Poruchy stability a chůze u extrapyramidových onemocnění / Balance and gait disorders in movement disorders

Gait and balance disorders and the resulting falls are a substantial part of Parkinson's disease (PD) and other movement disorders. Especially in the late stage of PD more than 80 % of the patients fall. History of falls remains the best predictor of falls nonetheless, but it can not be used in falls prevention. Dopaminergic pharmacotherapy improves postural stability and gait in PD only in the early stage and the dopaminergic responsiveness of these symptoms decreases significantly during the disease progression. The impact of this medication on future falls risk remains still unclear. The connection between balance and gait disorders and cognitive impairment in PD is also not fully understood. The current state of knowledge about gait and balance disorders and cognitive impairment in PD is not satisfactory. Therefore the aims of the experimental part of this thesis were prospective monitoring of risk factors and predictors of falls, observation of the impact of dopaminergic medication on future falls risk and verifying the relationship between gait and balance disorders and cognitive impairment in PD. The thesis concerns the topic of parental care for children with autism spectrum disorder. In the theoretical part there are described the main areas in which the autistic spectrum disorders manifest and the description of particular disorders. Next chapter describes this issue from point of view of parental care, it describes the difficulty of the situation, the impact on the lives of parents and whole families, factors affecting the adaptation to the situation and its overall management. The qualitative study was chosen for the research. The sample is composed of 11 mothers of children with autism spectrum disorder. Data for study was collected by half-structured interviews and supplemented by Logo-test. The results of study describe the impact of care for a child with autism spectrum disorder to the lives of parents, emotions in different situations, stressing and supporting aspects of the situation, scarce factors and external factors which the most affect the situation of parents. It seems that the situation causes numerous of changes in the lives of parents and affects their values and attitude to life. Theoretical part Currently accepted new conceptualization of women's sexual dysfunction is based on the model found in the American Psychiatric Association's Diagnostic and Statistical Manual (DSM-IV) and International Classification of Diseases (ICD). These classifications are extended and combined interpersonal, contextual, personalpsychological and biological factors. Recent DSM- IV definitions have focused on absence of sexual fantasies and sexual desireprior to sexual activity and arousal, even though the frequencyof this type of desire is known to vary greatly among women without sexual complaints. DSM-IV definitions also focus on genital swelling and lubrication, entities known to correlate poorly with subjective sexual arousal and pleasure. The structure of the new classification is based on the four categories of DSM-IV: disorders of desire, arousal, orgasm and pain. However these categories are newly assigned to the subjective dimension of satisfaction, stress and the presence of disorders during other sexual activities besides intercourse. This diploma thesis deals with the Achilles tendinopathy, and particularly with its etiopatogenetics. The theoretical part includes information about the Achilles tendon, tendinopathy of Achilles tendon and a summary of dysfunctions of the locomotor system. We assumed that the dysfunction of the locomotor system is the main etiopatogenetic factor of the Achilles tendinopathy, and that it results in changes of its position and loading. In the practical part we tried to reveal these dysfunctions by examination of seven patients, and to prove its main etiopatogenetic influence by a successfull therapy. Diploma thesis deals with the problems of sleep of the patients in intensive care. Sleep is the very basic of human needs and its correct pattern is crucial for any activity of an organism. A person who is sick has an increased need for sleep and are among the most frustrated needs. The theoretical part defines sleep as a basic human need to deal with physiology and sleep disorders. I include a chapter dealing with intensive care and aspects of the environment that affect sleep of the patients. The main objective is to determine whether the need for sleep of the patients is saturated or not. I also set milestones as to gain information regarding patients sleeping pattern at home.To find out, what influence nurses in approaching sleep as a matter and to look for factors that have an influence of some sort from the nurses point of view regarding the sleeping pattern same as from the patients view of the matter. In the empirical part, I reached the set goals through interviews and questionnaires. Thanks to interviews with patients, I discovered that need of sleep of patients in intensive care is not saturated.Out of the 30 respondents I interviewed, 17 of them therefore deemed their sleep as poor and insufficient. in English The thesis focuses on trainee teachers with specific learning disabilities. Its aim is to map their university study and teaching practice. The research part is supplemented with a short survey showing public opinion on teachers with learning disabilities. The thesis is based on empirical research of qualitative nature with quantitative information. Data were obtained from semi structured interviews with students of teaching with specific learning disabilities and from the questionnaire conducted which the public. Another analysis (qualitative and quantitative) and an interpretation of the data followed. All the findings were then compared with the data fixed in literature. The written work consists of three major parts. The theoretical part is predominantly based on abroad scientific sources of information about people with dyslexia studying at a university and employed. The second part of the thesis describes the methodology of the research and the analysis of the data. The two most important results of the research are represented by a number of features which describe a trainee teacher with specific learning disabilities, and the public opinion on such a person. It's goal is to analyze parent's strategy to cope with such burden in relation to taking care of a child with autistic spectrum disorder. Thesis is constructed as an emprical qualitativeresearch which also involves quantitative data. Data were acquired via half - structured interviews with parents of children with autism spectrum disorder.Furthermore, a questionnaire SVF 78 "Strategy of stress management" was used and filled out by the parents. The data were subsequently qualitatively analyzed and interpreted using content analysis. Finally, a comparison to specialized literature was performed. The text of the thesis itself is composed of three parts. First part includes theoretical solutions that are based on both Czech and foreign literature concerned with autism theory and coping with stress of parents bringing up an autistic child. Second part is focusing on the methodology and data analysis procedure. The theoretical part deals with the physiology of sleep and sleep disorders. The practical part were processed by means of qualitative and quantitative research . The aim of our thesis and our research was to determine the incidence of sleep disorders in different age categories. The aim of the qualitative part was to determine the causes and effects of sleep disorders in children and adolescents. In practice , the qualitative part certainly importance , especially for teachers , case reports indicate that some other sleep disorders affect child development , behavior and school performance. In this thesis we compare the quantitative survey targeted four designated groups of children . One group of preschool children are 1-6 years old . The second group consists of a sample of pupils of primary school 7 - 11 years . The penultimate group of children at the elementary school , aged 12 - 15 years. The last group are secondary school students aged 16-18 years . At the beginning of quantitative research, we set three hypotheses. Hypothesis 1 were not confirmed . For the method a questionnaire was chosen which includes semantic differentials and separate sentences. The respondents were selected based on availability. The 26 respondents included in this research were all women diagnosed with anorexia nervosa or bulimia nervosa. All were hospitalized in an eating disorder unit. The outcome of the study confirmed that video confrontation resulted in the patients describing the picture of their own body less ungainly and they were less ashamed about their own body.
