Einfluss des Proteins p8 auf das Immunsystem im Verlauf der akuten Pankreatitis im Mausmodell

Die Pathophysiologie der akuten Pankreatitis ist durch eine immunologische Dysregulation gekennzeichnet, deren genaue Mechanismen unvollständig verstanden sind. Zur Untersuchung des Einflusses des Stressproteins p8 wurde die akute experimentelle Pankreatitis in einer p8-defizienten Mauslinie induziert und das Immunsystem phänotypisiert. Auf die Entwicklung des Immun- und Organsystems hat eine p8-Deletion keinen Einfluss, jedoch ist die Immunseneszenz verbunden mit einer Splenomegalie, Milzsiderose und extramedullärer Hämatopoese. Die akute Pankreatitis verläuft bei Fehlen von p8 wesentlich schwerer, begleitet von einem stärkeren Verlust an CD4+ T-Zellen in Blut und Milz. In der Phase der Rekonvaleszenz kommt es unter p8-Defizienz zu einem gehäuften Auftreten lymphozytärer Apoptosen in der Milz, begleitet von einer schweren Mikroarchitekturstörung und Verlust von Lymphfollikeln. Es kann erstmals eine antiapoptotische Wirkung von p8 auf aktivierte Milzlymphozyten während der postinflammatorischen Immunrekonstitution postuliert werden, wobei die involvierten Signalwege weiter untersucht werden müssen.

The consequences of its use on the liver tissues of rats with or without diabetes mellitus (DM) have not been fully explored. The purpose of this multidisciplinary study was to evaluate the correlation between plasma hepatic enzyme levels and the presence of iron overload in the hepatic tissue of female Wistar rats with or without streptozotocin-induced DM and using TX. Female rats were studied in control groups: C-0 (non-drug users), C-V (sorbitol vehicle only) and C-TX (using TX). DM (diabetic non-drug users) and DM-TX (diabetics using TX) were the test groups. Sixty days after induced DM, blood samples were collected for glucose, alanine aminotransferase (ALT), aspartate aminotransferase (AST) alkaline phosphatase (ALP) and bilirubin measures. Hepatic fragments were processed and stained with hematoxylin and eosin (H&E), Masson s trichrome, Perls. The hepatic iron content was quantified by atomic absorption spectrometry. AST, ALT and ALP levels were significantly elevated in the DM and DM-TX groups, with unchanged bilirubin levels. Liver iron overload using Perls stain and atomic absorption spectrometry were observed exclusively in groups C-TX and DM-TX. There was positive correlation between AST, ALT and ALP levels and microscopic hepatic siderosis intensity in group DM-TX. In conclusion, TX administration is associated with liver siderosis in diabetic and non-diabetic rats. In addition, TX induced liver iron overload with unaltered hepatic function in 2 non-diabetic rats and may be a useful tool for investigating the biological control of iron metabolism / O tamoxifeno? Amostras hep?ticas foram coradas com hematoxilina e eosina, Tricr?mio de Masson e Perls. O conte?do hep?tico de ferro foi quantificado por espectrometria de absor??o at?mica. AST, ALT e ALP apresentaram-se significativamente elevados nos grupos DM e DM-TX, com bilirrubina n?o alterada. Siderose ? colora??o Perls e pela espectrometria de absor??o at?mica foram observados apenas nos grupos C-TX e DM-TX. Houve correla??o positiva entre os n?veis de AST, ALT e ALP e a intensidade da siderose hep?tica microsc?pica no grupo DM-TX. Em conclus?o, o uso de TX ? associada com siderose hep?tica em ratas diab?ticas ou n?o. Introdução: A hiperferritinemia (HF) pode refletir o estado inflamatório do paciente com doença hepática gordurosa não alcoólica (DHGNA) e obesidade, porém em cerca de 33% reflete uma real sobrecarga de ferro hepática. A síndrome dismetabólica relacionada ao depósito de ferro (SDDF) é caracterizada por HF, saturação de transferrina normal, alterações metabólicas e um discreto depósito de ferro hepático. O método padrão ouro para o diagnóstico de sobrecarga de ferro é a biópsia hepática. Por se tratar de método invasivo, novos métodos se tornam necessários. Dentre eles a ressonância nuclear magnética (RM) é o mais disponível. Pacientes e Métodos: O presente estudo avaliou pacientes com HF, excesso de peso, e DHGNA comprovada por biópsia hepática. Os pacientes realizaram RM pela técnica de relaxometria (análise de gordura/ferro - máquina 3T), dosagem de marcadores inflamatórios (IL-6 e TNF-alfa), análise da expressão das subunidades da ferritina (FP - cadeia pesada e FL - cadeia leve) e dosagem de hepcidina. Os dados foram comparados com a biópsia hepática. Estes pacientes foram classificados segundo os níveis de ferritina e siderose. Foram classificados em ferritina normal (FN), hiperferritinemia dismetabólica (HFD) e SDDF. O grupo de FN foi caracterizado por ferritina < 200 ng/mL em mulheres e 300 ng/mL em homens, já os pacientes do grupo HFD têm níveis elevados de ferritina e ausência de siderose hepática. Os pacientes também foram avaliados considerando somente a siderose hepática. Resultados: Foram avaliados 152 pacientes com DHGNA comprovada pela biópsia hepática, sendo 67 incluídos no estudo. A frequência de SDDF e HFD foi de 37% e 16%, respectivamente. O nível de corte da ferritina sérica, a fim de identificar depósito de ferro, foi de 180.4 ng/mL para mulheres (sensibilidade de 76,9%, especificidade de 64,3%) e 350,7 ng/mL para homens (sensibilidade de 72,7% e especificidade de 75,5%). Os pacientes com SDDF E HFD não apresentaram piores características metabólicas e histológicas. Os níveis de hepcidina foram maiores nos grupos SDDF E HFD, se correlacionando com o conteúdo de ferro hepático quando > 30,2 ng/mL. Os marcadores inflamatórios (IL-6 e TNF-alfa) foram semelhantes entre os grupos. A expressão de FP e FL se comportaram de modo semelhante entre os grupos. A FL se correlacionou com síndrome metabólica e circunferência abdominal, enquanto a FP se correlaciona com níveis maiores de esteatose. A RM foi capaz de identificar o conteúdo de ferro destes pacientes, sendo o ponto de corte de R2* 58,9s-1. A porcentagem de gordura, identificada pela RM, foi de 17% nos grupos HFD e SDDF e de 13% no grupo FN. Conclusão: 1) RM utilizando a técnica de relaxometria com correção da interferência de gordura se mostrou acurado na avaliação de discreto depósito de ferro em pacientes com SDDF; 2) HF pode refletir depósito de ferro hepático quando acima de 180,4 ng/mL para mulheres e 350,7 ng/mL para homens, não foi encontrada relação entre HF e características piores metabólicas ou histológicas; 3) FL se correlacionou com síndrome metabólica e circunferência abdominal, enquanto a FP se correlaciona com esteatose; 4) Hepcidina se correlacionou com siderose e ferritina sérica nos pacientes com DHGNA / Introduction: Hyperferritinemia (HF) may reflect the inflammatory status of patients with NAFLD and obesity, but about 33 % reflects a real hepatic iron overload. Pronostic à long terme des hémorragies intra-cérébrales / Long term prognosis of intracerebral haemorrhage

Contexte : Les hémorragies intracérébrales spontanées (HIC) sont grevées d’une mortalité élevée et d’un pronostic fonctionnel sombre. Les données concernant le pronostic à long terme des HIC sont rares. L’objectif principal de ce travail était d’étudier le pronostic au long cours des HIC en les abordant par le prisme de leur histoire naturelle.Méthodes : Nos populations d’étude sont issues de la cohorte prospective PITCH (Prognosis of IntraCerebral Haemorrhage) qui est une cohorte observationnelle ayant inclus de façon consécutive tous les patients admis au CHU de Lille pour une HIC spontanée entre 2004 et 2009. Nous avons étudié (i) l’incidence de la démence de novo post HIC ainsi que les facteurs prédictifs cliniques et neuroradiologiques associés à sa survenue ; (ii) la prévalence de la sidérose superficielle corticale (SSc) et les facteurs cliniques et radiologiques associés ; (iii) les facteurs prédictifs de récidive hémorragiques.Résultats : Nous avons mis en évidence qu’il existait un risque majeur de démence de novo chez les patients survivant à une HIC. Les facteurs prédictifs de démence identifiés tels que la localisation lobaire ou la SSc suggèrent une implication directe de l’angiopathie amyloïde cérébrale. Nous avons également montré qu’au sein de notre cohorte, un patient sur cinq avait de la SSc sur l’IRM cérébrale réalisée à l’admission. La SSc apparaissait être un facteur neuroradiologique prédictif majeur de récidive hémorragique.Conclusion : Les résultats de ce travail ont un impact important dans la prise en charge des patients ayant eu une HIC spontanée et permettront d’informer de façon adéquate les patients et leurs aidants. Ils apportent des informations nouvelles sur l’évaluation du risque de récidive hémorragique et sur d’éventuelles futures cibles thérapeutiques. / Background: The low frequency of spontaneous intracerebral haemorrhage (ICH) and its high mortality rate may explain the paucity of data in long term outcomes. The main objective was to study long term prognosis of ICH through the prism of their natural history.Methods: Our study populations were based on the PITCH (Prognosis of IntraCerebral Haemorrhage) cohort which is an observational study that included consecutively adults admitted at the Lille University Hospital for spontaneous ICH between 2004 and 2009. We aimed to determine (i) the incidence of new onset dementia and its clinical and radiological predictive factors; (ii) the prevalence of cortical superficial siderosis (cSS) and its associated factors; (iii) predictive factors of recurrent ICH.Results: We showed that the risk of new onset dementia is substantial after spontaneous ICH. Predictive factors of new onset dementia such as ICH lobar location and cSS suggest the implication of underlying cerebral amyloid angiopathy. We found that one out of five patients had cSS on baseline MRI. cSS was a strong predictive factor of recurrent ICH. Conclusion: These findings are of immediate clinical relevance in the management of ICH patients and will allow to adequately inform patients and caregivers. These results may provide additional information on ICH recurrence risk assessment and may contribute to the development of future therapeutic strategies.
