Using health-related quality of life instruments for children with long-term conditions : On the basis of a national quality registry system

Introduction: There has been a continuous development of new technologies in healthcare that are derived from national quality registries. However, this innovation needs to be translated into the workflow of healthcare delivery, to enable children with long-term conditions to get the best support possible to manage their health during everyday life. Since children living with long-term conditions experience different interference levels in their lives, healthcare professionals need to assess the impact of care on children’s day-to-day lives, as a complement to biomedical assessments. Aim: The overall aim of this thesis was to explore and describe the use of instruments about health-related quality of life (HRQOL) in outpatient care for children with long-term conditions on the basis of a national quality registry system. Methods: The research was conducted by using comparative, cross-sectional and explorative designs and data collection was performed by using different methods. The questionnaire DISABKIDS Chronic Generic Measure -37 was used as well as semi-structured interviews and video-recordings from consultations. Altogether, 156 children (8–18 years) and nine healthcare professionals participated in the studies. Children with Type 1 Diabetes (T1D) (n 131) answered the questionnaire DISABKIDS and children with rheumatic diseases, kidney diseases and T1D (n 25) were interviewed after their consultation at the outpatient clinic after the web-DISABKIDS had been used. In total, nine healthcare professionals used the HRQOL instrument as an assessment tool during the encounters which was video-recorded (n 21). Quantitative deductive content analysis was used to describe content in different HRQOL instruments. Statistical inference was used to analyse results from DISABKIDS and qualitative content analysis was used to analyse the interviews and video-recordings. Results: The findings showed that based on a biopsychosocial perspective, both generic and disease-specific instruments should be used to gain a comprehensive evaluation of the child’s HRQOL. The DISABKIDS instrument is applicable when describing different aspects of health concerning children with T1D. When DISABKIDS was used in the encounters, children expressed positive experiences about sharing their results with the healthcare professional. It was discovered that different approaches led to different outcomes for the child when the healthcare professionals were using DISABKIDS during the encounter. When an instructing approach is used, the child’s ability to learn more about their health and how to improve their health is limited. When an inviting or engaging approach is used by the professional, the child may become more involved during the conversations. Conclusions: It could be argued that instruments of HRQOL could be used as a complement to biomedical variables, to promote a biopsychosocial perspective on the child’s health. According to the children in this thesis, feedback on their results after answering to web-DISABKIDS is important, which implies that healthcare professionals need to prioritize time for discussions about results from HRQOL instruments in the encounters. If healthcare professionals involve the child in the discussion of the results of the HRQOL, misinterpreted answers could be corrected during the conversation. Concurrently, this claims that healthcare professionals invite and engage the child.

Evaluating and expanding knowledge and awareness of health professionals on the consumption and adverse consequences of Novel Psychoactive Substances (NPS) through innovative information technologic tools

Background: The rapid diffusion of Novel Psychoactive Substances (NPS) constitutes an important challenge in terms of public health and a novelty in clinical settings, where these compounds may lead to erratic symptoms, unascertained effects and multi-intoxication scenarios, especially in emergency situations. The number of NPS available on the illicit drug market is astonishing: official reports suggest the appearance of a new drug every week. NPS may be enlisted in many different families such as synthetic phenethylamines, tryptamines, cathinones, piperazines, ketamine-like compounds, cannabimimetics and other plant-derived, medical products and derivatives. Therefore, healthcare services and professionals are often called to face this unknown 'galaxy' where NPS users seem to perceive traditional services 'unfitting' for their needs, requiring an attention which is quite different from known classic drug abusers. In this context, the Recreational Drugs European Network (ReDNet), a research project funded the European Commission and led by the University of Hertfordshire, aimed to explore the NPS galaxy and develop information tools for vulnerable individuals and professionals working with them. This initiative reported specific Technical Folders on new drugs and disseminated the collected information through innovative communication technologies (e.g. multimedia tools, social networking and mobile phone services) internationally. Aim and objectives: The aim of this work is to evaluate and contribute to expand the knowledge of health professionals on NPS. The key objectives are: 1) to assess the level of knowledge on NPS amongst a sample of Italian healthcare professionals; 2) to evaluate the effectiveness of dissemination tools developed by ReDNet, including an SMS-Email/mobile service (SMAIL); 3) to understand the clinical impact of NPS by providing four Technical Folders and collecting two clinical cases on NPS. Methodology: According to the objectives, the methodological approach has been articulated in the following three phases. Phase 1: investigating knowledge and preferred channels of information via an online survey among health professionals in Italy. This first Italian study on NPS awareness had been online from February to July 2011, recruiting participants from Departments of Addiction, Psychiatry and other services. Phase 2: evaluating the ReDNet initiative. An evaluation questionnaire was designed and disseminated online to assess the various resources provided by ReDNet project; it had been online from April to July 2013, targeting professionals registered to ReDNet services. This phase also investigated the SMAIL service, a mobile application that was the latest technological tool developed by ReDNet team. Phase 3: promoting evidence based work in clinical practice through the preparation of four Technical Folders and two case reports. Technical Folders followed the methodology optimised during the ReDNet experience, organising NPS data under specific headings, measured for the need of health professionals. Case reports were collected in a Dual Diagnosis Unit in Italy ('Casa di Cura Parco dei Tigli'); assessed patients revealed for the first time the use of NPS; clinical interviews were conducted to collect a full anamnesis while for the first time psychopathological characteristics were measured in NPS abusers, using a psychometric instrument (MMPI-2). Results: In Phase 1 Italian services, in particular interviewees (n=243) from Departments of Psychiatry and Addiction, showed a strong interest for the subject but a poor understanding of NPS: 26.7% of respondents did not know if their patients ever used NPS; at the same time they considered this phenomenon as very relevant to their profession (e.g. psychomotor agitation [75.7%], errors in the assessment [75.7%], management of the clients [72%]); in addition less of a quarter of them had reliable information on new substances. Interviewees also reported the need for easily accessible channels of information to expand their expertise in the field (including emails [70%] and dedicated websites [51.9%]). The ReDNet initiative (Phase 2) reached professionals (n=270) from European countries and various other regions; they appreciated the website above all (48.5%), which provided access to other information (in form of academic papers, news, technical folders, etc.). The integration of technological-based and classic educational resources was used to self-educate professionals (52.6%) and supply information for research (33.7%) with up-to-date and 3 reliable information; in the same Phase the SMAIL service was analysed in its first 557 searches: in the pilot period 122 professionals used SMS inquiries (95%), asking information on NPS while highlighting the increasing number of NPS available on the market. Technical folders (Phase 3) described two new phenethylamines (Bromo-dragonfly and 25I-NBOMe), a novel ethno drug (Kratom) and a new synthetic cathinone (alpha-PVP) whose severe effects were also described in one of the clinical cases. The first case report (Alice) involved a clubber who used mephedrone and other NPS with a severe worsening of her psychiatric disturbances; the second one (Marvin) described a patient who was referred by a psychiatric service and revealed himself as a 'psychonaut' with an intense abuse of alpha-PVP. Conclusions: The exploration of the NPS galaxy is a new challenge for healthcare professionals. In this study, Italian services seemed to be unprepared to face the emergency and requested rapid access to reliable information; the ReDNet project provided both technology-based and traditional resources to expand knowledge on NPS, making professionals more aware of emerging issues and helping especially clinicians working in the field (e.g. via SMAIL service and Technical Folders). Overall, it can be observed that effective information services on NPS targeted at professionals initiatives should include an online interface integrating up-to-date information, describing NPS through specific Technical Folders and disseminating scientific literature; the use of technological tools, including mobile applications, is an important strategy to support health professionals in their activity. Finally, more 'visual' guidelines, possibly in the form of a 'map' of these heterogeneous compounds, could be a useful framework to describe NPS to physicians and other professionals who are often unprepared and unconfident to face such an expanding galaxy.

Školní úrazy při hodinách tělesné výchovy na 1. st. základní školy / School Injuries in Physical Education Classes at the Primary School

The aim of thesis is to provide a comprehensive picture of the selected areas of frequent school injuries in physical education classes in primary schools not only pedagogical, but also from a health point of view. The theoretical part deals with injuries of younger school age children, school injuries (especially in physical education classes) and their preventive measures, safety at work in physical education classes. It defines the most common types of injuries in physical education classes in primary schools, their prevention and following first aid. The work shows what to do in case of an accident of pupil and how to properly proceed directly at scene of the accident. To avoid legal penalties for teacher, we also mention the right way to report injury, injury record and we deal with book of injuries. Empirically oriented part and its questionnaire survey demonstrates what level of knowledges are reaching selected physical education teachers of elementary schools in the area of legal issues, injury prevention and laic first aid. The aim of this thesis is to show evidence and map a continuous educational development in the town of Loděnice in the Beroun region, where it is situated. The primary resources the author works with are school chronicles kept in the State regional archive in Beroun and school funds in Loděnice. The thesis has got a historical character, the author therefore uses work with sources, historical-comparative method, analysis, synthesis and interview. The thesis shows a specific period of this school, i.e. its development in 1782-1945. The author using the archive resources documents the educational development in Loděnice town in respect to the specified criteria. Furthermore, she maps the region of Beroun, its agricultural and industrial development. She highlights what influence the development in these areas had on education. The dissertation focuses on the assessment of the content of the inspection reports of the Czech School Inspectorate (CSI) in the context of the main stategical documents concerning the Czech educacional policy since the publication of the National Programme of the Development of Education in the Czech Republic (The White Book) in 2001. The thesis comprises of the historical introduction of the state supervision over schools in Bohemia and the extent of competency of the Czech School Inspectorate, which is currently conducting the schools external evaluation. The CSI provides the feedback to the schools through the inspection reports and the feedback to the Ministry of Education, Youth and Sports through the annual reports. The dissertation examines the compliance of the contents of the inspection reports and the strategical documents of the Ministry of Education.

Hodnoceni kvality a vykonnosti poskytovatelu zdravotni pece na Slovensku na bazi administrativnich udaju / Quality and performance assessment of healthcare providers in Slovakia on the basis of administrative data

The aim of this thesis was to explore options for hospital profiling in the Slovak republic. Sacrificing breadth of the study in favor of depth, the scope of the analysis was narrowed down to one quality indicator only - mortality. In the first step a mortality prediction model was constructed in order to predict expected probability of death on the basis of a set of risk factors in order to filter away variation in hospital outcomes that is caused by other factors than quality of care. Validation of the model was performed on a validation sample of 25% of data. Discriminative ability of the final model is very high - c-statistics over 0.9. Furthermore, we verified that hospitals differ in the risk structure of their patient populations significantly - mean predicted probability of dying for hospitals differed from 0.02% to 33%. In the second step hospital profiling was performed. Standardized mortality ratios were calculated for each hospital as a difference between observed and expected number of deaths. After introduction of risk-adjustment and calculation of confidence intervals 43% of hospitals were re-classified. 30-day mortality was selected as the best indicator for hospital profiling.

Syftet med denna studie är att ta reda på vilka skillnader det finns mellan Dagens Nyheter och Aftonbladets nyhetsrapportering av de Jugoslaviska krigen. De metoder som används i studien är en retorisk och semiotisk analys på lingvistisk och bilder. Teorier som används är framing och representation. Tre händelser i tre krig mellan åren 1991 och 1995 analyseras: Tiodagarskriget i Slovenien, belägringen av Dubrovnik och Srebrenicamassakern. Resultatet på frågeställningen; alltsaå vilka skillnader som finns mellan DN:s och Aftonbladets rapportering, är att mängden rapportering, textinnehåll och tidningarnas fokus var de största markanta skillnaderna. DN har rapporterat mer än Aftonbladet och har ett större fokus på att skriva och dokumentera det som politiskt eller krigiskt, samt intervjuar ledare och människor med makt. Aftonbladets rapportering innehåller mer vittnesskildringar och strävar efter att beröra folk samtidigt som de informerar. / The purpose of this study is to find out what kind of differences there are between Dagens Nyheter and Aftonbladets news reporting of the Yugoslavian wars. Methods that are used are a rhetorical- and semiotic analysis of both photos and text and the theories that are used are framing and representation. Three events of three wars between the years 1991 and 1995 are being analysed: The ten day war in Slovenia, the besiege of Dubrovnik and the massacre in Srebrenica. The results show that the amount of reporting, the content of the articles and their focus are the biggest differences between DN and Aftonbladet. DN reported more than Aftonbladet and aimed their focus on documenting politically or warlike and interviewed people with authority. Aftonbladets content is more about the point of view of the witnesses and aimed to affect readers as well as inform them.

Assessment is very much a process of measurement, whether the outcome is used for baseline, diagnostic, formative or summative purposes. When measurement is taken, in whatever form, a score is obtained. The score that is obtained forms the important part of assessment, because this score determines the outcome of the assessment, the decisions that are to be made regarding the student’s progress, curriculum changes and the evaluation of a course as a whole. Although a score is obtained from a test, the analysis thereof is frequently much neglected. The use of computers in education is not a new concept. The first computer application goes back a long way when computers were first used to do psychological testing. It then became clear that computers can be applied to more fields in education, especially in the field of testing. In the early days real progress was slow, since computers were expensive and were only used in large companies. However, the scenario has changed with the widespread availability of personal computers that has enabled educators to focus on the appropriate role of computerisation in the development, administration, scoring and interpretation of tests. The main objective of this study is to show the major advantage of using computers as a comprehensive assessment tool and to demonstrate the ability to construct and ‘bank’ test items to subsequently produce a standardised test. An added advantage was the computer’s ability to administer tests to students and manage student progress records. The research findings indicate that a Comprehensive Computer-Assisted Assessment Tool (CCAT) has the potential to contribute to the enhancement of assessment and that it can enable educators to prepare valid, reliable and fair test items which were more difficult and time-consuming without technology.

Finansiella och icke-finansiella målsättningar : En kvantitativ studie av börsnoterade företags målsättningar i årsredovisningar

I årsredovisningar kan företag frivilligt ge ut information utöver det som krävs av regler, principer och praxis. En vanlig typ av sådan frivillig information är målsättningar av olika slag. Men varför olika företag väljer att presentera olika antal målsättningar finns det dock inget tydligt svar på. I tidigare forskning har främst finansiella mål studerats, men då dagens informationssamhälle inte bara fokuserar på finansiella resultat finns en stor del övriga mål som företags intressenter kan komma att efterfråga. Syftet med denna studie är att förklara hur många och vilka målsättningar som börsnoterade företag väljer att redovisa samt förklara vilka samband som föreligger mellan antal målsättningar och variablerna: företagsstorlek, lönsamhet, skuldsättningsgrad, Tobins q och bransch. Denna studie är utförd genom en kvantitativ metod med en deduktiv ansats. Data samlades in från årsredovisningar genom en kvantitativ innehållsanalys. För att mäta sambanden mellan studiens beroende och oberoende variabler utfördes regressions- och korrelationsanalyser. Resultatet visade att 94 % av företagen redovisade någon målsättning, 88 % redovisade finansiella mål och 66 % icke-finansiella mål. Medelantalet för finansiella mål respektive icke-finansiella var 3 per företag. Antal totala målsättningar uppvisade ett positivt samband med företagsstorlek och lönsamhet. För finansiella målsättningar fanns ett positivt samband med skuldsättningsgrad medan icke-finansiella målsättningar inte uppvisade något signifikant samband med någon av variablerna. Även skillnader mellan olika branscher uppvisades, där konsumentvaror hade högst medelvärden, medan energi hade lägst. Dessa förhållanden gällde för målsättningar totalt samt uppdelat på finansiella och icke-finansiella. / In annual reports, companies can voluntarily disclose information that go beyond what rules, guidelines and praxis require. One kind of voluntary disclosure that is commonly occurring is different targets. But there is no distinct explanation to why different companies disclose different number of targets. The subject of earlier studies has mainly been financial targets, but in today’s information society financial results isn’t the only focus. Therefore, stakeholders may also be demanding non-financial information and targets. The aim of this study is to explain the number of and which targets listed companies choose to disclose. The aim is also to explain if the number of targets correlates with the variables: company size, profitability, leverage, Tobin’s q and industry. This study was executed through a quantitative method with a deductive approach. Data was collected from annual reports using a quantitative content analysis. To measure the correlation between the study’s dependent and independent variables, both regression and correlation analysis were performed. The result showed that 94 % of the companies disclosed some kind of target, 88 % disclosed financial targets and 66 % non-financial targets. The average number of financial targets was three per company, likewise for the non-financial targets. The number of targets in total proved to be positively correlated with company size and profitability. The financial targets proved to be positively correlated with leverage, while the non-financial targets didn’t show any significant correlation with any of the variables. Differences between industries were also shown, where consumer goods had the highest mean values, while energy had the lowest. These conditions did apply for the number of targets in total, financial targets and non-financial targets.
