The effects of short-term energy restriction in overweight/obese females on reproductive outcomes.

In the general population, one in five couples experiences difficulty in conceiving a child. The role of obesity on women’s fecundity has become a focus of attention in recent years. Successful treatment of infertility through Assisted Reproductive Technology (ART) is also compromised by the presence of obesity, which occurs in 30 % of women seeking treatment. A negative correlation exists between increased body mass index (BMI) and the number of collected oocytes and a lower birth rate after ART. Furthermore, a number of studies have established that weight loss improves natural conception rates in overweight women. Whether weight management can improve success rates in overweight / obese women undergoing in vitro fertilisation (IVF) has not been studied. The purpose of this project was to explore the role of short–term weight loss on potential pregnancy outcomes in overweight / obese women undergoing IVF programme. However, to establish this relationship, we proposed to carry out two studies to assess the following: (I) The feasibility of very low calorie diet (VLCD) during IVF treatment with respect to duration, level of restriction and tolerability of the diet during hormonal down regulation in women (Chapter 2). (II) How energy restriction may affect the quality of an early embryo in diet - induced obese mice with respect to various body weight and caloric intake (Chapter 3). In study (I), women preferred a shorter dietary intervention with greater energy restriction (456 kcal per day) to gradual energy restriction (1200 kcal / day for the first week, and afterward, 456 kcal / day) prior to oocyte transfer. Women were able to comply with the VLCD during IVF treatment and both dietary groups achieved a significant weight loss (mean 6.3 %). In study (II), by using obese mice, the effect of rapid weight loss (mean 12 %) was observed after 5 days of energy restriction. Ovulation rate was greater in the Obese group (HFD) (55.6%) and equal in both Control (CD) and Energy Restricted (HF / ER) (44.4 %) groups. The HF / ER group showed higher fertilisation rate (80 %) than HFD and CD (55% and 45.5%, correspondingly). The blastocyst stage was reached by half of the cultured embryos in both HF / ER and HFD groups and 33 % in the CD group. The quality of embryos that completed blastocyst formation did not differ between groups. However, postfertilisation development in females fed a high fat diet was slower compared to CD and HF / ER groups. In conclusion, this work illustrated a weight management prior conception and use of VLCD during IVF treatment in clinical study needs further investigation with regard to the dietary duration, level of energy restriction and how this combination will influence IVF treatment outcomes. (in English): A total of 62% of the Czech adult population currently suffer from overweight or obesity (BMI over 25). Overweight problems are mostly caused by bad eating and exercising habits. The web application Self-Coaching with cognitive behavioral therapy elements helps overweight or obese users to gradually change their bad habits. The objective of the research part of the thesis is to evaluate the effect of a 5-week use of the web application Self-Coaching and to find out whether or not this effect changes if the users are provided with a virtual intervention of an instructor trained in a cognitive behavioral approach to obesity treatment. I evaluated the percentage of weight loss as compared to the initial body weight, the energy and nutritional balance of the diet, vegetable intake, physical activity, the number of days of using the application and the use of the psychological section of the application (psycho-coaching). Weight loss was the identified main effect of the web application Self-Coaching. After five weeks, the users lost on average 2.5% of their initial weight. The intervention of the instructor helped to increase the number of days, for which the users used Self-Coaching.

Factors Impacting Body Mass Index of Hispanic Youth in a Weight Loss Program

Youth obesity prevalence rates in the United States are trending down; however, for Hispanic youth, rates are increasing despite efforts toward a reduction. The purpose of this study was to evaluate the impact of a culturally focused intervention program on weight reduction of Hispanic youth age 2 to 19 through examination of body mass index (BMI) percentile outcomes comparing age, gender, distance to program, time in program, and season of enrollment. Review of literature supports ecological and cultural approaches requiring behavior modification tailored to family and community as successful in decreasing youth obesity. However, researchers have yet to establish how various variables impact intervention outcomes making it difficult to determine what aspects of the cultural approach is effective. The epidemiology triangle and the social cognitive theory approach were used to relate to findings. Use of retrospective clinical archival participant data dating from November 7, 2008 to February 23, 2015 was analyzed through the use of various statistical applications. The analyses of this study indicated that only one age variable had significant results. The category for age 8 to12 had a significantly higher change in BMI than the other groups (p <.05) . This research has the potential to contribute to social change since it reveals that interventions tailored for the 8-12 age group may significantly improve the effectiveness of the program in reducing BMI percentile, thus decreasing obesity rates and associated disease along with morbidity and mortality. This knowledge can benefit educators, community collaborative efforts, practitioners, and other researchers.

Bakgrund: Det råder en brist på kunskap inom hälso-och sjukvården gällande människors upplevelser av att leva med övervikt och fetma. Tidigare forskning har fokuserat på specifika problemsituationer som uppstår inom vården. I litteraturen nämns sällan upplevelser som beskriver de överviktigas eget perspektiv. Detta trots den kraftigt ökande förekomsten av övervikt i samhället. Syfte: Syftet med studien är att belysa människors upplevelser av övervikt och fetma. Metod: Litteraturstudie med Kvantitativ och kvalitativ ansats. Resultat: Sju kategorier hittades: Skuld/ skamkänslor, sorgsenhet/sårad, Avvisad/isolerad, Förödmjukad/ oattraktiv, Glad/underbar, Lågt självförtroende och Oro i nära relationer. Slutsats. Studiens resultat visar att övervikt och fetma är ett lidande för många. Människor med övervikt och fetma utryckte även att lidandet påverkar deras livsstil. Klinisk betydelse: Genom att belysa om vad människor med övervikt och fetma upplever, kan detta leda till en utveckling av studier inom detta område och ökad medvetande inom omvårdnad. / Background: There is a lack of knowledge in health care regarding people's experiences of living with overweight and obesity. Previous research has focused on specific problem situations that arise in health care. Experiences that describes peoples overweight and obesity was uncommon in the literature. This is in spite of the rapidly increasing incidence of obesity in the society. Aim: The aim of this study is to highlight people's experiences of overweight and obesity. Method: It’s a study with quantitative and qualitative methods. Results: This study has seven categories: Guilt / shame, Sadness / hurt, Rejected / isolated, Humiliated / unattractive, Happy / wonderful, Low self-esteem, Anxiety in close relationships. Conclusion: The study results show that overweight and obesity are hardship for many. People with overweight and obesity mentioned that the emotion of suffering affects their lifestyle. Clinical significance: To illuminate the people`s experiences of overweight and obesity, will increase the awareness in caring and lead to a progress in research concerning nursing.

COMPARISON OF QUICK METHODS FOR DETERMINING BODY COMPOSITION IN FEMALE COLLEGIATE ATHLETES AND OBESE FEMALES

The Body Mass Index (BMI) is a tool used broadly by public health agencies to assess weight in populations. However, when differentiating between fat mass and fat free mass the formula (BMI = weight in kilograms/height in meters2) is not applicable. Research suggests that evaluating body fat percentage and adipose tissue deposition may provide a nuanced indication of overall health, making it more accurate on an individual basis. This study evaluated four methods (Body Mass Index, waist circumference, A Body Shape Index, and Waist to Stature Index) that assess body composition and their ability to predict body fat percentage in female collegiate athletes and overweight/obese females. The study also investigated if the CUN‐BAE formula could calculate body fat percentage accurately in comparison to air displacement plethysmography in both populations. The study found that the universality of these algorithms is uncertain in diverse populations and that the predictive power of anthropometric‐based formulas is inconsistent when considering body fat percentage.

Prevalence, perceptions and potential interventions : a mixed methods investigation of childhood overweight and obesity among a pro-poor cohort in Peru

Over the past 20 years there has been a clear shift in Peru’s mortality profile towards non-communicable diseases. As part of this transition, childhood overweight and obesity (O&O) has become a growing public health concern. In order to address this challenge, context-specific information is needed concerning prevalence, associated factors and culturally appropriate interventions. To this end, I conducted a mixed methods study using a socio-ecological framework to investigate childhood overweight and obesity in Peru. This study found a prevalence of 19.2% overweight and 8.6% obesity among children aged seven to eight in a pro-poor cohort in Peru. Factors associated with being overweight in this cohort included: a high socioeconomic status, living in metropolitan Lima, an O&O mother, being male and being an only child or having only one sibling. The quantitative analysis highlighted the prevalence in O&O in this population and revealed a number of relationships explored in more depth via interviews and focus group discussions. The qualitative component of this research explored these associations amongst a sub-sample of children, parents and teachers in three distinct geographical regions in Peru. My qualitative research showed that parents, teachers and children are aware of the health implications of childhood overweight and obesity and that they have a sophisticated understanding of the many factors that influence its aetiology. Participants also had many suggestions for ways to address this issue in their community. This information was complemented by a systematic review and meta-analysis of the evidence of childhood O&O interventions that took place in Latin America published between 1990 and 2011. A combination of diet, physical activity and pharmaceutical strategies have shown modest effect on BMI reduction in O&O children, but more evidence is needed for effective population-level prevention strategies. The findings highlight the need for comprehensive, multi-level interventions. Notable intervention components include: kiosco reform, junk food taxation, creation of recreational space and enhancement of school-based physical activity programmes. To accomplish this, communities need to be mobilised and work together with school administration, municipal and national government.

Temperament dispositions, problematic eating behaviours, and overweight in adolescents

Obesity, a common health condition in adolescence leading to severe medical complications, is assumed to be influenced by temperament factors. This paper investigates associations between reactive and regulative temperament, problematic eating behaviours, and excess weight. Several self-report instruments were completed by 130 adolescents (mean age 14.13 ± 0.61 years), including 27 overweight and obese individuals (20.8%). Bootstrap analysis revealed a mediating effect of restrained eating on the relation between reactive temperament and body mass index percentile, which differed according to gender: Restrained eating, which predicted weight gain, was more present in girls having a higher sensitivity to reward and in boys showing a higher sensitivity to punishment. No effect of regulative temperament was found. These results have important implications for weight management programs, as they suggest that reducing restrained eating by working on temperament may help to control weight.
