The development of an assessment protocol for activity participation in those suffering from mental illness

(Occupational Therapy), Faculty of Health Sciences, University of the Witwatersrand, 2011 / Aim: To compile an assessment protocol, comprising of appropriate assessment methods, which can be used to assess activity participation in those suffering from mental illness. Research methods: An audit of existing performance-based instruments was carried out, and thereafter focus groups with clinicians were conducted. The methods of assessment were collected through review of existing performance-based assessments and by means of discussion groups with occupational therapists in mental health care. Following this the Activity Participation Assessment protocol was compiled. The content validity was then investigated through expert review. Results: Assessment methods for the domains of the Activity Participation Assessment were determined. Common methods of assessment included interviewing, activities, collateral information and standardised assessments. The content validity indices for all domains of assessment were above the 0.80 recommendation for new instruments. Conclusion: The Activity Participation Assessment has good content validity and can be used in the clinical setting.

Counselling psychologists' use of self in the therapeutic relationship : the role of narcissism

This study explores the potential implications of narcissism in the way that Counselling Psychologists relate to themselves and their clients. Narcissism is understood as a situation that affects negatively one’s self-knowledge and ability to engage with others. This research examines therapists’ narcissism as it may manifest in their therapeutic practice and in relation to their ability to be aware of and use effectively the interpersonal dynamics of the therapeutic relationship. The researcher also explores how her own narcissism might appear in her relationships with others and her clients. The investigator chose a heuristic qualitative method as she thought that its focus on intrapersonal and interpersonal processes would help bring to light hidden dimensions of therapists’ narcissism in their practice as they were being reflected in the relationship between the therapists and the researcher. An interest in phenomenology and especially in aspects of dialogue and intersubjectivity, as well as the researcher’s commitment to constructionist epistemology shaped the way the methodology and method were implemented. The sample consisted of nine Counselling Psychologists, who participated in open-ended semi-structured interviews. The selection was based on their interest in the subject. The data collection arose from continuous interactions between participant and researcher as a result of the on-going relationship. The research design followed Moustakas’ (1990) phases of heuristic inquiry: initial engagement, immersion, incubation, illumination, explication and creative synthesis. The data were analysed through self-dialogue as well as the interaction between the researcher and the co-researchers. Drawing on hermeneutics helped the researcher unpack complex meanings about the participants’ experience of the phenomenon. The findings of this research suggest that narcissism manifests as a false, superior persona that serves to cover feelings of insignificance stemming from the early frustration of the individual’s needs for love and attention. Practitioners’ narcissistic needs often appear in their clinical work. Male therapists tend to ask for validation and mirroring from their clients and female therapists tend to deny their needs for admiration and approval behind a selfless facade. Nevertheless, both functions can be found in the same person. Narcissistic needs and vulnerabilities are frequently dissociated, denied and projected onto others, as their acknowledgement possibly causes a great amount of shame. By being open to their narcissism and their hidden needs for love and approval practitioners seem to become more able to reflect and relate openly to themselves and their clients. This increased ability can be associated with what Symington (1993) calls a “reversal” of narcissism. This research suggests that through acknowledging their narcissism and reflecting on their deep and hidden emotions practitioners can more easily engage in an honest and mutual exploration with their clients, which can help them to reach a greater knowledge about themselves. The researcher also found that her own narcissistic vulnerabilities, if unacknowledged can stop her from engaging openly and freely in the relationship with clients. The importance of looking at the therapists’ emotional baggage and commitment to achieve greater interpersonal growth, which is important to the field of Counselling Psychology, is emphasised in this study. Future researchers are encouraged to look at the clients’ perceptions of therapists’ ability to acknowledge their narcissistic vulnerabilities in order to attain a wider knowledge about the phenomenon.

A partir da práxis clínica e institucional, tentamos fundamentar uma modalidade de psicotherapia na qual a psicanálise do campo de Freud e Lacan é aplicada. Especificamos o enfoque desta reflexão no campo da Saúde Coletiva e na clínica do recalcamento (contexto das "neuroses"). Partimos do Dispositivo Intercessor, como um novo Modo de Produção de subjetividade e "conhecimento". De natureza transdisciplinar, o Dispositivo Intercessor parte, principalmente, da psicanálise e do Materialismo Histórico - bem como de inspirações da Análise Institucional francesa e da Filosofia da Diferença - para definir dois momentos de produção radicalmente diferentes: o momento da práxis clínica junto aos "sujeitos do tratamento" e da práxis institucional junto ao "coletivo de trabalho"; e o momento da reflexão teórica, produzida a posteriori, sobre o processo de produção realizado no primeiro momento. Se Freud idealizou a possibilidade de inserção da psicanálise nas instituições públicas, nossas reflexões pretendem demonstrar que essa empreitada é tão possível quanto eticamente necessária. An investigation of attitudes towardss the practice of school-based psychological services

The provision of school based psychological services in Victorian primary and secondary schools dates back to well before the Second World War. Since then, however, the activities that make up the role of school psychologists have changed substantially. School psychologists' roles have become more varied than the original psychometric focus and reflect a more systemic approach to the conceptualization of student problems. Within Australia, school psychologists can be found servicing single or multiple schools in the government, independent and Catholic school sectors, fulfilling a range of functions and dealing with a diversity of student issues. However, Australian academic research into the professional practice issues associated with the provision of school-based psychological services is rare. Therefore, this thesis sought to investigate a range of professional issues associated with the provision of school-based psychological services for Victorian school psychologists working in single and multiple schools in the government and non-government primary and secondary school sectors. In addition to surveying Victorian school psychologists, principals and teachers were also surveyed in order to ascertain their attitudes towards school-based psychological services. The sample consisted of 81 school psychologists, 21 principals and 86 teachers. The results revealed that school psychologists participate in a variety of activities, including a number of activities that reflect a systemic model of service delivery. They also deal with a broad range of student issues, some of which are quite serious in nature. However, the study also revealed a number of professional issues that were in need of improvement. Some of these included a lack of participation in regular supervision for school psychologists, school psychologists' dissatisfaction with some industrial and professional conditions associated with their role and differences in attitudes between psychologists, principals and teachers regarding the activities and responsibilities of school psychologists. Results from the study provide plausible evidence for the need to support school psychologists in the valuable work that they do within schools through improved industrial conditions, appropriate professional development, and regular supervision. Furthermore the results also reveal a need to educate and participate in dialogue with the educational community in order to increase understanding of school psychologists' roles and professional responsibilities.

O objetivo deste estudo é examinar, entre crianças que estiveram em atendimento psicológico numa clínica-escola, variáveis que possam ter influenciado no abandono de seus processos psicoterápicos. Para realizar este estudo foram analisados protocolos de atendimentos no arquivo de uma clínica-escola, localizada na região metropolitana de Porto Alegre, para examinar os casos de atendimento de crianças de 3 a 12 anos de idade, no período de 1999 a 2006. A inserção das entrevistas iniciais em uma instituição de saúde pública demandou a análise do funcionamento institucional, onde a organização das funções da instituição presentes em seu discurso é que produzem a subjetividade, sendo também produtoras da exclusão do indivíduo em decorrência dos conflitos institucionais entre as práticas instituídas e os movimentos em curso dentro da instituição. Concluiu o presente estudo que os pacientes aqui mencionados estabelecem a relação inicial em psicoterapia através da expressão de seus sintomas, sendo assim, o uso da técnica embasada no referencial psicanalítico será mais produtiva se efetuar a relação da linguagem dos sintomas e da história de vida de cada indivíduo, em detrimento da fala como expressão de sentimentos. / The mental health in the public health follows the historical trajectory that later had origin in European countries and U.S.A. in the postwar period and in the proper Brazilian movement and state native of São Paulo who searchs the humanização through antimanicomiais movements and of democratization of the services. In this historical context, the present research searched to understand as patient inserted in the services of public health, that present physical complaints, they establish the initial relation in psycotherapy of psychoanalític orientation. For in such a way we use studies of the psychosomatic one and we identify that the expression through the body becomes it main way of communication, where this form of representation of the internal contents constitutes the identity of the citizen, and for times, the abandonment of the treatment from the initial interviews occurs as form of maintenance of the symptoms and preservation of the identity constituted. The insertion of the initial interviews in an institution of public health demanded the analysis of the institucional functioning, where the organization of the functions of the institution gifts in its speech is that they produce the subjectivity, being also producing of the exclusion of the individual in result of the instituted institucional conflicts between the practical ones and the movements in course inside of the institution. It concluded the present study that the patients mentioned here establish the initial relation in psycotherapy through the expression of its symptoms, being thus, the use of the technique based in the psychoanalític referencial will be more productive to effect the relation of the language of the symptoms and the history of life of each individual, in detriment of speaks as expression of feelings.

Concluiu o presente estudo que os pacientes aqui mencionados estabelecem a relação inicial em psicoterapia através da expressão de seus sintomas, sendo assim, o uso da técnica embasada no referencial psicanalítico será mais produtiva se efetuar a relação da linguagem dos sintomas e da história de vida de cada indivíduo, em detrimento da fala como expressão de sentimentos. / Abstract: The mental health in the public health follows the historical trajectory that later had origin in European countries and U.S.A. in the postwar period and in the proper Brazilian movement and state native of São Paulo who searchs the humanização through antimanicomiais movements and of democratization of the services. In this historical context, the present research searched to understand as patient inserted in the services of public health, that present physical complaints, they establish the initial relation in psycotherapy of psychoanalític orientation. For in such a way we use studies of the psychosomatic one and we identify that the expression through the body becomes it main way of communication, where this form of representation of the internal contents constitutes the identity of the citizen, and for times, the abandonment of the treatment from the initial interviews occurs as form of maintenance of the symptoms and preservation of the identity constituted. The insertion of the initial interviews in an institution of public health demanded the analysis of the institucional functioning, where the organization of the functions of the institution gifts in its speech is that they produce the subjectivity, being also producing of the exclusion of the individual in result of the instituted institucional conflicts between the practical ones and the movements in course inside of the institution. It concluded the present study that the patients mentioned here establish the initial relation in psycotherapy through the expression of its symptoms, being thus, the use of the technique based in the psychoanalític referencial will be more productive to effect the relation of the language of the symptoms and the history of life of each individual, in detriment of speaks as expression of feelings. / Mestre
