Caracterizaçao molecular e diversidade clonal de Staphylococcus aureus, isolados de leite de vacas com mastite subclinica no estado de São Paulo

Orientador: Vera Lúcia Mores Rall / Resumo: Staphylococcus aureus é um agente comum de mastite bovina, responsável por grandes perdas econômicas na pecuária mundial. Esse micro-organismo possui fatores de virulência bastante conhecidos como a produção de hemolisinas, leucotoxinas e superantígenos como a toxina do síndrome do choque tóxico e enterotoxinas. O objetivo do estudo foi caracterizar molecularmente os isolados de S. aureus provenientes de leite de vacas com mastite subclínica, de várias regiões do estado de São Paulo, através de Multilocus Sequence Typing (MLST), spa typing, Pulse Field Gel Electrophoresis (PFGE) e agr. Também fizeram parte dos objetivos, testes fenótipicos e genotípicos de resistência a antimicrobianos, além da pesquisa de genes de alguns fatores de virulência como o pvl (toxina de Panton Valentine), tst (toxina da Síndrome do Choque Tóxico, , sea-see, seg, seh e sei, a fim de verificar quais os fatores de virulência mais envolvidos nesse quadro. Todos os isolados de S. aureus foram sensíveis a meticilina (MSSA), pois os genes mecA e mecC não foram encontrados. Entre os 12 antibióticos testados, observou-se resistência intermediaria à eritromicina e nenhuma das cepas foram resistentes à oxacilina, vancomicina e gentamicina. Os isolados resistentes à tetraciclina apresentaram o gene tetK . Na caracterização molecular, observou-se 23 spa types diferentes com prevalência dos tipos t605 e t127. A maioria das cepas (48,1%) eram pertencentes ao grupo agr II, seguido de 20,1% do grupo III e 8,1%... (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Staphylococcus aureus is a common agent of bovine mastitis, causing economic losses in Brazilian livestock. This microorganism has well known virulence factors such as the production of hemolysins, leukotoxins and superantigens such as toxic shock syndrome toxin and enterotoxins. The objective of the study was to characterize molecularly isolates of S. aureus from milk of cows with subclinical mastitis, of the several regions of São Paulo State, through Multilocus sequence typing (MLST), spatyping, Pulse Field Electrophoresis Gel (PFGE) and agr. Phenotypic and genotypic tests of antimicrobial resistance, as well as the search for genes of some virulence factors such as pvl (Panton Valentine toxin), tst (Toxic Shock Syndrome toxin). In the present study, all isolates of S. aureus were sensitive to methicillin (MSSA), as the mecA and mecC genes were not found. About 12 antibiotics tested, intermediate resistance to erythromicyn was observed and none resistance to oxacillin, vancomycin and gentamicin. The tetracycline resistant isolates showed the tetK gene. Molecular characterization showed 23 different spa types with prevalence of t605 and t127. The most of the strains (48.1%) presented as belonging to the agr II group, followed by 20.1% of the agr III group and 8.1% of the agrI group, and the agr IV group was not found. Apesar da adoção de práticas de manejo para minimizar a ocorrência desta, os resultados não são satisfatórios para o controle de Staphylococcus aureus, o principal agente causador. Além da importância veterinária, possui implicações na saúde humana devido ao uso extensivo de antibióticos no tratamento e controle da doença e devido ao potencial de disseminação zoonótica de S. aureus. A principal preocupação em relação à resistência aos antimicrobianos é referente à meticilina, mediada pelo gene mecA. Estudos moleculares são importantes para o conhecimento da diversidade genética dos agentes causadores de mastite. A tipagem por sequenciamento de um único locus, do gene da região X da proteína A de S. aureus (spa Typing), tem sido utilizada com sucesso para investigar estrutura populacional, sendo adequada para estudos epidemiológicos. O presente projeto teve como objetivo pesquisar o perfil de resistência aos antimicrobianos e a presença de genes de resistência à meticilina e de S. aureus isolados de mastite bovina, além de compreender a diversidade genética, identificar possíveis novos perfis e conhecer a distribuição destes isolados. Os resultados encontrados foram a ausência de resistência à meticilina no antibiograma, confirmada pela ausência do gene mecA, e a baixa prevalência de multirresistência aos antimicrobianos testados. Apesar de mais de 58,74% dos isolados (242/412) serem resistentes a pelo menos um antibiótico, 53,64% (221/412) correspondem a resistência à ampicilina. Foram determinados 44 spa tipos diferentes, sendo os três mais frequentes t605 (57,45%), t002 (8,4%) e t127 (8,13%). Não houve associação entre a distribuição dos spa tipos e as variáveis analisadas de ano, estado, município e fazenda, e perfil de resitência aos antibicrobianos. Este é o primeiro estudo que utiliza o spa typing para avaliar uma coleção temporalmente diversa e, portanto, representativa da região estudada ao longo de 22 anos. Logo, espera-se que a descrição dos tipos de S. aureus obtidos contribua para a compreensão dos aspectos epidemiológicos envolvidos na transmissão do patógeno e do papel dos hospedeiros como reservatórios para linhagens resistentes. Tal compreensão é imprescindível para a prevenção, controle e tratamento da mastite bovina. / Bovine mastitis is considered the disease of greatest impact in global dairy industry. Despite the adoption of management practices to minimize its occurrence, the results are not satisfactory for the control of Staphylococcus aureus, the major causative agent. Besides veterinary importance, there are also implications for human health due to the extensive use of antibiotics in treatment and control of the disease and due to the potential for zoonotic spread of S. aureus. The main concern regarding antimicrobial resistance is methicillin resistance mediated by the mecA gene. Molecular epidemiology studies are important for the understanding of the genetic diversity of the causative agents of mastitis. Typing by sequencing of a single locus of the X region gene of the S. aureus protein A (spa Typing) has been successfully used to investigate population structure. This project aims to investigate the antimicrobial susceptibility profile and the presence of methicillin resistance genes in S. aureus isolated from bovine mastitis, in addition to understanding the genetic diversity, identifying possible new profiles and knowing the distribution of these isolates. The results obtained to the present were the absence of methicillin resistance in the antibiogram, confirmed by the absence of mecA gene, and the low prevalence of multidrug resistance to antimicrobials. Even though resistance to at least one antibiotic was present in more than 58,74% of the isolates (242/412), ampicillin resistance corresponds to 53.64% (221/412). Different spa types were determined, the three most frequent being t605 (57.45%), t002 (8.4%) and t127 (8.13%). There was no association between the spa type distribution and the variables of year, state, municipality, farm, and antibiotic resistance profile analyzed. This is the first study that uses spa Typing to evaluate a collection as temporarily diverse and representative of the region studied in 22 years. Therefore, it is expected that the results of this study, by allowing comparison of the profile of S. aureus from different sources (animals and property), will contribute to the understanding of the epidemiological aspects involved in the transmission of the pathogen and the role of hosts as reservoirs for pathogenic strains. Genotypic characterization of Staphylococcus aureus isolates causing bacteraemia in patients admitted to Tygerberg Hospital, Western Cape Province, South Africa

The rate of MRSA infections are rapidly increasing worldwide. Currently, at Tygerberg hospital, approximately a third of S. aureus isolates are MRSA. This was the first epidemiological study of S. aureus conducted at Tygerberg Hospital that included prospective clinical data on patients with S. aureus bacteraemia together with spa typing of strains and the detection of the mecA and pvl genes in a multiplex PCR. Clonal cluster groups of S. aureus isolates were obtained by BURP analysis and compared to international important clones. The molecular epidemiology of hospital acquired (HA), health-care associated (HCA) and community acquired (CA) S. aureus bacteraemic strains at this hospital was examined. Lastly, repeat isolates of patients were collected to analyse any possible organism-related factors associated with persistent and recurrent bacteraemia. We investigated a total number of 113 S. aureus strains from 104 patients (70% MSSA, 30% MRSA). Repeat strains consisted of nine isolates (from 5 patients). All isolates were obtained from blood cultures collected during the period March 2008 to May 2009. Phenotypic and genotypic detection of methicillin resistance correlated well. According to the literature, most CA-MRSA strains are distinguishable from HA-MRSA strains based upon the presence of the PVL toxin. However, no CA-MRSA was detected in our study, therefore the association between HA-MRSA versus CA-MRSA strains could not be analysed. In this study, CA-MSSA was identified in 22% of all MSSA isolates versus 0% CA-MRSA. PVL positive strains were found in 22.7% of all MSSA isolates with no detection in MRSA isolates. It was noted that MRSA strains clustered in spa CC-701 and CC-012, whereas CC-002 only contained MSSA strains. Likewise HA-strains representing the majority of MRSA strains also clustered in spa CC-701 and CC-012. Forty nine spa types were identified in 89.3% of all isolates, whereas 9.7% of these strains were non-typeable. Five novel spa types were revealed. We detected a diverse number of spa-types that correlated to international clones. The most predominant spa type found in our setting was t037 (only in MRSA), followed by t891. According to the literature, t037 is associated to the Brazilian/Hungarian clone (SCCmec type III; ST 239). Our findings, as well as other South African studies, indicate that t037 has been identified in clinical strains from numerous provinces in South Africa. Interestingly, all isolates from spa type t891 were PVL positive MSSA. Bacteraemia cases were predominantly related to catheter sepsis, followed by skin and soft tissue infections (SSTI). Only one persistent bacteraemia case was identified related to a HA-SSTI. Recurrent bacteraemia cases were found in patients on dialysis for chronic renal failure and in burns patients related to intravascular catheter infections. The local epidemiology of S. aureus and the prevalence rate of different strains are important to investigate. The information provided contributes to the epidemiology of staphylococcal strains causing bacteraemia in our setting. These insights are useful for optimal diagnostic and therapeutic measures. The techniques developed can be used to identify outbreaks and recurrent infections. / AFRIKAANSE OPSOMMING: S. aureus veroorsaak ernstige infeksies in die hospitaalomgewing en in die gemeenskap. Wêreldwyd, neem metisillien-weerstandige S. aureus (MRSA) infeksies vinnig toe. Huidiglik by Tygerberg hospitaal is ongeveer ‘n derde van S. aureus isolate MRSA. Hierdie is die eerste epidemiologiese studie by Tygerberg hospitaal wat prospektiewe kliniese data van pasiënte met S. aureus bakteremie saam met spa tipering en aantoning van die mecA en pvl gene in ‘n multipleks PKR insluit. Klonale groepe (spa-CC) van MRSA en MSSA isolate is deur BURP analise verkry, en vergelyk met internasionaal belangrike klone. Die molekulêre epidemiologie van hospitaalverworwe (HA), gesondheidsorgverworwe (HCA) en gemeenskapsverworwe (CA) S. aureus bakteremie by hierdie hospitaal is ondersoek. Fenotipiese en genotipiese aantoning van metisillien weerstandigheid het goed gekorreleer. Volgens die literatuur kan die meeste CA-MRSA isolate van HA-MRSA isolate onderskei word op grond van die teenwoordigheid van die PVL toksien. Geen CA-MRSA is egter in ons studie gevind nie, dus kon die assosiasie tussen HA-MRSA en CA-MRSA isolate nie ondersoek word nie. CA-MSSA was in 22% van alle MSSA geidentifiseer teenoor 0% CA-MRSA. PVL is in MSSA isolate gevind (22.7% van alle MSSA) maar glad nie in MRSA nie. Dit is opgemerk dat MRSA isolate hoofsaaklik in spa CC 701 en CC-012 kloongroepe voorkom, teenoor kloongroep CC-002 wat slegs MSSA isolate bevat het. Soortgelyk het HA-isolate wat die meerderheid van MRSA isolate verteenwoordig het ook in kloongroepe 1 & 2 gegroepeer. Nege-en-veertig spa tipes is geïdentifiseer in 89.3% of alle isolate en 9.7% was nie-tipeerbaar. Vyf nuwe spa tipes is getoon. Ons het ‘n diverse aantal spa-tipes geïdentifiseer wat met internasionale klone gekorreleer het. Die mees dominante spa tipe in ons omgewing was t037 (slegs in MRSA), gevolg deur t891. Volgens die literatuur word t037 met die Brasiliaanse/Hongaarse kloon geassosieer (SCCmec tipe III; ST 239). Ons bevindings, asook ander Suid Afrikaanse studies, dui aan dat t037 in kliniese isolate vanaf talle provinsies in Suid-Afrika aangetoon is. Van belang is dat al die isolate van spa tipe t891 MSSA en PVL positief was. Bakteremiese gevalle was hoofsaaklik geassosieer met kateter-sepsis, gevolg deur vel en sagteweefsel infeksies (SSTI). Slegs een persisterende bakteremiese geval was geïdentifiseer geassosieer met HA-SSTI. Herhalende bakteremiese episodes is in pasiënte op dialise vir kroniese nierversaking en in brandwonde pasiënte met intra-vaskulêre kateter infeksies aangetoon. Die lokale epidemiologie van S. aureus en die prevalensie koers van verskillende stamme is van belang. Hierdie inligting dra by tot kennis van die epidemiologie van stafilokokkale stamme wat in ons omgewing bakteremie veroorsaak. Hierdie insigte is nuttig vir optimale diagnostiese en terapeutiese riglyne. Die tegnieke wat ontwikkel is, kan gebruik word om uitbrake en herhalende infeksies te identifiseer.

Characterization of Community-acquired Methicillin-resistant Staphylococcus aureus by Pulsed-field Gel Electrophoresis, Multilocus Sequence Typing, and Staphylococcal Protein A Sequencing: Establishing a Strain Typing Database

Staphylococcus aureus has long been recognized as a leading cause of nosocomial infection. However, several recent publications have demonstrated this pathogen as the cause of community-acquired severe wound infections and necrotizing pneumonia in otherwise healthy individuals. These highly virulent endemic clones have been reported in several locations in the United States and Canada. The rapid spread of the organism, the ability of certain clones to cause serious infection, and the antibiotic resistance of the endemic clones, illustrates the importance of infection control measures. In this study we examined three S. aureus typing techniques; pulsed-field gel electrophoresis (PFGE), multilocus sequence typing (MLST), and Staphylococcal protein A (spa) sequencing for subspeciation of community-acquired methicillin-resistant S. aureus (CA-MRSA). It is hypothesized that PFGE will result in a higher level of discrimination among the strains, while MLST and spa typing will result in highly portable data that lacks the discriminatory power of PFGE. Thirty CA-MRSA isolates that were obtained from Florida and Washington State were characterized by molecular typing methods. Whole genome restriction analysis was performed by PFGE using the SmaI enzyme. Sequence-based typing analyses, MLST and spa typing, were performed by polymerase chain reaction (PCR) followed by sequencing. PFGE data was analyzed using the BioNumerics® software package and sequence-based data was analyzed using DNAstar®. MLST Alleles were assigned using the online MLST database (www.mlst.net) and spa types were assigned using the Ridom SpaServer (www.ridom.de/spaserver). Molecular characterization of the 30 isolates resulted in 21 pulsotypes, four MLST sequence types (STs), and six spa types. Combining data from both MLST and spa typing resulted in only seven strain categories, many of which grouped isolates that are not epidemiologically linked. These data demonstrate that techniques such as MLST and spa typing are not well suited for tracking isolates with limited evolutionary diversity such as the CA-MRSA epidemic clones.

Antibiotic Resistance in Wastewater : Methicillin-resistant Staphylococcus aureus (MRSA)and antibiotic resistance genes / Resistenta gula stafylokocker (MRSA) och antibiotikaresistensgener förekommer i svenskt kommunalt avloppsvatten

A large part of the antibiotics consumed ends up in wastewater, and in the wastewater the antibiotics may exert selective pressure for or maintain resistance among microorganisms. Antibiotic resistant bacteria and genes encoding antibiotic resistance are commonly detected in wastewater, often at higher rates and concentrations compared to surface water. Wastewater can also provide favourable conditions for the growth of a diverse bacterial community, which constitutes a basis for the selection and spread of antibiotic resistance. Therefore, wastewater treatment plants have been suggested to play a role in the dissemination and development of antibiotic resistant bacteria. Methicillin-resistant Staphylococcus aureus (MRSA) is a large problem worldwide as a nosocomial pathogen, but knowledge is limited about occurrence in non-clinical environments, such as wastewater, and what role wastewater plays in dissemination and development of MRSA.   In this thesis we investigated the occurrence of MRSA in a full-scale wastewater treatment plant (WWTP). We also investigated the concentration of genes encoding resistance to aminoglycosides (aac(6’)-Ie+aph(2’’)), β-lactam antibiotics (mecA) and tetracyclines (tetA and tetB) in three wastewater-associated environments: (1) soil from an overland flow area treating landfill leachates, (2) biofilm from a municipal wastewater treatment plant, and (3) sludge from a hospital wastewater pipeline. In addition, concentrations of mecA, tetA and tetB were investigated over the treatment process in the WWTP. These investigations were performed to determine how the prevalence and concentration of MRSA and the antibiotic resistence genes are affected in wastewater and wastewater treatment processes over time. The occurrence of MRSA was investigated by cultivation and a commercially available real-time PCR assay. In order to determine concentrations of the genes aac(6’)-Ie+aph(2’’), mecA, tetA and tetB in wastewater we developed a LUXTM real-time PCR assay for each gene.   Using cultivation and real-time PCR we could for the first time describe the occurrence of MRSA in wastewater and show that it had a stable occurrence over time in a WWTP. MRSA could mainly be detected in the early treatment steps in the WWTP, and the wastewater treatment process reduced the number and diversity of cultivated MRSA. However, our results also indicate that the treatment process selects for strains with more extensive resistance and possibly higher virulence. The isolated wastewater MRSA strains were shown to have a close genetic relationship to clinical isolates, and no specific wastewater lineages could be detected, indicating that they are a reflection of carriage in the community. Taken together, these data indicate that wastewater may be a potential reservoir for MRSA and that MRSA are more prevalent in wastewater than was previously thought.   The real-time PCR assays, for aac(6’)-Ie+aph(2’’), mecA, tetA, and tetB that we developed, were shown to be sensitive, fast, and reproducible methods for detection and quantification of these genes in wastewater environments. The highest concentrations of all genes were observed in the hospital pipeline, and the lowest in the overland flow system, with tetA and aac(6´)-Ie+aph(2´´) detected in all three environments. In the full-scale WWTP, we continuously detected mecA, tetA and tetB over the treatment process and over time. In addition, it was shown that the treatment process reduces concentrations of all three genes. Cystic fibrosis (CF) is the most common autosomal recessive genetic disorder in Caucasians. Lower respiratory tract of CF patients is colonized by specific bacteria, often leading to chronic infection and lung tissue damage. In this thesis we characterized 338 isolates of S. aureus from 92 Czech CF patients isolated in 2011-2013. Using spa typing and PFGE we detected high clonal heterogenity of this collection with the exception of MRSA strains (resistant to oxacillin; 5% prevalence) which were clonally related. The prevalence of S. aureus MLSB resistance in our collection was high (69 %), which is a serious problem due to common usage of these antimicrobials in clinical practice. A half of the MLSB resistant strains lacked any known determinant of this resistance (ermA, ermC, ermT, msrA). Sequencing of the ribosomal genes revealed a high number of S. aureus strains carrying target site mutations resulting in MLSB resistance (37 %). This is new important information about the staphylococcal strains associated with chronic infections in Czech Republic. Die subklinische Staphylococcus aureus-Mastitis - Sanierung eines hessischen Milcherzeugerbetriebes und epidemiologische Untersuchungen mittels Staphylokokken-Protein A (spa)-Typisierung

In der vorliegenden Studie wurde die Sanierung einer durch S. aureus verursachten Eutergesundheitsstörung in einem hessischen Milchviehbestand mittels klassischer Sanierungsmaßnahmen über einen Zeitraum von 18 Monaten begleitet. Durch konsequente Einhaltung der Sanierungsmaßnahmen nach dem Fünf-Punkte-Plan und die räumliche Trennung der Herde in eine S. aureus-positive und -negative Gruppe sank die S. aureus-Prävalenz im Betrieb innerhalb von 15 Monaten von 30% auf <1%. Nach 18 Monaten waren erstmalig keine Neuinfektionen mehr mit S. aureus zu verzeichnen. Zusätzlich zu den im Abstand von drei Monaten durchgeführten Viertelanfangsgemelk (VAG)- Gesamtbestandsuntersuchungen wurden Umweltproben bakteriologisch auf das Vorhandensein von S. aureus untersucht. Diese Untersuchungen verliefen ausschließlich mit negativem Ergebnis. Die im Untersuchungszeitraum asservierten 218 S. aureus-Isolate aus diesem Betrieb wurden genotypisch mittels PCR untersucht. Thermonuklease-Gen, Protein A-Gen (IgG-bindende Region) und Polymorphismen bei Protein A-Gen (Xr-Region) sowie Koagulase-Gen ermöglichten die Unterscheidung in sechs Typen. Zusätzlich wurden 80 dieser Isolate mittels Pulsfeldgelelektrophorese (Pfge, Gold Standard) typisiert. Es konnten zwei Pfge-Typen gefunden werden: Pfge-Typ I mit insgesamt 10 Subtypen (bei 78 Isolaten) und Pfge-Typ II (bei zwei Isolaten). Der Pfge-Typ II wurde ausschließlich bei einem Zukaufstier nachgewiesen, das vor der Einstallung in diesen Betrieb nicht zytobakteriologisch untersucht worden war. Die 12 unterschiedlichen Pfge-Typen bzw. -Subtypen wurden zusätzlich mittels Staphylokokken-Protein A- (spa)-Typisierung untersucht. Dabei zeigte sich, dass alle Subtypen des Pfge-Typs I dem spa-Typ t2067 zugeordnet werden konnten und der Pfge-Typ II dem spa-Typ t2112 entsprach. 92 weitere bovine S. aureus-Mastitisisolate wurden möglichst randomisiert über das Landesgebiet Hessens entnommen und mittels dieser Typisierungsmethode untersucht. Die Isolate stammten ebenfalls aus S. aureus-Problembetrieben und wurden aus VAG von (sub-) klinischen Mastitiden in Reinkultur isoliert. Insgesamt konnten 28 spa-Typen unterschieden werden. Durch den Algorithmus BURP wurden 57 dieser Isolate dem Clonal Complex CC543 zugeordnet und waren demnach genetisch eng miteinander verwandt. Der in dem Sanierungsbetrieb vorherrschende spa-Typ t2067 war dem CC543 nicht zuzuordnen. Die Anwendung der spa-Typisierung in der Routinediagnostik und damit die Möglichkeit der laborübergreifenden, möglichst zentralisierten Datendokumentation der Ergebnisse könnten zukünftig die Identifizierung besonders häufig vorkommender und damit hochkontagiöser S. aureus-Stämme ermöglichen.
