O objetivo do presente trabalho foi realizar uma revisão bibliográfica sobre os efeitos que o hábito de fumar pode ter sobre o periodonto e o peri-implante, para alertar a classe odontológica sobre os cuidados que deve pacientes fumantes, bem como contribuir para o esclarecimento da área jurídica, carente desses conhecimentos, na solução dos problemas eventualmente envolvidos na avaliação da responsabilidade civil profissional do cirurgião-dentista. Como decorrência da revisão da literatura e da discussão empreendida, concluiu-se que: o tabagismo tem efeito deletério sobre a saúde bucal, constituindo-se no maior fator de risco para as doenças periodontais conhecido na atualidade; os principais efeitos nocivos do hábito de fumar sobre o periodonto são: diminuição da vascularização, alteração na resposta inflamatória e imunológica, bolsas periodontais mais profundas, maior perda de inserção periodontal e interferência na cicatrização pós-terapias; no que concerne ao peri-implante, o tabagismo provoca: vasoconstrição sistêmica, redução do fluxo sangüíneo, maior probabilidade de desenvolver peri-implantite e interferência na cicatrização pós-cirúrgica; o cirurgião-dentista precisa verificar se o paciente é fumante ou não, para esclarecê-lo e conscientizá-lo sobre os riscos que esse hábito traz à saúde e, conseqüentemente, aos tratamentos e trabalhos odontológicos reabilitadores, podendo levá-los ao insucesso, pois é o periodonto que os sustenta; o paciente deve manifestar-se sobre a vontade de abandonar esse hábito ou não, e o profissional deve documentar os esclarecimentos que lhe forneceu e a decisão que deles resultou, arquivando esse documento no prontuário odontológico, a fim de se resguardar no caso de eventuais ações impetradas contra si. / SUMARY The purpose of the present work was to perform a bibliographic revision on the effects that smoking habits might have on the periodontium and the peri-implant, in order to alert dentists about the care that must be taken in regard to the warning about the risks posed to the patients who smoke, as well as to advise legal professionals in need for such knowledge, in the solution of the problems that eventually arise when evaluating the legal responsibilities of the professional surgeon. As a result of the revision of the literature and the discussion enticed, it has been concluded that tobacco smoking has a deleterious effect on the dental health, making it the greatest risk factor for the periodontal diseases known today; the main noxious effects of the smoking habit on the periodontium are: lower vascularization, change in the inflammatory and immunological response, deeper periodontal pockets, greater loss of periodontal attachment and post-therapy healing interference; in regard to the peri-implant, tobacco smoking can cause: systemic vasoconstriction, reduction in blood flow, greater probability to develop peri-implantitis and interference in post-surgery healing; the surgeon needs to verify if the patient is a smoker or not, in order to warn him about the risks that this habit may pose to his health and, consequently, to the treatments and oral rehabilitation work, which can lead to failure, since it is the periodontium that supports it; the patient must indicate that he is willing to give up the habit or not, and the dentist must record the warning given and the decision made thereafter, filing this document in the patient’s records in order to prevent any law suits concerning mal-practice.

Part of the thesis are selected court rulings on professional responsibility. Finally, it is also dedicated to the comparison of observed professions.

Om lärarskapets moraliska dimension : ett perspektiv och en studie av lärarstuderandes nätbaserade seminariesamtal

In this dissertation, an educational perspective called the moral dimension of teaching is developed. The work includes a theoretically informed discussion from a pragmatist point of view in which the concept of pedagogical rhythm is introduced. The concept captures the need for teachers to regularly shift their intentions and occasionally act in contradictory ways as a consequence of the moral which emerges from interaction in pedagogical situations. Using this perspective, criteria are developed for the characteristics of discussions of the work of teachers, which are desirable in order for students in pre-service teacher education to have opportunities to develop their teachership. Secondly, the educational perspective as it is conceptualised serves as a theoretical framework for a study of discussions taking place in net-based seminars among students in teacher education. The study consists of 14 recorded seminars in which discussions of the work of teachers are analysed in terms of content and direction for reflection. The result of the analysis is a construction of four different focal points for processes of making judgements: existential, performative, critical and professional. Mainly the performative, and to some extent the critical, focal points appear to be supported by the net-based environment, although potential for the professional focal point is found when available tools in net-based settings are used in deliberate ways. Finally, based on these four focal points, possible future predispositions among student teachers are deliberated. Student teachers’ future opportunities to develop a moral and epistemological authority are discussed, as well as teachers’ general opportunities to exercise professional responsibility. The conclusion emphasises that a perspective such as the one developed in the dissertation is important, as it creates an understanding for the need to educate student teachers to exercise a form of responsibility that goes beyond being accountable to society.

Ethics in a shrinking world exploring the ethical implications of the proliferation of technology on world hunger

Even if they do not realize it, readers are perpetually in a condition that is strikingly similar to that of the Good Samaritan. Right now they have access to a vast network of communication that both enhances their senses and increases their sphere of influence. They can, for example, sit down at a computer and click on a certain combination of "sites" and the result will be that, in two weeks (or sooner), a DVD will arrive on their doorstep. Or, they can choose another combination, and the result will be that, in about two weeks (or less), a child will be saved from starvation and dehydration in some distant and destitute nation. Like the Good Samaritan, a reader of this thesis can see the desperate need of others and they have the ability to affect their condition. This perpetual Good Samaritan condition is directly a result of the recent changes the world has undergone as a result of technological advancement. This thesis is an exploration of the ethical implications of the potential perpetual good Samaritan. I will argue that (1) affluent individuals are able to affect positively the global poor and that they have a moral obligation to do so, (2) that this moral obligation is limited insofar as fulfilling the obligation requires a moral agent to sacrifice something of substantial significance (i.e., something that would cause a long term decrease in happiness), and (3) fulfilling this obligation requires specific actions on political, social, and individual levels.

Aim: To describe and interpret patients’, family members’ and ambulance personnel’s experiences with regard to survival, attendance, and caring at cardiac arrests and deaths, and to analyze ethical conflicts that arise in relation to families and how the personnel’s ethical competence can affect caring and the ability to handle ethical problems. Method: The three interview studies were guided by a reflective lifeworld approach grounded in phenomenology and analyzed by searching for the essence of the phenomenon in two studies and by attaining a main interpretation in one study. In the fourth study, the general approach was supplemented by “reflective equilibrium” that guided the ethical analysis. Results: The survivors are striving towards a good life by means of efforts to reach meaning and coherence, facing existential fear and insecurity as well as gratitude and the joy of life. Family members lose everyday control through feelings of unreality, inadequacy and overwhelming responsibility. Ambulance personnel’s care mediates hope and despair until the announcement of survival or death. After the event, family members risk involuntary loneliness and anxiety about the future. For the ambulance personnel, caring for families involves a need for mobility in decision making, forcing the personnel to balance their own perceptions, feelings and reactions against interpretative reasoning. To base decision making on emotional reactions creates the risk of erroneous conclusions and a care relationship with elements of dishonesty, misdirected benevolence and false hopes. Identification with family members can promote recognition of and response to their existential needs, but also frustrate meeting family members emotions’ and handling one’s own vulnerability and inadequacy. It was found that futile cardiopulmonary resuscitation, administered to patients for the benefit of family members, is not an acceptable moral practice, due both to norms of not deliberately treating persons as mere means and to norms of taking care of families. Conclusions: Ethical conflicts exist when it comes to conveying realistic hope, relief from guilt, participation, responsibility for decision making, and fairness in the professional role. Ambulance personnel need support to enhance ethical caring competence and to deal with personal discomfort, as well as clear guidelines on family support.
