A model to facilitate the mental health of student nurses working with mentally challenged individuals

(Psychiatric Nursing Science) / The researcher was involved in the clinical accompaniment of student nurses working with mentally challenged individuals during their psychiatric nursing practical training. In her role as advanced psychiatric nurse educator, she noticed that student nurses experienced working with mentally challenged individuals as a challenging working context. It created intense emotional discomfort for the student nurses, especially during their initial exposure to the relevant individuals. During the student nurses' last day of working with these individuals, they reflected with the advanced psychiatric nurse educator on their emotional growth and enrichment and how this experience had changed their views of life. Ineffective management of emotional discomfort may lead to emotional exhaustion or burnout and reflect negatively on a person's mental health. The main purpose of this research was to explore and describe the experiences of student nurses working with mentally challenged individuals. Subsequently, to develop, describe and write guidelines to operationalise and evaluate a model for the advanced psychiatric nurse educator to facilitate the mental health of student nurses working with mentally challenged individuals. A qualitative, explorative, descriptive, contextual and theory-generating research design was utilised to achieve the abovementioned purpose. The development of the model comprised four steps. Step one consisted of a concept analysis including identification and definition of central concepts in the model. A concept analysis was done by exploring and describing the experiences of student nurses working with mentally challenged individuals. Two focus groups, naive sketches, reflective journals, a reflective letter and field notes were used to explore their experiences. Focus groups were audiotaped as well as videotaped. Verbal consent was given by the student nurses to be videotaped and a letter of consent was signed to give permission for audiotaping of the focus groups. The audio tapes were transcribed verbatim. The video tapes were only used by the transcriber when she could not hear the sound on the audio tapes clearly. An independent coder utilised Tesch's method of open-coding to code and analyse the data. A consensus was reached between the researcher and the independent coder with regard to the themes and catogories represented by the data. During the concept analysis, engagement on a deeper emotional level was identified as the central concept. Step two consisted of the relationship statements of the model. During step three, a model for the facilitation of a process of engagement on a deeper emotional level for student nurses working with mentally challenged individuals was described. The structure of the model clarified the purpose, assumptions and context. The central concepts were defined and the relationship statements between the central and essential concepts were explained. The structure of the model focused on the relationship-, workingand termination phases within the process of engagement on a deeper emotional level. In step four guidelines were described to operationalise the model in practice. The model, as framework of reference for the advanced psychiatric nurse educator, focused on the facilitation of a process of engagement on a deeper emotional level for student nurses working with mentally challenged individuals. The process description of the model differentiated between three phases: the relationship phase, the working phase and the termination phase. Guidelines for the operasionalisation of the model focused on the objective of each phase as well as the strategy of actions for each different phase.

Bakgrund: Inom psykiatrisk vård är det viktigt att respektera patientens personliga förväntningar och mål för att uppnå goda behandlingsresultat och för att patienterna ska känna sig tillfreds med den vård de erhåller. Sjuksköterskan bör underlätta för patienter att kommunicera om sina besvär, stödja patienten att analysera destruktivt beteende, stödja patienten att skapa en normal daglig livsföring samt främja patientens integritet och egenvård. Trots detta upplever inte patienterna att detta uppfylls alla gånger. Syfte: Syftet med denna litteraturöversikt var att undersöka patienter med psykisk ohälsa med fokus på deras upplevelser av vården på psykiatriska vårdavdelningar.  Metod: I denna litteraturöversikt har författarna använt tio vetenskapliga artiklar för att besvara syftet. Dessa har granskats för att förstå skillnader och likheter och sedan delats in i teman. Resultat: Tre teman och tre underteman påvisades i denna litteraturöversikt. Ett tema var patienters upplevelser av att bli eller inte bli behandlad med respekt med undertemana patienters upplevelser av stigmatisering samt patienters upplevelser av vårdande miljö. Det andra temat var upplevelser av personalens förhållningssätt med undertemat patienters upplevelser av vårdande relationer. Det tredje och sista temat var patienters upplevelser av trygghet. Diskussion: I diskussionen diskuterar författarna patienters upplevelser på psykiatriska vårdavdelningar samt hur dessa påverkar dem. Författarna kom fram till fyra områden; patienters upplevelser av maktlöshet vid stigmatisering, vårdandets och miljöns påverkan på känslor av trygghet, självbestämmandets betydelse samt betydelsen av delaktighet. Dessa områden diskuterades och jämfördes med befintlig forskning och knöts an till Phil Barkers tidvattenmodell. / Background: In psychiatric care, it is important to respect the patients’ personal expectations and goals in order to achieve good treatment outcomes and to make patients feel satisfied with the care. The nurse should help patients to communicate about their inconveniences, support the patient to analyze destructive behaviors, support the patient to create a normal daily life routine and promote patients’ integrity and self-care. However, patients do not experience this in psychiatric care. Aim: The aim of this literature review was to explore patients with mental illness focusing on their experiences of care in psychiatric care units. Method: In this literature review the authors used ten scientific papers to answer the aim of the review. These articles have been examined to understand differences and similarities and then the authors performed a thematic analysis. Results: Three themes and three subthemes were identified in this literature review. The first theme was patients’ experiences of being or not being treated with respect with the associated subthemes patients’ experiences of stigmatization and patients’ experiences of the health care environment. The second theme was the experience of the staff's attitude with the associated subtheme patients’ experiences of caring relationship. The third and last theme was patients’ experiences of safety. Discussion: The authors discussed patients' experiences in psychiatric care units and how these experiences affect the patients. The discussion emphasized four areas; patients experiences of powerlessness in stigmatization, the healthcare’s and the environment's impact on feelings of safety, the importance of self-determination and the importance of participation. These areas were discussed and compared to existing research and also linked to Phil Barker's tidal model.

Bakgrund. Psykiatrisk slutenvård lider brist på kompetens och väl utnyttjade resurser för att möta patienternas komplexa behov. Specialistsjuksköterskor inom vårdformen sysslar huvudsakligen med grundläggande psykiatrisk omvårdnad. Deras breda kompetens underskattas och osynliggörs när slutenvården saknar omvårdnadsfokus. Forskning påvisar behov att tydliggöra specialistkompetensens betydelse inom psykiatrisk slutenvård, eftersom ökad förståelse inom området kan medföra bättre vård för patienten. Design. En kvalitativ design med fenomenografisk ansats användes. Metod. Fjorton semistrukturerade intervjuer med specialistsjuksköterskor verksamma i patientnära omvårdnadsarbete inom psykiatrisk slutenvård vid tre psykiatriska kliniker i norra Sverige genomfördes under tiden november 2015 – januari 2016. Intervjuerna transkriberades och analyserades sedan med kvalitativ analys. Resultat. Specialistsjuksköterskans kompetens möjliggör personcentrerad psykiatrisk omvårdnad och är betydelsefull för kunskapsspridning i personalgruppen. Betydelsen kan ökas genom att utrymme för mer patientnära arbete och utvecklingsarbete ges. Specialistkompetensen inom slutenvården behöver synliggöras och omvårdnad framhävas som behandlingsalternativ. Åtgärder för att öka specialistkompetensens betydelse kan påverka den psykiatriska slutenvårdens kvalitet och den enskilde patientens vård positivt. Slutsats. Specialistsjuksköterskans kompetens är betydelsefull för omvårdnadsarbetet inom psykiatrisk slutenvård, men får inte alltid nödvändigt utrymme för att möjliggöra vård av högsta kvalitet. Resultatet kan bidra till ökad förståelse för vikten av att framhäva specialistsjuksköterskans position, så att fler lockas att stanna kvar inom slutenvården och för att specialistkompetensen ska komma de patienter som är i störst behov av den till gagn. / The specialist nurse’s competence within the psychiatric inpatient care - An important resource that needs more space Aim. To illuminate the specialist nurse’s perception of significance of the own competence within the psychiatric inpatient care. Background. There is shortage of competence and well utilized resources to meet the patients’ needs within the psychiatric inpatient care. The competence of specialist nurses is underestimated and obscured as there is an absence of nursing focus. Clarifying the significance of the specialist competence could imply better care. Design. A qualitative design with a phenomenographic approach. Method. Fourteen semi-structured interviews with specialist nurses in patient-oriented inpatient nursing at three psychiatric clinics in north Sweden were conducted during November 2015-January 2016. The interviews were analysed by using a qualitative analysis. Result. The specialist nurse’s competence enables a person-centred psychiatric nursing and shares knowledge within the team. Enabling more patient-oriented and development related work could increase the significance of specialist competence. The specialist competence needs to be visualized and nursing highlighted as a treatment alternative. Increasing the significance of the specialist competence could affect the quality of care and influence the patient’s care positively. Conclusion. The specialist nurse’s competence is important within the inpatient care but needs more space to enable high quality care. The result demonstrates the significance to stress the specialist nurse’s position within psychiatric inpatient care. This could attract specialist nurses to stay within inpatient care and enable the competence to come to use for the patients who needs it the most.

The researcher conducted a research project based on more than 20 years of practice experience to define and describe the role and functions of the forensic nurse practitioner. A non-experimental descriptive study was done. Questionnaires and checklists were compiled to determine the perceptions of forensic psychiatric patients/clients (n =24) and nursing staff (n = 15) .The nursing activities in a selected forensic psychiatric unit was evaluated through nonparticipative observation. Results indicated that: • Patients experienced their environment as isolated and has a need for better support by the nursing staff; • Nursing staff indicated their need for more specific training in forensic psychiatric nursing; and • A need exists for specific protocols and procedures to guide his/her practice. The recommendations are made that: • A special training program for forensic psychiatric nursing needs to be planned and implemented; and • Procedures and guidelines must be established to guide the practice of the nurse practitioner. / AFRIKAANSE OPSOMMING: Daar bestaan in S.A. geen duidelike omskrywing van die rol en funksies van die forensiese psigiatriese verpleegkundige nie. Die navorser het gegrond op meer as 20 jaar praktykervaring 'n navorsingsprojek gedoen om die rol en funksies van die verpleegpraktisyn te definieer en te omskryf. 'n Nie-eksperimentele, beskrywende studie is gedoen. Vraelyste en kontrolelyste is opgestel om die persepsies van forensiese psigiatriese pasiënte/kliënte (n=24) en verpleegpersoneel (n=15) te bepaal. Die verpleegaktiwiteite in 'n geselekteerde forensiese psigiatriese eenheid is deur nie-deelnemende observasie geëvalueer. Resultate het daarop gedui dat: • Pasiënte hulle omgewing as geïsoleerd ervaar en het 'n behoefte aan beter ondersteuning deur die verpleegpersoneel; • Verpleegpersoneel het aangedui dat hulle 'n behoefte het aan spesifieke opleiding in forensiese psigiatriese verpJeging;en • Daar 'n behoefte is aan spesifieke protokolle en prosedures om sy/haar praktyk te rig . Die aanbevelings word gemaak dat: • 'n Spesiale opleidingsprogram vir forensiese psigiatriese verpleging beplan en geïmplementeer word; en • Prosedures en riglyne daargestel moet word om die praktyk van die verpleegpraktisyn te rig.
