A Retrospective Evaluation of Surgical Treatment of Peri-implantitis

Aim: To investigate retrospectively the outcome of surgical peri-implantitis treatment taking potentially relevant parameters into account. Material and method: Patients being treated for peri-implantitis during 2013-2019 by a single specialist with sufficient radiographic and clinical documentation and at least a nine-month follow-up were considered herein. Patient-, prosthodontic-, and implant-related data were collected from the journals and available radiographs at baseline and follow-ups. Surgical procedures were divided into open flap debridement (OFD), resective, regenerative and combined treatments. Treatment outcome was defined as: good (PPD≤5mm+no BoP); acceptable (PPD>5mm without BoP or BoP+PPD≤5mm); bad (PPD>5mm+BoP and/or suppuration); and failure (explantation or re-operation). Descriptive statistics, chi-square or Fisher’s exact test were performed to assess potential predictors for the combined outcomes good and acceptable, coined as “successful”, and bad and failure, coined as “unsuccessful”. Results: Thirty-seven patients and 69 implants with a follow-up of 9 to 63 months fulfilled the inclusion criteria. Overall, 71% of the treated implants showed a good outcome and 13% an acceptable outcome, i.e., in 84% of the cases a successful outcome was achieved. Regenerative treatment had the highest success rate (87%) followed by resective (84%), OFD (83%) and combined treatment (83%). One of the predictors (i.e., smoking) affected the successful outcome negatively with >10%. However, statistical analysis failed to prove significance. Conclusion: Based on this retrospective analysis of a relatively limited number of implants, surgical treatment of peri-implantitis was successful in 84% of the cases. For defining risk factors affecting the treatment outcome a larger sample size is required. The results were of no statical significance. / Syfte: Retrospektivt undersöka resultatet av kirurgisk peri-implantitbehandling med hänsyn till potentiellt relevanta parametrar. Material och metod: Alla patienter som kirurgiskt behandlades för peri-implantit under tidsperioden 2013-2019 av en enda specialist med minst nio månaders uppföljning beaktades här. Patient-, protes- och implantatrelaterade data samlades in retrospektivt från journaler och tillgängliga röntgenbilder pre- och postoperativt. Kirurgiska ingrepp delades in i open flap debridement (OFD), resektiv, regenerativ och kombinerad behandling. Behandlingsresultatet definierades som: good (PPD≤5mm+ingen BoP); acceptable (PPD>5mm utan BoP, eller BoP+PPD≤5mm); bad (PPD>5mm+BoP och/eller suppuration); och failure (explantering eller re-operation). Sammantaget visade 71% av de behandlade implantaten ett bra resultat och ytterligare 13% ett acceptabelt resultat, dvs. i 84% av fallen uppnåddes ett lyckat resultat. Regenerativ behandling hade den bästa lyckandefrekvensen (87%) följt av resektiv (84%), OFD (83%) och kombinerad behandling (83%). En av prediktorerna, mer specifikt rökning, påverkade lyckandefrekvensen negativt med >10%. Statistisk analys misslyckades dock med att bevisa statistisk signifikans. Slutsats: Baserat på denna retrospektiva analys av ett relativt begränsat antal implantat lyckades kirurgisk behandling av peri-implantit i 84% av fallen. För att definiera riskfaktorer som påverkar behandlingsresultatet krävs en större provstorlek. Resultaten visade ingen statistisk signifikans.

Assessment of students knowledge regarding patients suffering from microbial induced peri-implant diseases. Survey Study at the Faculty of Odontology, Malmö University

SyfteSyftet med studien var att undersöka vilken kunskap sistaårs tandläkarstudenter och sista-termins tandhygieniststudenter vid Odontologiska Fakulteten, Malmö Högskola har gällande diagnos, behandling och prognos av periimplantära sjukdomar.Material och MetodEn pappersenkät sammanställdes och delades ut till sistaårs tandläkar- och tandhygieniststudenter vid Odontologiska Fakulteten, Malmö Högskola.I enkäten inkluderades bakgrundsfrågor gällande erfarenhet av implantatinstallation samt implantatprotetik, kunskap om hur vävnaderna kring ett implantat undersöks, diagnostiska kriterier för periimplantär sjukdom, och prognos. Enkäten inkluderade även fyra unika fall.Resultat97 av 119 enkäter delades ut, 83 av 97 enkäter returnerades. Studien visade att studenterna i hög grad föreslår korrekt diagnos, behandlingsplan, och prognos. Resultaten visar även osäkerhet vid självutvärdering av den ställda prognosen.SlutsatsStudenterna angav i hög grad korrekt behandling, men att de träffar få patienter med periimplantär sjukdom under utbildningen. Studenterna är osäkra vilket resulterar i variationer mellan studenter gällande diagnos, behandling, och prognosbedömning vid periimplantär sjukdom.Majoriteten av studenterna föreslog att det är patientens nuvarande tandläkare som är ansvarig för uppföljning/utvärdering av implantatet. Detta är det mest lämpliga förslaget tack vare regelbundna kontroller till samma tandläkare (nuvarande).Då implantat används mer frekvent som ersättning för förlorade tänder finns ett behov av mer utbildning och färdighetsträning inom området vid Odontologiska Fakulteten, Malmö Högskola. / AimThe aim of this study is to examine the knowledge of last-year dental students and last semester dental hygienist students at the Faculty of Odontology, Malmö University regarding diagnosis, treatment, and prognosis of peri-implant diseases. Material and MethodA paper case based survey study was conducted and handed out among last-year dental and dental hygienist students at the Faculty of Odontology, Malmö University.The survey included background questions regarding experience of implant installation and implant prosthetics, and knowledge about how an examination is conducted, diagnostic criteria for peri-implant diseases, and prognosis. The survey also included four unique cases.Results97 of 119 surveys were handed out, 83 of 97 surveys were returned. The study showed that students to a high degree suggests a correct diagnosis, treatment plan and prognosis. The results also shows insecurities when self-evaluating the prognosis.ConclusionThe students planned the correct treatment, to a high degree, but they encounter few patients with peri-implant diseases during their education. The students are insecure, resulting in inter-student disagreement regarding diagnosis, treatment and assessment of prognosis of peri-implant diseases.The majority suggested the patients current dentist as responsible for follow-ups/evaluation, which also is the most appropriate suggestion because of regular check-ups to the same (current) dentist.Because implants are used more frequently as a replacement for lost teeth there is a need for more education and training in this area at the Faculty of Odontology, Malmö University.

Long-term Evaluation of Bone Augmentation Procedures with Autogenous Bone or Bone Substitutes in Association with Implant Treatment

Syfte. Syftet med denna systematiska litteraturöversikt var att undersöka det vetenskapliga underlaget avseende långsiktig stabilitet hos olika typer av benersättningsmaterial i kombination med implantatinstallation.Material och metod. En elektronisk sökning gjordes med hjälp av PubMed och Web of Science. Block med sökord konstruerades och skrevs in i sökmotorerna för att hitta studier med implantatbehandling i kombination med benaugmentation, med en radiologisk uppföljning efter minst 2 år. Funna artiklar granskades i förhållande till förutbestämda inklusions- och exklusionskriterier. Data från inkluderade studier registrerades. Risken för bias i de inkluderade studierna bedömdes enligt modifierade protokoll.Resultat. Den elektroniska sökningen resulterade i 1183 studier, varav 14 studier med antingen autogent, allogent, xenogent eller alloplastiskt benersättningsmaterial inkluderades i denna litteraturöversikt. Resultatet från studierna visade en stor skillnad i både tekniker och material.Slutsats. Den generella slutsatsen i denna studie är att den marginella benförlusten vid dentala implantat som placerats i augmenterat ben inte överstiger 1 mm inom två år. En mer exakt slutsats om den marginella bennivåns stabilitet kunde inte dras på grund av de stora skillnaderna mellan de inkluderade studierna. Det finns ett behov av fler högkvalitativa studier med långtidsuppföljning som undersöker och jämför den marginella bennivåns stabilitet för olika benersättningsmaterial. / Aim. The purpose of this systematic review was to investigate the literature available regarding long-term stability of different bone augmentation materials in combination with dental implant treatment.Material and methods. An electronic search was done using PubMed and Web of Science. Blocks with keywords were constructed and inserted into the search engines to find studies involving implant treatment in combination with bone augmentation, with a radiographic follow-up of at least 2 years. Found articles were reviewed in relation to predetermined inclusion- and exclusion criteria. Data from included studies was extracted. Included studies were assessed of risk of bias according to modified protocols. Results. The electronic search yielded 1183 studies. 14 studies using either autogenous, allogeneic, xenogeneic or alloplastic augmentation materials was included in this review. The results from the data extraction process showed a wide variation in both materials and techniques used.Conclusion. The general conclusion drawn from this study is that marginal bone loss around dental implants placed in augmented bone does not exceed 1 mm within two years post-operatively. A more precise conclusion regarding marginal bone level stability could not be drawn due to the heterogeneity of the included studies. There is a need for additional high-quality studies with long-term follow-up, investigating and comparing marginal bone level stability for different bone augmentation materials.

Evaluation of peri-implantitis treatment. Retrospective case control study

Tandimplantat utsätts för både tekniska och biologiska komplikationer; de senare innefattar mukosit och periimplantit. Ett stort antal periimplantitbehandlingar har utvärderats, ofta i samband med bensubstitut. Det finns dock ingen konsensus idag om effektiv periimplantitbehandling.Syfte: Syftet med studien är att utvärdera och jämföra om skillnader i behandlingsresultat föreligger när periimplantit behandlas kirurgiskt med bensubstitut eller utan bensubstitut.Material och Metoder: Trettiofyra patienter (22 kvinnor, 12 män) behandlades varav 12 patienter med bensubstitut och 22 patienter utan bensubstitut.Av totalt 198 implantat har 193 utvärderats: Avseende bennivån (100 implantat) Avseende fickdjup (PPD) och blödning vid sondering (BOP) (160 implantat).Resultat: Kirurgisk behandling i form av mekanisk depuration och användning av bensubstitut har bidragit till vinst av bennivån med en statistisk signifikans (p ≤ 0,05). Dessutom kan en statistiskt signifikant förbättring (p ≤ 0,05) av PPD noteras bland patienter som behandlades med bensubstitut jämfört med patienter som behandlats utan bensubstitut.Konklusion: Av de patientfallen i studien är slutsatsen att användning av bensubstitut vid kirurgisk periimplantitbehandling resulterar i betydligt större vinst i bennivå och PPD reducering i jämförelse med endast kirurgisk behandling. Nyckelord: Bensubstitut, Periimplantit, Kirurgisk behandling. / Dental implants are subjected to both technical and biological complications; the latter ones comprise peri-implant mucositis and peri-implantitis. Numerous peri-implantitis treatments have been evaluated, often involving implantation of a bone graft. However, as of today, no consensus exists regarding effective peri-implantitis treatment.Aim: The aim of the study is to retrospectively evaluate if there are differences in treatment results after surgical peri-implantitis treatment with or without a bone graft.Material and Methods: Thirty-four patients (22 female, 12 male) were treated whereas 12 patients were treated with bone graft and 22 patients were treated without bone graft. Out of a total of 198 implants, 193 were evaluated regarding peri-implant bone level (100 implants) and probing pocket depth(PPD) and bleeding on probing (BOP) (160 implants).Results: Surgical treatment including mechanical debridement and implantation of a bone graft contributed to statistical significant (p ≤ 0,05) bone level gain in comparison to the non-grafted group. Additionally, statistically significant (p ≤ 0,05) improvement in PPD could be noted among patients treated with bone graft compared to patients treated without a bone graft.Conclusion: Of the present case series, it can be concluded that implantation of a bone graft during surgical peri-implantitis treatment, results in significant larger bone level gain and PPD reduction comparing to only surgical treatment.Keywords: Bone graft, Peri-implantitis, Surgical treatment.

Vad påverkar om äldre personer genomgår behandling för periimplantit? : En explorativ litteraturstudie / What affects if older people undergo treatment for peri-implantitis? : An explorative literature study

Introduktion/Bakgrund: Andelen och antalet äldre personer ökar i hela världen. Dagens äldre personer åldras med flera naturliga tänder och tandimplantat än någonsin. Tandimplantat ersätter tänder som ofta förlorats på grund av orala sjukdomar. Riskfaktorerna för orala sjukdomar kan dock kvarstå och till exempel leda till periimplantit. Periimplantitbehandling är avgörande för implantatöverlevnad. För att ytterligare förstå vad som påverkar huruvida äldre personer genomgår perimplantitbehandling genomfördes följande litteraturgenomgång. Syfte var att utforska och kartlägga vilka faktorer som påverkar huruvida äldre personer genomgår periimplantitbehandling eller ej.  Metod: En litteraturstudie utfördes genom att systematiskt söka, identifiera, granska och sammanställa vetenskaplig litteratur gällande ämnet. Systematiska sökningar genomfördes i databaserna PubMed och Cinahl.Totalt identifierades 258 artiklar varav slutligen 9 artiklar inkluderas i resultatet. Urvalsprocessen understöddes av kvalitetsgranskning av artiklarna med hjälp av granskningsmallar. Resultat: Studiens resultat visar att kunskap var en viktig faktor förl att behandla periimplantit- såväl hos tandvårdspersonal som patienter. Andra faktorer som påverkade var tillgång till tandvård, patientens hälsotillstånd och ekonomiska situation. Slutsats: Periimplantit är ett komplext tillstånd och många faktorer påverkar hälsan. Ingen av de identifierade studierna har beskrivit mer samhälleliga faktorer som kan tänkas påverka om periimplantitbehandling genomförs eller ej. Studien visar att fortsatt forskning beträffande periimplantit behövs, till exempel interventioner gällande kunskap och kommunikation samt interventioner på övergripande, mer samhällelig nivå som kan tänkas påverka om periimplantitbehandling utförs eller ej. / Introduction/Background: The proportion and number of elderly people is increasing worldwide. Today´s elderly people are aging with more natural teeth and dental implants than ever before. Dental implants replace teeth that are often lost due to oral diseases. However, the risk factors for oral disease may persist and lead to peri-implantitis. Peri-implantitis treatments is essential for implant survival. To further understand whether older people undergo peri-implantitis treatment or not, this paper was written. Aim: The purpose of the study was to to explore and map factors that affect whether older people with  undergo peri-implantitis treatment or not.  Method: A literature study was carried out by systematically searching, identifying, reviewing and compiling scientific literature on the subject. Systematic searches were carried out in the PubMed and Cinahl databases. A total of 258 articles were identified and 9 articles are included in the results. These were quality reviewed using review templates. Results: The results of the study shows knowledge was an important factor in treatening peri-implantitis- both in staff and patients. Other influencing factors were access to dental care, the patient´s state of health and financial situation. Conclusion: The study shows that peri-implantitis is an area where further research is needed, especially in terms of communication, knowledge and regarding social factors that can affect whether peri-implantitis treatment is performed or not. Especially studies concerning  factors are lacking.

Estudo in vivo e in vitro da corrosão de ligas de Co-Cr e de Ni-Cr utilizadas em estruturas protéticas de implantes orais / In vitro and in vivo studies of corrosion of Co-Cr and Ni-Cr alloys used in oral implant prosthetic superstructures

Este trabalho avaliou o comportamento eletroquímico de três ligas de níquel-cromo e duas de cobalto-cromo in vitro e in vivo. Nos estudos in vitro foram fabricados eletrodos de trabalho a partir destas ligas. Estes eletrodos ficaram imersos em meio de NaCl 0,15 mol.L-1 a 36,5°C e foram submetidos às técnicas do potencial de circuito aberto, polarização potenciostática anódica e cronoamperometria. Para os estudos in vivo foram selecionados quinze pacientes com implantes já osseointegrados e aguardando a instalação das próteses. Esses pacientes foram distribuídos aleatoriamente em cinco grupos, correspondentes às cinco ligas diferentes. Deste modo, as estruturas protéticas dos implantes foram fundidas utilizando as cinco ligas. Foi utilizada a microscopia eletrônica de varredura (MEV) e a espectroscopia por dispersão de energia (EDS) como métodos de análise das superfícies linguais metálicas das próteses, antes da instalação em ambiente oral e após seis meses de permanência. Vistas por MEV, as ligas duas de cobalto-cromo Starloy C e Remanium 2001 e duas de níquel-cromo Wiron 99 e Remanium CSe apresentaram superfícies heterogêneas caracterizadas por uma fase semelhante à matriz da liga e a outra fase, composta por elementos de maior densidade atômica, correspondente à fase interdendrítica. Após seis meses, não houve sinais de corrosão e alterações significativas na composição destas ligas. As ligas de Co-Cr avaliadas, sob o ponto de vista eletroquímico, indicam comportamento catódico quando unidas ao implante sendo um fator indicativo de proteção à corrosão. Dentro dos limites dos estudos in vitro, as ligas de Ni-Cr apresentaram um filme passivo com ampla faixa de potencial em que se mantêm passiva e ausência de corrosão localizada. A liga de níquel-cromo Dan Ceramalloy apresentou a superfície heterogênea com quatro áreas distintas (área 1 rica em titânio; área 2 rica em níquel; área 3 correspondente à matriz; área 4 rica em molibdênio). Após seis meses, as três próteses metalo-cerâmicas apresentaram as regiões correspondentes à fase 2, rica em níquel, com depressões em baixo relevo, sugerindo a oxidação com a liberação de íons níquel. Nos estudos in vitro a liga Dan Ceramalloy apresenta potencial de corrosão mais negativo, uma faixa passiva de potencial mais estreita e um filme menos protetor, quando comparada com as demais ligas estudadas. Apesar de não ter sido observado clinicamente o processo de corrosão das ligas, os estudos in vitro e in vivo deste material, permitiram verificar o seu comportamento eletroquímico e sugerir a descontinuidade do seu emprego em estruturas protéticas de implantes orais. Os estudos in vitro e in vivo apresentaram concordância para todos os materiais estudados. / In this study, the electro-chemical behavior of three nickel-chrome and two cobalt chrome alloys was studied in vitro and in vivo. In the in vitro studies, working electrodes made of these alloys were fabricated. These electrodes were immersed in a medium of NaCl 0.15 mol.L-1 at 36.5°C and were submitted to the open circuit potential, anodic potentiostatic polarization and chronoamperometry techniques. For the in vivo studies, 15 patients whose implants had osseointegrated, and who were waiting for dental implant placement, were selected. These patients were randomly distributed into five groups, corresponding to the five different alloys. Thus the prosthetic implant superstructures were cast, using the five alloys. Scanning electron microscopy (SEM) and energy dispersive spectroscopy (EDS) were used as methods to analyze the metal lingual surfaces of the prostheses, before they were placed in the oral environment, and after six months in place. Visualized by SEM, the two cobalt-chrome alloys Starloy C and Remanium 2001, and two types of nickel-chrome - Wiron 99 and Remanium Cse - presented heterogeneous surfaces characterized by a phase similar to that of the alloy matrix, and the other phase composed of elements of greater atomic density, corresponding to the interdendritic phase. After six months, there were no signs of corrosion and significant alterations in the composition of these alloys. The Co-Cr alloys evaluated, from an electrochemical point of view, indicated a cathodic behavior when united to the implant, this being a factor indicative of protection against corrosion. Within the limits of the in vitro studies, the Ni-Cr alloys presented a passive film with an ample range of potential in which they remained passive, with absence of corrosion due to pitting. The nickel-chrome alloy Dan Ceramalloy presented a heterogeneous surface with four distinct areas (Area 1 rich in titanium; Area 2 rich in nickel; Area 3 corresponding to the matrix; Area 4 rich in molybdenum). After six months, the three metal-ceramic dental prostheses presented the regions corresponding to Phase 2, rich in nickel, with depressions in low relief, suggesting oxidation with nickel ion release. In the in vitro studies, the alloy Dan Ceramalloy presented a more negative corrosion potential, a narrower range of passive potential and a less protective film, when compared with the other alloys studied. In despite of the process of corrosion of the alloys not having been clinically observed, the in vitro and in vivo studies of this material allowed verification of its electrochemical behavior, and suggest that its use in prosthetic superstructures for oral implants should be discontinued. The in vitro and in vivo studies were shown to be in agreement for all the materials studied.

Análise in vitro da ação antibacteriana de filme nanoestruturado contendo partículas de prata aplicado em superfície de titânio / Analysis in vitro of antibacterial effect of nanostructured film with silver particles applied to titanium surface

Apesar do alto e significativo índice de sucesso dos implantes osseointegráveis, fracassos podem ser acarretados por diversos fatores, tais como falhas cirúrgicas, protéticas, trauma oclusal e por infecções nos tecidos ao redor do implante. As lesões inflamatórias desenvolvidas ao redor de implantes são conhecidas coletivamente como doença peri-implantar, que possui um papel muito importante na falência de implantes, sendo considerada uma das principais causas. O objetivo deste trabalho foi analisar a ação antibacteriana de filme nanoestruturado com partículas de prata aplicado em superfície de titânio de componentes protéticos conectados a implantes, em relação às bactérias predominantes na infecção peri-implantar. Para avaliar a ação antibacteriana do filme nanoestruturado com partículas de prata foi realizado teste microbiológico com cepas de Aggregatibacter actinomycetemcomitans (ATCC 43719), Fusobacterium nucleatum (ATCC 25586) e Porphyromonas gingivalis (ATCC 33277). Discos de titânio sem tratamento formaram o grupo controle (Ti) e discos com filme nanoestruturado com partículas de prata, o grupo teste (NT-Ag). Cada teste foi realizado em triplicata e repetido por três ocasiões separadas. Após 48 horas de incubação dos discos em caldo de infusão de cérebro e coração (BHI), previamente inoculado com as cepas, a 37 °C, em anaerobiose, os discos foram então retirados e colocados em um tubo contendo solução tampão fosfato-salina (PBS) e centrifugados por 1 minuto em aparelho tipo vortex. O PBS foi diluído em série até 1:100.000, distribuído em placas de ágar sangue e essas foram incubadas por 48 horas, a 37ºC, em anaerobiose e então foi feita a contagem de colônias sobre as placas para posterior calculo das unidades formadoras de colônia (UFC). A análise da adesão bacteriana foi realizada com cepas de Aggregatibacter actinomycetemcomitans (ATCC 43719), Fusobacterium nucleatum (ATCC 25586) e Porphyromonas gingivalis (ATCC 33277), através da leitura dos discos em microscópio eletrônico de varredura Leo 43, após imersão dos discos em BHI inoculado. O teste antibacteriano, realizado com Aggregatibacter actinomycetemcomitans, apresentou valor médio de UFC/mL do grupo teste (NT-Ag) semelhante ao obtido do grupo controle (Ti), não havendo diferença estatística (p<0,01) entre eles. Os testes realizados com Fusobacterium nucleatum e Porphyromonas gingivalis não apresentaram crescimento bacteriano, na etapa de leitura das placas, para os dois grupos, controle (Ti) e teste (NT-Ag). As imagens obtidas, através do microscópio eletrônico de varredura (MEV), demonstraram uma notável redução da adesão bacteriana, nos discos com filme nanoestruturado com partículas de prata (NT-Ag), em comparação com as imagens do grupo controle (Ti), para todas as cepas, porém não são conclusivas. O filme nanoestruturado com partículas de prata não foi efetivo na redução da adesão de A. actinomycetemcomitans. / Despite the high and significant success rate of dental implants, failures can be posed by various factors such as surgical and prosthetic failures, oclusal trauma and infections in the tissues around the implant. The inflammatory lesions developed around implants are collectively known as peri-implant disease, which has a very important role in implant failure and is considered one of the main causes. The aim of this study was to evaluate the antibacterial activity of nanostructured film with silver particles, applied on abutments connected to implants, in relation to the predominant bacteria in peri-implant infection. To evaluate the antibacterial activity of the nanostructured film, microbiological tests were performed with strains of Aggregatibacter actinomycetemcomitans (ATCC 43719), Fusobacterium nucleatum (ATCC 25586) and Porphyromonas gingivalis (ATCC 33277). Untreated titanium discs were the control group (Ti) and discs with the nanostructured film with silver particles formed the test group (NT-Ag). Each test was run in triplicate and repeated on three separate occasions. After 48 hours of discs incubation in brain heart infusion broth (BHI), previously inoculated with the strains, at 37 °C, under anaerobic conditions, the discs were then removed and placed in a tube containing phosphate buffered saline solution (PBS) and vortexed for 1 minute. The PBS was serially diluted up to 1:100,000, distributed in blood agar plates and incubated for 48 hours, at 37 ° C in anaerobiosis and then colonies were counted for subsequent calculation of colony forming units (CFU). The analysis of bacterial adhesion was performed with strains of Aggregatibacter actinomycetemcomitans (ATCC 43719), Fusobacterium nucleatum (ATCC 25586) and Porphyromonas gingivalis (ATCC 33277) by reading the discs surface in a scanning electron microscope Leo 43, after immersion of the discs on inoculated BHI. The antibacterial test, performed with Aggregatibacter actinomycetemcomitans, obtained CFU/mL medium value from the test group (NT-Ag) similar to the obtained in the control group (Ti), showing no statistical difference (p <0.01) between them. The tests performed with Fusobacterium nucleatum and Porphyromonas gingivalis showed no bacterial growth, in plates reading step, for both groups, control (Ti) and test (NT-Ag). The images obtained by scanning electron microscopy showed a remarkable reduction of bacterial adhesion, on discs with nanostructured film with silver particles (NT-Ag), when compared to the images of the control group (Ti), for all strains, but they were not conclusive. The nanostructured film with silver particles was not effective in reducing bacterial adhesion of A. actinomycetemcomitans.

Apesar do alto e significativo índice de sucesso dos implantes osseointegráveis, fracassos podem ser acarretados por diversos fatores, tais como falhas cirúrgicas, protéticas, trauma oclusal e por infecções nos tecidos ao redor do implante. As lesões inflamatórias desenvolvidas ao redor de implantes são conhecidas coletivamente como doença peri-implantar, que possui um papel muito importante na falência de implantes, sendo considerada uma das principais causas. O objetivo deste trabalho foi analisar a ação antibacteriana de filme nanoestruturado com partículas de prata aplicado em superfície de titânio de componentes protéticos conectados a implantes, em relação às bactérias predominantes na infecção peri-implantar. Para avaliar a ação antibacteriana do filme nanoestruturado com partículas de prata foi realizado teste microbiológico com cepas de Aggregatibacter actinomycetemcomitans (ATCC 43719), Fusobacterium nucleatum (ATCC 25586) e Porphyromonas gingivalis (ATCC 33277). Discos de titânio sem tratamento formaram o grupo controle (Ti) e discos com filme nanoestruturado com partículas de prata, o grupo teste (NT-Ag). Cada teste foi realizado em triplicata e repetido por três ocasiões separadas. Após 48 horas de incubação dos discos em caldo de infusão de cérebro e coração (BHI), previamente inoculado com as cepas, a 37 °C, em anaerobiose, os discos foram então retirados e colocados em um tubo contendo solução tampão fosfato-salina (PBS) e centrifugados por 1 minuto em aparelho tipo vortex. O PBS foi diluído em série até 1:100.000, distribuído em placas de ágar sangue e essas foram incubadas por 48 horas, a 37ºC, em anaerobiose e então foi feita a contagem de colônias sobre as placas para posterior calculo das unidades formadoras de colônia (UFC). A análise da adesão bacteriana foi realizada com cepas de Aggregatibacter actinomycetemcomitans (ATCC 43719), Fusobacterium nucleatum (ATCC 25586) e Porphyromonas gingivalis (ATCC 33277), através da leitura dos discos em microscópio eletrônico de varredura Leo 43, após imersão dos discos em BHI inoculado. O teste antibacteriano, realizado com Aggregatibacter actinomycetemcomitans, apresentou valor médio de UFC/mL do grupo teste (NT-Ag) semelhante ao obtido do grupo controle (Ti), não havendo diferença estatística (p<0,01) entre eles. Os testes realizados com Fusobacterium nucleatum e Porphyromonas gingivalis não apresentaram crescimento bacteriano, na etapa de leitura das placas, para os dois grupos, controle (Ti) e teste (NT-Ag). As imagens obtidas, através do microscópio eletrônico de varredura (MEV), demonstraram uma notável redução da adesão bacteriana, nos discos com filme nanoestruturado com partículas de prata (NT-Ag), em comparação com as imagens do grupo controle (Ti), para todas as cepas, porém não são conclusivas.
