved in extracellular matrix components degradation. The determining factors in the compliance of lateral muscular pharyngeal wall are also unknown. It is possible that the ECM has a relevant role in this aspect. Our objective was to determine and compare the density of the components of the extracellular matrix in the deep, structural lateral pharyngeal muscular walls in snorers, OSA and control patients and correlate with age and weight. In this prospective study we evaluated 61 patients older than 18 and nonobese with indications for pharyngeal surgery at the department of Otolaryngology of the Hospital das Clínicas of the São Paulo University, between 2005 and 2006. The patients were divided in three groups: controls (14 patients), 18 primary snorers and 29 OSA patients. We obtained specimens from the lateral pharyngeal muscular wall (superior constrictor muscle) during pharyngeal surgeries. Using histochemical and immunohistochemical analyses, we determined the fractional area of total collagen, elastic fibers, collagen I and III, matrix metalloproteinase 1 and 2, versican and fibronectin in the endomysium of these samples, and compared among groups. Results were further correlated with anthropometric data. Collagen type I was the most abundant component of the ECM within the pharyngeal muscle. There was a positive correlation between collagen I and age (r=0.419, p=0.01) and an inverse one with type III collagen (r=-0.284, p=0.027). Body mass index inversely correlated with collagen type III (r=-0,311, p=0,017). There were no statistically significant differences for any of the components of the ECM among groups. In summary, histological analysis of the lateral muscular pharyngeal wall of a large group of patients with sleep disturbances revealed that its extracellular matrix composition is age and weight related but may not be involved in the mechanisms leading to the collapsibility of the lateral wall of the pharynx.

Um dos tipos de câncer mais frequentes no mundo é o carcinoma epidermoide de cabeça e pescoço. A dificuldade de resposta eficiente aos tratamentos está relacionada à complexidade molecular deste tumor. MicroRNAs regulam uma parte significativa dos genes humanos, dentre os quais genes responsáveis por controlar proliferação e apoptose, caracterizando seu papel na progressão tumoral e resistência a quimioterápicos. O entendimento sobre como moléculas com ação antineoplásica atuam sobre a regulação de microRNAs pode contribuir para a compreensão de resultados terapêuticos. Neste estudo avaliamos o efeito dos triterpenos quinonametídeos maitenina e 22-β-hidroximaitenina sobre a viabilidade celular e sobre a regulação de microRNAs associados com proliferação e apoptose. O efeito destas moléculas foi avaliado frente a linhagens tumorais e em queratinócitos orais livres de tumor. Os resultados indicam que os dois triterpenos avaliados reduzem eficazmente a viabilidade das células cancerígenas e a regulação de microRNAs faz parte dos mecanismos envolvidos neste efeito. / Head and neck squamous cell carcinoma is one of the most frequent cancer types in the world. The difficulty of effective response to treatment is related to the molecular complexity of this tumor. MicroRNAs, small non-coding RNA molecules, regulate a significant proportion of human genes, among which genes involved in cell growth, apoptosis and response to chemotherapy. The comprehension on how anticancer molecules interfere with microRNA expression should contribute to the understanding of treatment outcome. This study evaluated the effect of two quinonamethide triterpenoids, maytenin and 22-β-hidroxymaytenin, on cell viability and on the regulation of microRNAs associated with proliferation and apoptosis. The effect of these molecules was evaluated in tumor cells and oral keratinocytes. The results show that the two triterpenoids effectively reduced cell viability of cancer-derived cell lines and that microRNA regulation is one of the mechanisms involved in this effect.

Associação entre a periodontite crônica e o câncer de boca e/ou orofaringe / Association between chronic periodontitis and oral cavity and/or oropharynx cancer

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / O objetivo desse trabalho foi determinar a prevalência, extensão e severidade da periodontite crônica em pacientes com câncer de boca e/ou orofaringe e verificar se essa forma de doença periodontal é indicadora de risco para essas neoplasias. Trinta e cinco pacientes com câncer de boca e/ou orofaringe, atendidos no Instituto Nacional de Câncer (INCA) e 40 pacientes e/ou acompanhantes do ambulatório do Instituto Nacional de Traumatologia e Ortopedia (INTO), não portadores de neoplasias de boca e/ou orofaringe, foram avaliados por meio de anamnese, exame periodontal e índice de dentes cariados perdidos e obturados (CPOD). A população estudada foi formada predominante por indivíduos do sexo masculino, da raça branca, casados, que cursaram até o ensino fundamental e possuíam renda familiar de até 6 salários mínimos. Não houve diferença estatística significante para esses parâmetros entre os grupos. No grupo caso havia significativamente mais pacientes fumantes e que ingeriam mais bebidas alcoólicas. Pacientes do grupo controle praticavam mais exercícios físicos e tinham o índice de massa corporal (IMC) significantemente mais elevado. O consumo de frutas, legumes e vegetais foi maior para o grupo controle (p>0,05), enquanto o consumo de carne vermelha foi maior no grupo caso (p>0,05). O histórico de casos de câncer na família era baixo em ambos os grupos. O grupo caso apresentou mais sítios com placa e sangramento a sondagem e menos sítios com sangramento gengival. Somente o índice de placa teve diferença estatisticamente significante. O número de dentes presentes era significantemente menor no grupo caso embora não houvesse diferença estatística significante entre os grupos para o índice CPOD. As médias de profundidade de bolsa a sondagem (PBS) e nível de inserção clínica (NIC) foram significantemente maiores para o grupo caso. A prevalência da periodontite crônica generalizada foi de 100% nos pacientes com câncer de boca e ou orofaringe. Oitenta e nove por cento dos pacientes do grupo caso apresentaram periodontite crônica severa. A extensão ou a severidade da periodontite crônica permaneceram indicadores de risco para o câncer de boca e/ou orofaringe independentes mesmo após os ajustes para fatores de confusão, álcool e fumo. / The aim of this study was to determine the prevalence, extension and severity of chronic periodontitis in patients with oral cavity and/or oropharynx cancer, as well as to verify whether this form of periodontal disease is a risk indicator for these neoplasms. Thirty five patients with oral cavity and/or oropharynx cancer from the National Cancer Institute (INCA) Hospital as well as 40 outpatients and/or their companions from the National Institute of Traumatology (INTO) were evaluated by means of anamnesis, periodontal examination and decayed, extracted and filled teeth index (DMFT). The predominant individual of the population in study was male, white, married, had completed grammar school and had a family income of up to 6 times the Brazilian minimum wage. There were no significant statistical differences for these parameters between the case and control groups. The case group had significantly more patients who smoked regularly and their individuals ingested more alcoholic beverages. Patients of the control group practiced more physical activities and had a significantly higher body mass index (BMI). The consumption of fruits and vegetables was higher in the control group (p>0,05) while red meat intake was higher in the case group (p>0,05). The family history for cancer was low in both groups. The case group presented more sites with plaque and bleeding on probing but less sites with gingival bleeding. Only the difference in the plaque index was statistically significant. The number of teeth present in the mouth was significantly lower in the case group although there was no statistically significant difference in the DMTF index between the groups. The mean probing pocket depth and clinical attachment level were significantly higher for the case group. The prevalence of chronic generalized periodontitis was 100% in patients with oral cavity and/or oropharynx cancer. Eighty nine percent of the patients in the case group presented chronic severe periodontitis. The extension and severity of chronic periodontitis remained as a risk indicator for oral cavity and/or oropharynx cancer even after the adjustments for confusion factors, i.e. smoke and alcohol.

Avanços tecnológicos, como a tomografia computadorizada de feixe cônico (TCFC), têm sido utilizados com o propósito de se observar tridimensionalmente, o crânio, a face e o espaço aéreo faríngeo, sendo possível a avaliação das mudanças do espaço aéreo induzidas pelos aparelhos ortopédicos propulsores mandibular. O intuito deste estudo é apresentar um método de avaliação das vias aéreas de fácil reprodutibilidade, e avaliar, mediante a TCFC, os efeitos do aparelho de Herbst bandado no espaço aéreo orofaríngeo (EAOF), nasofaríngeo (EANF) e faríngeo superior (EAFS), em indivíduos com má oclusão classe II divisão 1ª. A amostra foi composta por imagens digitais de 15 indivíduos (idade média inicial de 15,7 anos ± 1.7, após surto de crescimento pubertário) com má oclusão classe II divisão 1ª, tratados com aparelho Herbst bandado (8 meses). Os exames tomográficos foram realizados antes (T1) e após (T2) o tratamento. A fase inicial foi realizada com a orientação do nos planos; axial, coronal e sagital. Foram mensurados: comprimento no software Dolphin 3D ®: comprimento, maior área no corte coronal, maior área no corte sagital, área mínima da secção transversal e volume, de EANF, EAOR e EAFS (orofaringe e nasofaringe). Dois avaliadores previamente calibrados mensuraram duas vezes apenas um tempo das tomografias, para observar a reprodutibilidade (inter-examinador) e replicabilidade (intra-examinador). Sendo confirmada a reprodutibilidade do método, foi avaliado o efeito do aparelho Herbst, analisando os períodos pré (T1) e pós (T2) tratamento ortopédico. Technological advances, such as cone beam computed tomography (CBCT), have been used in order to observe three-dimensionally: the skull, face and pharyngeal airway space, with possible assessment of airspace changes induced by orthopedic activators. The purposes of this study are to present a method for assessing airway that is easily reproducted, and evaluate through the CBCT the effects of banded Herbst appliance in the oropharyngeal airspace (EAOF), nasopharyngeal (EANF) and upper pharynx (EAFS) in individuals with malocclusion class II division 1. The sample consisted of digital images of 15 individuals (initial age of 15.7 years ± 1.7, after pubertal growth spurt) with malocclusion class II division 1, treated with banded Herbst appliance (8 months). CBCT scans were performed before (T1) and after (T2) treatment. The initial phase was conducted with the guidance of planes: axial, coronal and sagittal. The following were measured: the largest area in the coronal and sagittal, minimum cross-sectional area and volume of EANF, and EAOR EAFS (oropharynx and nasopharynx) by length in Dolphin 3D® software. Two calibrated evaluator mesured twice time of CBCT to observe the reproducibility (inter-examiner) and repeatability (intra-examiner). Once the reproductability of the method was coinfirmed, the effect of the Herbst applicance was evaluated before (T1) and after (T2) the orthodontic treatment. The statistical analysis was performed in two steps. Head and neck squamous cell carcinomas (HNSCC) remain a significant cause of morbidity worldwide, with approximately 550,000 new cases diagnosed each year. The main etiological factors include smoking and alcohol consumption. The incidence of non-oropharyngeal HNSCC is gradually decreasing while the incidence of squamous cell oropharyngeal carcinoma (OPSCC) is still on the rise. This increasing incidence can be most likely attributed to an increasing prevalence of human papillomavirus (HPV) infection. From the clinical point of view the most significant fact is that patients with HPV positive OPSCC have better prognosis. HNSCC is linked to an alteration in the immune system. Only a limited number of studies have correlated both the immunological parameters and HPV status with patient prognosis. Therefore, we focused on the research of the immunological profile of patients with HNSCC of viral and non-viral etiology. In our study, 110 patients with HNSCC were enrolled. They were divided into HPV-positive and HPV-negative groups based on the expression of HPV 16 E6 mRNA detected in the tumor tissue. Basic lymphocyte subpopulations (CD3+, CD4+ CD25+ Treg, CD4+ CD25+ FoxP3 Treg, CD4+, CD8+, CD19, and CD3- CD16+ CD56+ cells) were determined by flow cytometry in the peripheral blood (PB). OSA patients and snorers have a narrower pharynx which is more susceptible to collapse and is associated with muscle thickening in the lateral pharyngeal wall, largely formed by superior pharyngeal constrictor. The endomysial extracellular matrix of skeletal muscle has an intimate relationship with the muscle cells and has an important influence on its mechanical and biological properties and can modify their behavior and function. Collagen and elastic fibers form the scaffolding of the connective tissue and proteoglycans and structural glycoproteins (fibronectin) have important roles in hydration, resiliency and adhesive properties of the tissues and the matrix metalloproteinases are involved in extracellular matrix components degradation. The determining factors in the compliance of lateral muscular pharyngeal wall are also unknown. It is possible that the ECM has a relevant role in this aspect. Our objective was to determine and compare the density of the components of the extracellular matrix in the deep, structural lateral pharyngeal muscular walls in snorers, OSA and control patients and correlate with age and weight. In this prospective study we evaluated 61 patients older than 18 and nonobese with indications for pharyngeal surgery at the department of Otolaryngology of the Hospital das Clínicas of the São Paulo University, between 2005 and 2006. The patients were divided in three groups: controls (14 patients), 18 primary snorers and 29 OSA patients. We obtained specimens from the lateral pharyngeal muscular wall (superior constrictor muscle) during pharyngeal surgeries. Using histochemical and immunohistochemical analyses, we determined the fractional area of total collagen, elastic fibers, collagen I and III, matrix metalloproteinase 1 and 2, versican and fibronectin in the endomysium of these samples, and compared among groups. Results were further correlated with anthropometric data. Collagen type I was the most abundant component of the ECM within the pharyngeal muscle. There was a positive correlation between collagen I and age (r=0.419, p=0.01) and an inverse one with type III collagen (r=-0.284, p=0.027). Body mass index inversely correlated with collagen type III (r=-0,311, p=0,017). There were no statistically significant differences for any of the components of the ECM among groups. In summary, histological analysis of the lateral muscular pharyngeal wall of a large group of patients with sleep disturbances revealed that its extracellular matrix composition is age and weight related but may not be involved in the mechanisms leading to the collapsibility of the lateral wall of the pharynx.

Head and neck squamous cell carcinomas (HNSCC) remain a significant cause of morbidity worldwide, with approximately 550,000 new cases diagnosed each year. The main etiological factors include smoking and alcohol consumption. The incidence of non-oropharyngeal HNSCC is gradually decreasing while the incidence of squamous cell oropharyngeal carcinoma (OPSCC) is still on the rise. This increasing incidence can be most likely attributed to an increasing prevalence of human papillomavirus (HPV) infection. From the clinical point of view the most significant fact is that patients with HPV positive OPSCC have better prognosis. HNSCC is linked to an alteration in the immune system. Only a limited number of studies have correlated both the immunological parameters and HPV status with patient prognosis. Therefore, we focused on the research of the immunological profile of patients with HNSCC of viral and non-viral etiology. In our study, 110 patients with HNSCC were enrolled. They were divided into HPV-positive and HPV-negative groups based on the expression of HPV 16 E6 mRNA detected in the tumor tissue. Die lokale Rezidiv- und Überlebensrate von Patienten mit Plattenepithelkarzinomen des Oropharynx, Hypopharynx und Larynx

Die vorliegende retrospektive Studie untersuchte die lokale Rezidiv- und Überlebensrate von Patienten mit Plattenepithelkarzinomen in Abhängigkeit vom Randstatus und Resektionsabstand der Tumorresektion sowie von der postoperativen Therapie. Dabei wurden Daten von 351 Patienten mit Plattenepithelkarzinomen des Oropharynx, Hypopharynx und Larynx ausgewertet, welche zwischen 2005 und 2009 reseziert wurden. Der gesundheitliche Zustand der Patienten wurde anschließend bis Dezember 2011 beobachtet. Im Ergebnis konnte kein Einfluss des Randstatus und des Resektionsabstandes auf die lokale Rezidivrate nachgewiesen werden. Beide Faktoren hatten jedoch einen signifikanten Einfluss auf die Letalitätsrate. Die postoperative Therapie zeigte einen positiven Einfluss auf die lokale Rezidivrate. Sie konnte aber die negativen Prognosefaktoren, die eine Indikation dieser Therapie darstellen, bezüglich der Letalitätsrate nicht ausgleichen. Weiterhin war ein positiver Einfluss der postoperativen Therapie auf das rezidivfreie Überleben (DFS-disease-free survival) sowohl uni- als auch multivariat nachweisbar. Die Gesamtüberlebensrate (OS-overall survival) wurde signifikant positiv durch einen geringeren Randstatus und einen zunehmenden Resektionsabstand (ASR Klassifikation) beeinflusst. Das krankheitsspezifische Überleben (DSS-disease-specific survival) sank bei Auftreten eines lokalen Rezidivs. Aus diesen Ergebnissen kann geschlussfolgert werden, dass eine postoperative Therapie zur Kontrolle lokaler Rezidive von hoher Bedeutung ist. Da lokale Rezidive die Überlebensrate signifikant senken, hat die postoperative Therapie indirekt einen positiven Effekt auf das Überleben. Des Weiteren zeigt sich bei Patienten mit freien Resektionsrändern die höchste Überlebensrate, wobei diese mit zunehmendem Abstand des Karzinoms vom Resektionsrand steigt.

Modèle cellulaire de carcinogenèse pour les cancers de l’oropharynx induits par le VPH

Problématique: Le virus du papillome humain (VPH) est présent dans près de 50% des cancers de l’oropharynx. Le potentiel oncogénique du VPH est encodé dans les oncoprotéines E6 et E7, qui agissent en modulant différents gènes, dont les gènes suppresseurs de tumeur p53 et pRb. Les cellules VPH positives démontrent une altération au niveau de la signalisation de la réponse aux dommages à l’ADN (RDA), un mécanisme de contrôle dans l’arrêt de la croissance des cellules ayant subit des dommages au niveau de leur ADN. Hypothèse et objectifs : Nous croyons que les défauts au niveau de la RDA des cancers VPH positifs peuvent être exploités afin de sensibiliser préférentiellement les cellules cancéreuses aux traitements de radiothérapie. Cette stratégie de recherche nécessite l’élaboration d’un modèle cellulaire de carcinogenèse isogénique pour le cancer de l’oropharynx que nous proposons de développer et de caractériser. L’étude vise à dériver des lignées isogéniques à partir de kératinocytes primaires et cellules épithéliales de l’oropharynx pour ensuite valider la carcinogenèse de notre modèle in vitro & in vivo Méthodologie : Des lignées cellulaires de kératinocytes primaires et de cellules épithéliales de l’oropharynx ont été successivement modifiées par transduction afin de présenter les mutations associées aux cancers de l’oropharynx induits par le VPH. Les cellules ont été modifiées avec des lentivirus codants pour la télomérase (hTERT), les oncogènes E6, E7 et RasV12. Afin de valider la cancérogenèse in vitro de notre modèle, des études d’invasion en matrigel et de croissance sans ancrage en agar mou ont été réalisées. Les populations cellulaires transformées ont été ensuite introduites dans des souris immunodéficientes afin d’évaluer leur tumorogénicité in vivo. Résultats : À partir des plasmides recombinés construits par méthodes de clonage traditionnelle et de recombinaison « Gateway », nous avons produit des lentivirus codants pour la télomérase humaine (hTERT), les oncogènes viraux E6 et E7 et l’oncogène Ras. Les kératinocytes primaires et cellules épithéliales de l’oropharynx ont été infectés successivement par transduction et sélectionnés. Nous avons validé l’expression de nos transgènes par méthode d’immunofluorescence, de Western Blot et de réaction de polymérisation en chaîne quantitative en temps réel (qRT-PCR). Nous avons établi trois lignées des cellules épithéliales de l’oropharynx (HNOE) à partir d’échantillons tissulaires prélevés lors d’amygdalectomie (HNOE42, HNO45, HNOE46). Les cellules transduites avec le lentivirus exprimant le promoteur fort CMV/TO de l’oncogène RasV12 ont présenté un changement morphologique compatible avec une sénescence prématurée induite par l’oncogène Ras. En exprimant des quantités plus faibles du RasV12 mutant, la lignée cellulaire HEKn hTERT-E6-E7 PGK RasV12 a réussi à échapper à la sénescence induite par l’oncogène Ras. La population cellulaire exprimant HEKn hTERT-E6-E7-PGK RasV12 a présenté un phénotype malin en culture et à l’étude d'invasion, mais n’a pas démontré de résultats positifs à l’étude de croissance sans ancrage en agar mou ni en xénogreffe en souris immunodéficientes. Conclusion : Nos résultats démontrent qu’en présence des oncogènes viraux E6 et E7, il y a un troisième mécanisme suppresseur de tumeur qui médie la sénescence induite par l’oncogène Ras. Nous avons identifié que la présence de E6 seule ne suffit pas à immortaliser les kératinocytes primaires humains (HEKn). Nous n’avons pas réussi à créer un modèle in vitro de carcinogenèse pour les cancers de l’oropharynx induits par le VPH. / Background: Human papillomavirus (HPV) is present in almost 50% of all oropharyngeal cancers. The oncogenic potential of HPV is encoded by the E6 and E7 oncoproteins, which act by modulating different genes, including tumour suppressor genes p53 and pRb. The process of inactivation of p53 and pRb is largely responsible for the genomic instability that contributes to malignant transformation of cells. HPV-positive cancer cells show an alteration in their DNA Damage Response (DDR) signalling pathway that allows them to inhibit key tumour suppressor genes and to ignore DNA damage signals. Hypothesis and objectives: We believe that these DDR defects can be exploited to preferentially sensitize cancerous cells to radiotherapy by using a defined cell culture model. We propose to characterize a defined cell culture model for HPV induced oropharyngeal cancer. Derive isogenic cell culture lines from primary skin keratinocytes and oropharyngeal epithelial cells. and to validate the carcinogenesis of our model in vitro and in vivo. Methods: We propose to use primary skin keratinocytes and oropharyngeal epithelial cells which will be sequentially modified by transduction using a Gateway Lentiviral System to present the mutations associated with HPV induced oropharyngeal cancer. The cells will be modified with lentivirus encoding the human telomerase (hTERT), E6, E7 and Ras oncogenes. To validate the in vitro carcinogenesis of our model, we will assess anchorage independent growth and invasiveness by means of soft agar medium and matrigel studies. To assess in vivo tumorigenicity, the transformed cell populations will be introduced into immunodeficient mice. Results: We constructed recombinant lentivector plasmids using traditional cloning and Gateway recombination methods. Using the constructed lentivectors, we generated lentiviruses encoding the catalytic subunit for human telomerase (hTERT), the E6 and E7 viral oncogenes and Ras oncogene. Primary keratinocytes and oropharyngeal epithelial cells were infected successively by transduction with the above-mentioned lentiviruses and then underwent selection. We validated the expression of our transgenes by methods of immunofluorescence, Western blot and real-time quantitative polymerase chain reaction (qRT-PCR). We have successfully established and kept in culture three lines of epithelial oropharyngeal cells (HNOE42, HNOE45, HNOE46) from tissue samples collected during tonsillectomy. Cells transduced with lentivirus expressing CMV/TO, a strong promoter for the RasV12 oncogene, showed morphological changes compatible with premature Ras oncogene induced senescence. Cell line HEKn hTERT-E6-E7 PGK RasV12 managed to escape Ras oncogene induced senescence by expressing lower amount of mutated RasV12. The HEKn hTERT-E6-E7-PGK RasV12 cell line presented a malignant phenotype in culture and on matrigel invasion assay. However, soft agar assay for anchorage independent cell growth and xenograft assay in immunodeficient mice were both negative for tumorigenicity. Conclusion: Our results demonstrate that in the presence of E6 and E7, a third tumor suppressor mechanism mediates Ras oncogene induced senescence. Furthermore, we have found that the presence of E6 alone is not sufficient to immortalize primary human keratinocytes (HEKn). We have not managed to create an in vitro carcinogenesis cell culture model for HPV induced oropharyngeal cancer.
