The change in oral health related-quality of life among adolescents and their families after orthodontic treatment

Objective: Assess the changes in oral health-related quality of life (OHRQoL) of adolescents and their parents after overjet reduction. Materials and Methods: 53 patients between the ages of 11-18 years with increased dental overjet (≥ 6mm) and their parents were selected, of which 28 were pre- and 25 were post-treatment with dental overjet reduced to within normal limits. The data collection instrument was the Child Oral Health Quality of Life (COHQoL) Questionnaire. Results: Adolescents and their parents reported poorer quality of life before orthodontic treatment than after. The improvement in oral health-related quality of life was statistically significant for all health domains except for the social well-being domain. Parental reports on (OHRQoL) were in agreement with their children’s. No statistically significant differences were evident in (OHRQoL) between pre- and post-treatment groups. Conclusions: Adolescents with increased dental overjet ≥6mm experienced substantial psycho-social impacts. Adolescents with increased overjet can accurately recall the initial negative effects of the original malocclusion on their lives, even after a time lapse of five years. O propósito do estudo foi avaliar um novo procedimento para o tratamento precoce da Classe II Subdivisão com envolvimento mandibular, método proposto por Sakima 85 em 2003. A amostra foi dividida em: Grupo 1 - Portadores de má oclusão Classe II Subdivisão com mordida cruzada posterior unilateral (D) ou (E) e Grupo 2 - Portadores de má oclusão Classe II Subdivisão sem cruzamento. Todos os indivíduos foram submetidos a tratamento ortodôntico de interceptação para correção da subdivisão, sendo que o Grupo 1 teve procedimento prévio para descruzamento da mordida cruzada posterior unilateral (MCPU). A avaliação dos resultados foi realizada mediante tomada de radiografias extra-orais Submento-vertex, os cefalogramas foram traçados e os dados compilados para análise em computador. Os traçados cefalométricos obtidos das imagens radiográficas da Submento-vertex (SMV) foram digitalizados no programa Radiocef. Os dados analisados, submetidos à metodologia estatística e os resultados discutidos. O método estatístico empregado, além da utilização da média e desvio padrão, contou com o Teste t de Student, Teste F de Snedecor e probabilidade p, utilizou-se também o teste não paramétrico de Mann-Whitney-Wilcoxon, os dados estatísticos foram utilizados para avaliar os resultados do tratamento ortodôntico realizado. Após a discussão dos resultados, concluiu-se que: 1)Houve presença de rotação mandibular significante para ambos os grupos estudados quando relacionada maxila e mandíbula; 2) A evolução dos grupos estudados foi semelhante tanto clínica como cefalométrica; 3) Os movimentos dentários constatados melhoram o... . / The purpose of this study was to evaluate a new approach to the early orthodontic treatment of Class II, division 1, subdivision, in the mixed dentition and determined the utility of approach. The sample consisted of 23 subjects divided in two groups (group 1 and group 2). The average was 9.3 years considered in both groups. Measurements were obtained with the use of Submento-vertex Cephalometric Radiographs. The Submento-vertex Radiographs symmetry was assessed by measuring the differences in the spatial positions of dental and Skeletal landmarks between right and left side, determined asymmetric (A) and normal (N) sides. The Angle's 2 Class II subdivision was evaluated in the mixed dentition. Statistic method was based on the skewness statistics, the distributions were normal, allowing means and standard deviations to be used to describe central tendencies and dispersion. We also used the Student t test, the Snedecor p test and Mann-Whitney-Wilcoxon tests were used to statistically comparation.The results showed lower jaw replacement and dento-alveolar displacement in order to correct this malocclusion. The conclusions about the present study were: 1) There was significant mandibular rotation from the asymmetric side to the symmetric side in the two groups when maxilar and mandibular relation was investigated; 2) The two groups evolution was clinical and cephalometrical similar; 3) The teeth movements provide evidence to better relation of the occlusion; 4) The midline line of the teeth has a clinically consistent to normalization and this can better investigated with other evaluation methods.

Assim foi propósito no presente trabalho de pesquisa verificar por meio de radiografias cefalométricas laterais e fotografias frontais e laterais a relação entre a proporção áurea e estética facial antes e depois do tratamento ortodôntico. Foram utilizadas radiografias cefalométricas laterais, fotografias frontais e de perfil de 67 indivíduos antes e depois do tratamento ortodôntico. Foi desenvolvido um programa de cefalometria em Delphi 7.0, contendo a análise cefalométrica deste trabalho de pesquisa. Concluímos: no Grupo 1(melhora após o tratamento ortodôntico) as razões A-Pog/V1S-C1MS, Ena-Enp/V1S-C1MS e V1S-C1MS/C1MS-DM16 diferiram da forma estatisticamente significante comparando antes e depois do tratamento ortodôntico. A razão V1S-C1MS/C1MS-DM16 antes do tratamento não se apresentava em proporção áurea e depois do tratamento passou a se apresentar em proporção áurea. No grupo 2 (não ocorreu melhora após o tratamento) apenas a razão N-Ena/V1S-DM16 diferiru de forma estatisticamente significante comparando antes e depois do tratamento. / The beauty is connected to the proportionality and this constant proportion gives us the impression of guiding the growth, the harmony, the reproduction and the stability of the forms in the nature, what is being evidenced in history for studious as: philosophers, mathematicians, sculptors, painters, architects and Orthodontists. Pithagoras, a mathematics studious, established the proportions from the beauty standards and aesthetic harmony. It was designated as golden proportion. From this thought comes the fact of the use of the golden proportion in the evaluation of cranium facial structures intending to generate an individualized analysis, bringing for each person proportions based in it own measures. It was the purpose in the present research work to verify by lateral cephalometry radiographies, frontals and laterals photographs the relation between golden proportion and aesthetic facial before and after the orthodontic treatment. It was used lateral cephalometry radiographies, frontal and lateral photographs of 67 individuals before and after the orthodontic treatment. It was developed a program of cephalometry in Delphi 7.0, having the cephalometry analysis of this research work. It was concluded: in the Group 1 (improvement after the orthodontic treatment) the ratios A-Pog/V1S-C1MS, Ena-Enp/V1SC1MS and V1S-C1MS/C1MS-DM16 had differed from the statically significant form comparing before and after the orthodontic treatment. The ratio V1SC1MS/ C1MS-DM16 before the treatment was not presented in aureate proportion and after the treatment becomes to present itself in aureate proportion. In group 2 (no improvement after the treatment) only the ratio N-Ena/V1S-DM16 had differed from the statically significant form comparing before and after the treatment. The ratio A-Pog/V1S-DM16, before the treatment, was presented in golden proportion before the treatment and after the treatment it was not presented in golden proportion.

Avaliação comparativa do padrão de normalidade do perfil facial em pacientes brasileiros leucodermas com norte o americano / Standard profiles normality comparative study between caucasian brazilian measures and north american measures

Com a grande procura da sociedade moderna pela estética perfeita a cirurgia ortognática para conseguir melhores resultados e mais precisos, começou a planejar e diagnosticar os casos clínicos utilizando medidas obtidas de grandezas do perfil tegumentar dos pacientes com o auxílio de imagens digitais empregadas em softwares de planejamento, Neste estudo foi proposto aferir as medidas de brasileiros leucodermas de descendência européia e compará-las com as medidas já padronizadas por ARNETT, com o intuito de criar novas medidas para serem seguidas por brasileiros utilizando-se o software Dolphin Imaging 9.0 de predicção cirúrgica. Foi utilizado na metodologia radiografias cefalométricas de 60 pacientes com oclusão classe I de Angle e harmonia facial. Todas as radiografias foram digitalizadas e inseridas no software Dolphin 9.0 e um total de 16 pontos de tecido mole e 22 pontos do esqueleto facial foram marcados, seguindo exatamente as marcações da análise de Arnett & McLaughlin presentes no programa. Os resultados obtidos foram avaliados estatisticamente e mostram que o perfil do brasileiro é quase totalmente diferente do perfil norte-americano, exceção feita a apenas quatro pontos para os homens e outros quatro, para as mulheres. Os brasileiros apresentam uma face menos protruída, um perfil mais convexo e menor proeminência do queixo do que o grupo controle. Os dados obtidos mostram a necessidade de realizar algumas mudanças nas grandezas numéricas para que um perfeito diagnóstico e planejamento possa ser realizado em brasileiros, criando assim o padrão do perfil facial do brasileiro leucoderma de descendência européia. / Considering that modern orthognatic surgery is mainly concerned in planning and diagnosing clinical cases by the use of patients\' soft tissue measurements obtained from digital images employed in planning software, the aim of this study is to establish Caucasian Brazilians measures and to compare them to the standard North American measures. For this, a total of 60 Angle\'s class I patients not previously submitted to orthodontic treatment and presenting harmonious facial features were invited to participate in the study as volunteers. The patients were initially photographed in frontal and lateral views and, afterwards, cephalograms were obtained. The images were digitized, adjusted and indexed in Dolphin\'s image software. In order to procede cephalometric analysis, a total of 16 soft tissue profile points and 22 hard tissue profile points were demarcated, accordingly to North American original measurements used in Dolphin\'s digital image program. After, all cephalograms were again evaluated to determine intra-examiner error according to paired Student\'s t test. Mean and standard error measures were obtained and compared to American measures by unpaired Student\'s t test with 95% confidence level. The results obtained have shown that all measurements, except for four points in male and four points in female, are significantly different (p < 0.05) between Brazilian and American samples. Datas show that Brazilians\' soft tissue profile is almost completely different from North Americans\', with a less protrusive face, shorter chin projection and a more convex profile. This suggests the necessity of performing some changes in numerical values to obtain an ideal diagnosis and planning of orthognatic surgery in Caucasian Brazilians.

Achados incidentais em radiografias panorâmicas de pacientes pré e pós-tratamento ortodôntico / Incidental findings in panoramic radiographs comparing pre and postorthodontic treatment

A radiografia panorâmica é o exame complementar de imagem mais solicitado pelos cirurgiões-dentistas e parte essencial da documentação utilizada pelos ortodontistas no planejamento do tratamento ortodôntico. Durante a transição da dentadura mista para a permanente, o processo biológico é complexo, podendo ocorrer falhas ou irregularidades odontogênicas, que resultam em anomalias dentárias. Entretanto, outras alterações podem ser vistas nas imagens radiográficas panorâmicas, que modificam densidade óssea e alteram o trabeculado ósseo no período entre o início e o término do tratamento ortodôntico. Algumas destas alterações podem interferir ou modificar o diagnóstico durante o tratamento ortodôntico. O número de imagens radiográficas obtidas durante o tratamento ortodôntico depende do protocolo estabelecido caso a caso. Neste estudo interpretamos 500 imagens panorâmicas, sendo 250 pré-tratamento ortodôntico e 250 pós-tratamento ortodôntico, de pacientes com idade de 11 a 18 anos, pertencentes ao Departamento de Ortodontia da Faculdade de Odontologia de Bauru FOB USP. A interpretação radiográfica foi realizada por um único examinador. Nosso objetivo foi avaliar imagens que foram obtidas no início e no final do tratamento ortodôntico e os resultados nos permitiram concluir que no grupo referente à transição da dentadura mista para a dentição permanente, as imagens panorâmicas possuíram o maior número de achados incidentais; a remodelação apical ortodôntica dos incisivos foi encontrada em 180 casos pós-tratamento ortodôntico; houve diferença estatisticamente significante (Teste de Wilcoxon; p<0,05) nos resultados da ossificação do complexo estilo-hióideo bilateral e realização de tratamento endodôntico pré e pós-tratamento ortodôntico; e, muitos dos achados necessitam de intervenção durante o tratamento. / Panoramic radiograph is the most requested imaging exam by dentists and it plays essential role in documentation for orthodontic treatment planning. During the transition from mixed dentition to permanent dentition, the biological process is complex and may experience odontogenic failures or irregularities that result in dental abnormalities. However, other changes can be seen in the panoramic radiography that modify bone density and trabecular bone when it is compared before and after orthodontic treatment. Some of these changes may affect and modify the diagnosis during orthodontic treatment. The number of radiographic images obtained in orthodontic treatment depends on the protocol established for each case. In this study we interpret 500 panoramic radiograph, of patients with age of 11 to 18, belonging to the Department of Orthodontics, Bauru Dental School. The radiographic interpretation was performed by a single examiner. Our objective was to evaluate images that were taken at the beginning and end of orthodontic treatment and the results allowed us to conclude that the most incidental findings were observed in panoramic images in the group concerning the transition from mixed dentition to permanent dentition; apical root resorption of the incisors was found in 180 orthodontic post-treatment cases; there was a statistically significant difference (Wilcoxon Test; p<0.05) between the results of ossification of complex bilateral stylohyoid and endodontic treatment before and after orthodontic treatment; and many of the findings require intervention during treatment.

âEstudo da severidade das mÃs oclusÃes e da necessidade de tratamento ortodÃntico em estudantes do curso de odontologia da UFCâ / âStudy of the malocclusion severity and orthodontic treatment need in students of dentistry of the UFC"

FundaÃÃo Cearense de Apoio ao Desenvolvimento Cientifico e TecnolÃgico / A mà oclusÃo à considerada um problema de saÃde pÃblica pois, alÃm de ser bastante prevalente, à capaz de causar importantes prejuÃzos estÃticos e/ou funcionais. Entretanto, os adultos raramente sÃo incluÃdos em programas ou aÃÃes de saÃde pÃblica que estudam problemas de oclusÃo dentÃria, apesar de estarem suceptÃveis Ãs consequÃncias destes distÃrbios da mesma maneira, ou com maior intensidade, que os mais jovens. AlÃm disso, na maior parte das pesquisas que buscam estimar a prevalÃncia da mà oclusÃo e a necessidade de tratamento ortodÃntico, observa-se a exclusÃo de pessoas jà tratadas ortodonticamente, pois se supÃe que elas apresentariam necessidade significativamente menor do que as que nunca foram tratadas. Assim, esta dissertaÃÃo, composta por dois artigos, teve como objetivos: verificar a prevalÃncia e a severidade das mÃs oclusÃes, alÃm de determinar a necessidade de tratamento ortodÃntico, em um grupo de adultos jovens e, em um segundo momento, investigar se existia diferenÃa entre indivÃduos com e sem tratamento prÃvio. Para isso, foi avaliada uma amostra composta por 241 indivÃduos, com idade entre 17 e 33 anos, regularmente matriculados no Curso de Odontologia da Universidade Federal do CearÃ. Os estudantes foram divididos em dois grupos (com e sem tratamento ortodÃntico prÃvio) e examinados de acordo com o Dental Aesthetic Index (DAI). No artigo 1, estudou-se a prevalÃncia e a severidade das mÃs oclusÃes, e tambÃm a necessidade de tratamento ortodÃntico, dos indivÃduos nÃo tratados. No artigo 2, estas mesmas variÃveis foram comparadas entre os grupos tratado e nÃo tratado. Os resultados do artigo 1 demonstraram que o desalinhamento mandibular anterior e o apinhamento dental foram as mÃs oclusÃes mais prevalentes com 94,6% e 91,9%, respectivamente. A mÃdia do escore do DAI foi 25,94 (IC 95% 25,07; 26,81). Pouco mais da metade dos estudantes (51,4%) foram classificados como sem necessidade de tratamento ortodÃntico, enquanto 15,3% da amostra apresentou mÃs oclusÃes severas ou muito severas, o que implica em necessidade de tratamento ortodÃntico altamente desejÃvel ou obrigatÃria. No segundo artigo, observou-se que, diferentemente dos problemas decorrentes da falta de espaÃo, os quais se mostraram menos prevalentes em estudantes tratados previamente (p<0,001), a maioria das mÃs oclusÃes nÃo foi diferente entre os dois grupos (p>0,05). O DAI mÃdio do grupo tratado foi 24,77 (IC 95% 24,03; 25,51) e o do grupo nÃo tratado foi 25,94 (IC 95% 25,07; 26,81). Pode-se concluir que as mÃs oclusÃes relacionadas à falta de espaÃo foram mais prevalentes, embora sem muita severidade. A necessidade de tratamento ortodÃntico nÃo apresentou diferenÃa estatisticamente significante entre estudantes tratados e nÃo tratados (p>0,05), apesar de a severidade da mà oclusÃo dos tratados ter sido menor (p<0,05). / The malocclusion is considered a problem of public health because, beyond being enough prevalent, it is capable to cause important aesthetic and/or functional damages. However, the adults rarely are enclosed in programs or action of public health that to study problems of dental occlusion, although to be susceptible to the consequences of these disturbances in the same way, or with bigger intensity, that the younger. Moreover, in the most part of the research which search to esteem the prevalence malocclusion and the orthodontic treatment need, it is observed the exclusion of orthodontic treated already people, therefore it supposes that they would present significantly lesser need than the ones that had been never treated. Thus, this dissertation, comprised by two manuscripts, had as objectives: to verify the prevalence and the severity of the malocclusions, beyond determining the orthodontic treatment need, in a group of young adults and, in a second moment, to investigate if to exist difference between individuals with and without orthodontic treatment. For this, it was evaluated a sample composed for 241 individuals, with 17-to-33-years old, regularly registered in the College of Dentistry of the Federal University of the CearÃ. The students were divided in two groups (with and without previous orthodontic treatment) and examined according with the Dental Aesthetic Index (DAI). In article 1, it was studied the prevalence and the severity of the malocclusions, and also the orthodontic treatment need, of the individuals untreated. In article 2, these same variables were compared between the treat and untreated groups. The results of the manuscript 1 demonstrated that the anterior irregularity on the mandible and the crowding were the most prevalent malocclusions with 94,6% and 91,9%, respectively. The mean DAI score was 25,94 (CI 95% 25,07; 26,81). Little more than the half of the students (51,4%) were classified as without orthodontic treatment need, while 15,3% of the sample presented severe or very severe malocclusion, which implies a highly desirable or mandatory orthodontic treatment need. In the second article, it was observed that, differently of the problems deriving of lack of the space, which it showed lesser prevalent in previously students treated (p<0,001), the majority of the malocclusions was not different between the two groups (p>0,05). The mean DAI of the treated group was 24,77 (IC 95% 24,03; 25,51) and the untreated group was 25,94 (IC 95% 25,07; 26,81). It can be concluded that the malocclusions related with the lack of the space were the most prevalent, although without very severity. The orthodontic treatment need presented no difference statistically significant between treated and untreated students (p>0,05), in spite of the severity malocclusion of the ones treated had been lesser (p<0,05).

OBJETIVOS: avaliar a ocorrência de Aggregatibacter actinomycetemcomitans na saliva total em indivíduos antes e depois da instalação do aparelho ortodôntico fixo metálico. METODOLOGIA: Foram avaliados 15 pacientes da Clínica de Especialização em Ortodontia da Faculdade de Odontologia – UFJF e de consultórios particulares, do sexo feminino e masculino, com idade média de 18 anos e 3 meses (14-18 anos); 7 homens e 8 mulheres. Amostras de saliva total foram coletadas um mês antes (T1), imediatamente antes (T2), 15 dias antes (T3) e 1 mês após a instalação de aparelho fixo Edgewise Standard (Morelli 10.30.900) nos dentes superiores e inferiores, com exceção dos segundos e terceiros molares. A coleta de saliva foi realizada com o dispositivo Salivete Cortisol (Sarstedt ® Nümbrecht, Alemanha), com swab neutro de poliéster e microtubo. A partir das amostras coletadas, foi realizada a extração do DNA para identificação da bactéria (Aa) por meio do método Reação em Cadeia Polimerase em tempo real. RESULTADOS: Após as análises, não havia a presença da bactéria Aggregatibacter actinomycetemcomitans em nenhum dos intervalos das amostras testadas. Não houve deterioração da condição periodontal (índice de placa visível, profundidade de sondagem e sangramento à sondagem) após T4. CONCLUSÕES: a) não foi possível identificar a ocorrência da bactéria Aggregatibacter actinomycetemcomitans na saliva nem antes (um mês antes), nem no dia da colagem, nem 15 e um mês após a colagem do aparelho fixo; b) o uso do aparelho fixo não contribuiu para o aumento dos índices de placa, profundidade de sondagem e sangramento à sondagem. / Orthodontic treatment has potential to cause changes in periodontal due to biofilm accumulation, due to the difficulty of conducting a proper oral hygiene in the presence of bands and brackets. OBJECTIVES: Evaluate the occurrence of Aggregatibacter actinomycetemcomitans in whole saliva in individuals before and after the installation of metal orthodontic braces and assess whether there has been deterioration of periodontal status (visible plaque index, probing depth and bleeding on probing) after installing braces. METHODS: We evaluated 15 patients of Specialization in Clinical Orthodontics, School of Dentistry - UFJF and private practices, female and male, with an average age of 18 years and 3 months (14-18 years); 7 men and 8 women. Whole saliva samples were collected one month before (T1), immediately before (T2), 15 days before (T3) and one month after the installation of fixed Edgewise Standard unit (Morelli 10.30.900) on the upper and lower teeth, except the second and third molars. The saliva collection was done with the Salivete Cortisol device (Sarstedt ® Nümbrecht, Germany), with neutral swab polyester and micro tube. From samples collected, extraction of DNA for bacteria identification was carried out (Aa) by Polymerase Chain Reaction method in real time. RESULTS: After the analysis, there was no presence of bacteria Aggregatibacter actinomycetemcomitans in any of the intervals of the samples tested. There was no deterioration of the periodontal status (visible plaque index, probing depth and bleeding on probing) after T3. CONCLUSIONS: a) Unable to identify instances of Aa bacteria in saliva either before or after bonding of fixed appliances; b) the use of braces did not contribute to increase plaque index, probing depth and bleeding on probing.

Retinerade överkäkshörntänder -En retrospektiv jämförelse mellan två typer av kirurgiska friläggingar inför en ortodontisk behandling

Hörntänderna i maxillan är efter visdomständerna de tänder som oftast blir retinerade. Prevalensen varierar mellan 1-3%. En retinerad överkäkshörntand har ofta en ektopisk placering. Detta kan leda till att eruptionsriktningen blir felaktig och därmed kan tanden orsaka skador på de angränsande incisivernas rötter. För att minska risken för detta krävs tidig diagnostik, i 9-11 års ålder.Den vanligaste åtgärden för palatinalt displacerade hörntänder är extraktion av mjölkhörntänderna för att underlätta spontaneruption av de efterföljande permanenta hörntänderna. Om enbart denna behandling inte är tillräcklig eller om den permanenta hörntanden ligger för djupt måste kirurgisk friläggning ske. Detta sker i kombination med efterföljande ortodontisk behandling.I Sverige används idag två typer av kirurgisk friläggning; öppen och sluten. Vilken typ av friläggning som används tycks vara lokalt betingat. I Malmöregionen används oftast öppen friläggning medan i Jönköpingsregionen sluten friläggning är vanligast.I denna studie jämförs behandlingsutfallet hos patienter där öppna och slutna friläggningar gjorts. Framförallt utvärderas om det finns någon signifikant skillnad i behandlingstid, från friläggning till dess att den ortodontiska behandlingen är färdig. Hänsyn tas till typ av friläggning och om tanden ligger palatinalt eller buckalt retinerad. Det undersöks även vid vilken ålder friläggning gjorts samt vilken typ av retentionapparatur som använts.I gruppen med öppen friläggning var patienterna i genomsnitt 15,2 år respektive 12,9 år i gruppen med sluten friläggning. Den totala behandlingstiden från friläggning till avlägsnande av apparatur visade sig vara signifikant kortare i gruppen som fick öppen friläggning jämfört med gruppen som fick sluten friläggning. / The maxillary canines are after the third molars, the most frequently impacted teeth. The incidence varies between 1-3%. Impacted maxillary canines are often related to ectopic position, which can lead to an incorrect direction of eruption and thus cause damage to the adjacent incisors roots.The most common treatment for palatally displaced canines is extraction of the primary canines to facilitate spontaneous eruption of the succeeding permanent teeth. If this treatment is not sufficient or if the permanent canine lies too deep, surgical exposure is needed. This occurs in combination with orthodontic treatment.In Sweden two types of surgical exposures are practiced; open and closed. Which type of exposure is used seems to be locally induced. In Malmö the open exposure is most frequently practiced while in Jönköping the closed exposure is the treatment of choice.The aim of this study is to compare the treatment outcome in two groups of patients, treated either with an open or a closed exposure. It was evaluated whether any significant difference was found in the overall treatment time, from exposure until completed orthodontic treatment. Consideration was given to type of exposure and weather the tooth was palatally or buccally impacted. The age of the patients when the exposures occurred and the type of retention used in the different groups were also examined.The mean age for the patients treated with an open exposure was 15.2 years compared to 12.9 years in the other group. The total time of treatment from exposure to removal of the appliances was significantly shorter in the group with open exposure.
