Eccentric training in the treatment of tendinopathyJonsson, Per January 2009 (has links)
Chronic painful tendinopathies are common, not only in sports and recreationally active people, but also among people with a sedentary lifestyle. Both the lower and upper limbs are affected. There is lack of knowledge about the etiology and pathogenesis to tendinopathy, and many different treatments options have been presented. Unfortunately, most treatments have not been tested in scientific studies. Conservative (non-surgical) treatment has since long shown unsatisfactory results and surgical treatment is known to give unpredictable results. The aim of this thesis was to evaluate new models of painful eccentric training for the conservative treatment of different chronic tendinopathies. After promising results in a pilot study, using painful eccentric calf muscle training in patients with chronic mid-portion Achilles tendinopathy, we investigated if these results could be reproduced in a larger group of patients with both mid-portion and insertional Achilles tendinopathy (study I). After 12 weeks, 89% of the patients with pain from the mid-portion were satisfied and back in previous activities. In the group with insertional Achilles tendinopathy the results were poor. A new model for eccentric training was designed for patients with insertional Achilles tendinopathy. The eccentric calf muscle training was done from tip-toe to floor level (study II). With this new regimen 67% of the patients were satisfied and back in previous activities. The next step was to investigate the effects of painful eccentric quadriceps training on patients with jumper´s knee/patellar tendinopathy (study III). Two different training protocols were used. Eccentric training performed on a 250 decline board showed promising results with reduced pain and a return to previous activities, while eccentric training without the decline board had poor results. In a following prospective study, patients with jumper´s knee/patellar tendinopathy were randomised to either concentric or eccentric painful quadriceps training on a 250 decline board (study IV). After 12 weeks of training, there were significantly better results in the group that did eccentric training. In a pilot study (study V), we investigated painful eccentric deltoideus and supraspinatus muscle training on a small group of patients on the waiting list for surgical treatment of subacromial impingement syndrome. After 12 weeks of training, 5 out of 9 patients were satisfied with the results of treatment and withdrew from the waiting list for surgery. In conclusion, the present studies showed good clinical results with low risks of side effects and low costs. Thus, we suggest that painful eccentric training should be tried in patients with Achilles and patellar tendinopathy before intratendinous injections and surgery are considered. For patients with chronic painful impingement syndrome, the results of our small pilot study are interesting, and stimulates to randomised studies on larger materials.

Šunų kelio girnelės išnirimo klinikinis pasireiškimas ir gydymo metodų apžvalga / Clinical manifestation and treatment review of patella luxation in dogsDubickaitė, Ieva 05 March 2014 (has links)
Darbo tikslas: Išsiaiškinti skirtingo amžiaus ir veislių šunų sergamumą kelio girnelės išnirimu, įvertinti klinikinio pasireiškimo formas, aptarti dažniausiai taikomus gydymo metodus.
Tiriamojo darbo metu buvo renkami sertifikuojamų veislinių šunų kelio sąnario tyrimo metu gauti rezultatai ir nagrinėjami duomenys apie šunų kelio girnelės išnirimą. Darbo metu buvo siekiama nustatyti šunų sergamumo dažnumą atsižvelgiant į veislę, pažeidimo laipsnį, amžių, lytį. Taip pat buvo praktiškai susipažinta su dažniausiai taikomais kelio girnelės išnirimų gydymo metodais.
Renkant duomenis, dėmesys buvo kreipiamas į gyvūnų šunų amžių, veislę, lytį, girnelės nestabilumo laipsnį ir pažeidimo charakterį.
Kelio girnelės nestabilumas ir jo laipsnis nustatytas, atliekant šuns užpakalinių kojų ortopedinį tyrimą, ypatingą dėmesį kreipiant į kelio girnelių padėtį ir jų paslankumo laipsnį. Gauti duomenys registruoti, suskirstant kelio girnelės išnirimo atvejus pagal pažeidimo laipsnį. Duomenys statistiškai apdoroti paskaičiuojant procentinę išraišką. Statistinių duomenų apdorojimui buvo pasirinktas Microsoft EXCEL 2003 programinis paketas.
Išanalizavus 2009 – 2013 metų surinktus duomenis, nustatyta, kad iš 390 tirtų atvejų, kelio girnelės išnirimu sirgo 40 % šunų. Dažniausiai išnirimai diagnozuoti Jorkšyro terjerams, prancūzų buldogams, mopsams. Nustatyta, kad abejose galūnėse labiausiai paplitęs 1 laipsnio girnelės išnirimas. Pagal gautus duomenis, visi išnirimai diagnozuoti į vidinę... [toliau žr. visą tekstą] / The goal of the work was to find out the prevalence of patellar luxation in dogs of various breeds and age, evaluate the clinical signs and discuss the treatment methods.
During the investigation the data from dogs of certified breeds concerning the results of knee joint clinical examination regarding patellar luxation was obtained and analyzed. Attempts were made to establish the connection between the age, breed and the prevalence of patellar luxation. Practical studies were performed concerning methods of treatment.
When collecting data attention was paid to the age, breed, sex of the animal and the type of patellar luxation.
The type of patellar instability and its degree were determined by clinically examining the position of the patellae and their stability. The data obtained was registered, the cases of patellar luxation were divided according to the luxation degree/ The data was processed statistically, using Microsoft Excel 2003.
The analysis of the data collected during 2009-2013 showed that from 390 cases analyzed 40% were diagnosed with patellar luxation. The breeds affected the most were Jorkshire terriers, French bulldogs, Pugs. Bilateral luxations in most cases were grade 1. All luxations were found to be medial, bilateral luxations were diagnosed in 51,9% cases. The most affected dogs were found to be the ones of 1-3 years old (93,6% of all cases). Females were more affected than males (71,2%).

Após o deslocamento medial da patela, foi realizado o aprofundamento do sulco troclear com o auxílio de uma lima cilíndrica para metal, removendo-se a cartilagem articular e tecido ósseo subcondral , até que o sulco adquirisse uma profundidade equivalente a pelo menos a metade da espessura da patela. Após o aprofundamento do sulco troclear foi realizado o implante de um grampo metálico confeccionado com um pino intramedular de Steinmann, imediatamente proximal à crista troclear medial, dando continuidade à mesma, numa extensão equivalente à trajetória da patela. Em seguida a cápsula articular foi suturada em um padrão Sultan, iniciando-se pelo ligamento femuropatelar lateral, e a pele com pontos em U com fio de nylon 3-0. O tempo médio de apoio do membro foi de 5 dias, sendo que aos 10 dias de pós-operatório todos os cães apoiavam normalmente o membro. As avaliações radiográficas no período pós-operatório imediato, aos 30, 60 e 90 dias não revelaram alterações referentes a presença do grampo metálico, tais como áreas de osteólise ou migração do grampo. / This work shows a new technique for correction of patellar luxation, mainly for the pacients with high hiding patella. Twelve adult and healthy mongrel dogs with average weigh of 10 Kg were used for this study. The surgical technique consisted of skin and articular capsule parapatellar incision and after that was done the medial displacement of patella from trochlear groove which was deepened using a cylindrical rasp for metal, to remove the articular cartilage and subchondral bone tissue in a depth equivalent to the half of the patella thickness. After this, an implant of a metallic staple, confectioned using a Steinmamm intramedullary pin, was used proximal to the medial tochlear crest giving continuity in an extension equivalent to patella trajectory. Then the articular capsule was sutured using Sultan pattern from the lateral femuropatellar ligament and the skin closure was performed by U pattern, both with nylon 3-0. The average time of support of the affected limb was five days and after ten days of postoperative period, all dogs supported it normally. The radiological evaluations at immediate postoperative period and at 15, 30, 60 and 90 days, didn't show any alterations related to the presence of the metallic staple, like osteolytic lesions or migration of the staple. The histopathological evaluations of the femoral tochlea of the limb submitted to the surgery presented changes of the cartilaginous tissue for a fibrocartilagenous one. The histology of the patella and the implant site of the metallic staple didn't show any abnormalities when compared to the contra-lateral limb.

Avaliação do tendão quadríceps e ligamento patelar pela ultrassonografia e ressonância magnética em jogadores de futebol assintomáticos / Ultrasonography and magnetic resonance imaging for assessment of the quadriceps tendon and patellar ligament in asymptomatic soccer playersOlger de Souza Tornin 07 February 2013 (has links)
INTRODUÇÃO: A ultrassonografia (US) e a ressonância magnética (RM) representam os principais métodos de diagnósticos por imagem na avaliação do mecanismo extensor do joelho. Há carência de trabalhos que avaliem por US e RM a presença de alterações no mecanismo extensor de jogadores de futebol assintomáticos. OBJETIVO: Avaliar, por meio de RM e US, o tendão quadríceps e o ligamento patelar de jogadores de futebol assintomáticos e dos participantes do grupo-controle. Identificar se há alterações de imagens ou lesões. Determinar se essas sofrem influências pelos seguintes critérios: canhoto ou destro, quanto à dominância ao chute, posição em campo, idade do atleta e tempo de prática esportiva. MÉTODO: Foram avaliados um total de 248 joelhos: 112 eram de 56 de jogadores de futebol assintomáticos e 136 eram do grupo-controle. O grupo composto por jogadores de futebol apresentava idades entre 14 e 34 anos. Já o grupo-controle era composto por indivíduos com idade também entre 14 e 34 anos, principalmente, universitários e funcionários do hospital, com gênero igual aos dos jogadores selecionados, mas sem atividades físicas de impacto, como vôlei, futebol e salto, por exemplo. RESULTADO: Constatou-se a presença de alterações de imagens ou de lesões em nove ligamentos patelares e apenas duas nos tendões quadríceps de jogadores de futebol assintomáticos, tanto pela US quanto pela RM, e nenhum caso alterado no grupo-controle. Houve concordância significativa entre RM e US quanto aos resultados obtidos. Além disso, os jogadores de futebol apresentaram anormalidades (alterações de imagens ou lesões) no tendão quadríceps ou no ligamento patelar significativamente (p<0,05) maior do que no grupo-controle; a idade média dos jogadores com alguma anormalidade é significativamente menor do que a dos jogadores sem lesão (p<0,003); o tempo médio de prática desportiva dos jogadores que apresentam anormalidade é menor do que o tempo médio dos jogadores que não têm anormalidade (p<0,001); os jogadores destros apresentaram anormalidades, enquanto que os jogadores canhotos não (p<0,05). Dentre os jogadores, há correlação entre mais alterações de imagem ou lesão de joelho e menor tempo como jogador e menor idade (R² de Nagelkerke = 0,700). Destros têm risco aumentado de ter anormalidade nas referidas estruturas (Razão de Chances = 15,204) quanto menor for o tempo como jogador de futebol. Os jogadores da defesa têm 4,76 vezes mais chance de desenvolver anormalidade do que atacantes e goleiros (p<0,04). CONCLUSÃO: Os jogadores de futebol assintomáticos apresentam anormalidades no tendão do quadríceps e ligamento patelar detectadas pela RM e US. As alterações estão relacionadas a menor idade, menor tempo de prática desportiva, ser destro e ser jogador de defesa / INTRODUCTION: Ultrasonography (US) and magnetic resonance imaging (MRI) are the main diagnostic imaging methods used in evaluation of the extensor mechanism of the knee. Theres a scarcity of works assessing alterations on the extensor mechanism of the knee of asymptomatic soccer players using US and MRI. OBJECTIVE: To assess, by means of MRI and US imaging, the quadriceps tendon and patellar ligament of asymptomatic soccer players and healthy control group; identify abnormal imaging findings or injuries; ascertain whether these are affected by leg dominance (left or right), field position, age, and time spent practicing the sport. METHOD: A total of 248 knees were evaluated: 112 of 56 asymptomatic soccer players and 136 of healthy controls. The players ages ranged from 14 to 34 years. The control group was composed mostly of college students and hospital staff members, also with ages between 14 and 34, matched for gender to the selected soccer players, and who did not engage in high-impact activities such as volleyball, soccer, or jumping. RESULTS: Abnormal imaging findings or injuries were detected, by US and MRI alike, in nine patellar ligaments and two quadriceps tendons of asymptomatic soccer players. No abnormal findings were detected in the control group. CONCLUSION: There was significant consistency between MRI and US findings. Soccer players were much more likely (p<0.05) to have abnormal imaging findings or injuries of the quadriceps tendon and patellar ligament as compared with the control group; the average age of players presenting some abnormality is significantly lower than of those without injuries (p<0.003); the average time spent practicing the sport for players presenting abnormalities is lower than those who dont present injuries (p<0.001); a correlation between the dominant leg and the affected knee was observed, with the players with a dominant right leg presenting injuries while the ones with a dominant left leg not showing abnormalities (p<0.05). Amongst the players there is a correlation between more abnormal imaging findings or injuries on the knee and less professional practicing time and age (Nagelkerke R² = 0.700).Players with a dominant right leg have an increased chance of presenting abnormalities on the quadriceps tendon and patellar ligament (Odds Ratio = 15.204) with a lower average time spent practicing the sport. Defense players have 4.76 times more chance to develop abnormalities than forward players and goalkeepers (p<0.04). CONCLUSION: The asymptomatic soccer players present abnormalities on the quadriceps tendon and patellar ligament identified using ultrasonography (US) and magnetic resonance imaging (MRI). Previous issue date: 2016-01-22 / Medial patellar dislocation is a common condition in small dogs and should be diagnosed and treated as early as possible in order to prevent further deformities and secondary osteoarthritis. The objective of this article was to study the measures and the anatomic relationships of the trochlear groove and patella in radiographic examinations of healthy dogs predisposed to medial patellar dislocation breeds, analyzing and comparing with the trochlear groove and patellar of patients with such disease. 40 knees of predisposed pure breeds dogs were used which were divided into two groups: group 1 (G1) consisting of 18 knees that do not show patellar dislocation and group 2 (G2) consisting of 22 knees showing patella in the medial grade II or III. Depth and width trochlear groove, height and thickness of the patella, a percentage of the patella in the groove, groove angle troclear were measured and calculated the ratios of patellar width and trochlear groove and depth of the trochlear groove and thickness of the patella. All parameters studied showed statistically significant differences, in comparison between healthy and affected dogs, compared to being evaluated by Student's t test. Through evaluation and measurements on radiographs G1 dogs, it were developed predictor formulas of broadening (Lsd = Lpp/0,89 – Lsp) and deepening (Psd = 0,65Ep –Psp) required the trochlear groove to perform trochleoplasty block. / Luxação medial da patela é uma afecção comum em cães de pequeno porte, devendo ser diagnosticada e tratada o mais cedo possível com objetivo de evitar maiores deformações e alterações articulares secundárias. O objetivo deste trabalho foi estudar as medidas e as relações anatômicas do sulco troclear e da patela através de exames radiográficos de cães sadios de raças predispostas à luxação patelar medial, analisando e comparando com o sulco troclear e patela de pacientes portadores desta enfermidade. Foram utilizados 40 joelhos de cães de raças predispostas que foram divididos em dois grupos, sendo o grupo 1 (G1) composto de 18 joelhos que não apresentavam luxação de patela e o grupo 2 (G2) composto de 22 joelhos que apresentem luxação de patela medial em graus II ou III. Foram realizadas mensurações de profundidade e largura do sulco troclear, altura e espessura da patela, percentual da patela dentro do sulco, ângulo do sulco troclear, sendo calculadas as razões entre largura patelar e do sulco troclear e profundidade do sulco troclear e espessura da patela. Todos os parâmetros estudados demonstraram diferença estatística significativa, na comparação entre cães sadios e portadores da afecção, ao serem avaliadas pelo teste t de Student. Através dessas medições realizadas nas radiografias dos cães do G1 foram desenvolvidas fórmulas preditivas do alargamento (Lsd = Lpp/0,89 – Lsp) e aprofundamento (Psd = 0,65Ep –Psp) do sulco troclear, para realização de trocleoplastia em bloco.

It is characterized by the medial displacement of the patella from its normal position in the trochlear groove, and surgical treatment is recommended from grade two dislocation. The objective of this study was to test if the creation of a complementary patellar ligament, manufactured from shredding of surplus retinaculum is a good technique to reinforce the patellar stabilization for the treatment of medial patellar luxation in dogs. We used 16 knees of 13 dogs, regardless of gender or age who had medial patellar luxation grades two, three and four, and among these, 8 were part of the B group in which we used the technique of synthetic patellar ligament for comparison. The animals were evaluated 30 and 90 days after surgery, for the parameters of pain, willingness to lift the contralateral limb, lameness and weight-bearing on the affected limb by the nonparametric Wilcoxon test for differences between ordered pairs. Since all animals showed functional recovery in 90 days, it is concluded that the proposed technique is also an appropriate surgical option for the strengthening of patellar stabilization in the treatment of medial patellar luxation in dogs. / A luxação patelar medial é um transtorno ortopédico frequentemente visto na clínica veterinária, acometendo especialmente cães de pequeno porte. Caracteriza-se pelo deslocamento medial da patela de sua posição normal no sulco troclear, sendo seu tratamento cirúrgico recomendado a partir do grau dois de luxação. Objetivou-se neste trabalho verificar se a criação de um ligamento patelar complementar confeccionado através do retalhamento do retináculo excedente constitui uma boa técnica de reforço da estabilização patelar para o tratamento da luxação medial de patela em cães. Foram utilizados 16 joelhos de 13 cães, sem distinção de sexo ou idade que apresentavam luxação de patela medial de graus dois, três e quatro, sendo que entre esses, 8 fizeram parte do grupo B onde se utilizou a técnica do ligamento patelar sintético para fazer a comparação. Os animais foram avaliados com 30 e 90 dias após a cirurgia, em relação aos parâmetros dor, disposição para levantar o membro contra-lateral, claudicação e suporte de peso no membro afetado. As diferenças entre os pares para esses parâmetros foram submetidas ao teste não paramétrico de Wilcoxon, evidenciando aos 30 dias melhor evolução estatística no grupo B, mas igual desempenho para ambos os grupo aos 90 dias. Conclui-se então, que a técnica proposta também constitui opção cirúrgica apropriada de reforço na estabilização patelar para o tratamento da luxação patelar medial em cães.

Bakgrund: Patellar tendinopati (PT) är framförallt en vanlig diagnos bland aktiva inom idrotter där hopprörelser ofta förekommer. Diagnosen innebär ofta överbelastning av knäskålsenan, vilket leder till smärta och nedsatt funktionsförmåga. Fysioterapeutisk behandling syftar till att minska smärtan och återställa funktionsförmågan. Infrapatellara remmar (IPR) har använts sedan många år för symptomlindring vid idrottsaktiviteter men det saknas forskning som har sammanställt effekter av användning, vilket föranleder behovet av en systematisk litteraturöversikt. Syftet var att genom en systematisk litteraturstudie beskriva hur IPR används och utvärderas i studier vid PT, rapporterade effekter samt studiekvalitet och preliminär evidensstyrka. Metod: Artikelsökning genomfördes i databaserna PubMed och PEDro. Fem studier analyserades utifrån litteraturstudiens syfte och frågeställningar samt kvalitetsgranskades enligt SBU:s granskningsmallar. Resultat: Av studierna undersökte tre IPR:s effekt på smärtintensitet, två proprioception och två indirekt uppmätt senbelastning. Endast kortsiktiga utfall mättes och när dessa var statistiskt signifikanta var kliniska relevansen ofta oklar. Sammantagen preliminär evidensstyrka bedömdes som måttligt stark. Slutsatser: Studierna visade på varierande effekt av att använda IPR avseende förbättring av proprioception, smärtintensitet och indirekt uppmätt senbelastning. Fler högkvalitativa randomiserade studier behövs och framtida forskning bör undersöka långsiktiga effekter vid användning av IPR. / Background: Patellar tendinopathy (PT) is common in jumping athletes. It’s mostly related to excessive loading of the patellar tendon, which leads to pain and decreased functional capacity. Physiotherapy treatment aims to reduce pain and restore functional capacity. Infrapatellar straps (IPS) have been utilized for many years to alleviate symptoms during sports activities but there is a lack of research that has compiled reports on effects of usage, which leads to the need of a systematic review. Objective: To describe use and evaluation of IPS in studies on patients with PT and describe reported effects, study quality and preliminary strength of evidence. Method: The PubMed and PEDro databases were searched for articles. Five studies were analyzed considering the review’s objective. Study quality was assessed according to SBU’s checklists. Results: Among the studies three investigated the effect of IPS on pain intensity, two on proprioception and two on indirectly measured tendon load. Short-term outcomes were investigated and the clinical relevance was often unclear when outcomes were statistically significant. Total preliminary strength of evidence was assessed as moderate. Conclusion: Variable effects are shown from IPS usage regarding pain intensity, proprioception and indirectly measured tendon load. More high-quality randomized trials and investigations of long-term effects are needed.

A Comparison of Kinesio® Taping Methods for Subjects with Patellar TendonitisGallais, Kathleen Yvette January 2020 (has links)
This project investigated the effects of Kinesio® Tape on pain, kinesiophobia, and proprioception in participants with patellar tendonitis. Thirty participants with patellar tendonitis were divided into three groups, the first received a supportive Kinesio® Tape application at the knee, the second received a facilitative application at the hip, and the third received both. A Visual Analog Score, Tampa Scale for Kinesiophobia score, and a proprioceptive score quantified through the Biodex Balance System were obtained both immediately after application, and 24 to 36 hours following. Statistically significant improvement in VAS scores and in proprioceptive ability with eyes closed 24 to 36 hours following Kinesio® Tape application was observed under all interventions. Kinesio® Tape application for pain, cause of pain and proprioception may assist patients with patellar tendonitis.
