Komunitní péče v psychiatrii / Community care in psychiatry

Abstract Diploma thesis deals with the issue of community care in psychiatry and its objective is to describe the importance of a nurse in providing this care. Community psychiatric care is a very wide area of an intermediary patient assistance designed to help the patients in any areas of their lives. Although in the Czech Republic community care has not achieved the appropriate development yet, its benefits for patients are important now and its importance has been growing. In the theoretical part, after a brief introduction to community care issues, history, principles, but also community care connection with nursing care, are mentioned. Subsequently, attention is paid to particular areas of community care that are important for the mentally ill and the role of a nurse in these areas is described. The approach to the mentally ill, the issue of stigmatization and psychiatric care organizations are mentioned. In the theoretical are part, psychiatric illnesses that may occur in community care are also analyzed and it is explained how community care may be beneficial for psychiatric patients. In the practical part, the objective was to find out the level of psychiatric nurses´ awareness of community care and to determine if they see the sense of community care in psychiatry. Making a survey of community care in which nurses can be involved was another objective of the work. The nurses´ responses to the stated hypotheses were statistically analyzed within quantitative research. The purpose of hypotheses stated was to determine whether nurses with the work experience longer than ten years are more likely to believe that psychiatric care is more beneficial for patients than hospitalization., And also, if nurses with an education higher than secondary have better awareness of community services provision. Other hypotheses were focused on finding out whether nurses consider the most common community care services issue in the Czech Republic to be housing promotion and if nurses aged over thirty years are more aware of the nurses´ role importance in community care. None of those hypotheses were confirmed. The hypothesis assuming that nurses obtain more information on community care from the Internet and literature than at workshops was confirmed. Psychiatric nurses working in mental homes in the South Moravian region and the Vysočina region took part in this research. A component of the practical part is also an analysis of community services in these regions. In both the regions 13 civic associations and community centers were mapped.

The ward sister round presents an integral part of the nursing profession. The ward sister round comprises a regular visit of the nurse directly at the patient´s bed. Its objective consists in determining and satisfying the unsaturated needs of patients, further in ensuring and providing a high quality and efficiency nursing care in a close link with medical care. Nursing handover can be divided into two main sections: written and oral handover. The oral handover takes place directly in the patients´/clients´ room under the use of nursing documentation. Its advantage consists in the direct involvement of the patient/client in the handover process. The written handover is then entered by the nurses into the Handover Book. The significance of my diploma work consists in increasing the nurses´ standard of knowledge related to the modernization of ward sister rounds and nursing handover as an integral part of the nursing profession.

This work is focused on communication improvement in tracheostomized and laryngektomized patients. I have chosen the topic because, according to my opinion, communication between people is very important. If there is an obstacle on communication, it brings about a lot of problems in human life which is very stressful indeed. By tracheostomy and laryngectomy performing, a barrier in spoken verbal communication arises and therefore it is important to address this issue and focus ourselves on the help to these patients. After tracheostomy the function of vocal cords is disabled and laryngectomy means a surgical removal of the larynx. In these clients, either temporarily or permanently, disruption in spoken verbal communication occurs. There are several ways how to compensate the spoken verbal communication. As a substitute, however, the most frequently used is the written verbal communication. According to my opinion it would be appropriate to include pictograms into the care of these patients. Pictograms are cards with pictures. When they are used for communication in hospital, they are supplemented with words or short phrases that clearly express the particular notion or feeling. The cards are supposed to help a client who has problems with verbal communication, but also health care workers. Therefore we set a goal to create pictograms and bring them into practice, then to determine whether the use of pictograms in practice contribute to a better communication between nurses and a tracheostomized or a laryngectomized patient and to find out whether the use of pictograms has an influence on cooperation between the family of a tracheostomized or laryngectomized patient and members of the health care team. We also wondered if patients and nurses would recommend or would not recommend the continued use of pictograms in practice.

This thesis adverts to an educational problems of parents, how do they perceive manner and content of information presented, how do they adapt to their child?s disease, what do they lack and what restricts them from satisfying their needs. Complex care for small patients suffering from musculoskeletal disorder should also encompass a degree of care towards parents. A nurse must, likewise, refer to their needs, knowledge, attitude, care about their mental condition, and provide them during the whole time with professional help and mental encouragement throughout coping with their difficult and demanding life situation, which the disease, the musculoskeletal disorder of their child, undoubtedly is. An outlook of nurses of this problem is also a part of this thesis.

The aim of this thesis is, through questionnaire survey, to compare the position of men and women working as general nurses. Then, on the basis of gained results and determined hypothesis evaluate, if men are perceived same as women, whether their low number is caused by gender prejudices and if healthcare personnel and the general public want more men on this position. Theoretical part is devoted to gender, history of nursing, education and specification of general nurse's work and position of men and women in health care in the Czech Republic and the world. The research part deals with the analysis and interpretation of results from the questionnaire surveys designed not only for general nurses, but also for the public.

The aim of thesis was to map the care of nurses and paramedics for patients on continuous renal replecemant therapy. Find out what nurses/paramedics have difficulty taking care of the patients and what would have welcomed into practice to improve care. The theoretical part focuses on the description of the method of elimination, nursing care and competence of health professionals in the care of patients on elimination method. The research was compiled by the method of quantitative research, for whose realization was used a questionnaire. The questionnaire was intended for nurses and paramedics caring for patients on continuous elimination method. For statistical data processing was used responses from 105 respondents working in the Institute of Clinical and Experimental Medicine. The results of the survey indicate that nurses and paramedic extensively involved in the specialized care of patient with continuous elimination method. Medical staff perceived as the most difficult managing a large number of skills associated with the preparation and handling of elimination device in this area. On the basis of these findings was prepared brochure focusing on continuous elimination method, which is the output of this work. Intensive care unit delirium from nurse's perspective Introduction: Delirium is a disturbance of consciousness, one of the most frequent organ dysfunctions in the ICU. Delirium occurs in 60 - 80% patients who receive mechanical ventilation and is independently associated with increased mortality and development of post-ICU neurocognitive deficit in adult ICU patients. It is recommended to monitor delirium routinely with validated screening tools (CAM-ICU or ICDSC). Experienced and well educated nurse, armed with appropriate diagnostic tools, can actively participate in prevention, diagnosis and therapy od delirium. Objective: This Master diploma thesis objective was to get to know current knowledge of nursing staff working in Czech ICUs about delirium and their clinical praxis. Method: Quantitative research was conducted with a questionnaire filled by nursing staff of ICUs for adult patients in two university and one regional hospital in Czech Republic from August to September 2017. Results: Altogether, 110 questionnaires from 130 have returned (84% return). The outcomes showed that 63% of nurses are sufficiently informed on ICU delirium, 66% would appreciate more education on the subject. (v AJ) Introduction to Issue: The examined issue of the thesis is the influence of burnout and its development on the sexuality of women in health care in emergency care. Since there are a few nurses working in hospital facilities, I think they are in great danger of burnout. Therefore, it is necessary to investigate this issue and discover possible solutions to avoid burnout. The objective of the thesis is to find out whether burnout syndrome and its development influence the sexuality of non-medical health workers. Methodology: The research was conducted by means of a quantitative method using the online questionnaire on the website Ošetřovatelství.info. The data were collected using two questionnaires, the first focused on burnout (the Czech version of the Shirom and Melamed scale) and the second, which investigated the sexual function of the respondents (KRATOCHVÍL, Stanislav. Treatment of Sexual Dysfunctions). All nurses and female paramedics working in intensive care were included in the research sample.
